Multisystemic eosinophilic epitheliotropic disease in a horse: attempted treatment with hydroxyurea and dexamethasone.

Equine multisystemic eosinophilic epitheliotropic disease is rare in horses. The clinical signs vary according to the organs affected, the skin and gastrointestinal tract being most commonly involved. This paper gives the first reported description of a horse with multisystemic eosinophilic epitheliotropic disease in the United Kingdom and the attempts to treat it. The horse showed dermatological, gastrointestinal, hepatic, pulmonary and pancreatic involvement. Some improvement was seen when the horse was treated with corticosteroid and hydroxyurea.

Differential effects of hydroxyurea on the survival of UV- and MNNG-treated adenovirus 5.

The effects of hydroxyurea on plaque formation by UV-irradiated and MNNG-treated adenovirus 5 were investigated. Hydroxyurea blocked the recovery of UV-irradiated viruses in all cases studied, but the effect was less when fibroblasts from a patient with xeroderma pigmentosum were used. This fact supports the notion that hydroxyurea blocks excision repair of UV-produced damage. The recovery of MNNG-treated viruses was not blocked by hydroxyurea when viruses were used to infect normal human fibroblasts, but was blocked if the cell strain used as viral host were deficient in repair of O6-methylgu

Comparison of teratogenic effects of aspirin and hydroxyurea in the Fischer 344 and Wistar strains.

The Fischer 344 rat is being used increasingly in toxicology studies. There have been few reports in which rats of this strain were used in teratology and reproduction studies, but comparison of teratologic data with other toxic and points and kinetic information would be greatly facilitated by using the same strain. Therefore, the embryotoxic effects of two positive teratogens, aspirin and hydroxyurea, were compared in Fischer rats and in the commonly used Wistar rats. Aspirin was administered in single oral doses of 500 and 625 mg/kg on d 10; hydroxyurea was injected ip at 500 mg/kg on d 11.

An exploration of the role of hydroxyurea injection time in fetal growth and teratogenesis in rats.

Pregnant Wistar rats were injected with hydroxyurea (HU) intraperitoneally (IP) t one of several 6-hour intervals on days 9-10.75 to study the role of circadian growth variations in the teratologic response of the fetuses. Two stocks of rats were studied and the results in each compared. The fetal response to HU, as observed at day 21, was not detectably modified by circadian fetal growth variation. No correlations between hour of HU administration and fetal weight, placental weight, resorption, or total malformation rates were found. Cyclic variations in the incidence of hydronephrosis and le

Hydroxyurea pharmacokinetics in children with sickle cell anemia across different global populations.

Hydroxyurea provides effective disease-modifying treatment for people with sickle cell anemia (SCA), especially when escalated to maximum tolerated dose (MTD), which has wide interpatient dosing variability due to pharmacokinetic (PK) differences. Whether hydroxyurea PK parameters differ among children with SCA in different global regions is unknown. We compared hydroxyurea PK parameters among children with SCA from 5 clinical trials: HUSTLE (United States), TREAT (United States), NOHARM (Uganda), REACH (Uganda and Kenya), and EXTEND (Jamaica). Key hydroxyurea PK parameters were determined usi

Severe Relapsing Hailey-Hailey Disease Displaying a Durable Complete Response to Hydroxyurea.

Familial benign chronic pemphigus, also known as Hailey-Hailey disease, was first described by the Hailey brothers in 1939 (1). It represents a chronic autosomal-dominant genetic skin disorder with incomplete penetrance, usually diagnosed in children and young adults. As a result, family history of this disorder can be elicited in only about 66% of patients. We describe herein a patient with Hailey-Hailey disease who received treatment with hydroxyurea for a new diagnosis of polycythemia vera, with a surprising outcome. A 54-year-old man was diagnosed with Hailey-Hailey disease at age ten when

Upregulation of interferon-alpha receptor expression in hydroxyurea-treated leukemia cell lines.

