Interpregnancy intervals and the risk for infant mortality: a case control study of Arizona infants 2003-2007.

There is well-documented evidence on how interpregnancy interval (IPI) is associated with adverse perinatal outcomes and how short and long IPIs are associated with increased risk for preterm birth, low birth weight, and intra-uterine growth restriction. However, the extremes of IPI on infant mortality are less well documented. The current study builds on the existing evidence on IPI to examine if extremes of IPI are associated with infant mortality, and also examines if IPI is associated with both neonatal and post-neonatal mortality after adjusting for several known confounders. Association of Special Supplemental Nutrition Program for Women, Infants, and Children With Preterm Birth and Infant Mortality.

IMPORTANCE: Nearly 4 in 10 expectant mothers in the United States received Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) benefits during pregnancy between 2011 and 2017. Despite public support for the program, empirical evidence of the success of the program varies substantially. OBJECTIVE: To assess the association of WIC program participation during pregnancy by low-income expectant mothers covered by Medicaid with infant mortality by gestational age at birth and by maternal race/ethnicity in comparison with their counterparts who did not receive WIC benefits

Therapeutic hypothermia and in-hospital mortality in 35-week infants with encephalopathy.

OBJECTIVES: Therapeutic hypothermia (TH) improves outcomes in infants ≥36 weeks with HIE, although safety at 35weeks remains uncertain. We compared TH utilization and associated outcomes in 35- versus 36-week infants with HIE. STUDY DESIGN: Using the 2016-2022 National Inpatient Sample, we identified 35- and 36-week infants with HIE. The primary outcome was in-hospital mortality; secondary outcomes included coagulopathy and other complications. Logistic regression adjusted for covariates; Cochran-Armitage tested trends in TH use. RESULTS: Among 1,397,680 infants, HIE occurred in 2354 (0.48%)

A new mortality score in preterm infants: the vasoactive inotropic score.

OBJECTIVES: The aim of this study is to evaluate the predictive value of the vasoactive inotropic score (VIS) for mortality in extremely low birth weight (ELBW) preterm infants and to compare this value with the well-known Clinical Risk Index for Babies-II (CRIB-II) score. METHODS: This study was designed as a retrospective study. A total of 280 preterm infants weighing under 1000 g who were admitted to the neonatal intensive care unit over a five-year period were included in the study. For each patient, a CRIB-II score and VIS were calculated, and their ability to predict mortality was compa

Continuous glucose monitoring for the prevention of morbidity and mortality in preterm infants.

BACKGROUND: Preterm infants are susceptible to hyperglycemia and hypoglycemia, conditions which may lead to adverse neurodevelopment. The use of continuous glucose monitoring devices (CGM) might help keeping glucose levels in the normal range, and reduce the need for blood sampling. However, the use of CGM might be associated with harms in the preterm infant. OBJECTIVES: Objective one: to assess the benefits and harms of CGM alone versus standard method of glycemic measure in preterm infants. Objective two: to assess the benefits and harms of CGM with automated algorithm versus standard metho

Utility of echocardiography in predicting mortality in infants with severe bronchopulmonary dysplasia.

OBJECTIVE: To determine the relationship between interventricular septal position (SP) and right ventricular systolic pressure (RVSP) and mortality in infants with severe BPD (sBPD). STUDY DESIGN: Infants with sBPD in the Children's Hospitals Neonatal Database who had echocardiograms 34-44 weeks' postmenstrual age (PMA) were included. SP and RVSP were categorized normal, abnormal (flattened/bowed SP or RVSP > 40 mmHg) or missing. RESULTS: Of 1157 infants, 115 infants (10%) died. Abnormal SP or RVSP increased mortality (SP 19% vs. 8% normal/missing, RVSP 20% vs. 9% normal/missing, both p < 0.

Laryngotracheobronchial anomalies in infants and the related risk factors of in-hospital mortality.

BACKGROUND: Laryngotracheobronchial anomalies (LTBAs) may cause respiratory problems during early childhood, and increase the risk of hospitalization or mortality in diseased children. This study investigated the initial hospitalization age and risk factors for in-hospital mortality in infants diagnosed with LTBAs during their first 5 years of life. METHODS: Hospitalized infants diagnosed with LTBAs were retrieved from Taiwan's National Health Insurance Research Database from 2003 to 2005. Their medical claim data were traced up to 59 months of age. The age distribution of all LTBA cases was

Assessing mortality risk in very low birth weight infants.

INTRODUCTION: Preterm birth is the most important univariant risk factor of neonatal mortality. Assessment of risk factors affecting mortality in preterm infants with very low birth weight is important for the treatment of this highly vulnerable population. OBJECTIVE: Detection of risk factors for neonatal mortality in very low birth weight premature infants. METHODS: The current study was conducted in a tertiary research and educational hospital, NICU, Pediatric Clinic KCU Sarajevo, from January 2010 to December 2010. After admission CRIB score was determined to every hospitalized infant wi

Hyperbilirubinemia in infants with Gram-negative sepsis does not affect mortality.

BACKGROUND: Sepsis is associated with an increased production of oxidant species and a decrease in endogenous antioxidant defenses. Mortality is high, especially when endotoxins are involved, e.g., in infants with Gram-negative sepsis. Yet, chronic as well as acute unconjugated hyperbilirubinemia has been shown to protect against endotoxin-induced shock in vivo in rats and in mice. We hypothesized that hyperbilirubinemia in infants with Gram-negative sepsis improves survival and/or mitigates the inflammatory response. OBJECTIVE: To assess the relationships between serum bilirubin concentratio

Racial disparities in mortality among infants with Dandy-Walker syndrome.

BACKGROUND: Congenital malformations are the major cause of infant mortality in the United States, but their contribution to overall racial disparity--a major public health concern--is poorly understood. We sought to estimate the contribution of a congenitally acquired central nervous system lesion, Dandy-Walker Syndrome (DWS), to black-white disparity in infant mortality. METHODS: Data were obtained from the New York State Congenital Malformations Registry, an ongoing population-based validated surveillance system. We compared black to white infants with respect to infant, neonatal, and post

Transanal protrusion of intussusception in infants is associated with high morbidity and mortality.

BACKGROUND: Although anal protrusion of intussusception in infants is well recognised, it is rarely reported and confusion with rectal prolapse often results in delayed diagnosis and treatment. This report highlights the problems of diagnosis and the morbidity and mortality associated with this condition. METHOD: A retrospective case series of five infants presenting to Ahmadu Bello University Teaching Hospital (ABUTH), Zaria, Nigeria with anal protrusion of intussusception over a period of 5 years. During that time, 17 children were treated for intussusception at ABUTH. RESULTS: The anal pr

Prophylactic antibiotics to reduce morbidity and mortality in ventilated newborn infants.

BACKGROUND: Intubation is associated with bacterial colonisation of the respiratory tract and, therefore, may increase the risk of acquiring an infection. The infection may prolong the need for mechanical ventilation and increase the risk of chronic lung disease. The use of prophylactic antibiotics has been advocated for all mechanically ventilated newborns in order to reduce the risk of colonisation and the acquisition of infection. However, there is the possibility that the harm this may cause might outweigh the benefit. OBJECTIVES: To assess the effects of prophylactic antibiotics on morta
