Social Determinants of Health (SDoH) are an important part of the exposome and are known to have a large impact on variation in health outcomes. In particular, housing stability is known to be intricately linked to a patient's health status, and pregnant women experiencing housing instability (HI) are known to have worse health outcomes. Most SDoH information is stored in electronic health records (EHRs) as free text (unstructured) clinical notes, which traditionally required natural language processing (NLP) for automatic identification of relevant text or keywords. A patient's housing status

BACKGROUND: Youth experiencing homelessness and housing instability exhibit disparities in tobacco use and co-use of tobacco and cannabis products compared to their stably housed peers. Interventions to address these disparities have had limited success to date. This may be related to a lack of fit between intervention approaches and the unique needs and preferences of this marginalized population. Leveraging strategies to facilitate authentic engagement of youth in intervention research, including collaborating with youth in the cocreation of intervention goals and strategies, presents an opp

Youth experiencing homelessness and housing instability (YEH) face disproportionate health risks and structural barriers to health equity, which were intensified during the COVID-19 pandemic. We conducted a community-based participatory research study with 21 adolescents (ages 13-17) and transitional-aged youth (ages 18-26) experiencing homelessness or housing instability in San Diego, California, USA. Between February and July 2023, we conducted six "arts-based engagement sessions" (5 in English, 1 in Spanish) that blended expressive arts and focus group approaches to examine how YEH accessed

Homelessness continues to be one of the most pressing policy issues in Los Angeles County, California. Although substantial investments have been made to address it over the past several years, the number of people experiencing homelessness is still large. It is important to address transition age youth, ages 18 to 25, who are experiencing homelessness because resolving homelessness at an early age may prevent chronic homelessness and the consequences of living unsheltered, such as increased morbidity and earlier mortality. This study is the second publication from a project designed to better

BACKGROUND: Substance use increases risk of cardiovascular events, particularly among women with additional risk factors like housing instability. While multiple substance use is common among unstably housed individuals, relationships between multiple substance use and cardiovascular risk factors like blood pressure are not well characterized.
METHODS: We conducted a cohort study between 2016 and 2019 to examine associations between multiple substance use and blood pressure in women experiencing homelessness and unstable housing. Participants completed six monthly visits including vital sign

OBJECTIVES: Both homelessness and incarceration are associated with housing instability, which in turn can disrupt continuity of HIV medical care. Yet, their impacts have not been systematically assessed among people living with HIV/AIDS (PLWHA).
METHODS: We studied a retrospective cohort of 1,698 New York City PLWHA with both jail incarceration and homelessness during 2001-05 to evaluate whether frequent transitions between jail incarceration and homelessness were associated with a lower likelihood of continuity of HIV care during a subsequent one-year follow-up period. Using matched jail, s

Third places-public and community settings like parks and libraries-are theorized to serve as sources of social connection, community, and play. Yet, young people who hold marginalized identities, such as those experiencing homelessness and housing instability, often endure discrimination in third place settings. This study used game-based inquiry to partner with recently housed young people who have experienced chronic housing instability (N = 21) to understand how they would (re)imagine future third places. We then analyzed transcripts from these game sessions through qualitative thematic an

Despite advances in biomedical treatments, women living with HIV (WLH) who experience homelessness and housing instability suffer suboptimal HIV outcomes, even when linked to treatment. The purpose of this study was to explore experiences of housing instability among WLH and to understand its role in their ability to adhere to antiretroviral medication and remain retained in care. Sixteen women who were linked to Ryan White Program HIV care in South Florida participated in in-depth interviews. The findings focus around four larger themes: difficulty storing medication, privacy- and stigma-rela

Homelessness is a well-known social determinant of health, but health status also correlates with housing instability, a broader term that encompasses everything from trouble paying rent to living in overcrowded conditions and to moving frequently. UnitedHealthcare Community Plan-Arizona gives beneficiaries shelter with the hope that health care spending on them will go down.