BACKGROUND: Interferon-alpha (IFN-alpha) shows its antitumor effect through binding to specific cell surface receptors. A DNA synthesis inhibitor, hydroxyurea (HU), has been successfully combined with IFN-alpha to improve the efficiency of IFN therapy for chronic myelogenous leukemia (CML). To understand the mechanism of this combination effect, expression of IFN-alpha receptors on the CML cell line, K562, was studied before and after treatment with HU. METHODS: Cells were treated with HU at a dose of 0, 0.1, 0.2, or 0.4 mmol/L for 48 hours. Binding assays were performed using 125I-labeled IF

Evaluation of cytogenetic conversion to Ph- haemopoiesis in long-term bone marrow culture for patients with chronic myeloid leukaemia on conventional hydroxyurea therapy, on pulse high-dose hydroxyurea and on interferon-alpha.

Long-term bone marrow culture (LTBMC) has been used successfully in autologous transplantation in chronic myeloid leukaemia (CML). However, variation between patients in the recovery of Ph- cells in culture limits the application of this procedure to a minority. Treatment that effectively reduces in vivo tumour burden prior to initiation of LTBMC may improve the selection of Ph- cells in culture. To test this hypothesis we evaluated the frequency and degree of cytogenetic conversion to Ph- haemopoiesis in LTBMC from four independent groups of CML patients: Untreated (n = 19); conventional dosa

Successful Topical Treatment of Hydroxyurea-Induced Ulcers Without Drug Discontinuation: A Case Report.

Hydroxyurea, an antineoplastic agent, is widely used to treat essential thrombocythemia and polycythemia vera. Although hydroxyurea-induced skin ulcers are uncommon, they are a recognized long-term adverse effect. Current management generally recommends discontinuation of hydroxyurea as the mainstay of treatment; however, drug withdrawal may aggravate the underlying hematologic disease. We report a case of bilateral medial malleolar ulcers associated with hydroxyurea therapy in a 65-year-old man with essential thrombocythemia, who showed marked improvement after two months of topical treatment

3D printed hydroxyurea for pediatric use: toward personalized formulations and reduced exposure risk.

Sickle cell anemia is one of the most frequent severe monogenic disorders in the world, with the highest prevalence in Sub-Saharan Africa, India, the Middle East, and in populations with ancestry from these regions. Currently, hydroxyurea is recommended as a primary prevention from 9 to 12 months of age in infants affected by this condition, with doses adjusted upon weight and biological parameters. Because treatment is initiated early and maintained throughout life, the lack of age-adapted, palatable formulations poses major challenges to long-term adherence. To address this issue in children

Hydroxyurea attenuates hepatic stellate cell proliferation in vitro and liver fibrogenesis in vivo.

Liver fibrosis results from excessive proliferation of, and collagen production by hepatic stellate cells (HSCs) that is caused by chronic liver injury. No drugs are available to cure liver fibrosis. Hydroxyurea is an anti-proliferative drug that is used in benign and malignant disorders. Here, we studied the effect of hydroxyurea on primary HSCs and its anti-fibrotic effect in the CCl4 mouse model of liver fibrosis. Primary rat HSCs were cultured in the absence or presence of hydroxyurea (0.1-1.0 mmol/L). CCl4 or vehicle was administered to C57BL/6/J mice for 4 weeks, with or without hydroxyu

Advanced clinical parameters: A complementary hydroxyurea adherence evaluation in sickle cell anemia treatment.

OBJECTIVES: Given the complex pathology of sickle cell anemia (SCA) and low adherence to hydroxyurea (HU) treatment, there is a need to seek parameters that identify recent changes in patient status. The advanced clinical parameters (ACPs) allow an early analysis of hematopoiesis. We aimed to draw the demographic profile of non-adherent SCA patients and to verify the use of ACPs as a measure of HU treatment adherence. METHOD: In a cross-sectional study, we divided 83 SCA subjects treated with HU into Children (<12 years old) and adolescents/adults (≥12 years old). Their hemogram with the ACPs

Health Care Costs and Thromboembolic Events in Hydroxyurea-Treated Patients with Polycythemia Vera.

