Insurance status as a mediator of clinical presentation, type of intervention, and short-term outcomes for patients with metastatic spine disease.

BACKGROUND: It is well established that insurance status is a mediator of disease management, treatment course, and clinical outcomes in cancer patients. Our study assessed differences in clinical presentation, treatment course, mortality rates, and in-hospital complications for patients admitted to the hospital with late-stage cancer - specifically, metastatic spine disease (MSD), by insurance status. METHODS: The United States National Inpatient Sample (NIS) database (2012-2014) was queried to identify patients with visceral metastases, metastatic spinal cord compression (MSCC) or pathologi

Stimulants associated with reduced risk of hospitalization for motor vehicle accident injury in patients with obstructive sleep apnea-a nationwide cohort study.

BACKGROUND: The risk of injury directly related to hospitalization for motor vehicle accidents (MVAs) in the obstructive sleep apnea (OSA) patients has not been thoroughly understood. Our study aimed to examine the association between the OSA and the hospitalization for an MVA injury. METHODS: This retrospective cohort study used Taiwan's National Health Insurance Research Database (NHIRD) between 2000 and 2015. The OSA patients aged ≥20 years by age, sex, and index-year matched by non-OSA controls were enrolled (1:3). We used the Cox proportional regression model to evaluate the association

Preoperative resource utilization prior to minimally invasive repair of pectus excavatum.

BACKGROUND: Preoperative testing to assess the physiologic impact of pectus excavatum is sometimes ordered to meet third-party payor preauthorization requirements. This study describes the utility of physiologic testing prior to minimally invasive repair of pectus excavatum (MIRPE). METHODS: We retrospectively reviewed patients that underwent MIRPE from 1/2012-7/2016 at two academic children's hospitals. Data collected included demographics, insurance, Haller Index (HI), pulmonary function tests (PFTs) and echocardiograms (ECHO) obtained, and preauthorization denials. RESULTS: A total of 360

Impact of insurance and implant coverage on arthroscopic shoulder surgery patients: a prospective multicenter analysis.

BACKGROUND: Advances in implant technology for arthroscopic shoulder surgery allow patients to achieve similar success rates as traditional approaches with reduced morbidity and quicker recovery. However, in the U. S. health care system, insurance coverage for these implants remains variable. Unlike commercial carriers, patients with governmental insurance typically do not have coverage for arthroscopic shoulder implants. How this disparity impacts the health care of patients with shoulder pathology remains unclear. METHODS: We performed a prospective multicenter study analyzing the effects o

Provider-Sponsored Health Plans Negotiated Lower Prices For Health Services Compared With Traditional Insurers, 2024.

Although hospitals and insurers have traditionally operated as separate organizations, an increasing number of health systems have begun offering vertically integrated insurance plans, known as provider-sponsored health plans. We examined differences in negotiated prices for forty-six mandatory reported health care services between provider-sponsored health plans and nonintegrated insurance plans, using hospital price transparency data. Of the 639 health systems in the United States in 2023, 179 (28.0 percent) offered an insurance plan, most often Medicare Advantage plans. Across health care s

Burden of Racial Disparities in Pediatric Traumatic Brain Injury Among American Indian/Alaska Native Children: A Scoping Review of the Literature.

BACKGROUND: Traumatic brain injury (TBI) is a leading cause of morbidity and mortality in children worldwide. In the United States, and in the state of Alaska in particular, American Indian/Alaska Native (AI/AN) children experience a disproportionate burden of healthcare disparities. Understanding the epidemiology, mechanisms of injury, access to care, and long-term clinical outcomes in this vulnerable population is crucial for guiding management strategies and prevention initiatives. OBJECTIVES: This scoping review aimed to characterize the epidemiology of pediatric TBI among AI/AN children

Alternative sources of injury account recovery.

Obtaining full, prompt payment for healthcare services rendered is critical for physicians and healthcare organizations. A little-known type of insurance--medical payments insurance--can help providers obtain payment for services provided to injured patients. Medical payments insurance riders are attached to some automobile insurance policies and to virtually all residential and commercial property insurance policies. In some instances, medical payments insurance can be collected directly from an injured patient's insurer, and determining fault is not an issue.

