Cardioplegia strategies in ALCAPA surgery: A narrative review on practical comparison of techniques.

Anomalous left coronary artery from the pulmonary artery (ALCAPA) is a rare yet life-threatening congenital cardiac anomaly in which the left coronary artery originates from the pulmonary trunk, leading to severe myocardial ischemia, infarction, and progressive heart failure if untreated. Definitive management requires surgical reimplantation of the anomalous artery into the aorta, supported by meticulously tailored cardioplegia to protect the vulnerable pediatric myocardium, whose perfusion is further complicated by right-to-left coronary collaterals. This manuscript reports the findings of a

Temperature Safeguards and Peri-Procedural Strategies for Stem Cell Collection in Cold Agglutinin Disease: A Case Report and Literature Review.

Cold agglutinins can result in red blood cell (RBC) agglutination and lysis at low temperatures. Temperature sensitivity during stem cell mobilization, processing, and infusion poses unique procedural challenges in patients with active cold agglutinins. A 49-year-old man presented at an outside hospital with severe hemolysis and was found to have an anti-I cold agglutinin and a component of warm autoimmune hemolytic anemia, together with a positive COVID and Rhinovirus test. He was treated with RBC transfusions, steroids, and Rituximab. Bone marrow evaluation several months later at our instit

Tracheal agenesis without esophageal fistula: genetic, resuscitative, and pathological issues.

An exceptional case of tracheal agenesis with no communication with the esophagus is described. This malformation needs surgical airway approach and is hardly classifiable. We analyzed the literature and our institutional data: this resulted to be the first case of such anatomical variant. Genetic and pathological issues are reviewed: recent genetic data seem to explain this malformation. We also reviewed the available literature about prenatal presentation. Because prenatal diagnosis is difficult to achieve and current guidelines for neonatal resuscitation do not provide any recommendation, t

Pediatric craniofacial reconstruction: an overview of perioperative management.

Pediatric craniofacial defects as a result of congenital malformation, tumor, or trauma are devastating to both the child and the family. These patients require extensive education, preparation, careful monitoring and psychological support. With continuing advances in surgical technology and technique, the effectiveness and utilization of pediatric craniofacial surgery is increasing. As this type of surgery is currently available at relatively few institutions, it has created a new nursing experience. A multi-disciplinary team approach has evolved at Texas Children's Hospital which necessitate

Establishing a Uterus Transplantation Program: Institutional and Clinical Framework for Implementation.

Uterus transplantation (UTx) is an emerging treatment for absolute uterine factor infertility, enabling gestation in individuals without a functional uterus. Since the first live birth in 2014, more than 150 procedures and over 80 live births have been reported worldwide. Unlike conventional solid organ transplantation, UTx is a temporary intervention aimed at restoring reproductive capacity, requiring integration across transplant surgery, reproductive endocrinology, and maternal-fetal medicine. Despite rapid clinical progress, structured guidance for program development remains limited. UTx

Special considerations in managing pain and psychosocial distress in patients with opioid use disorder and cancer: the role of the supportive care and psycho-oncology interdisciplinary team.

People with a substance use disorder (SUD) have shortened lifespans due to complications from their substance use and challenges engaging with traditional health care settings and institutions. This impact on life expectancy is especially prominent in patients with co-occurring SUDs and cancer, and often has a much worse prognosis from the cancer than a similar patient without a SUD. Palliative care teams are experts in serious illness communication and symptom management and have become increasingly embedded in the routine care of patients with cancer. We argue that the skill set of palliativ

Identification and stratification of placenta percreta with gynecologic oncologist management.

OBJECTIVE: Gynecologic oncologist involvement in the surgical team of patients with placenta percreta has shown improved patient outcomes. Yet, stratification of cases is dependent on identification of placenta percreta by ultrasonography which has a poor detection rate. To allow patients to receive optimal team management by pre-operative stratification our objective was to identify the pre-operative characteristics of patients with previously underdiagnosed placenta percreta. METHODS: A retrospective single institution case-control study was performed from January 2010 to December 2022 of s

Datopotamab deruxtecan-associated select adverse events: clinical practices and institutional protocols on prophylaxis, monitoring, and management.

OBJECTIVE: Datopotamab deruxtecan (Dato-DXd) is an antibody-drug conjugate comprised of a topoisomerase I inhibitor payload and a monoclonal antibody directed to trophoblast cell-surface antigen 2, a protein that is broadly expressed in several types of solid tumors. Dato-DXd demonstrated statistically significant improvement in median progression-free survival (mPFS) over docetaxel in patients with previously treated metastatic non-small cell lung cancer (mNSCLC) and over the investigator's choice of chemotherapy in patients with previously treated post-endocrine therapy hormone receptor-posi

A Guide for Initiating and Managing Chimeric Antigen Receptor T Cell Therapy Clinical Trials in Autoimmune Rheumatic Diseases.

