Remote Ischemic Preconditioning in a Cirrhotic Patient Undergoing Major Hepatectomy.

Remote ischemic preconditioning (RIPC) has been shown to reduce ischemic reperfusion injury for patients undergoing hepatectomy for colorectal liver metastasis. We present a case of a 69-year-old male who underwent right hepatectomy for a multifocal hepatocellular carcinoma of the right liver and concomitant liver cirrhosis (Child-Pugh stage A). We performed portal vein embolization prior to surgery and intraoperative RIPC of the iliac vessels. The postoperative course after major hepatectomy went uneventful.

Treatment strategy for isolated bile leakage after hepatectomy: Literature review.

Isolated bile leakage (IBL) after hepatectomy is intractable, and various treatment methods for it have been reported. This review aimed to clarify the treatment strategy for IBL by summarizing studies on IBL after hepatectomy without extrahepatic bile duct resection. Thirty-three cases of IBL were reported. The incidence of IBL is very low, accounting for 0.1%-1% of all hepatectomy cases. The risk factors for IBL are unclear; however, several reports mention that biliary anomaly is associated with a high risk of IBL, with preoperative and intraoperative confirmation of biliary tree anatomy be

Laparoscopic Right Hepatectomy Via an Anterior Approach for Hepatocellular Carcinoma.

BACKGROUND AND OBJECTIVES: In the past, right hepatectomy via the anterior approach has been regarded as one of the many standard approaches for hepatectomy. However, total laparoscopic right hepatectomy from the anterior approach has been regarded as technically challenging. We report our experience in using the anterior approach in total laparoscopic right hepatectomy for hepatocellular carcinoma (HCC). METHODS: From June 2013 through December 2015, five consecutive patients underwent total laparoscopic right hepatectomy using the anterior approach, but without the hanging maneuver. RESULT

Preoperative Thrombocytopenia May Predict Poor Surgical Outcome after Extended Hepatectomy.

BACKGROUND: It is a novel idea that platelet counts may be associated with postoperative outcome following liver surgery. This may help in planning an extended hepatectomy (EH), which is a surgical procedure with high morbidity and mortality. AIM: The aim of this study was to evaluate the predictive potential of platelet counts on the outcome of EH in patients without portal hypertension, splenomegaly, or cirrhosis. METHODS: A series of 213 consecutive patients underwent EH (resection of ≥ five liver segments) between 2001 and 2016. The association of preoperative platelet counts with posthe

Liver hypertrophy: Underlying mechanisms and promoting procedures before major hepatectomy.

Various procedures can promote hypertrophy of the future liver remnant (FLR) before major hepatectomy to prevent postoperative liver failure. The pathophysiological situation following portal vein embolization (PVE), hepatic artery ligation/embolization or hepatectomy remains unclear. On one hand, the main mechanisms of hepatic regeneration appear to be driven by hepatic hypoxia (involving the hepatic arterial buffer response), an increased portal blood flow inducing shear stress and the involvement of several mediators (inflammatory cytokines, vasoregulators, growth factors, eicosanoids and s

Risk Factors for Cholangiocarcinoma After Initial Hepatectomy for Intrahepatic Stones.

BACKGROUND: Aggressive hepatectomy is effective in treating intrahepatic stones and may minimize the deleterious consequences of subsequent cholangiocarcinoma (S-CCA). The risk factors of S-CCA after different methods of hepatectomy may vary with the resection scope of stone-affected segments. METHODS: We reviewed the records of 981 patients of primary intrahepatic stones with elective hepatectomy from January 2000 to December 2010. The clinical characteristics of patients in the S-CCA group (n = 55) and the control group (n = 926) were compared. The uniformity between extent of liver resecti

Scarless hepatectomy: natural orifice specimen extraction after left lateral sectionectomy.

