Patients re-engaging with HIV care in Guatemala: Prioritizing CD4 counting and screening for histoplasmosis and tuberculosis.

BACKGROUND: Discontinuation of antiretroviral therapy (ART) significantly contributes to the development of advanced HIV disease (AHD) and opportunistic infections. This study analyzed data from patients who re-engaged in care after ART interruption and compared the cohort with patients with newly diagnosed HIV, focusing on the burden of tuberculosis and histoplasmosis. METHODS: A diagnostic package for opportunistic infections was implemented in Guatemala in 2017, encompassing tuberculosis and histoplasmosis. From 2017 to 2019, we enrolled 1379 adults re-engaging in care and 3412 patients wi

Factors Associated With Major Lower Extremity Amputations in Diabetic Foot Infections at a County Hospital in Guatemala.

INTRODUCTION: Outcomes from diabetic foot infections (DFIs) at the major referral hospital (Hospital Nacional de San Benito) in El Petén, Guatemala have not been analyzed. We hypothesized that poor diabetic control might be associated with a high rate of major lower extremity amputations (mLEAs; above the ankle). METHODS: We performed a retrospective analysis at Hospital Nacional de San Benito between (8/14 and 6/23) in patients presenting with DFIs. Patients receiving mLEAs were compared with all others (AO = [trans-metatarsal amputations, toe amputations, incision and drainage, and antibiot

Epidemiology and Mortality of Cryptococcal Disease in Guatemala: Two-Year Results of a Cryptococcal Antigen Screening Program.

Cryptococcal disease is an important opportunistic infection among people living with HIV. The cryptococcal antigen (CrAg) can be detected before the clinical onset of meningitis and its screening is recommended. Here, we evaluated CrAg frequency, and describe the epidemiological characteristics and mortality at 180 days in a cohort of HIV patients from Guatemala. A total of 3457 patients were screened with a CrAg lateral flow assay in serum between January 2017 and December 2018. CrAg positivity was 11.5% in patients with ≤100 CD4/mm3, 8.7% in patients with <200 CD4/mm3, and 6.3% in patients

Use of Postpartum Birth Control in Rural Women in Southwest Guatemala: Analysis of a Quality-Improvement Database.

OBJECTIVE: Our objective was to observe the prevalence of postpartum contraceptive use in a population of rural women in Southwest Guatemala by type, and to determine characteristics associated with long-acting reversible contraceptive (LARC) use and sterilization. METHODS: We conducted a secondary analysis of prospectively collected quality improvement data from a cohort of postpartum women. We compared women intending to use or already using contraception to those not intending to utilize a method; bivariate comparisons were used to determine if there were differences in characteristics bet

A Rapid Screening Program for Histoplasmosis, Tuberculosis, and Cryptococcosis Reduces Mortality in HIV Patients from Guatemala.

Opportunistic infections (OIs) and advanced HIV disease (AHD) contribute to HIV-related mortality. Here, we analyzed the situation of AHD and OIs in a cohort of newly diagnosed HIV patients from Guatemala. We included 2127 adult patients from 13 facilities across the country during 2017 to 2018. Patients were screened for tuberculosis (TB), nontuberculous mycobacteria (NTM), histoplasmosis, and cryptococcal disease, independently of their CD4 cell count. Of the 2127 enrolled patients, 1682 (79.1%) had a CD4 cell count available; of which 52% presented with AHD. Of the Mayan population, 65% had

The effect of shared genetic and environmental factors on periodontal disease parameters in untreated adult siblings in Guatemala.

BACKGROUND: Increasing evidence supports the role of genetic factors in susceptibility to infectious diseases, including chronic periodontitis. The role of genetic factors in phenotypic expression can be estimated from the degree of resemblance between relatives, as compared with that of unrelated members of a population. Heritability is an estimate of the proportion of total phenotypic variation of a quantitative trait, which is attributable to genetic factors, and is based on the variance within versus between family members. The aim of this study was to determine whether there is a familial

Husbands and maternal health matters in rural Guatemala: wives' reports on their spouses' involvement in pregnancy and birth.

This paper explores the content, prevalence, and determinants of husbands' involvement in two aspects of their wives' maternal health: (1) providing advice and care during pregnancy and (2) attending birth. Using primarily women's reports of their spouses' involvement taken from qualitative and survey data from a sample of rural Guatemalan households, I find that husband participation is relatively high compared to some expectations and that the dynamics of male involvement differ between the two aspects. The analysis of receiving advice or care from husbands during pregnancy, an analysis done

Identification of the fragmentary, burned remains of two U.S. journalists seven years after their disappearance in Guatemala.

The contribution of a physical anthropologist to a forensic investigation is generally associated with analysis of skeletal remains in a laboratory. This case, which deals with the identification of two U.S. journalists who disappeared in Guatemala in March 1985, shows that the observations of a physical anthropologist in the field--at the site where the skeletal remains are located--can be essential to the success of an investigation. In this case, there was a deliberate attempt to mislead the investigators, but the physical anthropologists on the team discovered the deception. Subsequently,

Plants used in Guatemala for the treatment of gastrointestinal disorders. 1. Screening of 84 plants against enterobacteria.

