Community-acquired meningitis caused by a CG86 hypervirulent Klebsiella pneumoniae strain: first case report in the Caribbean.

BACKGROUND: Community-acquired bacterial meningitis due to Klebsiella pneumoniae has mainly been described in Southeast Asia and has a poor prognosis. Severe invasive infections caused by K. pneumoniae, including meningitis, are often due to hypervirulent strains (hvKP), which are characterized by capsular serotypes K1 and K2, a gene responsible for hypermucoviscosity, and the cluster for synthesis of the siderophore aerobactin. CASE PRESENTATION: A 55 year old man with a history of essential hypertension, benign prostate hyperplasia, hyperlipidemia, obstructive sleep apnea, and chronic alcoh

Lipid profiles in French West Indies sickle cell disease cohorts, and their general population.

BACKGROUND: The pathophysiology of sickle cell disease (SCD) and the variability of its clinical expression remain not fully understood, whether within or between different SCD genotypes. Recent studies have reported associations between lipid levels and several SCD complications. If lipid levels have been previously described as low in sickle cell anemia (SCA), few data have been provided for sickle cell SC disease (SCC). We designed our epidemiological study to isolate lipid levels and profiles by genotype in Guadeloupian cohorts of SCA and SCC adult patients, at steady state. We compared SC

Chikungunya virus in Europe: A retrospective epidemiology study from 2007 to 2023.

BACKGROUND: Chikungunya virus (CHIKV), a mosquito-borne alphavirus, is responsible for disease outbreaks worldwide. However, systematic knowledge of spatiotemporal distribution and risk patterns of CHIKV in mainland Europe remains unclear. Our aim was to decipher the epidemiological characteristics, diversity, and clinical manifestations of CHIKV. METHODS: In this retrospective study, we retrieved the surveillance bulletins of chikungunya infections reported in Europe during 2007-2023 to depict the epidemiological characteristics. We performed genotyping and phylogenetic analyses to examine t

Climate-driven models of leptospirosis dynamics in tropical islands from three oceanic basins.

BACKGROUND: Leptospirosis is a neglected zoonosis which remains poorly known despite its epidemic potential, especially in tropical islands where outdoor lifestyle, vulnerability to invasive reservoir species and hot and rainy climate constitute higher risks for infections. Burden remains poorly documented while outbreaks can easily overflow health systems of these isolated and poorly populated areas. Identification of generic patterns driving leptospirosis dynamics across tropical islands would help understand its epidemiology for better preparedness of communities. In this study, we aim to m

Metallome deregulation and health-related impacts due to long-term exposure to recent volcanic ash deposits: New chemical and isotopic insights.

Volcanic ash exposure can lead to significant health risks. Damage to the respiratory and pulmonary systems are the most evident toxic side effects although the causes of these symptoms remain unclear. Conversely, the effects on other organs remain largely under-explored, limiting our understanding of the long-term volcanic ash-related risk at the whole-body scale. The metallome i.e. metal concentrations and isotopic compositions within the body, is suspected to be affected by volcanic ash exposure, having thus the potential for capturing some specificities of ash toxicity. However, the means

Association of UGT1A1 polymorphism with prevalence and age at onset of cholelithiasis in sickle cell anemia.

BACKGROUND AND OBJECTIVES: High levels of erythrocyte destruction in sickle cell anemia (SCA) result in chronic hyperbilirubinemia, with cholelithiasis occurring in a subset of patients. We investigated whether susceptibility to cholelithiasis in SCA was associated with the promoter polymorphism of the 5?-diphosphate-glucuronosyltransferase 1A1 (UGT1A1) gene encoding a key enzyme in bilirubin catabolism. DESIGN AND METHODS: We determined the frequencies of UGT1A1 promoter alleles in 171 SCA children and 153 SCA adults regularly followed for a number of years at the Guadeloupe sickle cell cent

Leptospirosis in the Caribbean Region between 2000 and 2022: A scoping review of morbidity and mortality.

BACKGROUND: Leptospirosis remains an important yet underreported public health concern in the Caribbean. Urbanisation, biodiversity loss and human encroachment into natural habitats have contributed to shifts in its epidemiological patterns. However, accurate assessment of disease burden is hindered by limited diagnostic capacity, surveillance challenges, and scarce research. We aim to describe geographical and temporal distribution of leptospirosis epidemiology in Caribbean Island Countries and Territories (CRICTs) and identify patterns and gaps in knowledge. METHODOLOGY/PRINCIPAL FINDINGS:

Frailty and 12-month mortality among older adults with type 2 diabetes in nursing homes: A longitudinal study.