BACKGROUND: The use of laparoscopy in liver surgery is well established and considered as the gold standard for small resections. The laparoscopic resections have lower morbidity and better cosmetic results, but still require an incision to remove the surgical specimen. The possibility of remove the specimen through natural orifices and avoid an abdominal incision may further improve the benefits offered by minimally invasive procedures. AIM: To describe the technique of transvaginal extraction of the specimen after laparoscopic liver left lateral sectionectomy. METHOD: The laparoscopic live

Central hepatectomy with pancreatoduodenectomy for diffusely spread bile duct cancer.

BACKGROUND: Major hepatectomy with concomitant pancreatoduodenectomy (major-HPD) is the only procedure that provides a curative resection for diffusely spread extrahepatic bile duct cancer (DSEBDC). We sometimes encounter patients who cannot undergo major-HPD because of a poor functional hepatic reserve. The aim of the present study was to assess the feasibility of central hepatectomy with pancreatoduodenectomy (central-HPD) for patients with DSEBDC as an alternative to major-HPD. METHODS: Between 2002 and 2010, six patients with DSEBDC underwent central-HPD. The hepatectomy procedures for ce

Optimal central venous pressure during partial hepatectomy for hepatocellular carcinoma.

BACKGROUND: Low central venous pressure (CVP) affects hemodynamic stability and tissue perfusion. This prospective study aimed to evaluate the optimal CVP during partial hepatectomy for hepatocellular carcinoma (HCC). METHODS: Ninety-seven patients who underwent partial hepatectomy for HCC had their CVP controlled at a level of 0 to 5 mmHg during hepatic parenchymal transection. The systolic blood pressure (SBP) was maintained, if possible, at 90 mmHg or higher. Hepatitis B surface antigen was positive in 90 patients (92.8%) and cirrhosis in 84 patients (86.6%). Pringle maneuver was used rout

Impact of localized congestion related to venous deprivation after hepatectomy.

OBJECTIVE: We sought to assess the impact of localized venous congestion related to venous deprivation on liver function recovery and regeneration after hepatectomy, using the living donation model. Harvesting the middle hepatic vein (MHV) optimizes the venous drainage of right grafts but could lead to donor segment IV congestion. METHODS: In a series of 44 donors, 25 underwent right liver harvesting without the MHV and 19 with the MHV. The venous drainage anatomy of segment IV was defined as type I if exclusive through the MHV and type II if shared through the left hepatic vein. We prospecti

Repeated hepatectomy for recurrent intrahepatic cholangiocarcinoma: report of two cases.

Repeated hepatectomy for remnant liver recurrence of primary intrahepatic cholangiocarcinoma (ICC) is a seldom-encountered surgical technique because of its poor prognosis. Here, we present two long-term survivors of recurrent ICC by repeated hepatectomy. One patient underwent five hepatectomies in 6 years; first, extended left hepatectomy for a primary mass-forming-type ICC and then four partial hepatectomies for independent recurrent tumors all developing in segment V. The other patient underwent re-hepatectomy for a very large recurrent tumor 7 years after the first left hepatectomy for a p

Surgical treatment for extrahepatic recurrence after hepatectomy for colorectal metastases.

BACKGROUND/AIMS: Recurrence of the remnant liver or extrahepatic sites after hepatic resection for colorectal metastases is concerned as a significant prognostic factor. While regional chemotherapy may reduce recurrence of the liver, appropriate therapy for extrahepatic metastases needs to be elucidated. METHODOLOGY: We identified 207 patients undergoing hepatectomy for colorectal metastases. Patient characteristics, clinicopathological features, sites of extrahepatic recurrence, and outcome are examined. RESULTS: Recurrence of the lung, brain, bone, and other site after hepatic resection wa

Colonic varices ruptured via drainage catheter after extended right hepatectomy.

Extended right hepatectomy was performed on a 69-year-old woman with bile duct carcinoma of the hepatic hilum. Three weeks after the operation, an abscess was detected at the resected surface and a drainage catheter was inserted. The patient was discharged with the drainage catheter in place and followed-up at a local hospital. Three months later, the drainage catheter was removed. Bleeding immediately occurred through the fistula, and fistulography revealed varices. Angiography demonstrated right colonic varices, and the fistula was embolized with coils. There was no further bleeding. In conc
