[Recent advances in geriatric vitaminology].

The recent report concludes that elderly people have satisfying the recommended daily allowance in nutritional consumption except for calcium. However, those who are institutionalized, of low income, alcoholic and living alone are at high risk of vitamin deficiency. The high incidence in vitamin deficiency is shown when vitamin levels were measured with biological activity. Physiological range of vitamin seems to be enough to normalize this deficiency and nutritional supplementation would be useful. Those who are with the lack of calcium consumption or of time spent under the sunshine are at h

Dental implants and the geriatric patient.

Dental care of the aging patient presents a number of problems not encountered in younger patients. Most of these problems result from tissue changes that occur during aging. Dental implants and implant-retained and/or implant-supported prostheses are viable treatment options for older patients. The majority of elderly patients in their younger retirement years are relatively healthy and not limited in activity. The clinician must be aware of the physical, metabolic, and endocrine changes associated with aging and how these changes may affect implant treatment. The elderly deserve the best car

Geriatric gynecologic care of minorities.

African-American and Hispanic-American women, as well as women of other minorities are susceptible to the same gynecologic diseases as white women. Elderly minority patients need gynecologic care; they reluctantly seek care because they harbor both fear and distrust. Gynecologists who care for the elderly are expected to provide the same quality of care to whites and minorities. However, ethnicity, cultural differences, and socio-economic status can impact negatively on outcomes. This racial/ethnic disparity is most clearly demonstrated in the poor prognosis, increased mortality, and decreased

Laparoscopic cholecystectomy in the geriatric population.

Although the role of laparoscopic cholecystectomy (LC) as a safe and cost effective procedure has been ascertained, its role in the geriatric population, the majority of whom present with coexistent diseases, has yet to be defined. We retrospectively reviewed outcome parameters of 144 consecutive patients over age 65 undergoing LC, for both acute cholecystitis and symptomatic cholelithiasis. These results were compared with 72 patients having open cholecystectomy (OC) during the same time period and in the year preceding the introduction of LC. Groups were well matched with respect to age, age

Recent advances in geriatric psychopharmacology.

Psychopharmacotherapy of the elderly must take into account the effects of age-related changes in the structure and function of the brain and various organs. In general, older people are more sensitive than young people to both the therapeutic and toxic effects of psychotropic medications, necessitating lower doses and longer dosage intervals. This holds true for the treatment of 5 major types of psychiatric illness (depression, bipolar disorder, anxiety, psychotic disorders and dementia). The tricyclic antidepressants, although efficacious, inexpensive, and backed by 30 years of experience, a

Pharmacologic considerations in the geriatric patient.

A physician has the primary responsibility of first causing no harm. Unfortunately, the myriad of pharmacologic agents available to the clinician at times may make this directive difficult. When the variable of age-related changes is introduced, the clinician's task may seem almost impossible. The most important aspect of gerontology is that there are no constants. Age-related changes are inconsistent. Although liver function may vary little among the general population, virtually every other system is affected by the aging process. Additional factors that complicate treatment of the geriatric

Craniomandibular disorders in the geriatric patient.

This paper represents a general review of basic age-related changes that take place in the craniomandibular apparatus and the most frequently presenting conditions associated with craniomandibular disorders (CMD) in the elderly. The evaluation of geriatric patients with signs or symptoms of CMD must consider (1) normal age-related changes in the craniomandibular apparatus and their impact on both normal function and responses to stress; (2) the role of dentition status and dental prosthesis in CMD; and (3) the contribution of malignant disease, psoriasis, arthritic conditions, pseudogout, gran

The geriatric auditory and vestibular systems.

Age-related declines in auditory and vestibular function are becoming more prevalent and better understood. The personal and societal impact of these disorders is impressive, and present techniques of amplification or balance training can be of great benefit. However, many issues await investigation, and continued integration of ever-increasing technologies promises many new answers, and questions, in the future.

[Out-of-hospital geriatric facilities].

