Hypernatremia induced by low-dose Tolvaptan in a Patient with refractory heart failure: A case report.

RATIONALE: Tolvaptan (TLV) is a selective vasopressin type 2 receptor antagonist, which has an active effect on patients with congestive heart failure especially combined with hyponatremia. Increasingly, evidence has demonstrated that low-dose tolvaptan can dramatically relieve patients' dyspnea and the dose would not cause severe electrolyte abnormalities. Even hypernatremia is a major adverse effect of tolvaptan, treatment with tolvaptan shows good security and is well-tolerated. Few cases have reported that patients who developed severe hypernatremia induced by low-dose Tolvaptan. PATIENT

Inhibition of vasopressin-release during developing hypernatremia and plasma hyperosmolality: an effect of intracerebroventricular glycerol.

In non-hydrated goats prolonged (3 h, 0.02 ml/min) intracerebroventricular (IVT) infusion of 0.35 M glycerol depressed the plasma vasopressin level during the entire infusion period which resulted in a conspicuous water diuresis outlasting the infusion by about 20 min. Since no compensatory drinking occurred during this sustained water diuresis it gradually induced pronounced dehydration (loss of greater than 1 liter of total body water causing 5% increase in plasma [Na+] and osmolality). The same degree of dehydration was in other experiments induced by water deprivation. It then caused a 5-f

Effect of Desmopressin on Attaining Therapeutic Hypernatremia with Hypertonic Saline Continuous Infusion in Intracranial Hemorrhage.

BACKGROUND: Platelet dysfunction in the setting of intracranial hemorrhage may potentiate worsening hemorrhage. Desmopressin may be used to treat platelet dysfunction in intracranial hemorrhage through von Willebrand factor release. Hypertonic saline is often used in this population to treat cerebral edema and elevated intracranial pressure. Free water reabsorption associated with desmopressin may attenuate the effect of hyperosmolar therapy. The effect of desmopressin on continuous infusion 3% sodium chloride (3%) when targeting a specific sodium goal has not been definitively elucidated. We

Successful treatment of hypodipsic/adipsic hypernatremia in a cat with lobar holoprosencephaly using oral desmopressin.

CASE SUMMARY: A 2-year-old female spayed domestic shorthair cat was presented with a history of collapse, possible hypodipsia/adipsia, severe dehydration and hypernatremia. MRI of the brain revealed a failure of separation of the cerebral hemispheres as characterized by an absence of the rostral part of the corpus callosum, fornix and septum pellucidum and the presence of a single fused lateral ventricle. A diagnosis of hypodipsic/adipsic hypernatremia with lobar holoprosencephaly was made. Dietary management of the cat's condition was attempted by increasing oral water intake, but the cat's h

Adipsic hypernatremia in a young Sudanese child, challenges in a limited-resource setting: a case report.

Adipsia is a rare condition characterized by a lack of thirst due to a defect in specific osmoreceptors located in the hypothalamus. The disorder is characterized by failure to maintain the body's normal plasma osmolality (POSM), resulting in chronic or recurrent severe hypernatremia and dehydration. Adipsia is usually accompanied by central diabetes insipidus (DI). Isolated adipsia (without DI) is very rare, with causes ranging from congenital central nervous system malformations to acquired anterior hypothalamic lesions. The diagnosis and management of the condition are considerably challeng

Osmotic Demyelination Syndrome Associated with Hypernatremia Caused by Lactulose Enema in a Patient with Chronic Alcoholism.

A 44-year-old man with chronic alcoholism presented with seizure and loss of consciousness. He was diagnosed with alcoholic hepatic encephalopathy, and his neurologic symptoms recovered after lactulose enema treatment. His initial serum sodium level was 141mEq/L. However, his mental state became confused after treatment with lactulose enema for five days, and his serum sodium level increased to 178mEq/L. After five days of gradual correction of serum sodium level from 178mEq/L to 140mEq/L, the patient's mental state recovered, but motor weakness in both limbs remained. Therefore, magnetic reso

Salty, Sweet and Difficult to Treat: A Case of Profound Hypernatremia in the Setting of Hyperosmotic Hyperglycemic State.

Hyperosmolar hyperglycemic state (HHS) is a disorder that occurs most frequently in type 2 diabetics and is associated with high mortality - up to 50%. Hypernatremia, when associated with HHS, worsens the prognosis. Encephalopathy is evident at a serum sodium level greater than 160 mOsm/kg. Additional symptoms include lethargy, weakness, seizures, and coma. Rhabdomyolysis can rarely occur in hyperosmolar states. Here we describe a case of severe hypernatremia in the setting of HHS leading to profound encephalopathy and report to the best of our knowledge the highest serum sodium level publishe

Hypopituitarism: Case Study Involving Hypernatremia Prompting Discovery of a Pituitary Disorder in a Patient With Lymphoma.

