By means of a consultation with patients is reported about satisfaction and quotes of success at 195 hallux valgus deformity-operations. As a most used method has proved the 2/3 resection according Brandes, which has a high quotes of success. It is reported and discussed about alternative operations, which more respect different variations and claims of feet.

Fifty Swanson single stemmed metatarsophalangeal silastic great toe prostheses in 38 patients inserted between 1974 and 1984 in the Orthopaedic University Clinic of Innsbruck were reviewed. The authors compare results of hallux valgus correction and of hallux rigidus correction by the Swanson prostheses and conclude, that implant arthroplasty should not be performed to correct hallux valgus, but is an excellent procedure in the treatment of hallux rigidus, if the hallux valgus angle is not higher than 18 degrees.

HAV is a joint deformity that has some very characteristic anatomic features but that may be developed from a variety of interactive causes, both anatomic and functional. In the normal condition, the first metatarsal bone and hallucal phalanges form an axial array of bones. At the first metatarsophalangeal joint this arrangement is stabilized by a set of muscles whose tendons surround the joint. The pathology is associated with the deviation of the bones from their axial position and the related destabilization of the muscle group around the joint, specifically: (1) The hallux deviates and rot

Osteoplasty of the first metatarsal is a form of "plastic surgery" of bone. It is technically demanding and requires a thorough knowledge of anatomy and morphology of bone. Direct dissection techniques are recommended rather than layering, which unnecessarily devascularizes the tissues. Understanding the principles of osteosynthesis and absolute stability is important and will allow the surgeon confidence in knowing that these operative procedures can be performed safely with a high degree of predictability in the correction. These procedures open a new area for consideration in the management

The authors report on experience gained in the surgical treatment of eight patients with congenital hallux varus associated with polydactyly of the big toe. Aside from a description of the "radiological anatomy", the paper focuses in particular on the intraoperative soft tissue findings. In addition to the supernumerary bone structures, muscle and tendon anomalies were found in all the patients. In four patients the cause of the varus malposition was found to be fibrous stands extending from the rudimentary big toe to the tarsus. In five patients the abductor hallucis muscle was found to be sh

The oblique osteotomy of metatarsale I according to Ludloff allows good and various corrections in hallux valgus deformities. Various modifications of the osteotomy are described. In case of simultaneous osteoarthritis of the metatarsophalangeal joint the additional resection according to Keller-Brandes is recommended.

Hallux metatarsophalangeal joint cheilectomy is a joint-sparing technique that involves resection of the dorsal metatarsal head osteophytes; this may be achieved through minimally invasive and arthroscopic techniques. General indications for minimally invasive surgery (MIS) cheilectomy are mild-to-moderate hallux rigidus (Grades I-II) with symptomatic dorsal osteophytes causing dorsal impingement and/or shoe wear irritation in those who have failed extensive nonoperative management. The literature confirms equivalent outcomes to open cheilectomy; however, it is somewhat inconsistent regarding

The following are clinical consensus statements (CCS) on the topic of hallux rigidus sponsored by the American College of Foot and Ankle Surgeons. A core panel synthesized the data and divided the topic in to twelve sections, each section contained a variable number of consensus statements, based upon complexity. Overall there were 24 consensus statements synthesized for this subject matter. The 24 statements were provided to the expert panel with all available evidence to come to a consensus utilizing all available evidence.

Hallux rigidus is a degenerative arthritic condition affecting the first metatarsophalangeal joint. Prevalence in patients aged 50 years and above is estimated at 20% to 30%, with a portion being symptomatic. Conservative treatment's efficacy is linked to initial pain levels; though shoe modifications and insoles are commonly recommended, their true effectiveness lacks strong evidence. Injection therapy, including corticosteroids and hyaluronic acid, demonstrates varied outcomes, with about 50% of patients undergoing surgery within 1 to 2 years. The condition's etiology remains elusive, but re

Iatrogenic hallux varus is a dreaded complication of hallux valgus surgery, consisting in 1st-ray deformity in the form of medial malalignment of the 1st phalanx with respect to the metatarsal axis. Such over-correction results from imbalance between excessive medial capsule retraction or tensioning and excessive lateral laxity or soft-tissue release. There may be loss of medial stability of bone origin due to excessive "exostosectomy" or excessive intermetatarsal angle closure. Following excessive lateral release, the imbalance gradually induces a varus deformity of the 1st phalanx due to tra

BACKGROUND: Although more than 1500 publications on hallux valgus can be found in the current literature, none of them have reported on the course of pain resolution after hallux valgus surgery. Thus, this study aimed to investigate pain resolution after hallux valgus surgery and to identify predictive factors associated with residual pain at 6 months after surgery.
METHODS: We prospectively followed up 308 patients who underwent hallux valgus surgery at a tertiary hospital at 6 months and 2 years after surgery. Multivariate logistic regression analysis was performed to evaluate the risk fact

Hallux rigidus is a painful condition of the great toe characterized by restriction of the metatarsophalangeal joint arc of motion and progressive osteophyte formation. Precise cause of hallux rigidus remains under debate. Anatomic variations and historical, clinical, and radiographic findings have been implicated in the development and progression of hallux rigidus. Radiologic findings associated with hallux rigidus include metatarsal head osteochondral defects, altered metatarsal head morphology, and an elevated hallux interphalangeus angle measure. Associated historical findings include a p

Hallux rigidus, the most common degenerative disorder of the foot, is accountable for abnormality of gait and restriction of activity levels and daily function. This article describes and reviews the available literature on nonoperative modalities available in the treatment of hallux rigidus, including manipulation and intra-articular injections, shoe modifications and orthotics, physical therapy, and experimental therapies.

BACKGROUND: The purpose of this study was to report outcomes of congenital hallux varus deformity after surgical treatment.
METHODS: We evaluated ten feet of eight patients with a congenital hallux varus deformity, including four feet combined with a longitudinal epiphyseal bracket (LEB). There were seven male patients and one female patient with a mean age of 33 months (range, 7 to 103 months) at the time of surgery. Two patients were bilaterally involved. The mean duration of follow-up was 5.9 years (range, 2.3 to 13.8 years). Clinical outcomes were assessed according to the criteria of Phe

Metatarsalgia, pain around the metatarsophalangeal joints, may arise from mechanical causes spontaneously or iatrogenically. Nonunion or malunion of the first metatarsal can transfer weight-bearing forces and overload the lesser metatarsals. Transfer metatarsalgia after failed hallux valgus surgery is troublesome and more prevalent than would be expected. Clinical examination of the patient allows identifying the type of transfer metatarsalgia and pathomechanics involved. This review focuses on transfer metatarsalgia after hallux valgus surgery and provides a basic understanding of the pathome

With mounting attention focused on decreasing postsurgical pain and dysfunction, emphasis has been placed on approaching disorders using minimally invasive techniques. Surgical procedures of the hallux, such as hallux valgus correction, have earned the reputation for high postsurgical pain and prolonged recovery. Arthroscopic hallux procedures have the advantages of minimizing pain, swelling, and disability. Certain conditions, such as synovitis, loose bodies, and early-grade hallux rigidus, are better addressed arthroscopically. With the correct indications, hallux metatarsophalangeal arthros
