Role of ultrasound and informed consent in the evaluation of elevated maternal serum alpha-fetoprotein.

Informed consent of medical procedures should include a discussion of both the risk of the procedure and the probability of the malady in question. Many centers in the United States currently recommend amniocentesis for all women with elevated levels of maternal serum alpha-fetoprotein (MSAFP) unexplained by targeted ultrasound examination. Prospective clinical data from our unit support the use of an algorithm that provides for a 90% reduction of the predicted risk of neural tube defect from MSAFP alone. The predicted risk is revised only if the ultrasound shows normal fetal cranial size and

Guidance on communication and informed consent with patients and their families for experimental individualized treatments.

With advanced tools and techniques, it is now possible to genetically diagnose an increasing number of individuals with rare diseases for which no disease-modifying treatment exists. For a subset of affected individuals, it is possible to develop individualized interventions to modulate and/or correct the underlying genetic defect. Communicating to such individuals and their guardians about the potential risks and benefits of such experimental therapies poses special challenges. In addition to typical procedures used for consenting subjects of clinical trials or off-label therapy, individualiz

Procedural sedation in dentistry: a scoping review and proposal for an ad hoc informed consent.

BACKGROUND: The healthcare aspects of dentistry are framed within a legal environment. Dentists are obliged to obtain their patients’ written informed consent (IC) before undertaking any procedures. A failure to fulfill IC obligations or to fulfill such obligations correctly can have legal repercussions related to professional liability. Procedural sedation (PS) in dental patients requires an independent health treatment. This is usually successfully provided by authorized sedative drugs that induce anxiolysis and sedation, allowing the patient to tolerate uncomfortable procedures while mainta

AI-generated versus standard informed consent forms in otolaryngology: a blinded multirater pilot feasibility study.

BACKGROUND: Informed consent documents often exceed recommended readability levels, potentially compromising patients' decision-making. AIMS/OBJECTIVES: This pilot feasibility study evaluated two large language models (LLMs), ChatGPT-5.2 and Gemini 3 Pro, as tools for generating more comprehensible otolaryngology surgical consent forms, and estimated effect-size parameters for a future trial. MATERIALS AND METHODS: In a prospective, double-blind design, 123 Turkish consent documents (41 procedures × 3 sources: official forms, ChatGPT-5.2 and Gemini 3 Pro) were evaluated. Three independent ot

Optimizing Informed Consent for Australian Newborn Bloodspot Screening and Research: Consensus Workshop Insights and Recommendations.

Informed consent is fundamental to Australian newborn bloodspot screening (NBS), but emerging genomic screening technologies pose new challenges to clinical care and research. Optimizing consent processes is necessary to support ethical practice and maintain public trust as NBS evolves. This study aimed to identify gaps and opportunities to improve NBS consent processes in Queensland, Australia, while also exploring preliminary insights into the evolving complexity of consent in the context of genomic NBS (gNBS) and NBS-related research. A qualitative study design was used, with two facilitate

Trust, autonomy, and informed consent: a qualitative investigation of youth experiences accessing contraception in Canada.

We sought to understand, "What are the contraception access experiences, beliefs, attitudes, knowledge and needs of youth in Canada, from the perspectives of youth and youth service providers?" We explore person-centred contraception care and highlight the importance of fostering trust, autonomy, and informed consent. We conducted one-on-one semi-structured interviews with youth (aged 15-25) and healthcare providers (HCPs) across Canada, representing a diversity of rural and urban settings, gender, age, and ethnicities. Using social constructivist grounded theory, we identified key themes rela

Understanding patient and surgeon attitudes towards informed consent for sensitive intraoperative exams in colorectal surgery.

BACKGROUND: There is limited understanding of how consent is obtained for digital rectal (DRE) and intraoperative pelvic exams (IPE) in colorectal surgery. METHODS: Two-pronged survey of colorectal patients and surgeons. Participants self-rated their agreement with various statements regarding consent for surgery, DRE, and IPE. RESULTS: Out of 90 patients, 77 % agreed that surgeons may perform additional procedures other than those explicitly consented to preoperatively. 89 % agreed that DRE was implicit in the consent but 65 % preferred for it to have been explicitly discussed preoperati

Informed Consent in Vulnerable Populations: The Case of Detained Persons with Attention Deficit Hyperactivity Disorder.

Informed consent (IC) is a critical component in research involving human participants, yet participants' understanding of consent information remains underexplored, particularly in vulnerable populations. This study aimed to assess whether attention deficit hyperactivity disorder (ADHD) was associated with understanding and willingness to sign the IC among detained individuals. This secondary analysis used data from a randomised trial conducted in a Swiss prison (n = 183). Statistical analyses included regression models. There was no significant difference in understanding of the IC between t

Informed consent: is it more than a formality? a quality improvement project in surgical practice.

Informed consent is a fundamental element of clinical practice. A well-structured and understandable consent form not only upholds patients' rights but also protects clinicians against medicolegal risks. This quality improvement project aimed to identify deficiencies in the consenting process for surgical procedures and implement changes to enhance patient care. The project was conducted in the Ear Nose Throat Department at Holy Family Hospital, Rawalpindi, a large tertiary care centre serving a predominantly low socioeconomic population in Pakistan. A total of 80 patients were included across

Facing the new diagnostic and treatment options of Alzheimer's disease: The necessity of informed consent.

With advances in biomarker-based detection of Alzheimer's disease (AD) and new treatment options with disease-modifying treatments (DMTs), we are heading toward a new conceptualization of diagnostics and therapy in the early stages of AD. Yet consensus guidelines on best clinical practices in predictive AD diagnostics are still developing. Currently, there is a knowledge gap regarding counseling and disclosure practices in early symptomatic disease stages, its implications for dementia risk estimation, and DMTs with associated risks and benefits. The crucial feature is the capacity of patients

Practices of Informed Consent for Emergency Procedures at a Tertiary Care Hospital in Lahore, Pakistan.

Background The purpose of obtaining informed consent is to ensure that patients undergoing any medical or surgical intervention are neither deceived nor coerced. Accurately estimating surgical risks is critical for shared decision-making and informed consent. Probable complications and alternative procedures should be presented to the patient so that they can freely choose an operative option. However, this factor is difficult to carry on in emergencies where an urgent decision is required. Objective This study aimed to assess the ongoing clinical practices of informed consent in emergency sur

Parturients feel capable of giving informed consent for epidural analgesia: A qualitative and quantitative analysis.

INTRODUCTION: The patient's right to autonomy confirmed by informed consent is a cornerstone in modern medicine. Epidural analgesia is increasingly popular in obstetric analgesia, but physicians disagree whether labour pain impairs parturient decision-making. We investigated the fraction of parturients feeling capable of giving informed consent including their knowledge of risks. METHODS: Bedside survey postpartum women at the Herlev Hospital, Denmark. The inclusion criteria were recipient of epidural analgesia during labour. A power calculation based on the recognition of genuine and false s

Informed consent is poorly documented when obtaining toxicology testing at delivery in a Massachusetts cohort.

BACKGROUND: Positive toxicology testing at delivery can have enormous consequences for birthing persons and their families, including charges of child abuse or neglect and potential loss of custody for the birthing parent. Therefore state and national guidelines stipulate that, clinicians must obtain consent before toxicology testing at delivery. OBJECTIVE: This study aimed (1) to determine clinician documentation of patient consent for peripartum toxicology testing and (2) to characterize the extent to which patient and hospital characteristics were associated with documented consent.