BACKGROUND: Frailty is highly prevalent among older adults with type 2 diabetes mellitus (T2DM) and may contribute to adverse health outcomes, particularly in institutionalized settings. Despite its clinical relevance, the prognostic value of frailty among nursing home residents with T2DM remains underexplored. This study aimed to assess the association between frailty, assessed using the Frailty Index (FI), and 12-month all-cause mortality among older adults with T2DM residing in French Caribbean nursing homes. METHODS: Data were drawn from the KASEHPAD (Karukera Study on Aging in Nursing Ho

Desert dust episodes during pregnancy are associated with increased preterm delivery in French Guiana.

Preterm deliveries are a major multifactorial public health problem in French Guiana. Desert dust episodes have been associated with preterm delivery in Guadeloupe, a territory with similarities to French Guiana. We thus tried to replicate this finding in the context of French Guiana. A retrospective ecological cohort study combined daily PM10 concentration measurements during pregnancy and term at delivery extracted from French Guiana's computerized pregnancy delivery registry. Daily PM10 concentrations during the course of pregnancy were analyzed as mean concentrations and as the proportion

Genome-Wide Scan and Test of Candidate Genes in the Snail Biomphalaria glabrata Reveal New Locus Influencing Resistance to Schistosoma mansoni.

BACKGROUND: New strategies to combat the global scourge of schistosomiasis may be revealed by increased understanding of the mechanisms by which the obligate snail host can resist the schistosome parasite. However, few molecular markers linked to resistance have been identified and characterized in snails. METHODOLOGY/PRINCIPAL FINDINGS: Here we test six independent genetic loci for their influence on resistance to Schistosoma mansoni strain PR1 in the 13-16-R1 strain of the snail Biomphalaria glabrata. We first identify a genomic region, RADres, showing the highest differentiation between su

Computer-assisted analysis of Mycobacterium avium fingerprints using insertion elements IS1245 and IS1311 in a Caribbean setting.

A total of 33 clinical isolates of the Mycobacterium avium complex from 25 patients, identified by means of biochemical and cultural characteristics, the Accuprobe system and DT1/DT6 PCR, were further analysed using novel insertion elements IS1245 and IS1311 in a French Caribbean setting. PvuII-cleaved DNA and non-radioactive Southern hybridization and detection systems were used for fingerprinting with both IS elements. The data confirmed the specificity of the two probes for M. avium in our setting and highlighted a significant proportion of M. intracellulare-infected patients in this region

Estimating the burden of leptospirosis in the Caribbean: Insights from environmental and sociodemographic factors.

INTRODUCTION: Previous studies up to early 2000s found that leptospirosis incidence in humans was high across the Caribbean region (CR), yet up-to-date and reliable surveillance data are scarce. Limited research capacity in the region has further contributed to less robust characterisation of transmission drivers, perpetuating a cycle of neglect. To address these gaps and support evidence-based public health responses, this study aims to update incidence estimates in the CR by integrating data from multiple data sources, including peer-reviewed publications, surveillance reports and environmen

Bacterial microbiota management in free-living amoebae (Heterolobosea lineage) isolated from water: The impact of amoebae identity, grazing conditions, and passage number.

Free-living amoebae (FLA) are ubiquitous protozoa mainly found in aquatic environments. They are well-known reservoirs and vectors for the transmission of amoeba-resistant bacteria (ARB), most of which are pathogenic to humans. Yet, the natural bacterial microbiota associated with FLA remains largely unknown. Herein, we characterized the natural bacterial microbiota of different FLA species isolated from recreational waters in Guadeloupe. Monoxenic cultures of Naegleria australiensis, Naegleria sp. WTP3, Paravahlkampfia ustiana and Vahlkampfia sp. AK-2007 (Heterolobosea lineage) were cultivate

Magnetic resonance imaging lesion pattern in Guadeloupean parkinsonism is distinct from progressive supranuclear palsy.

In the Caribbean island of Guadeloupe, patients with atypical parkinsonism develop a progressive supranuclear palsy-like syndrome, named Guadeloupean parkinsonism. Unlike the classical forms of progressive supranuclear palsy, they develop hallucinations and myoclonus. As lesions associated with Guadeloupean parkinsonism are poorly characterized, it is not known to what extent they differ from progressive supranuclear palsy. The aim of the present study was to determine the structural and metabolic profiles of Guadeloupean parkinsonism compared with progressive supranuclear palsy and controls u