The economic disruption of the COVID-19 pandemic combined with a pre-existing housing affordability crisis to threaten a wave of evictions from rental housing in the United States. Eviction and housing loss were associated with a range of adverse outcomes even prior to the pandemic; during the public health emergency, housing instability could have additionally increased opportunities for viral spread. Mitigating eviction risk was therefore an important form of prevention. We evaluate one federal policy response to the potential eviction crisis, the Emergency Rental Assistance program (ERA). U

Parolees experience numerous barriers that may impact successful reintegration into society. Residential instability may further add to these obstacles, as there may be limited opportunities for housing given their criminal history. The present study aimed to examine the impact of residential instability on suicidal ideation among parolees. Results indicated that residentially stable and unstable individuals had similar risk factors which were significantly associated with suicidality, such as age and having perceived unmet mental health needs. Other risk factors differed among the two groups,

Housing insecurity can take multiple forms, such as unaffordability, crowding, forced moves, multiple moves, and homelessness. Existing research has linked homelessness to increased emergency department (ED) use, but gaps remain in understanding the relationship between different types of housing insecurity and ED use. In this study, we examined the association between different types of housing insecurity, including detailed measures of homelessness, and future ED use among a cohort of patients initially seen in an urban safety-net hospital ED in the United States between November 2016 and Ja

PURPOSE OF REVIEW: People experiencing homelessness (PEH) remain an understudied and underserved population in cancer care, particularly regarding access to urologic cancer treatment. This review aims to examine barriers and facilitators PEH encounter when accessing treatment for urologic malignancies.
RECENT FINDINGS: PEH are found to have increased odds of delayed or disrupted urologic oncological treatment and experience deviations from guideline-based therapy. Problems arise in treatment coordination and organisation, with communication and logistical challenges posing major barriers. Com

Severe alcohol use disorder (AUD) in the context of housing instability remains one of the most complex health and social issues. Homelessness is related to increased vulnerability to stigma, marginalization and harmful ways of alcohol consumption, including non-beverage alcohol use (NBA). As a result, severe intoxication, alcohol poisoning, injury and death are common occurrences. Although harm minimization strategies have been readily proposed and examined in the context of drug use, applying the same principles to severe AUD remains controversial within the research and treatment community.

Little is known about predictors of housing instability for young adults participating in a Boston-area rapid rehousing program over the course of 1 year. We analyzed de-identified data from young adults enrolled in a rapid rehousing program between April 2020 and March 2022. The primary outcome was housing success, defined as acquiring independent housing or securing a second-year rapid rehousing lease. Mental health, substance use, service utilization, employment, experiences of interpersonal violence, and pregnancy were collected at baseline and at 3-month intervals for 1 year. Data were an

OBJECTIVE: Homeless adults make extensive use of emergency department (ED) services. This study examined factors associated with moderate and high ED use in a cohort of chronically homeless individuals.
METHODS: A cross-sectional analysis identified factors related to ED use in a cohort of 755 individuals at 11 sites at entry into the Collaborative Initiative to Help End Chronic Homelessness (CICH). Bivariate analyses identified sociodemographic, housing status, health status, and service-related factors associated with moderate and high ED use. Independent risk factors were then identified b

BACKGROUND: Food insecurity (FI) is an important public health issue for US veterans. For many veterans, civilian life is fraught with service-incurred health issues and socioeconomic challenges, each risk factors for FI. The FI literature on veterans is limited due to insufficient coverage of the topic's complexity and the methods used to study it in this population. No published analysis has evaluated how FI has been examined in US veterans.
OBJECTIVES: We assessed how FI has been examined in US military veterans by identifying (1) the major content areas, or domains, studied in association

People living with HIV (PLHIV) in under-resourced rural regions face intersecting social and structural barriers that intensify their vulnerability. This pilot study explored how overlapping marginalized identities and socioeconomic hardship shape experiences of discrimination among PLHIV in Southern Illinois. Twenty-two participants completed a community-informed survey that captured racial, sexual, and gender identities alongside indicators of stigma across healthcare, housing, employment, and community settings. The findings reveal that most participants experienced at least one form of dis

Preparing for the future is a major developmental task during adolescence and early adulthood. However, youth experiencing homelessness face additional challenges relating to economic instability, housing insecurity, and a lack of social support. Semi-structured interviews were conducted with 38 youth, from Ottawa and Toronto, who were experiencing homelessness and participating in a randomized controlled trial of Housing First for Youth (HF4Y). This qualitative study explores youths' visions for the future, including their goals, aspirations, plans, and barriers to achieving them. Overall, fi
