The distal attachments of the extensor hallucis longus (EHL) tendons in 47 amputated legs and in eight cadavers were examined. The EHL had two tendons in 34 of the amputated legs and bilaterally in five cadavers. The lateral tendon was inserted to the middle of the dorsal aspect of the base of the distal phalanx of the hallux and the medial tendon to the medial side of the insertion of the lateral tendon. The length and thickness of these two tendons were measured and compared in order to obtain data for using these tendons in tendon repair and hallux varus corrections by autogenous tendon tra

Osteotomy is a good method for correcting a metatarsus varus and a hallux valgus if the first metatarsophalangeal joint has little or no arthritis, no luxation and is mobile. Osteotomy of the neck of the first metatarsal is recommended when the articular surface of the first metatarsophalangeal joint has a certain lateral obliquity. In these cases osteotomy of the base of the metatarsal, which exaggerates this obliquity, should be avoided. Osteotomy allows correction in all planes. As osteotomy of the diaphysis of the proximal phalanx is indicated in cases where the hallux is too long, is pron

INTRODUCTION AND AIMS: The first metatarsophalangeal joint arthrodesis is indicated for the treatment of various pathologies as a technique to reduce pain and improve the support of the first radius. Numerous surgical techniques and fixation methods have been described, with the combination of a dorsal plate and an interfragmentary screw being the one that has shown to be the most stable construct in biomechanical studies. Our aim is to analyze the radiological results after metatarsophalangeal arthrodesis of the hallux using a dorsal plate associated or not with an interfragmentary screw. The

A brief review of pedal anatomy and the mechanisms enabling feet to function as organs of locomotion is presented. The importance of the subtalar joint is emphasized. Four major abnormalities disturbing normal foot function are noted, and the most commonly encountered of these alterations, rearfoot varus, is discussed in some detail. A series of pedal abnormalities in rearfoot varus occurs because in the foot's attempt to overcome its imbalance, it pronates excessively and causes development of callus, corn, hallux valgus, and even ingrown toenail. Haglund's deformity, soft corn, and tailor's

A new method was devised for the evaluation of medial and lateral splaying of the foot on dorsoplantar radiographs made while the patient is bearing weight. This method involves use of a two-dimensional coordinate system; the axis of the shaft of the second metatarsal is the x axis, the intersection of the x axis with the proximal end of the second metatarsal is the point of origin, and the perpendicular to the x axis that passes through the point of origin is the y axis. This method was used to study the radiographs of 177 feet of 112 female patients who had symptomatic hallux valgus and thos

BACKGROUND Hallux valgus (HV), characterized by lateral deviation of the first metatarsophalangeal joint (MJ) and medial metatarsal protrusion, frequently causes pain and gait dysfunction. Conventional treatments face limitations in efficacy or invasiveness. Acupotomy is a minimally invasive microsurgery technique that uses a needle knife and microtrauma to detach tissue associated with contractures. It may be used with or without ultrasound guidance. This report describes the case of a 52-year-old woman with varus deformity of the second toe and valgus deformity of the left first metatarsopha

PURPOSE: Our purpose was to evaluate the clinical and radiologic results of arthroscopy-assisted hallux valgus deformity correction with percutaneous screw fixation.
METHODS: Ninety-four feet underwent arthroscopy-assisted hallux valgus deformity correction. Patients in whom the 1,2-intermetatarsal angle could be reduced manually and who had no significant abnormality of the distal metatarsal articular angle were included, and an endoscopic distal soft tissue procedure was performed. Those patients with first tarsometatarsal hypermobility, in whom the 1,2-intermetatarsal angle cannot be reduc

Background The Lapidus procedure has become a popular procedure in correcting hallux valgus deformities and has undergone several modifications in an effort to improve the efficacy of the procedure. The senior author modifies this procedure with the addition of an intermetatarsal and intercuneiform fusion. Our hypothesis is that this will improve the procedure outcomes and decrease deformity recurrence. Methods We reviewed patient charts who underwent the procedure between 2014 and 2017 performed by the senior author. This yielded 47 reviewable cases, with 34 meeting study criteria. The cases

This is a review of 55 metatarsal osteotomies for hallux valgus and metatarsus primus varus in patients under 15 years of age. The results were analysed after the patients were reviewed clinically and radiologically. Complications of the osteotomies and the factors that caused them are discussed. Poor results, irrespective of the type of osteotomy, were due to excessive shortening of the first metatarsal and/or dorsal tilting of the metatarsal head. In both instances there was persistent metatarsalgia.