BACKGROUND: The endocrine system contributes to numerous physiologic processes. Compensatory mechanisms are in place that can assist when endocrine dysfunction occurs, which may make it difficult to identify pathologic states. OBJECTIVES: The case study in this article presents a 74-year-old woman with diffuse large B-cell lymphoma who was admitted to an oncology critical care unit with neutropenic fevers and suspicion of gastrointestinal bleeding. She was placed on NPO status (no oral intake) overnight and quickly developed hypernatremia. This prompted further investigation, resulting in the

Pathogenesis of chronic hypernatremia with dehydrated and non-dehydrated states in hypothalamic space-occupying lesions.

The present study was undertaken to determine pathophysiology of body water control in hypernatremic subjects with hypothalamic space-occupying lesions. Eight subjects with hypothalamic space-occupying lesions were divided into two groups of hypernatremia in the presence or absence of body water deficit. In 5 dehydrated hypernatremic subjects whose ages ranged from 20 to 67 years, serum sodium (Na) levels were 156.4 +/- 3.1 mmol/l; plasma osmolality (Posm), 320.6 +/- 9.8 mmol/kg; and urinary osmolality (Uosm), 246.8 +/- 46.7 mmol/kg under ad libitum water drinking. Severe Hypernatremia Due to Postoperative Diabetic Ketoacidosis and Transient Central Diabetes Insipidus: A Case Report.

We report a case of acute severe hypernatremia secondary to diabetic ketoacidosis (DKA) and transient central diabetes insipidus (CDI) in a postoperative patient. Diagnosis of DKA may be delayed in patients on sodium-glucose cotransporter 2 (SGLT2) inhibitors, particularly in the postoperative state. Hypernatremia was refractory to medical management and corrected with slow low-efficiency dialysis (SLED) and desmopressin supplementation. This case highlights the importance of suspecting diabetes insipidus in refractory hypernatremia and demonstrates the role of SLED for controlled sodium corre

Osmotic Demyelination Syndrome Associated With Severe Acute Hypernatremia Secondary to Poorly Controlled Central Diabetes Insipidus.

Osmotic demyelination syndrome is a rare disorder caused by destruction of the myelin sheath, particularly in the pontine region, generally secondary to rapid osmotic variations. Osmotic demyelination syndrome due to hypernatremia is rarely described in the literature. The prognosis has improved thanks to a better understanding of its pathophysiology and advances in diagnosis and therapy. In this article, we report the case of centropontine and extrapontine myelinolysis associated with severe acute hypernatremia at 181 mEq/l secondary to abrupt discontinuation of desmopressin prescribed in the

Hypernatremia is associated with poorer outcomes following aneurysmal subarachnoid hemorrhage: a nationwide inpatient sample analysis.

BACKGROUND: Hypernatremia is one of the most common electrolyte disturbances following aneurysmal subarachnoid hemorrhage (aSAH) and has been correlated with increased mortality in single institution studies. We investigated this association using a large nationwide healthcare database. METHODS: We performed a retrospective analysis of adults between 2002 and 2011 with a primary diagnosis of aSAH using the Nationwide Inpatient Sample (NIS). Patients were grouped according to whether or not an inpatient diagnosis of hypernatremia was present. The primary outcome was the NIS-SAH outcome measure

Effect of Hypernatremia on Outcomes After severe Traumatic Brain Injury: A Nationwide Inpatient Sample analysis.

OBJECTIVE: Induced hypernatremia is frequently used to reduce intracranial pressure in patients with severe traumatic brain injury (TBI). This technique is controversial, and some studies have independently associated hypernatremia with worse outcomes after TBI. We sought to investigate this potential association in a large healthcare database. METHODS: The Nationwide Inpatient Sample was used to obtain data on all adults who had been discharged from 2002 to 2011 with a primary diagnosis of TBI who required mechanical ventilation, intracranial pressure monitoring, or craniotomy/craniectomy. Plasma exchange successfully treats central pontine myelinolysis after acute hypernatremia from intravenous sodium bicarbonate therapy.

BACKGROUND: Osmotic demyelination syndrome (ODS) primarily occurs after rapid correction of severe hyponatremia. There are no proven effective therapies for ODS, but we describe the first case showing the successful treatment of central pontine myelinolysis (CPM) by plasma exchange, which occurred after rapid development of hypernatremia from intravenous sodium bicarbonate therapy. CASE PRESENTATION: A 40-year-old woman presented with general weakness, hypokalemia, and metabolic acidosis. The patient was treated with oral and intravenous potassium chloride, along with intravenous sodium bicar
