Vestibular cortex lesions affect the perception of verticality.

Seventy-one patients with unilateral supratentorial infarctions were evaluated with respect to static vestibular function in the roll plane, including determinations of the subjective visual vertical, skew deviation, and ocular torsion. Since animal studies have revealed at least four different areas of the parietal and temporal cortex involved in vestibular function, we tried to identify cortical areas in humans responsible for vestibular function in the roll plane. Infarcted areas, as demonstrated in magnetic resonance and computed tomography scans, were projected onto the appropriate sectio

Brain Activation and Functional Connectivity During Performance of Lumbopelvic Motor Tasks in Individuals With and Without Low Back Pain: A Functional Magnetic Resonance Imaging Study.

Mounting evidence suggests that individuals with chronic low back pain exhibit changes in brain activity. However, changes in brain activity during the performance of salient motor tasks have not been fully described. Therefore, the purpose of this study was to investigate the differences in cortical activation and functional connectivity between individuals with and without chronic low back pain while performing condition-specific motor tasks. Twenty-three individuals with chronic low back pain and 19 asymptomatic individuals participated in this study. Whole brain activity and functional con

Operculo-Insular Epilepsy: Scalp and Intracranial Electroencephalographic Findings.

PURPOSE: Operculo-insular seizures are heterogeneous and may resemble seizures originating from the temporal, frontal, or parietal lobe. Although surface and invasive EEG recordings are often necessary to detect operculo-insular seizures, electrophysiological features of operculo-insular epilepsies remain poorly characterized. This study describes the EEG findings of patients with operculo-insular epilepsy. METHODS: We reviewed electrophysiological data of all patients (n = 9) with operculo-insular seizures revealed by intracranial EEG and for whom operculo-insular epilepsy was confirmed by g

Imaging of odor perception delineates functional disintegration of the limbic circuits in mesial temporal lobe epilepsy.

Metabolic and neuro-receptor abnormalities within the extrafocal limbic circuits are established in mesial temporal lobe epilepsy (MTLE). However, very little is known about how these circuits process external stimuli. We tested whether odor activation can help delineate limbic functional disintegration in MTLE, and measured cerebral blood flow with PET during birhinal smelling of familiar and unfamiliar odors, using smelling of odorless air as the baseline condition. Patients with MTLE (13 left-sided, 10 right-sided) and 21 controls were investigated. In addition to odor activation, the analy

Association of blood pressure with brain structure in youth with and without bipolar disorder.

BACKGROUND: We previously found that blood pressure (BP) is elevated, and associated with poorer neurocognition, in youth with bipolar disorder (BD). While higher BP is associated with smaller brain structure in adults, studies have not examined this topic in BD or youth. METHODS: Participants were 154 youth, ages 13-20 (n = 81 BD, n = 73 HC). Structural magnetic resonance imaging and diastolic (DBP), and systolic (SBP) pressure were obtained. Region of interest (ROI; anterior cingulate cortex [ACC], insular cortex, hippocampus) and vertex-wise analyses controlling for age, sex, body-mass-ind

The Central Sulcus of the Insula: A Highly Reliable Radiographic Landmark for Identification of the Rolandic Sulcus.

BACKGROUND: Anatomic studies have suggested that the central insular sulcus (CIS) runs in line with the Rolandic sulcus (RS). The radiographic relationship between the RS and CIS has not been systematically studied. This study aims to evaluate the applicability of using the CIS as a radiologic landmark to identify the RS. METHODS: We retrospectively reviewed 100 consecutive normal magnetic resonance imaging (MRI) scans (200 hemispheres) performed at a single institution. MRI scans with any intracranial pathology or finding were excluded. Sagittal and axial fluid-attenuated inversion recovery

Cortico-prefrontal afferents in the guinea pig.

The prefrontal cortex of the guinea pig was investigated in a search for afferents from cortical areas outside the frontal lobe using the horseradish peroxidase retrograde tracing technique. Based on afferents from the medial, the lateral, and from both parts of the thalamic mediodorsal nucleus, three prefrontal subregions were defined, namely the sulcal (within and above the rhinal sulcus), the medial (anterior and superior to the genu of the corpus callosum), and the fronto-polar subfield. Only the fronto-polar region was found to receive afferents from insular, cingulate, retrosplenial, par

Sensing the Self: The Role of the Insula and Interoception in Body Image.

This chapter explores the insula's role in shaping body image through interoception - the neural process of sensing, interpreting, and integrating internal bodily signals to facilitate a coherent sense of self. Drawing on A.D. (Bud) Craig's hierarchical model, which emphasizes the insula's integration of sensory input into higher-order self-representations, we examine how disruptions in interoceptive processing may contribute to body image disturbance. This framework is applied to mental health conditions such as anorexia nervosa, body dysmorphic disorder, as well as physical health conditions

A case report of agoraphobia following right parietal lobe surgery: changes in functional and structural connectivities of the multimodal vestibular network.

Agoraphobia is a visuo-vestibular-spatial disorder that may involve dysfunction of the vestibular network, which includes the insular and limbic cortex. We sought to study the neural correlates of this disorder in an individual who developed agoraphobia after surgical removal of a high-grade glioma located in the right parietal lobe, by assessing pre- and post-surgery connectivities in the vestibular network. The patient underwent surgical resection of the glioma located within the right supramarginal gyrus. The resection interested also portions of the superior and inferior parietal lobe. Str

Effective connectivity analysis of iEEG and accurate localization of the epileptogenic focus at the onset of operculo-insular seizures.

