"Silent" rupture of unscarred gravid uterus with subsequent pelvic abscess: successful laparoscopic management.

Intrapartum rupture of an unscarred uterus is rare in current times. However, it is still associated with significant maternal and fetal mortality and morbidity. Unlike rupture or dehiscence of a previous cesarean scar, which is occasionally bloodless, complete rupture of a gravid unscarred uterus frequently results in fetal jeopardy and significant maternal intraperitoneal bleeding, causes acute abdomen, and demands emergency surgical (laparotomy) intervention. Laparoscopy generally has no role in such circumstances due to the generally unstable maternal hemodynamic condition and the necessit

The direct depressant effects of desflurane and sevoflurane on spontaneous contractions of isolated gravid rat myometrium.

Our purpose was to investigate the direct depressant effects of desflurane and sevoflurane at 0.5, 1 and 2 minimum alveolar concentrations (MAC) on spontaneous contractions of isolated gravid rat myometrium. Ten gravid, albino Wistar rats, weighing 240-310 g and at 19-20 days' gestation were used. Sixty myometrial strips were obtained from 10 rats, and randomly assigned into six groups of 10. After obtaining spontaneous myometrial contractions in de Jalon solution for 45 min, 0.5, 1 or 2 MAC of desflurane or sevoflurane were continuously bubbled in the bath for 15 min and myometrial contractio

Life event stress and the association with spontaneous abortion in gravid women at an urban emergency department.

In this study, the authors hypothesized that life event stress is associated with an increased risk of spontaneous abortion. Using a nested case-control design in an emergency department (N = 970), stress was measured using a life event inventory and a sample drawn from R. B. Ness et al.'s (1999) Early Pregnancy Study. Gestational age at time of fetal loss served as a marker of chromosomal status. Women experiencing more than one life event used more alcohol and public assistance. Spontaneous abortion at 11 weeks or greater was associated with more life event stress (adjusted odds ratio 2.9, 9

Conservative obstetric management of a gunshot wound to the second-trimester gravid uterus. A case report.

BACKGROUND: Gunshot wounds to the abdomen during pregnancy are becoming increasingly common. When the gravid uterus is injured but repairable and the fetus is either previable or dead, conservative management is an option, even if laparotomy is required for management of other injuries. CASE: A pregnant woman sustained a gunshot wound to the abdomen during the second trimester of pregnancy. The bullet injured the rectum and traversed the uterine cavity, with resultant rupture of membranes and direct, fatal injury to the fetus. Laparotomy was required for treatment of the maternal injuries; ho

High-velocity penetrating wounds of the gravid uterus: review of 16 years of civil war.

OBJECTIVE: To evaluate the value of selective laparotomy in pregnant women with penetrating abdominal injuries. METHODS: A retrospective survey was carried out at our center over 16 years of civil war, extending from 1975 to 1991. Fourteen pregnant women had uterine injuries secondary to high-velocity abdominal penetrating trauma. The corresponding management was evaluated carefully with respect to maternal and fetal outcomes. RESULTS: Two maternal deaths occurred, neither resulting solely from intra-abdominal injuries. Visceral injuries were present when the entrance of the missile was in e

Early childhood psychosis and bleeding during pregnancy. A prospective study of gravid women and their offspring.

Children with infantile autism and childhood psychoses were identified in the National Institute of Neurological Diseases and Stroke Collaborative Perinatal Study. They were matched with two control groups one of normal-IQ and the other of low-IQ children. Perinatal data, which had been collected prospectively, were then rated blindly. The relationship between uterine bleeding in mothers and their subsequently delivered children, who developed the syndromes of autism and childhood psychosis, was found to be significant. Mid-trimester bleeding was especially prevalent. The bleeding was usually

Gravidity and malaria trends interact to modify P. falciparum densities and detectability in pregnancy: a 3-year prospective multi-site observational study.

BACKGROUND: Low-density Plasmodium falciparum infections prevail in low transmission settings, where immunity is expected to be minimal, suggesting an immune-independent effect on parasite densities. We aimed to describe parasite densities in pregnancy, and determine how gravidity and antibody-mediated immunity affect these, during a period of declining malaria transmission in southern Mozambique. METHODS: We documented P. falciparum infections at first antenatal care visits (n = 6471) between November 2016 and October 2019 in Ilha Josina (high-to-moderate transmission area), Manhiça (low tra

Is gravidity 4+ a risk factor for oral clefts? A case-control study in eight South american countries using structural equation modeling.

Background : There is disagreement about the association between cleft lip with or without cleft palate and multigravidity, which could be explained by differences of adjusting for maternal age, Amerindian ancestry, and socioeconomic status. Objective : The aim was to evaluate gravidity 4+ (four or more gestations) as a risk factor for cleft lip with or without cleft palate in South America. Design : We used a matched (1:1) case-control study with structural equation modeling for related causes. Data were obtained from 1,371,575 consecutive newborn infants weighing ≥500 g who were born in the

Gravidity-dependent protection against placental malaria is associated with enhanced natural killer cell-mediated antibody-dependent cellular activation: a nested cohort study.

BACKGROUND: Placental malaria disproportionately affects primigravid women and is a leading cause of morbidity in endemic settings. IgG specific to VAR2CSA, a placenta-binding Plasmodium falciparum protein, accumulates with successive pregnancies, but protective mechanisms remain incompletely understood. We investigated whether natural killer antibody-dependent cellular activation (NK-ADCA) was acquired in a gravidity-dependent manner and associated with improved birth outcomes. METHODS: We characterised second trimester NK-ADCA and VAR2CSA-specific antibody features among pregnant women in E

Spontaneous Reduction of an Incarcerated Gravid Uterus With a Giant Uterine Fibroid in Late Pregnancy: A Case Report.