In France there are many and varied housing structures for the elderly. They were created after the war in response to the progressive ageing of the population, bringing in its wake an ever increasing dependence. These structures consist of accommodation equipped for almost complete nursing care. There is a large number of such establishments situated between individual homes and long-term hospital care: homes in the form of small family type reception units (such as sheltered housing), living with a family, rest-homes and sun-homes for the least disabled. Service-flats provide a further large

The acute abdomen in the geriatric patient.

Evaluating an elderly patient with an acute abdomen is a challenging clinical problem. THe diagnosis is more difficult to secure in this age group, because it is difficult to obtain an accurate history, and physical findings are more subtle. The major factors that increase mortality include associated medical diseases, emergent operations, and delay in recognition and treatment of the problem. Patients with widespread malignancy and generalized peritonitis fare poorly, but operative mortality in the geriatric group has improved significantly because of more prompt diagnosis, aggressive resusci

Diabetes mellitus in geriatric females.

A study was carried out on 36 geriatric diabetic females (above 60 years). Marked exhaustion and significant loss of weight were common presenting complaints (60%) besides usual symptomatology. Only 25% patients were asymptomatic for diabetes. Generalised itching (20%) and pruritus vulvae (33.3%) were other common presenting complaints. Neuropathy was found to be the commonest complication being present in 77.7% patients, followed by retinopathy (50%) and nephropathy (27.7%). Hypertension was found to be associated in 44% patients and evidence of ischaemic heart disease was found in 42%. Devel

Wertheim hysterectomy in the geriatric population.

A series of 38 patients 65 years of age or older who underwent Wertheim hysterectomy for primary treatment of stages IB and IIA (FIGO) carcinoma of the cervix is reported. There were no ureteral fistulas and no perioperative mortality, but 4 of the patients required reoperation. A control population of 320 patients less than age 65 was identified who had been treated in the same fashion and who resembled the older group in regard to stage, histologic features, grade, and incidence of pelvic nodal metastasis. Transfusion requirements and incidence of postoperative lower extremity edema were sim

Atypical presentation of geriatric infections.

Infection is a common problem in elderly patients, the most common infections being those in the urinary tract and the respiratory tract. In geriatric practice, infection is most frequently seen in combination with many other problems. The geriatric presentation is described in which patients may present with an increased dependency upon care providers due to the development of fresh problems like falls, immobility, confusion, incontinence of urine, and fecal impaction with incontinence. It is important to recognize this presentation and not to miss the diagnosis. It is well recognized that pn

Geriatric depression and treatment with trazodone.

With the growing number of Americans over the age of 65 years, the high incidence of geriatric depression has become a major concern in the United States. Age-related circumstances--increased incidence of illness, bereavement, financial difficulties, and institutionalization--may contribute to an increased rate of depression in this age group. The signs and symptoms of depression in elderly patients are similar to those seen in younger individuals; therefore, standard Diagnostic and Statistic Manual III (DSM-III) criteria are reliable for making a diagnosis. However, symptoms such as insomnia,

Pharmacokinetics of temazepam in geriatric patients.

A single dose of temazepam 10 mg, as a solution in soft gelatin capsules, was given to 10 fasting geriatric in-patients (mean age 83 years) in a stable clinical condition. The mean peak plasma concentration was 306 ng/ml, with a median time of 0.75 h to peak concentration. Temazepam was eliminated from plasma in a biexponential manner, with a distribution phase (mean t1/2 alpha = 0.7 h) predominating for 3 h. The drug had a mean elimination half-life of 8.7 h. In a chronic study, in which temazepam 10 mg p.o. was given nightly to 13 patients, the plasma concentrations on Days 3, 5, 8, 12 and 1

Cardiac arrests in a geriatric unit.

The outcome of cardio-pulmonary resuscitation within an acute geriatric unit was studied in 95 patients (mean age 77 years, range 65-90) who were involved in 106 consecutive cardio-respiratory arrest calls. In 58 patients (61%), initial resuscitation was unsuccessful and a further 21 (22%) died later in hospital. Fourteen patients (15%) were alive 3 months after hospital discharge, a success rate comparable to that of published series in younger patients. The probability of successful resuscitation was greater in patients in the High-dependency Unit, and ventricular fibrillation and a short du
