Assessing the Use of Medical Insurance Claims and Electronic Health Records to Measure COVID-19 Vaccination During Pregnancy.

INTRODUCTION: The safety and effectiveness of COVID-19 vaccination during pregnancy are commonly assessed using administrative health records, which may misclassify vaccine exposures. OBJECTIVE: The aim of this study was to compare the capture of COVID-19 vaccines in commercial insurance claims records and electronic health records (EHR) before and during pregnancy in a cohort in the United States and demonstrate the implications of observed misclassification on vaccine safety and effectiveness estimates. METHODS: Analysis of de-identified data from the Optum Labs Data Warehouse (OLDW) inclu

Epidemiological disease burden and annual, nationwide health insurance treatment cost of female infertility based on real-world health insurance claims data in Hungary.

BACKGROUND: Infertility affects more than 50 million couples worldwide, resulting in a significant burden on individuals and society. Its prevalence ranges from 8-12% among developed countries. The growing number of patients poses an increasing challenge to the healthcare system and its funding. Our quantitative, descriptive, and cross-sectional study aimed to analyze the prevalence and annual nationwide health insurance treatment cost of female infertility in Hungary in 2019. METHODS: We used claims data obtained from the Hungarian National Health Insurance Fund Administration (NHFIA). The n

Optimizing identification of Lyme disease diagnoses in commercial insurance claims data, United States, 2016-2019.

BACKGROUND: Commercial insurance claims data are a stable and consistent source of information on Lyme disease diagnoses in the United States and can contribute to our understanding of overall disease burden and the tracking of epidemiological trends. Algorithms consisting of diagnosis codes and antimicrobial treatment information have been used to identify Lyme disease diagnoses in claims data, but there might be opportunity to improve their accuracy. METHODS: We developed three modified versions of our existing claims-based Lyme disease algorithm; each reflected refined criteria regarding a

Epidemiology of overdose episodes from the period prior to hospitalization for drug poisoning until discharge in Japan: An exploratory descriptive study using a nationwide claims database.

BACKGROUND: Little is known about the nationwide epidemiology of the annual rate, causative substance, and clinical course of overdose-related admission. We aimed to describe the epidemiology of overdose episodes from the period prior to hospitalization for drug poisoning until discharge to home. METHODS: We assessed all cases of admission due to overdose (21,663 episodes) in Japan from October 2012 through September 2013 using the National Database of Health Insurance Claims and Specific Health Checkups of Japan. RESULTS: The annual rate of overdose admission was 17.0 per 100,000 population

Clinical and healthcare burden of myotonic dystrophy type 1 (DM1) in the United States: a claims-based study.

OBJECTIVES: Myotonic dystrophy type 1 (DM1) is a multi-systemic disease affecting skeletal and smooth muscle, the eye, brain, and heart. No disease-modifying therapies are available. Prior studies on clinical care and economic burden combined DM1 and DM type 2. This study describes the US DM1 population aged ≥12 years, assessed organ system involvement over time, estimated healthcare resource utilization (HCRU), associated costs, and identified predictors of high-cost status. METHODS: Linked open claims and EHR data (Clarivate) from 1 January 2015 to 25 August 2023 were analyzed. Eligible ind

All-cause healthcare cost differences across ambulatory cardiac monitoring modalities in Medicare beneficiaries: a real-world claims-based study using entropy balancing.

OBJECTIVES: Arrhythmias, including atrial fibrillation and ventricular tachycardia, are associated with substantial morbidity and healthcare utilization in older adults. Ambulatory cardiac monitoring (ACM) is used to evaluate suspected or known arrhythmias; however, monitoring modalities vary in duration, diagnostic yield, and cost. We evaluated differences in total cost of care across major ACM modalities using real-world Medicare claims data. METHODS: We conducted a retrospective cohort analysis using the Merative MarketScan Medicare Claims Database to estimate per patient per month (PPPM)

Healthcare claims and health interview survey data for chronic disease surveillance: agreement and comparative validity of prevalence indicators for 20 chronic conditions in a general population sample in France.

Healthcare claims data are increasingly used to derive chronic condition (CC) surveillance indicators, although comparative evidence with self-reported data remains scarce. We explored the agreement and comparative validity (concurrent and predictive) of 20 CC prevalence indicators independently constructed using the French National Health Data System (SNDS) and Health, Health Care, and Insurance Survey (ESPS 2010-2014). Individual data from 5039 ESPS participants aged ≥25 years, representative of the 2010 French general population, were linked to the SNDS. Follow-up data included a 2014 healt

Risk of Hypertension After Renal Trauma: An Analysis of a Large Insurance Claims Database.