BACKGROUND: Patients with polycythemia vera (PV) are at increased risk of thromboembolic events (TEs), which are key contributors to reduced overall survival compared with the age- and sex-matched general population. In addition to aspirin and phlebotomy to maintain hematocrit level < 45%, many patients receive cytoreduction with hydroxyurea (HU), which is associated with improved survival and may reduce the risk of cardiovascular events and TEs. However, 1 in 4 patients become resistant to or intolerant of HU. In the general population, prophylaxis and treatment following arterial and venous

Hydroxyurea in Pediatric Patients With Sickle Cell Disease: What Nurses Need to Know.

Sickle cell disease (SCD) is an inherited disorder in which sickled red blood cells occlude the small vessels of the body, reducing oxygen delivery to tissues and ultimately negatively affecting many of the body's major organs. Hydroxyurea has proven beneficial in the treatment of SCD and prevention of disease-related complications. The 2014 guidelines put forth by the National Heart, Lung, and Blood Institute recommend hydroxyurea treatment in infants 9 months and older, children, and adolescents with SCD-SS or SCD-Sβ(0) thalassemia regardless of clinical severity. This is a change from the 2

Parental and other factors associated with hydroxyurea use for pediatric sickle cell disease.

BACKGROUND: Hydroxyurea (HU) is highly effective treatment for sickle cell disease (SCD). While pediatric use of HU is accepted clinical practice, barriers to use may impede its potential benefit. PROCEDURE: A survey of parents of children ages 5-17 years with SCD was performed across five institutions to assess factors associated with HU use. RESULTS: Of the 173 parent responses, 65 (38%) had children currently taking HU. Among parents of children not taking HU, the most commonly cited reasons were that their hematology provider had not offered it, their child was not sufficiently symptomat

A multidisciplinary team approach to hydroxyurea-associated chronic wound with squamous cell carcinoma.

Hydroxyurea (HU) has been shown to induce a variety of cutaneous adverse reactions, including severe leg ulcers. This report shows a successful treatment of a HU-induced chronic wound associated with squamous cell carcinomas (SCC). A 62-year-old patient affected with polycythemia vera and treated with HU for 10 years, presented with a non healing ulcer on a left heel. The patient gave a history of suffering from the wound for over 2 years. Biopsy showed evidence of invasive SCC. The patient underwent Mohs surgery and a greater saphenous vein ablation for polycythemia vera-associated vascular c

Hydroxyurea therapy increases plasma erythropoietin in patients with essential thrombocythaemia or polycythaemia vera.

The determination of serum/plasma erythropoietin (EPO) concentration has gained widespread use in the diagnosis of polycythaemia vera (PV). A reduced EPO concentration in a newly diagnosed essential thrombocythaemia (ET) seems to be a risk factor for thromboembolic events. In this study plasma EPO concentration was determined before and after initiated hydroxyurea (HU) therapy, 14 patients with PV or ET were included. After 1 month on HU therapy 11 of 14 patients had increased their EPO concentration compared with prior to medication. The plasma EPO was increased in all, except one patient, af

Perimalleolar ulcers in hydroxyurea treated patients with concomitant chronic venous disease: diagnostic pitfalls.

We report three male patients, with duplex confirmed chronic venous disease, who were on treatment with hydroxyurea for chronic myelogenous leukaemia (CML) and polycythemia vera (PV), referred to us for the management of non-healing perimalleolar ulcers of varying durations. Treatment of the ulcers, based on standard venous ulcer treatment protocols, proved futile, and one patient actually underwent surgery for varicose veins. The ulcers healed only after the discontinuation of hydroxyurea. In another patient, the ulcer continued to progress along with emergence of ankle oedema, venous eczema

Hydroxyurea-induced splenic regrowth in an adult patient with severe hemoglobin SC disease.

Hydroxyurea has been extensively used in patients with sickle cell anemia and severe sickle cell-hemoglobin C (SC) disease to reduce the severity of their diseases. We report here our experience with an adult patient with severe SC disease who developed symptomatic splenomegaly requiring splenectomy while being treated with hydroxyurea. This case suggests that hydroxyurea might restore some splenic function in functionally asplenic patients with sickle cell anemia or SC disease, but also raises the clinical concern that hydroxyurea may induce splenic regrowth, resulting in symptomatic splenome