Gastrointestinal disorders are important causes of morbidity in developing countries. Natural healing is the traditional way of treating these diseases in Guatemala. Ethnobotanical surveys and literature reviews showed that 385 plants from 95 families are used in Guatemala for the treatment of gastrointestinal disorders. The activity of 84 of the most commonly used plants was screened in vitro against five enterobacteria pathogenic to man (enteropathogenic Escherichia coli, Salmonella enteritidis, Salmonella typhi, Shigella dysenteriae and Shigella flexneri). Results indicate that 34 (40.48%)

Chemical control of Simulium ochraceum Walker (Diptera: Simuliidae) larvae in an onchocerciasis endemic area of Guatemala.

Chemical control against larvae of Simulium ochraceum, the principal vector of onchocerciasis, was carried out from 1979 to 1984 in a 91.3 km2 area of Guatemala where onchocerciasis is endemic. The control operation was divided chronologically into three phases according to the different tactics employed. Phase 1 (1979-1981), using briquettes of fat and detergent containing 10% temephos, was effective only in perennial streams. Phase 2 (1981-1982), which limited the application target to small streams with discharges of 0.1-1 litre/sec, was not effective. Successful control was achieved by Pha

Prevalence and incidence of onchocerciasis as baseline data for evaluation of vector control in San Vicente Pacaya, Guatemala.

The aim of this study was to estimate the incidence rate of onchocerciasis as measured by skin biopsy during field surveys carried out in San Vicente Pacaya, Guatemala from 1977 through 1979. The age standardized prevalence rates for males and females were 44.6% and 19.9%, respectively, in 1979. There were annual incidence rates of 0.08 and 0.04 for males and females, respectively. The incidence rates seem to increase with age. Since the effect of false negatives will be greater in females than in males, these rates should be interpreted cautiously. Extremely high reversion rates (proportion o

Seasonal variation in the microfilarial skin density of Onchocerca volvulus and in the biting activity of Simulium spp. in Guatemala.

The density of Onchocerca volvulus microfilariae in the skin of 11 carriers was examined by carrying out a monthly examination in a single endemic area of Guatemalan onchocerciasis. The results obtained suggested the possibility of a seasonal variation in the microfilarial skin density. Almost all of the volunteers participating had shown a peak concentration of microfilariae in September, followed by August or October. The highest biting densities of Simulium ochraceum, the principal vector of onchocerciasis in Guatemala, occurred from September to the following January. It thus appears that

Observations on filarial infections and parous rates of anthropophilic blackflies in Guatemala, with reference to the transmission of Onchocerca volvulus.

In Guatemala, in areas of different onchocerciasis endemicity and outside the endemic zone, the main anthropophilic blackfly species Simulium ochraceum, S. metallicum, S. callidum and S. gonzalezi were examined for infections with Onchocerca volvulus, other filariae, and non-filarial parasites. Third stage larvae, indistinguishable from those of O. volvulus were found only in 3 females of S. ochraceum out of 3,513 examined. In S. metallicum, 2 females out of 3,121 dissected harboured infective larvae of another filaria species, morphologically different from O. volvulus. Thoracic infections we

Factors associated with hand hygiene adherence among healthcare workers in Guatemala during the COVID-19 pandemic.

BACKGROUND: Healthcare-associated infections are prevalent in low- and middle-income countries and may be reduced through proper hand hygiene (HH) adherence during patient care. AIM: We produced and distributed alcohol-based hand rub (ABHR) to 19 public primary- and secondary-level healthcare facilities in Quetzaltenango, Guatemala, and carried out HH observations to assess healthcare workers' (HCWs) HH adherence, and to identify factors associated with this practice. HH adherence was defined as washing hands with soap and water or using ABHR. METHODS: Observations were conducted before (202

Socioeconomic and environmental factors associated with dengue fever incidence in Guatemala: Rising temperatures increase dengue risk.

Dengue fever is a mosquito-borne illness that infects 390 million people annually. Dengue outbreaks in Guatemala have been occurring more often and at increased rates since the first dengue outbreak in Guatemala in the 1970s. This study will examine environmental and socioeconomic factors associated with dengue in Guatemala at the municipality (county) level. Socioeconomic factors included population density, Mayan population, economic activity, and attending school. Environmental factors included average minimum annual temperature and annual precipitation. The relationship between environment

Placebo-controlled clinical trial of sodium stibogluconate (Pentostam) versus ketoconazole for treating cutaneous leishmaniasis in Guatemala.

To determine the relative efficacy and toxicity of stibogluconate and ketoconazole for the treatment of cutaneous leishmaniasis, a comparative trial was conducted in which 120 Guatemalan men with parasitologically proven cutaneous leishmaniasis were randomly divided into three treatment groups: sodium stibogluconate (20 mg of antimony per kilogram per day intravenously for 20 days), ketoconazole (600 mg per day orally for 28 days), and placebo. Treatment outcome was influenced by species. Among patients infected with Leishmania braziliensis, 24 (96%) of 25 in the stibogluconate group but only

Search for persistent epizootic Venezuelan encephalitis virus in Guatemala, El Salvador and Nicaragua during 1970-1975.

Evidence was sought during 1970-1975 of persistence of equine-virulent Venezuelan encephalitis (VE) virus in regions of Central America that were heavily involved in the epidemic-equine epizootic of 1969. (a) Four sentinel horses were exposed in an arid, upland region of the Atlantic drainage of Guatemala during August-October 1970, but no horse became infected. (b) The epicenter region of the 1969 outbreak, in southwestern Guatemala and southwestern El Salvador, was studied during July 1970-February 1974; no antibody developed in sentinel horses, sentinel hamsters did not die, mosquitoes yiel
