Addressing Disparities in Pediatric Congenital Heart Disease: A Call for Equitable Health Care.

While significant progress has been made in reducing disparities within the US health care system, notable gaps remain. Patient-Reported Outcomes Among Transition-Age Young Adults With Juvenile Idiopathic Arthritis in the Childhood Arthritis and Rheumatology Research Alliance Registry.

OBJECTIVE: To evaluate patient-reported care utilization and outcomes among young adults with juvenile idiopathic arthritis (JIA), including factors associated with complete transfer to adult rheumatology. METHODS: We included young adults with JIA enrolled in the Childhood Arthritis and Rheumatology Research Alliance (CARRA) Registry from 2015 to 2019 with age ≥ 18 years at their last clinical site visit. We used data from the CARRA Registry Long-term Follow-up program, which follows inactive CARRA Registry patients and collects patient-reported information through phone surveys. We compared

Barriers and promoters to adapting research findings to clinical care in hereditary angioedema in the United States: A qualitative study.

Treatment options for hereditary angioedema (HAE) have evolved due to discoveries in basic and clinical research. HAE clinical guidelines emphasize optimizing quality of life through attack prevention strategies that include long-term prophylaxis. We sought to identify barriers to and promoters of research translation into HAE clinical practice that may inform efforts to improve patient care. We interviewed US allergy/immunology clinicians who completed an online continuing medical education activity on HAE or were identified through healthcare provider directories. Interviews focused on clini

Factors affecting the amount of long-term restorative dental treatment provided to 100 patients by 20 dentists in 3 Adelaide private practices.

Apart from the effects of the frequency of patient attendance for dental examination, and of a change of dentist, little is known of those factors which may influence the amount of treatment provided in dental practice. The present study investigated during 1992 the effects of seven factors on the amount of treatment provided for 100 adult patients treated by 20 male dentists over many years in three selected Adelaide private practices. The amounts of particular types of restorative treatments provided were significantly related to patient age, gender and frequency of attendance, and to practi

Maintaining A Long-Term Practice of Daily Pelvic Floor Muscle Exercises: What Do Childbearing Women Think?

INTRODUCTION: To be effective, pelvic floor muscle therapy (PFMT) exercises should be intense, be practiced consistently, and include clinical support. Many women do not adhere to a consistent PFMT program, compromising the resolution or prevention of urinary incontinence (UI). This study aimed to answer 2 key questions: What prevents women from performing PFMT long term, and what can health care providers do to support women to perform them? METHODS: Postpartum women from 4 sites in the United States completed a questionnaire about experiences with PFMT or Kegel exercises during and after pr

Less Invasive Surfactant Administration: A Viewpoint.

The standard of care in treating respiratory distress syndrome in preterm infants is respiratory support with nasal continuous positive airway pressure or a combination of continuous positive airway pressure and exogenous surfactant replacement. Endotracheal intubation, the conventional method for surfactant administration, is an invasive procedure associated with procedural and mechanical ventilation complications. The INSURE (intubation, surfactant administration, and extubation soon after) technique is an accepted method aimed at reducing the short-term complications and long-term morbiditi

[Toward establishment of an economical basis for neurological care in Japan: appropriate assessment of techniques for diagnosis and treatments].

Appropriate assessments for medical techniques of clinical diagnostic examination have been one of long-standing issues in social insurance system in Japan. Assessments of those in the surgical fields in which the most of assessments are evaluated by surgical operations including related materials as well as surgeons' techniques, is rather easy to evaluate quantitatively. Comparing with this, medical fields including neurology, quantitative assessments of medical techniques either diagnostic or treatment is not so easy. In order to evaluate the burden in each technique in the medical fields, s

Occupational and Non-occupational Injuries Can Result in Prolonged Augmentation of Psychiatric Disorders.

BACKGROUND: The long-term effects of occupational injury (OI) on psychiatric diseases are unclear. This study assessed and compared the effects of OI, no injury (control), and non-OI (NOI) on the development of psychiatric diseases. METHODS: We used Taiwan's National Health Insurance Research Database to investigate the incidence of psychiatric disorders in OI, NOI, and control groups. The subjects were aged 20-50 years, actively employed in 2000, and did not have history of injury or psychiatric disorders. All subjects were followed from 2000 and were classified into OI, NOI, and control gro

Asphyxia due to substandard obstetric care in Finland: An 11-year patient insurance registry study of compensated patient claims.

BACKGROUND: Asphyxia is a major cause of neonatal mortality, often linked to inadequate perinatal care, especially misinterpretation of cardiotocography. In Finland, the Patient Insurance Centre manages medical claims, offering a no-fault compensation model. Utilizing these claims could enhance obstetric safety and newborn health. AIM: The aim of this study was to analyze the compensated patient claims and identify the substandard care leading to asphyxia. METHODS: A nationwide retrospective registry-based study was conducted. Data included all compensated patient insurance claims for neonat

Unequal Gains? A Literature Review on the Affordable Care Act's Effects on Healthcare Utilization Across Racial and Ethnic Groups.

The Affordable Care Act (ACA), implemented in 2010, aimed to expand healthcare access, reduce costs, and address long-standing disparities in the U.S. healthcare system, particularly among racial and ethnic minorities. This paper reviews the ACA's impact on healthcare utilization for these populations, with a focus on insurance coverage, preventive services, and health outcomes. While Medicaid expansion significantly reduced uninsured rates and increased access to care in states that adopted the expansion, millions of low-income individuals, many of whom are racial and ethnic minorities, remai

BACKGROUND: Adults with traumatic brain injuries (TBI) frequently experience cognitive, emotional, physical and communication deficits that require long-term rehabilitation and community support. Although access to rehabilitation services is linked to positive outcomes, there can be barriers to accessing community rehabilitation related to system navigation, referral processes, funding, resource allocation and communications required to ensure access. Changes in social participation between ages 65 and 70 and incident dementia: a Bayesian competing risk analysis.

BACKGROUND: Social participation may protect against dementia, but few studies have examined whether cessation of social participation during the period surrounding retirement is associated with subsequent dementia risk accounting for the competing risk of death. This study examined whether changes in social participation between ages 65 and 70 are associated with incident dementia in community-dwelling Japanese older adults. METHODS: This prospective cohort study analysed 1,681 participants of the NISSIN Project (Nisshin City, Aichi Prefecture, Japan) enrolled at age 65 (1996-2005), re-exami