Recognition of insular epilepsy may sometimes be challenging due to the rapid speed at which insular seizures can spread throughout the cortex via extensive connections to surrounding cortices. The spectrum weighted adaptive directed transfer function, a multivariate causality-based effective connectivity measure, was applied to intracranial electroencephalography recordings to identify generators of seizure activity. A non-parametric test based on surrogate data testing was used to validate statistical significance of causal relations. Outflow and inflow of seizure activity were extracted fro

Insulotaxy: Navigating the Human Insula with a Novel Stereotactic Framework.

INTRODUCTION: The insula is an increasingly important target for functional neurosurgery given its involvement in a range of neurological and neuropsychiatric disorders, including epilepsy and chronic pain. As this practice evolves, optimal targeting will require standardized outcome measures that relate electrode or laser trajectory to postprocedural outcome. Traditional whole- brain registration approaches fail to capture the substantial person-to-person variability in insular gyral configuration, including the relative internal rotation of the insular gyri with respect to standard stereotac

Blunted perception of breathlessness in three cases of low grade insular-glioma.

Better understanding of breathlessness perception addresses an unmet clinical need for more effective treatments for intractable dyspnoea, a prevalent symptom of multiple medical conditions. The insular-cortex is predominantly activated in brain-imaging studies of dyspnoea, but its precise role remains unclear. We measured experimentally-induced hypercapnic air-hunger in three insular-glioma patients before and after surgical resection. Tests involved one-minute increments in inspired CO2, raising end-tidal PCO2 to 7.5 mmHg above baseline (38.5 ± 5.7 mmHg), whilst ventilation was constrained (

FDG-PET in Cotard syndrome before and after treatment: can functional brain imaging support a two-factor hypothesis of nihilistic delusions?

Introduction: Cotard syndrome is a neuropsychiatric entity recognised by the presence of nihilistic delusions. Although different models have been proposed for the development of monothematic delusions, including Cotard syndrome, functional neuroanatomical models are lacking. Methods: A case report of a 19-year old male with autoimmune encephalitis and Cotard syndrome, in whom Positron Emission Tomography (18F-FDG-PET) scans were performed before and after successful treatment with electroconvulsive therapy (ECT), is presented. Literature review on brain imaging is provided to discuss a functio

Alterations to metabolic hormones in amyotrophic lateral sclerosis and frontotemporal dementia postmortem human tissue.

Metabolic changes are observed in patients with both amyotrophic lateral sclerosis (ALS) and frontotemporal dementia (FTD). Although regulation of metabolic processes in the CNS is predominantly carried out within the hypothalamus, extra-hypothalamic CNS areas contain metabolic hormone receptors, including those for leptin (LEPR), insulin (INSR), and neuropeptide Y (NPY), indicating that they may play a role in biological processes underlying pathogenic disease processes. The status of these hormones within regions vulnerable in ALS/FTD is not well described. This study sought to determine whe

Gray and white matter reduction in hyposmic subjects--A voxel-based morphometry study.

The absence of olfactory input causes structural brain remodelling in humans. Mainly, the olfactory bulb and cortical olfactory areas are involved in this process. The aim of our study was to investigate volume changes of the gray and white matter in a group of subjects with an impaired but not complete loss of olfaction (hyposmia). Magnetic resonance images of hyposmic subjects and an age- and sex-matched control group were acquired on a 3T scanner. Voxel-based morphometry (VBM) was performed using VBM8 toolbox and SPM8 in a Matlab environment. The analysis revealed significant gray matter vo

Estimation of the Composition of the Resting State fMRI Networks in Subjects with Mild Depression and Healthy Volunteers.

Depressive disorders can be associated with changes in not only interaction between neural networks, but also in their composition. Resting state fMRI scanning was performed for 4 min twice for each subject and the results of patients with mild depression (N=15) and healthy subjects (N=19) were analyzed. The fMRI signal was reduced into the independent components and the contrasts between the groups and between the first and second records were constructed for each component. During the first scanning, the auditory network of individuals with depression involved greater volume in the left insu

Topographical map of subjective states evoked by focal seizures and electrical stimulation of the human insula.

OBJECTIVE: The goal of this study was to investigate how the topographical map of human subjective experiences induced by intracranial electrical stimulation (iES) compares to the map of subjective auras experienced by patients during seizures involving the same cortical areas (here, the insular cortex). METHODS: We recruited 14 patients with insular epilepsies confirmed with intracranial electroencephalography in the United States (N = 7) and France (N = 7). We identified insular regions involved early in seizures (i.e., presumed seizure-onset zones [SOZs]), and documented the auras reported

Suanzaoren decoction and estazolam improve insomnia symptoms by altering insular subregional functional connectivity in chronic insomnia disorder.

Chronic insomnia disorder (CID) is characterized by dysregulation of the insula, a key hub integrating emotional, cognitive, and autonomic processes. This study investigated insular subregional functional connectivity (FC) abnormalities in CID, longitudinal FC modulation following treatment with Suanzaoren Decoction (SZRD) or estazolam, and the associated molecular signatures. We recruited 82 patients with CID and 80 healthy controls. Patients underwent clinical assessments and resting-state functional magnetic resonance imaging at baseline and after six weeks of treatment. Twelve insular subr

Evaluating the rare cases of cortical vertigo using disconnectome mapping.

In rare cases, cortical infarcts lead to vertigo. We evaluated structural and functional disconnection in patients with acute vertigo due to unilateral ischemic cortical infarcts compared to infarcts without vertigo in a similar location with a focus on the connectivity of the vestibular cortex, i.e., the parieto-opercular (retro-)insular cortex (PIVC). Using lesion maps from the ten published case reports, we computed lesion-functional connectivity networks in a set of healthy individuals from the human connectome project. The probability of lesion disconnection was evaluated by white matter
