Heparin-induced thrombocytopenia (HIT) is a life-threatening complication most commonly associated with exposure to heparin. Timely recognition of suspected HIT is vital, as early intervention is associated with improved outcomes and a decreased mortality rate. Spontaneous HIT is a rare form that occurs in the absence of heparin exposure. Here, we report the case of an 83-year-old patient diagnosed with spontaneous HIT after undergoing knee replacement surgery.

Heparin, a commonly used anticoagulant, has been associated with several cutaneous adverse effects, including skin necrosis, bruising/ecchymosis, urticaria, angioedema, red plaques, nodular lesions, and allergic contact dermatitis. Bullous hemorrhagic dermatosis (BHD) caused by heparin is a rare skin-related side effect. A 34-year-old woman diagnosed with anti-phospholipid syndrome was recently observed to develop hemorrhagic bullous dermatosis distant from the site of intravenous unfractionated heparin. Heparin therapy was continued for 5 days, followed by daily oral warfarin 2 mg, along with

PURPOSE OF REVIEW: Fibroblast growth factor (FGF) 23 is a bone-derived hormone that regulates phosphate and vitamin D metabolism by targeting the kidney. When highly elevated, such as in chronic kidney disease (CKD), FGF23 can also target the heart and induce pathologic remodeling. Here we discuss the mechanisms that underlie the physiologic and pathologic actions of FGF23, with focus on its FGF receptors (FGFR) and co-receptors.
RECENT FINDINGS: Klotho is a transmembrane protein that acts as an FGFR co-receptor for FGF23 on physiologic target cells. Klotho also exists as a circulating varian

BACKGROUND: The cause of hyperkalemia is frequently iatrogenic. Patient's prescriptions should therefore be checked in the analysis of the hyperkalemia. Low-molecular-weight heparin is not often suspected to cause this.
CASE DESCRIPTION: A 64-year-old man, hospitalized because of a complicated clinical course of pancreatitis, developed an acute severe hyperkalemia. Further analysis was susceptive for hypoaldosteronism, which was confirmed with biochemical testing. The only drug that could cause hyperkalemia in this case was nadroparin, which was prescribed because of vena lienalis and a super

INTRODUCTION: Heparin resistance during cardiopulmonary bypass poses a significant intraoperative dilemma. Antithrombin deficiency related heparin resistance is well described, but less common causes are still poorly understood and inadequately managed.
CASE REPORT: We present a case of heparin resistance during cardiopulmonary bypass in a gentleman with no previous haematological history or thrombotic risk factors. The patient required three times the regular dose of unfractionated heparin to achieve acceptable conditions to initiate and maintain bypass. The patient was found to have elevate

Heparin-induced thrombocytopenia is a rare complication of treatment with both unfractionated heparin (UFH) and low molecular weight heparin (LMWH). Antibodies against the complex heparin-platelet factor 4 are the main cause of pathogenesis, resulting in the activation of thrombocytes, coagulation, endothelium, monocytes, neutrophils and subsequent highly prothrombotic state. The prothrombotic state can result not only in venous but also in arterial thrombosis at different locations (which is manifested apart from venous thromboembolic disease also by acute limb ischemia, acute myocardial infa

Introduction Heparin-induced thrombocytopenia (HIT) is by far the most relevant pathological association of it encountered by clinicians. It is an immune-mediated phenomenon caused by antibodies directed against complexes of heparin molecules and platelet factor 4 (PF4). HIT is a considerable side effect in patients of acute coronary syndromes (ACS). Its prevalence and associated outcomes in ACS patients have not been studied sufficiently except for clinical trials. The objective of this study was to assess the frequency of HIT in patients presenting with ACS. Methods This was an observational

Heparin, a well known drug for anticoagulant therapy and prophylaxis of deep vein thrombosis and coronary syndromes, is also involved in numerous pathological processes such as inflammation, immune cell migration, tumor cell metastasis, smooth muscle cell proliferation etc. Though heparin is a clinically used anticoagulant with minimal side effects and drug interactions, its clinical use is limited due to parenteral administration. Alternatively, noninvasive delivery approaches such as oral, nasal, pulmonary or transdermal route are being explored that may deal with problems associated with pa

