Empowering patients or general practitioners? A randomised clinical trial to improve quality in reproductive health care in Belgium.

BACKGROUND: Evidence-based clinical guidelines on contraceptive use were developed and distributed among all Flemish general practitioners (GPs) in Belgium. OBJECTIVES: To evaluate the effectiveness of two strategies for implementing a guideline on oral contraceptives in order to enhance the quality of the first contraception consultation in general practice, by either empowering patients or by introducing a computer decision support system (CDSS). METHOD: A random sample of 45 GPs was distributed among three study groups. One group was visited by an 'empowered patient', one group's electron

PRISM (Program of Resources, Information and Support for Mothers) Protocol for a community-randomised trial [ISRCTN03464021].

BACKGROUND: In the year after birth one in six women has a depressive illness, and 30% are still depressed, or depressed again, when their child is 2 years old, 94% experience at least one major health problem (e.g. back pain, perineal pain, mastitis, urinary or faecal incontinence), 26% experience sexual problems and almost 20% have relationship problems with partners. Women with depression report less practical and emotional support from partners, less social support overall, more negative life events, and poorer physical health. Their perceptions of factors contributing to depression are la

RETURN Randomized Controlled Trial to Increase Adult Routine Dental Visits.

Groups with low socioeconomic status are less likely to make planned dental visits even though routine attenders have fewer caries and less tooth loss. Because behavioral interventions positioned in urgent dental care may improve health and inequalities, RETURN, which involved a nurse conversation supported by videos, booklets, and goal/action-plan setting, was developed. The objective of this study was to investigate the effectiveness of the RETURN intervention at increasing planned dental visits and lessening the oral health impact. Adults attending 14 National Health Service urgent dental s

[Preparing for Pandemics].

The current COVID-19 pandemic has contributed to millions of deaths globally and it is estimated that the hit to the global economy could reach more than twelve trillion US-dollars. Disease outbreaks have often pushed weak health systems to a breaking point, as witnessed during cholera, Ebola and Zika virus upsurges. The preparation of a plan involves the analysis of a scenario divided into the disaster cycle's four phases: preparation, response, recovery, and mitigation. Several levels of planning are recognised according to the goals to be reached: strategic plans are directed to define the

Implementation of an HIV Pre-exposure Prophylaxis Strategy Into Abortion and Early Pregnancy Loss Care.

BACKGROUND: Family planning and abortion clinics routinely address sexual health. We sought to evaluate implementation outcomes of an HIV pre-exposure prophylaxis (PrEP) care strategy for patients seeking management of induced abortion and pregnancy loss. SETTING: Single-center, urban, academic, hospital-based family planning service. METHODS: We used a multifaceted implementation strategy directed toward family planning providers comprised of educational sessions, an electronic medical record-prompted verbal assessment of HIV risk, electronic medical record shortcuts for PrEP prescription,

Caregiver Outcomes from the BRITE Study: Secondary Outcomes of a Multisite Pragmatic Randomized Controlled Trial of Post-Rehabilitation Discharge Transitional Support after Traumatic Brain Injury.

Caregivers of individuals admitted to inpatient rehabilitation following traumatic brain injury (TBI) often experience high burden and unmet needs after hospital discharge. Transitional periods are challenging, and standard discharge planning with referrals to outpatient care is typically not enough support for this critical phase. We sought to evaluate the impact of a post-inpatient rehabilitation patient/caregiver intervention (rehabilitation transition planning; RTP) on caregiver burden and quality of life, compared to standard rehabilitation discharge planning (RDP). Of 925 randomized pati

The impact of perceived lack of support provided by health and social care services to caregivers of people with motor neuron disease.

Our objective was to investigate the relationship between support by health and social care services and caregiver well-being. A survey, including a generic health status measure (SF-12), a disease-specific measure for patients (ALSAQ-40), the Carer Strain Index (CSI) for caregivers and questions on experiences of health and social care services, was sent to patient members of the MND Association (UK) and their caregivers. A single 'problem score' was calculated from the experience questions and the relationship between the problem score with caregiver and patient well-being was analysed. Most

Adapting the Fiscal Mapping Process to improve the financial sustainability of implementation support and mental health delivery organizations: protocol for a mixed methods study.

BACKGROUND: Sustainability of evidence-based mental health intervention (EBI) implementation is a challenge to increasing access to mental health services. Funding is a common barrier to sustainment. EBIs, while cost effective, are expensive to implement. The Fiscal Mapping Process is a strategic planning tool designed for behavioral health agencies to plan the financial sustainment of specific EBIs. Financial planning at an organizational level is also required to enable implementation support organizations (ISOs) and mental health delivery organizations (MHDOs) to sustainably operate. Projec

Building the foundation: Training, stakeholder engagement, and planning for Common Elements Treatment Approach (CETA) implementation in Uganda.