The impact of comorbidities, regional trends, and hospital factors on discharge dispositions and hospital costs after acoustic neuroma microsurgery: a United States nationwide inpatient data sample study (2005-2009).

OBJECT: Hospitalization cost and patient outcome after acoustic neuroma surgery depend on several factors. There is a paucity of data regarding the relationship between demographic features such as age, sex, race, insurance status, and patient outcome. Apart from demographic factors, there are several hospital-related factors and regional issues that can affect outcomes and hospital costs. To the authors' knowledge, no study has investigated the issue of regional disparity across the country in terms of cost of hospitalization and discharge disposition. METHODS: The authors analyzed the Natio

A detailed audit of reimbursement for abdominal CT in an academic practice.

RATIONALE AND OBJECTIVES: Declining fee schedules, decreasing operating margins, and increasingly stringent compliance regulations create a need for intense scrutiny and optimization of a radiology organization's billing and collection procedures. The authors' goal was to analyze the effectiveness of departmental professional billing procedures, identify controllable factors, and intervene when they could be improved. MATERIALS AND METHODS: A detailed audit of professional claims and payments was performed for all patients who underwent abdominal-pelvic computed tomography (CT) during July 19

When inequity impacts clinical care: an analysis of length of stay and reimbursement rates for medical stabilization for anorexia nervosa based on insurance coverage.

BACKGROUND: The recent COVID-19 pandemic has had a significant effect on the rates and severity of anorexia nervosa (AN) leading to rises in admissions for medical stabilization. To improve care and effectively allocate limited resources, we need a better understanding of the factors that lead to prolonged hospitalization in patients with AN. METHODS: We performed a retrospective chart review of 139 adolescent and young adult patients admitted a combined 196 times for medical instability due to AN at a large pediatric hospital in the Midwest region of the United States. We used multiple linear

Group-Based Medical Mistrust and Healthcare Utilization Among Racially and Ethnically Minoritized Adults.

BACKGROUND: Group-based medical mistrust is a critical yet understudied factor associated with healthcare utilization among Black, African American, Hispanic/Latino, Asian/AAPI, or Indigenous working adults. How mistrust relates to primary care (GP/PCP), urgent care/emergency room (ER), and hospital-based care, and whether these associations vary by health perceptions or insurance coverage, remains unclear. OBJECTIVES: This study examined whether higher group-based medical mistrust was associated with lower primary care use, greater urgent care/ER use, and hospitalizations, and whether health

Reduction in Hospitalizations for Esophageal Reflux in a Decade with Minimal Increases in Other Functional and Motor Disorders.

BACKGROUND: Functional and motility disorders (FMDs) are common conditions that cause significant morbidity and economic loss. A comprehensive analysis of these disorders and their impact has not been done in an inpatient setting. AIMS: We seek to evaluate adult hospitalization trends for FMDs in the USA. METHODS: The National Inpatient Sample between 2005 and 2014 was analyzed. Poisson regression was used to assess hospitalization trends for FMDs referenced to non-FMD hospitalizations. Linear regression was used to assess cost per hospitalization and length of stay (LOS). All models were ad

Impacts of Electroconvulsive Therapy on 1-Year Outcomes in Patients With Schizophrenia: A Controlled, Population-Based Mirror-Image Study.

OBJECTIVES: Despite the decline in the use of electroconvulsive therapy (ECT) in patients with schizophrenia, ECT augmentation is still recommended for those with poor response to standard pharmacological intervention. However, the effectiveness of augmentation of antipsychotics with ECT on long-term clinical outcomes needs to be verified in an expanded sample. METHODS: Patients who were hospitalized for schizophrenia and received ECT for the first time during that hospitalization were identified from the total population health insurance database in Taiwan between 2002 and 2011. Utilization and unexpected hospitalization rates of a pediatric emergency department 23-hour observation unit.

OBJECTIVES: The 23-hour observation units (OUs) may be used to avoid unnecessary hospital admissions. However, unexpected hospitalizations from the 23-hour OUs involve transfer of care and may decrease the efficiency and safety of care of the patient and the unit itself. The primary objective of this study was to determine the predictors of unexpected hospitalization for admissions to a pediatric 23-hour OU. METHODS: This is an observational prospective cohort study of patients admitted to a pediatric 23-hour OU. Bivariate and multivariate regression analyses identify factors associated with