Thromboembolic complications are the most common preventable cause of mortality and morbidity in trauma patients. Thrombosis is also the common cause of ischemic heart disease (acute coronary syndrome), stroke, and venous thromboembolism. Heparin, as a potent anticoagulant, has been used in clinical practice for more than five decades and remains the major medicine for the prevention and treatment of venous thromboembolism. However it binds to the endothelium and has a high affinity for plasma proteins resulting in a short half-life and unpredictable bioavailability. Transdermal drug delivery

A 71-year-old man underwent total arch replacement for the true aortic arch aneurysm. On the post-operative day (POD) 10, right hemiplegia and motor aphasia occurred, and it was revealed that there were multiple cerebral infarction in brain computer tomography scan and magnetic resonanse imaging. Furthermore, platelet count has declined significantly from POD 15, so we suspected that heparin-induced thrombocytopenia might occurred. Then, we stopped continuous injection of heparin and administered argatroban and warfarin. In blood examinations, anti-platelet factor 4(PF4)/ heparin antibody meas

Many critically ill patients receive heparin, either before intensive care unit (ICU) admission (e.g., postcardiac surgery), for prophylaxis/treatment of thrombosis, for hemodialysis/filtration, or even incidentally (e.g., flushing of intravascular catheters), and are therefore at risk for developing immune heparin-induced thrombocytopenia (HIT), a prothrombotic drug reaction caused by platelet-activating antiplatelet factor 4 (PF4)/heparin antibodies. However, HIT explains at most 1 in 100 thrombocytopenic ICU patients (HIT frequency 0.3-0.5% vs. 30-50% background frequency of ICU-associated

New treatment strategies based on the availability of alternative anticoagulants have improved outcomes for patients with heparin-induced thrombocytopenia (HIT). Patients with HIT are at an extreme risk of thrombosis even after the discontinuation of heparin. Maintaining a high degree of awareness and instituting alternative anticoagulation immediately on suspicion of HIT are the two keys to preventing death, limb amputation, and other complications such as pulmonary embolism, myocardial infarction, or stroke; stopping heparin alone is inadequate. Warfarin should not be used early or unopposed

BACKGROUND: Venous thromboembolism (VTE) remains a frequent complication in orthopedic and trauma patients, despite the use of prophylactic antithrombotic drugs like standard or low molecular weight heparin. A new synthetic pentasaccharide with specific anti-Xa activity (fondaparinux sodium) has been shown to reduce the overall risk of VTE in major orthopedic surgery by 50% without increasing the risk of clinically relevant bleeding, when compared with enoxaparin. Beyond their traditional role as anticoagulants, heparin and low molecular weight heparin possess potent immunomodulatory effects.

INTRODUCTION: Heparin-induced thrombocytopenia is due to the development of IgG antibodies specific to platelet factor 4. More frequently observed after administration of unfractionated heparin, this complication is characterized by a delayed decrease in platelet count (after the 5th day of treatment) associated with venous and/or arterial thromboses.
CURRENT KNOWLEDGE AND KEY POINTS: It is often difficult to discard another potential cause for thrombocytopenia and to demonstrate by Elisa or platelet activation tests the presence of heparin-dependent antibodies is therefore mandatory in every

Aspirin, heparin and the coumarins are the classical anti-thrombotic agents. They represent the platform upon which newer drugs holding the promise of greater efficacy and less toxicity are being developed. Even as such newer drugs arrive into clinical practice, the older agents remain remarkable for their decades-long pre-eminence. All derive from natural sources, and none from a search for therapeutic anti-thrombotic agents; they have saved countless lives but also served as essential probes into basic mechanisms of thrombosis. Testament to their clinical importance is that these agents are

Potential therapeutic strategies in acute ischaemic stroke include reperfusion, prevention of thrombus extension and rethrombosis and neuroprotection. Heparins inhibit thrombin and factor X and therefore may have useful effects on arterial and venous thrombosis; they are effective in the prevention and treatment of deep vein thrombosis (DVT) and pulmonary embolism (PE) and in the prevention of myocardial infarction (MI) in patients with unstable angina. Low molecular weight heparins (LMWHs) and heparinoids are known to be more effective and have a safer profile than unfractionated heparins in