Polydactyly of the foot is not an uncommon foot problem. However, it is not well understood and not often reported. Its clinical manifestations vary, as do treatment methods and results. During the period of 1979 to 1994, 54 children with polydactyly of 65 feet were treated in our clinic. Bilateral involvement was seen in 11 patients, and six had accompanying polydactyly of the hand. By Blauth and Olason's classification, duplication of the fifth ray (43 feet) was most common, followed by first ray duplication (20 feet) and other ray duplication (2 feet). A rudimentary form was found in one fo

Background. Lapidus surgery involving arthrodesis of the first metatarsocuneiform (MTC) joint is an effective procedure for the correction of moderate to severe hallux valgus. The aim of this study was to collect and analyze radiographic data from our institution and determine the extent of first metatarsal shortening associated with the Lapidus procedure. Materials and Methods. A total of 53 patients (54 feet) who underwent arthrodesis of the first MTC joint combined with modified McBride bunionectomy for correction of moderate to severe hallux valgus deformity, between 2010 and 2015 were inc

Primary arthrodesis is a thoroughly studied treatment option for end-stage pathologic entities of the first metatarsophalangeal joint. It is a commonly accepted treatment of many pathologic conditions, including hallux rigidus, severe hallux valgus, hallux varus, and other conditions pertaining to the first ray. Numerous fixation techniques are available for this procedure. Fixation constructs range from simple crossing Kirschner wires to plate and screw fixation or, even, external fixation. We propose a simple and cost-effective fixation technique using an intramedullary Steinmann pin with cr

Twenty patients underwent 25 basal medial opening wedge osteotomies of the first metatarsal stabilized using a low-profile wedge plate in combination with a distal soft tissue release, distal metatarsal osteotomy and Akin osteotomy as required for correction of a hallux valgus deformity. The mean clinical and radiographic follow-up was 12.2 months. Pre- and post operative radiographs available in 15 cases showed that the median hallux valgus angle (HVA), intermetatarsal angle (IMA) and distal metatarsal articular angle (DMAA) were corrected from 45.5 to 13.1, 17.7 to 9.2 and 243 to 10.0 degree

The authors describe their experience of valgus osteotomy of the first metatarsal in hallux valgus - metatarsus primus varus (HV-MTPV). It is based on the Stamm technique consisting in the resection of one third of the base of the proximal phalanx of the great toe. From the resected part they make a wedge-shaped bone graft. Subsequently, they extend the section and perform a two-third transversal osteotomy of the first metatarsal (MT) 1 cm distal to the cuneometatarsal joint. The rest of the bone is broken into valgus position by pressure. Afterwards, the bone graft is inserted in the open ost

BACKGROUND: Recurrent hallux valgus is a relatively common, yet challenging, condition for both the patient and the surgeon. The literature on the treatment of recurrent hallux valgus is sparse. The purpose of this study was to evaluate prospectively the functional outcome and patient satisfaction following the Lapidus procedure for the treatment of recurrent hallux valgus deformity.
METHODS: Twenty-four patients with a total of twenty-six symptomatic recurrences of hallux valgus after previous procedures for treatment of the deformity were included in the study. Exclusion criteria included p

This study aims to assess a novel minimally invasive surgical technique that addresses hallux valgus accompanied by metatarsus adductus. We retrospectively analysed the results of 20 patients (21 feet) that underwent a newly developed percutaneous osteotomy procedure of the lesser metatarsal bones in order to correct hallux valgus deformities accompanied by metatarsus adductus. We used x-ray studies in order to evaluate changes in the hallux valgus angle, the first intermetatarsal angle, and the metatarsal angle (using the modified Sgarlato method). We also compared the pre- and postoperative

Implantation of a femoral stem in a dysplastic femur poses many problems. Most common abnormalities are excessive anteversion and valgus angle of the femoral neck. This makes it impossible to use conventional stems. Suitable options are distally fixed stems with small metaphysis or proximally modular stems. Femurs with previous varus or valgus osteotomies show more severe deformities that sometimes require performing an osteotomy to obtain adequate correction. Again, distally fixed stems or proximally modular stems are suitable, the latter having the advantage that, by loading the femur proxim