OBJECTIVE: To evaluate the risk of new-onset hypertension (HTN) after renal trauma (rT) using a large insurance claims database. METHODS: We conducted a retrospective analysis of Merative MarketScan records from 2011-2017. Patients with rT were compared to controls with isolated bladder or urethral trauma. Patients were excluded if they had both trauma types, pre-existing HTN, <1 year of pre-trauma enrollment data, or age <18 years. Kaplan-Meier curves and multivariate Cox proportional hazards models assessed HTN risk between groups. RESULTS: A total of 7882 patients (4703 rT) were included.

Validation of a Claims-Based Algorithm for Specialist Palliative Care Delivery in Metastatic Cancer.

CONTEXT: The lack of valid methods to identify specialist palliative care (PC) delivery in population-level data impedes comprehensive understanding of its use. OBJECTIVE: To develop and validate a claims-based algorithm to identify receipt of specialist PC in Medicare beneficiaries with metastatic cancer. METHODS: We developed a claims-based algorithm to identify specialist PC, using a physician billing claim from a known PC clinician as the gold standard, retaining candidate variables with a positive predictive value (PPV) >60%. We evaluated algorithm performance and conducted simulation t

Increased risk of adverse events in non-cancer patients with chronic and high-dose opioid use-A health insurance claims analysis.

BACKGROUND: Chronic and high dose opioid use may result in adverse events. We analyzed the risk associated with chronic and high dose opioid prescription in a Swiss population. METHODS: Using insurance claims data covering one-sixth of the Swiss population, we analyzed recurrent opioid prescriptions (≥2 opioid claims with at least 1 strong opioid claim) between 2006 and 2014. We calculated the cumulative dose in milligrams morphine equivalents (MED) and treatment duration. Excluded were single opioid claims, opioid use that was cancer treatment related, and opioid use in substitution programs

Diagnosis and medical care for congenital cytomegalovirus infection: An observational study using claims data in Japan, 2010 to 2017.

Although early detection and intervention may improve the outcome of the congenital cytomegalovirus (cCMV) infection, few studies assessed the real-world clinical practice for cCMV patients. We analyzed medical claims data to assess the patterns of diagnoses and medical care for cCMV patients. We used a subset of medical claims database (JMDC Claims Database) in Japan, covering 207,547 newborns between April 2010 and March 2017 and observed for at least 6 months. The diagnosis of cCMV and related symptoms and sequelae and medical care, including essential examinations and antiviral treatment, w

A Contemporary Medicolegal Analysis of Injury Related to Peripheral Nerve Blocks.

BACKGROUND: Closed malpractice claims can provide insight into low-frequency adverse events in many areas of perioperative and chronic pain care. Over the last decade, there have been changes in surgical and regional anesthetic practice, likely impacting adverse event patterns. Given the wide variability and low frequency of complications associated with peripheral nerve blocks, the study of closed malpractice claims offers an opportunity to examine adverse events, and the patient, technical, and provider factors that led to the claim. Knowledge gained from examination of closed claims has alr

Do falls and falls-injuries in hospital indicate negligent care -- and how big is the risk? A retrospective analysis of the NHS Litigation Authority Database of clinical negligence claims, resulting from falls in hospitals in England 1995 to 2006.

BACKGROUND: Accidental falls are very common in older hospital patients -- accounting for 32% of reported adult patient safety incidents in UK National Health Service (NHS) hospitals and occurring with similar frequency in settings internationally. In countries where the population is ageing, and care is provided in inpatient settings, falls prevention is therefore a significant and growing risk-management issue. Falls may lead to a variety of harms and costs, are cited in formal complaints and can lead to claims of clinical negligence. The NHS Litigation Authority (NHSLA) negligence claims da

The Fake Bad Scale and MMPI-2 F-family in detection of implausible psychological trauma claims.

We tested the validity of the Lees-Haley Fake Bad Scale (FBS) and the family of MMPI-2 F scales (F-family; F, F(p), and F-K scales) in predicting improbable psychological trauma claims in an applied setting. Litigants reporting implausible symptoms long after minor scares and nonlitigants clinically referred following severe stressors completed the MMPI-2. Both groups were naturally matched on social class. The FBS demonstrated sensitivity, specificity, and positive predictive power in the detection of atypical problems but the F-family showed poor utility. FBS cutting scores derived from logi