Early and ongoing stakeholder engagement is critical to the development and implementation of scalable behavioral health interventions. Our overarching goal in this implementation research study was to understand and document the process of planning and rolling out Common Elements Treatment Approach (CETA), a transdiagnostic mental health intervention, in Uganda. Guided by the Consolidated Framework for Implementation Research (CFIR) 2.0, we conducted in-depth interviews with multilevel stakeholders (healthcare providers, MOH members, clinic managers, implementing partners, n = 20) to understa

Artificial intelligence and the future of maternal and newborn health in low-income countries: advancing equity, early detection, and health system resilience.

Maternal and newborn mortality remain major public health challenges in low-income countries, where poverty, health workforce shortages, limited access to quality care, and weak health systems contribute to preventable deaths and adverse outcomes. Artificial intelligence (AI) has emerged as a promising tool for strengthening maternal and newborn health through improved risk prediction, early diagnosis, clinical decision support, and health system planning. This narrative review examines the potential of AI to advance equity, enhance early detection of complications, and improve health system r

Using Planned and Unplanned Adaptation to Implement Universal Alcohol Screening and Brief Intervention to Prevent Alcohol-Exposed Pregnancies in Four Primary Care Health Systems.

BACKGROUND: The United States Preventive Services Task Force recommends annual alcohol screening and brief behavioral intervention (alcohol SBI) with general adult and pregnant populations. Implementation of alcohol SBI in primary care has encountered numerous barriers to adapting procedures and infrastructure to support its routine delivery. This collection of case studies describes the implementation strategies used by 4 academic health system teams that were funded by the Centers for Disease Control and Prevention to implement alcohol SBI within healthcare systems to prevent alcohol-exposed

A Decade of Discovery: Methodological Innovation, Capacity Building, and Global Leadership in Hidden Population Research in Iran.

BACKGROUND: In the early 2010s, Iran lacked reliable estimates of key populations at increased risk of HIV, including people who inject drugs, female sex workers, and men who have sex with men. This information gap limited evidence-based planning, resource allocation, and monitoring of HIV prevention programs. METHODS: This paper reviews the development and achievements of Iran's population size estimation program over more than a decade. Drawing on program records, published studies, training activities, and policy contributions, we summarise the program's impact across research, education,

Committee Opinion No. 654 Summary: Reproductive Life Planning to Reduce Unintended Pregnancy.

Approximately one half (51%) of the 6 million pregnancies each year in the United States are unintended. A reproductive life plan is a set of personal goals regarding whether, when, and how to have children based on individual priorities, resources, and values. A lack of reproductive life planning, limited access to contraception, and inconsistent use of contraceptive methods contribute to unintended pregnancy. The American College of Obstetricians and Gynecologists strongly supports women's access to comprehensive and culturally appropriate reproductive life planning and encourages obstetrici

Implementing business continuity effectively within the UK National Health Service.

Whereas major incident planning is very well established within National Health Service (NHS) organisations in the UK, business continuity management (BCM) planning, in many cases, is a relatively new activity; however, a combination of factors including the emergence of H1N1 influenza, has led to growing interest in the subject. This paper draws on both the personal experience of the authors and published research in relevant fields to make a number of specific recommendations about the effective implementation of BCM within NHS organisations. These include the need to define the BCM project

Consumer-oriented health care reform strategies: a review of the evidence on managed competition and consumer-directed health insurance.

CONTEXT: For many years, leading health care reform proposals have been based on market-oriented strategies. In the 1990s, a number of reform proposals were built around the concept of "managed competition," but more recently, "consumer-directed health care" models have received attention. Although price-conscious consumer demand plays a critical role in both the managed competition and consumer-directed health care models, the two strategies are based on different visions of the health care marketplace and the best way to use market forces to achieve greater systemwide efficiencies. METHODS:

Needle-exchange participation, effectiveness, and policy: syringe relay, gender, and the paradox of public health.

Needle-exchange programs (NEPs) have been politically controversial, and most studies have focused on evaluating their effectiveness on human immunodeficiency virus (HIV) transmission rates with little emphasis on the process of how they are used. This article shows that the way intravenous drug users use NEPs may influence their effectiveness. Using data from Baltimore's NEP, participants (N = 2,574) were classified as low, medium, and high users based on the volume, frequency, and duration of contact with the NEP. Higher NEP use was associated with shorter syringe circulation times and less
