Investigation of the morphometry of the pre-lacrimal recess of the maxillary sinus for the pre-lacrimal approach of the maxillary sinus and paramedian skull base. A computed-tomography study.

The pre-lacrimal recess approach is modernly used for lesions of the anterior maxillary wall and for reaching paramedian cranial base regions. In this computed-tomography study, we assessed the pre-lacrimal recess types as well as the angles between the anterior and medial maxillary walls and between the anterior maxillary wall and the lateral margin of the nasolacrimal canal to show the feasibility of the pre-lacrimal recess approach in reaching lesions of the infratemporal and pterygopalatine fossae, using 30 computed-tomography studies (60 sides). A type I pre-lacrimal recess was identified

Chondroblastoma of the temporal bone.

Chondroblastomas are highly destructive tumors that are derived from immature cartilage cells. The occurrence of this tumor in the temporal bone or skull base is uncommon. Approximately 70 cases have previously been reported, several of which have involved the temporomandibular joint (TMJ). We report here the case of a 67-year-old woman who presented with right-sided mixed hearing loss, a right external auditory canal mass, ear fullness, otalgia, blood-stained otorrhea, and pain around the TMJ, associated with difficulty in opening the mouth. CT and MRI revealed a mass involving the TMJ, infra

Surgical management of previously untreated glomus jugulare tumors.

The treatment of glomus jugulare tumors is controversial. Changes in the surgical treatment of glomus jugulare tumors at The House Ear Clinic have allowed complete resection in 85% of patients with minimal morbidity and no surgical mortalities. Our experience with 52 previously untreated patients with glomus jugulare tumors is reviewed. Two primary surgical techniques were used. The mastoid/neck approach was used in 9 patients with small tumors limited to the jugular bulb. The infratemporal fossa approach was used in the remaining 43 tumors. Lower cranial nerve preservation was possible in the

Anatomic Considerations Guiding Single Versus Multiportal Endoscopic Approaches for Resection of Juvenile Nasopharyngeal Angiofibroma: Cases Series With Graded Multicorridor Resections.

BACKGROUND: Juvenile nasopharyngeal angiofibromas (JNAs) are characterized by expansive and destructive growth, often invading the midline/paranasal sinuses, pterygopalatine fossa, and infratemporal fossa and can extend into the orbit, cavernous sinus, or intracranially. OBJECTIVE: To evaluete the major benefits of the extended endoscopic endonasal approach (EEA) for JNA resection as compared with more traditional and invasive transpalatal and transfacial approaches. When JNAs extend into lateral anatomic compartments, the optimal operative trajectory often requires additional approach strate

Allergic fungal rhinosinusitis presenting with intracranial spread along large sphenoidal emissary foramen.

A 13-year-old girl was admitted for headache, right periorbital swelling and erythema. CT imaging demonstrated right orbital preseptal cellulitis, severe pansinusitis and suspected epidural abscess. Brain MRI and sinus CT confirmed an epidural abscess in the right middle cranial fossa and a second extra-cranial abscess in the right infratemporal fossa along with large right sphenoidal emissary foramen. Drainage from sinus surgery confirmed allergic fungal rhinosinusitis. She was treated with prednisone and voriconazole.

Nasopharyngeal angiofibroma: a concise classification system and appropriate treatment options.

OBJECTIVES: To describe a clear and simplified classification system for juvenile nasopharyngeal angiofibroma (JNA), and to describe suitable management options. STUDY DESIGN: Retrospective medical record review. METHODS: The clinical and imaging materials of 51 cases of JNA diagnosed at our hospital between 1981 and 2011 were collected and studied. Based on our experiences, we prefer to divide JNAs into three types. Type I includes JNAs fundamentally localized to the nasal cavity, paranasal sinus, nasopharynx, or pterygopalatine fossa. Type II is a JNA extending into the infratemporal fossa

Nasopharyngeal teratoma involving the temporal bone.

Teratoma is the most common nasopharyngeal tumor in neonates. The tumor is usually limited to the oropharynx and is manifest by stridor and respiratory distress. Complete excision affects a cure. An unusual case of nasopharyngeal teratoma extending into the temporal bone, causing facial paralysis and conductive hearing loss, in addition to stridor, is presented. Stridor was relieved by transoral partial excision shortly after birth. Complete removal of the tumor by way of a subtemporal and infratemporal fossa approach was performed at 14 months of age. The defect was filled with a rectus abdom

Biportal Endoscopic TransOrbital and transMaxillary Approach to the Cranio-Orbital Region and Middle Cranial Fossa: A Preliminary Analysis of Maneuverability.