Incarceration of the gravid uterus is a rare obstetric complication causing serious maternal and fetal morbidity. This report describes the case of a 43-year-old gravida 3, para 0 woman with a giant uterine fibroid who was diagnosed with an incarcerated gravid uterus at 24 gestational weeks based on transvaginal ultrasonography and magnetic resonance imaging (MRI). The largest fibroid measured 15 cm in maximum diameter and was thought to limit uterine mobility; therefore, conservative management with a planned cesarean delivery was selected. Repeat MRI at 34 weeks unexpectedly demonstrated spo

Minimally Invasive Adhesiolysis for the Incarcerated Gravid Uterus: A Case Report of Early Second Trimester Intervention.

Incarceration of the gravid uterus is a rare obstetric complication in which the retroverted uterus becomes trapped in the pelvis, often resulting in maternal and fetal morbidity if not recognized and managed early. We present the case of a 36-year-old multigravida at 15 weeks' gestation with no prior abdominal surgeries who initially presented with urinary retention and was diagnosed with an incarcerated gravid uterus. Multiple attempts at manual reduction under spinal and general anesthesia were unsuccessful. Intraoperative transabdominal ultrasound demonstrated a live intrauterine pregnancy

Management of Acute Urinary Retention due to Bladder Outlet Obstruction in the Setting of Incarcerated Gravid Uterus.

OBJECTIVE: An incarcerated gravid uterus (IGU) is a rare condition that occurs when a retroverted gravid uterus becomes trapped between the sacral promontory and pubic symphysis. If untreated, IGU could have devastating consequences including uremia, renal failure, sepsis, peritonitis, uterine wall necrosis/rupture, and ultimately maternal and/or fetal death. Our objective is to provide education on the presentation, diagnosis, and management of a rare diagnosis to assist clinicians in better recognizing and treating IGU. METHODS: We present two cases of gravid females with acute urinary rete

Regulatory mechanisms of HMGB1 and its receptors in polycystic ovary syndrome-driven gravid uterine inflammation.

High-mobility group box 1 (HMGB1) is critical for inflammatory homeostasis and successful pregnancy, and there is a strong association among elevated levels of HMGB1, polycystic ovary syndrome (PCOS), chronic inflammation and pregnancy loss. However, the mechanisms responsible for PCOS-driven regulation of uterine HMGB1 and its candidate receptors [toll-like receptor (TLR) 2 and 4] and inflammatory responses during pregnancy remain unclear. In this study, we found a gestational stage-dependent decrease in uterine HMGB1 and TLR4 protein abundance in rats during normal pregnancy. We demonstrated

Efficacy of well-planned management in patients with incarcerated gravid uterus: A case series and literature review.

OBJECTIVE: Incarcerated gravid uterus is a condition in which uterine myoma and intraperitoneal adhesion lead to persistent uterine retroversion. Accurate diagnosis before cesarean section is crucial so that the procedure can be planned with regard to the spatial relationship between the uterine incision and other organs. This study investigated the effects of well-planned management on the outcome of cesarean sections. MATERIALS AND METHODS: Four patients with incarcerated gravid uterus who received well-planned management and preoperative magnetic resonance imaging were compared with three

Efficacy of laboratory tests and ultrasonography in the diagnosis of acute appendicitis in gravid patients according to the stages of pregnancy.

BACKGROUND: Normal physiologic changes during pregnancy result in similar laboratory and symptomatology changes as those during acute appendicitis (AA), making the diagnosis extremely difficult. The aim of the present study was to analyze the efficacy of conventional laboratory and radiologic tests in the diagnosis of AA according to different stages of pregnancy. METHODS: Twenty-five pregnant patients with pathologically confirmed AA operated at our department between 2012 and 2017 were retrospectively analyzed in terms of changes in conventional laboratory parameters as well as neutrophil-t

Incarceration of early gravid uterus with adenomyosis and myoma: report of two patients managed with uterine reduction.

Although gravid uterine incarceration is typically diagnosed during the early second trimester, we encountered two unusual cases in early pregnancy. A 34-year-old multiparous woman with adenomyosis presented at 7 + 2 weeks of gestation with increased urinary frequency and a sensation of incomplete bladder emptying. The uterine incarceration was successfully reduced by manual reduction and pessary insertion, and she delivered a normal infant at term. In the second case, a 31-year-old nulliparous woman with a large myoma complained of dysuria, acute urinary retention, and intense back pain at 6

Maternal pre-gravid cardiometabolic health and infant birthweight: A prospective pre-conception cohort study.

BACKGROUND AND AIMS: Both low birthweight and high birthweight have been associated with the development of cardiometabolic disease in adulthood, possibly reflecting the effect of intrauterine fetal programming. As developmental programming can begin before conception, pre-gravid factors that predict birthweight may be relevant in this context. However, little is known about such factors. Thus, we established a pre-conception cohort to identify maternal pre-gravid cardiometabolic determinants of infant birthweight. METHODS AND RESULTS: In this prospective observational cohort study, 1484 newl

Midtrimester spontaneous torsion of unruptured gravid rudimentary horn: Presurgical diagnosis on magnetic resonance imaging.

Unicornuate uterus with rudimentary horn occurs due to failure of complete development and partial fusion of one of the Müllerian ducts. Pregnancy in a non-communicating rudimentary horn is extremely rare, with a reported incidence of 1 in 76 000-150 000 pregnancies, and usually terminates in rupture during the first or second trimester. Clinical diagnosis of rudimentary horn pregnancy in a woman with history of normal vaginal delivery in prior gestations is difficult. The role of sonography, and more recently, magnetic resonance imaging, in the presurgical diagnosis of rudimentary horn pregna