Chimeric antigen receptor (CAR) T-cell therapy, long transformative in oncology, is now rapidly emerging as a frontier in autoimmune rheumatic diseases, particularly systemic lupus erythematosus (SLE), driven by accumulating evidence of deep B-cell depletion, immune "resetting," and durable drug-free remission in early studies, yet its translation into rheumatology demands mastery of formidable logistical, regulatory, clinical, and ethical complexities that span institutional readiness, multidisciplinary team formation, stringent regulatory compliance, sophisticated operational workflows, comp

From De-escalation to Restraint: A Qualitative Study of Emergency Clinician Decision-Making in Patient Agitation.

INTRODUCTION: Managing agitation in the emergency department (ED) is challenging and frequently involves decisions about physical restraints and emergent medications. While institutional protocols and behavioral response teams exist, little is known about how individual clinicians make decisions about restraint use-decisions with significant implications for safety, ethics, and equity. The objective of the study was to explore how emergency physicians, residents, and nurses make decisions about physical restraint and emergent medication use during episodes of patient agitation in the ED. Crisis Resource Management and Non-Technical Skills Training in Anesthesia: A Narrative Review.

Adverse medical events remain a serious concern worldwide; deficiencies in human factors contribute to a large proportion of medical incidents and preventable patient harm. In anesthesiology, where high-risk and complex clinical events are routine, non-technical skills (NTS)-including communication, teamwork, situational awareness, and decision-making-play a critical role in preventing errors during high-acuity emergencies. Furthermore, Crisis Resource Management (CRM) provides a structured framework for enhancing team performance and coordination during medical crises by emphasizing effective

Integration of the CLEFT-Q Patient-Reported Outcome Tool into a Multidisciplinary Cleft and Craniofacial Clinic: A Proof of Concept.

Implementation of patient-reported outcome (PRO) tools has increased internationally in a variety of clinical settings, with emerging evidence suggesting benefits for patient satisfaction, improved patient-provider communication, and management of chronic conditions. However, integrating PROs into clinical workflow remains a barrier to implementation, with common challenges including patient completion, provider review of results, and future accessibility of data. Cleft lip and/or palate presents with a variety of aesthetic, functional, and psychosocial sequelae optimally managed by a multidis

Transformation of an interprofessional learning strategy through organizational change principles and patient partnership.

Interprofessional learning (IPL) is required for health profession students to develop appropriate collaborative competencies. In 2024, in response to accreditation recommendations and evolving best practices, the Royal College of Surgeons in Ireland (RCSI), University of Medicine & Health Sciences undertook a comprehensive redesign of its original IPL strategy. Guided by Kotter's eight-step model for organizational change, an interprofessional leadership team of academic staff, student, and patient partners reviewed and updated their approach to IPL. This new strategy integrates formal, infor

A multidisciplinary approach to the management of liver disease and alcohol disorders in psychiatric settings (Review).

Society is burdened with the uncontrolled use of alcohol, an ongoing issue, with a substantial associated morbidity and a pressing economical reverberation. It is inevitable that a series of psychiatric patients who display alcohol disorders will be admitted to hospital while also suffering from health conditions, such as liver disease, due to the consumption of alcohol. Managing comorbid patients in a psychiatric facility is a delicate matter that requires a collaborative team. The aim of this systematic paper is to highlight the following: The possibility of treating alcohol use disorder (AU

Deep brain stimulation: a return journey from psychiatry to neurology.

Deep brain stimulation (DBS) has emerged as an effective neurosurgical tool to treat a range of conditions. Its use in movement disorders such as Parkinson's disease, tremor and dystonia is now well established and has been approved by the National Institute of Clinical Excellence (NICE). The NICE does, however, emphasise the need for a multidisciplinary team to manage these patients. Such a team is traditionally composed of neurologists, neurosurgeons and neuropsychologists. Neuropsychiatrists, however, are increasingly recognised as essential members given many psychiatric considerations tha

Kidney transplantation in HIV-positive patients: report of our first 7 cases.

INTRODUCTION: Infection with human immunodeficiency virus (HIV) is associated with end-stage renal disease (ESRD). Although many teams initially were reluctant to offer kidney transplantation as a therapeutic option in HIV-positive patients with ESRD, new drug regimens introduced in the late 1990s have dramatically improved the life expectancy in these patients. OBJECTIVE: To report the results of the first 7 kidney transplantation procedures in HIV-positive patients at our institution. PATIENTS AND METHODS: Patients were selected to minimize the risks of HIV disease progression, opportunist