BACKGROUND AND OBJECTIVES: Traditional and well-established transcranial approaches to the spheno-orbital region and middle cranial fossa guarantee optimal intracranial exposure, and additional orbital and zygomatic osteotomies provide further control over extracranial components to be resected; however, these techniques come at the cost of additional morbidity. The introduction of minimally invasive endoscopic approaches and the conceptualization of the so-called "multiportal" paradigm might provide an alternative route. This preliminary study investigates the feasibility of the combined Bipo

The value of transmandibular approach to total maxillectomy in the management of T3-T4 carcinoma of the maxillary sinus.

Surgical excision of the maxilla either as a primary procedure or after radiotherapy continues to play an important role in the management of carcinoma of the maxillary sinus. A majority of these cancers are seen in T3-T4 stage. Recurrence at the infratemporal fossa has been recognized as a major cause of failure. Although the importance of inclusion of the pterygopalatine fossa in the field of excision has been recognized, there is no uniform approach for en bloc removal of the pterygopalatine fossa. Transmandibular approach has been described for purpose and practiced at the Bangalore Instit

The orbitozygomatic infratemporal approach: a new surgical technique.

Lesions in the parasellar region and the interpeduncular fossa, including medial-third sphenoid wing meningiomas, petroclival meningiomas, trigeminal neurinomas, and basilar tip aneurysms, are very difficult to approach for radical procedures. To minimize brain retraction and achieve excellent exposure in the shortest possible distance for safe manipulation within these regions, the authors have developed a new surgical technique, an orbitozygomatic infratemporal approach. Sixteen patients with parasellar tumors, nine patients with basilar tip aneurysms, and one patient with a P-1 distal aneur

Transmandibular exposure of the skull base.

The combined transmandibular-transcervical approach to the skull base ensures a wide field exposure to the lateral and midline compartments of the middle cranial fossa with attendant vascular control. Splitting the lip and mandible in the midline and dividing the floor of the mouth structures along the lateral border of the tongue exposes the parapharyngeal space, infratemporal fossa (lateral compartment), and clivus, nasopharynx, and cervical spine (midline compartment). A variety of benign and malignant intracranial and extracranial skull base lesions have been treated using this approach.

Orbital neurofibromatosis with enophthalmos.

Two patients with orbital neurofibromatosis associated with enophthalmos are presented. Assessment using 3D-CT scan shows an increase in the size of the orbital cavity and an enlargement of the inferior orbital fissure, which allows orbital fat to prolapse into the infratemporal fossa, causing enophthalmos.

The surgical management of advanced malignancies of the parotid gland.

Advanced malignancies of the parotid gland frequently invade the parapharyngeal space and the infratemporal fossa. The majority of these lesions have not been cured by surgery and/or radiotherapy, and palliation or cure can only be achieved by en bloc resection of this region. Stylohamular dissection is a systematic method for en bloc resection of the infratemporal fossa and lateral skull base. From January 1980 until December 1987, 18 patients with advanced parotid malignancies underwent stylohamular dissection. Pathologic examination revealed the following diagnosis: adenocarcinoma (7), squa

[Comparison of computed tomographic scanning and magnetic resonance imaging in the diagnosis of trigeminal schwannoma. Report of four cases].

Four cases of trigeminal schwannoma with different clinical presentations are reported. Two patients had root-type tumors and two ganglion-type tumors. Computed tomography demonstrated the mass in each case, but extension of the tumor into Meckel's cave, displaced but uninvolved seventh and eighth cranial nerves, extension of the tumor into the orbit or into the infratemporal fossa, and other anatomical details were far more clearly shown by magnetic resonance imaging, which provided confirmation of the mass lesions in the base of the brain and within the posterior fossa.

Advances of neuroimaging, skull base technique and embolization improved outcome in patients who present with tumor of the glomus jugulare. Catecholamine secreting subgroup, however, is considered to be extremely high risk because of potentially serious complication of an intra- and perioperative hypertension crisis. The authors present detailed description of treatment strategies and perioperative management with a catecholamine secreting glomus jugulare.
