BACKGROUND: The diagnostic criteria for primary stabbing headache (PSH) in the 3rd beta edition of International Classification of Headache Disorders (ICDH-3 beta) were recently revised. In the ICDH-3 beta, PSH is defined as short-lasting head pain spontaneous occurring as a single stab or series of stabs without autonomic symptoms and involving all head areas (i.e., not limited to the ophthalmic branch region of the trigeminal nerve). The aim of this study was to investigate the validity of the ICHD-3 beta criteria for PSH in a clinic-based setting.
METHODS: We prospectively collected data f

BACKGROUND: Headache attributed to anxiety or depressive disorders was added to the appendix of the International Classification of Headache Disorders, 3rd edition (ICHD-3) in 2018. However, its specific clinical features have not yet been systematically characterized, limiting its early recognition and optimal clinical management.
METHODS: We conducted an observational study between March 2024 and June 2025 at a headache center in China. Following stringent exclusion of primary headache disorders and other secondary headache causes, 101 patients meeting diagnostic criteria for headache attri

The relationship between headaches and psychiatric disorders among psychiatric outpatients remains insufficiently characterized. In this study, the prevalence of primary headaches was estimated, and the factors associated with headache occurrence and quality of life (QoL) in this population were examined. A total of 1,014 patients were evaluated. Psychiatric diagnoses were determined using the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV), and primary headaches were classified according to the International Classification of Headache Disorders (ICHD-3). Key variables included

BACKGROUND: Headache disorders, including migraine, tension-type headache, and cluster headache, are among the most prevalent and disabling neurological conditions. Although genetic factors contribute to their pathogenesis, they fail to fully account for their clinical heterogeneity, episodic nature, and varied treatment responses. This review synthesizes current evidence on the role of epigenetic mechanisms—including DNA methylation, histone modifications, non-coding RNAs, and RNA modifications—in the pathophysiology of headache disorders. PRINCIPAL FINDINGS: Evidence from both human studies

Military veterans are at increased risk for headache disorders compared to the general population, yet the prevalence and burden associated with headache disorders among veterans is not yet well understood. In this electronic medical record study, we examined the prevalence of headache disorders among veterans seen in a northeastern network of Veterans Health Administration (VHA) primary care during 2017-2018. We also examined rates of psychiatric comorbidity and health care utilization of veterans with headache disorders for the year following the date of the first headache code in the medica

BACKGROUND: Headache disorders, particularly migraine and tension-type headache (TTH), are among the most prevalent global public-health problems. Medication-overuse headache (MOH) is a common sequela of mismanagement of these. Migraine and MOH are highly disabling. Formulation of responsive health policy requires reliable, locally-derived, population-based data describing both individual and societal impact of headache disorders. South-East Asia is the only one of WHO's six world regions in which no such national data have yet been gathered.
METHODS: In a nationwide population-based cross-se

OBJECTIVE: Earlier studies have provided evidence of genetic inheritance of headache, especially migraine, but no familial occurrence has been found regarding temporomandibular disorders (TMD). In adults, headache and TMD have been found to be associated with each other, but studies on children are few. The aim of the present study was to test the hypothesis that there is no association between signs of TMD in 13-year-old headache children and their mothers.
MATERIAL AND METHODS: The study population was a nested case-control study of the population-based Finnish Family Competence Study origi

BACKGROUND: Recurrent or chronic facial pain may be a diagnostic challenge. Applying the diagnostic criteria of the second edition of the International Classification of Headache Disorders (ICHD-II) leaves a considerable number of patients unclassifiable.
OBJECTIVE: The aim of this study was to establish and evaluate revised criteria of trigeminal neuralgia and persistent idiopathic facial pain.
METHODS: Based on the diagnostic value of 12 clinical features of trigeminal neuralgia and 15 features of persistent idiopathic facial in 97 patients referred for facial pain to a neurological tertia

The aim was to investigate the association between temporomandibular disorders (TMD) and overall muscle tenderness, depressive symptoms, sleep difficulties, headache frequency and related symptoms in children with primary headache in comparison with controls. Based on an unselected population sample of 1135 Finnish schoolchildren classified according to the type of headache at age 12, altogether 297 children aged 13-14 from different headache groups and healthy controls were randomly selected for an interview and clinical examinations. Children with migraine had more TMD signs than children wi

OBJECTIVE: To investigate the frequency of depressive and anxiety disorders in patients with chronic daily headache.
BACKGROUND: There is a lack of data in the literature on the extent of psychiatric comorbidity in patients with different subtypes of chronic daily headache.
METHODS: We recruited consecutive patients with chronic daily headache seen in a headache clinic from November 1998 to December 1999. The subtypes of chronic daily headache were classified according to the criteria proposed by Silberstein et al. A psychiatrist evaluated the patients according to the structured Mini-Intern

Clinical features and pathophysiological mechanisms of primary (idiopathic) headaches are revisited in the light of the new classification of headache, recently proposed by the Ad Hoc Committee of the International Headache Society. This computer-based classification is hierarchically constructed and offers operational diagnostic criteria for all headache disorders. However, further investigations are required in some cases (e.g. in case of tension-type headache) to better define the clinical form.

Headache disorders are common, including in the working population. Clinicians caring for patients with headache need to be aware of work-related factors as potential causes or triggers of headache disorders, and consider the impact of headache on fitness-to-work, especially in safety-sensitive and decision-critical roles. Such fitness-to-work determination should include individualized consideration of the nature of the headache disorder itself, the pattern of the headache, the impact of sleep deprivation on the headache as it relates to fitness to do shiftwork, medication and substance side

BACKGROUND: Cerebral venous sinus thrombosis (CVST) is a rare diagnosis and is estimated at 1.32-1.57 per 100,000 cases. Patients with CVST often present with headache, seizures, focal motor deficits, encephalopathy, or aphasia, and can be fatal in 3%-15% of cases. Newer diagnostic criteria indicate functional neurological disorder (previously known as conversion disorder) as a "rule in" diagnosis that should be given only if patients meet strict criteria.
CASE REPORT: A 26-year-old woman with a past medical history of depression during pregnancy, anxiety, and migraines, presented to a large

OBJECTIVE: Headache disorders are a prevalent yet frequently underestimated issue in patients with unruptured intracranial aneurysms (UIAs). The primary aim of this study is to systematically examine the incidence, specific characteristics, and associated risk factors of headache disorders in the context of individuals diagnosed with UIAs. Through this investigation, we hope to contribute valuable insights to the current understanding of this complex relationship and potentially inform future diagnostic and treatment approaches.
METHODS: Data from 146 consecutive patients harboring UIAs were

There are relevant clinical overlaps between some of the painful temporomandibular disorders (TMD) and headache conditions that may hamper the diagnostic process and treatment. A non-systematic search for studies on the relationship between TMD and headaches was carried out in the following databases: PubMed, Cochrane Library and Embase. Important pain mechanisms contributing to the close association and complex relationship between TMD and headache disorders are as follows: processes of peripheral and central sensitisation which take place in similar anatomical areas, the possible impairment
