Critical care medicine has matured greatly as a field in the past decade. Much has been learned concerning the institution of life support therapies to sustain patients with diverse and multiple organ failures, thus providing patients with a window of opportunity to recover from potentially life-ending insults. The management of critically ill patients has increasingly involved creation of a highly controlled environment by care providers, with patients immobilized, tethered to devices, and receiving multiple drugs to facilitate the entire process. Although it has been assumed that such contro

We report the first series of indigenous European patients with severe leptospirosis in need of intensive care because of acute respiratory distress syndrome, coma and shock. The all-cause hospital mortality was 16.7%, and may have been influenced by relatively early diagnosis, indicating the need for heightened clinical suspicion in Europe.

OBJECTIVES: This monocentric, observational and retrospective survey was performed to check the appropriateness between aminoglycoside prescriptions and inhibitor quotient to be reached, in Intensive Care Unit (ICU) patients. We identified variability factors for aminoglycoside plasmatic concentrations at peak such as standardized index of gravity (IGS2 scale), age, sex, weight, and severity of sepsis.
PATIENTS AND METHOD: Eighty-seven ICU patients received an antibiotic combination mandatorily including an aminoglycoside (amikacin or gentamicin) as curative treatment for a severe infection.

Sleep is essential to human life. Sleep patterns are significantly disrupted in patients who are hospitalized, particularly those in the intensive care unit. Sleep deprivation is pervasive in this patient population and impacts health and recovery from illness. Immune system dysfunction, impaired wound healing, and changes in behavior are all observed in patients who are sleep deprived. Various factors including anxiety, fear, and pain are responsible for the sleep deprivation. Noise, light exposure, and frequent awakenings from caregivers also add to these effects. Underlying medical illnesse

OBJECTIVES: Infections at the Intensive Care Units (ICU) are a substantial clinical problem due to their high incidence and significant impact on patient mortality, as well as on the duration of their treatment within ICUs. The objective of the present study was to analyse the infections occurring at our department and to identify those micro-organisms responsible for infections, with consideration of their susceptibility to antibiotic treatment. Moreover, an evaluation was performed in respect of correlation between infection incidence, patient mortality and duration of stay at the department

Transesophageal echocardiography provides unique diagnostic capabilities, allowing for a very precise look at the structure and hemodynamics of the human heart. It is minimally invasive and portable, and quickly diagnoses sudden hemodynamic changes in intensive care patients. It provides invaluable and precise information about myocardial dysfunction and intracardiac volume status. It can diagnose dynamic left ventricular outflow obstruction, infrequent but serious complication of aortic valve replacement, septal myectomy, or mitral valve repair. Transesophageal echocardiography examination ca

Neuromuscular disorders encountered in the ICU can be categorized as muscular diseases that lead to ICU admission and those that are acquired in the ICU. This article discusses three neuromuscular disorders can lead to ICU admission and have a putative immune-mediated pathogenesis: the Guillian-Barré syndrome, myasthenia gravis, and dermatomyositis/polymyositis. It also reviews critical care polyneuropathy and ICU acquired myopathy, two disorders that, alone or in combination, are responsible for nearly all cases of severe ICU acquired muscle weakness.

Many pressures play a role in escalating the emergence of antibiotic-resistant infections in intensive care units (ICUs), oncology and bone marrow transplant wards, and dialysis units. One is horizontal cross-transmission of antibiotic-resistant infections; another is the frequent use of broad-spectrum antibiotics. Among the serious infections acquired by patients in ICUs are bloodstream infections and nosocomial pneumonia. This article discusses potential multidisciplinary interventions to prevent bacterial resistance. Among these wide-ranging strategies are adequate decontamination between p

Continuous renal replacement therapy (CRRT) was first described in 1977 for the treatment of diuretic-unresponsive fluid overload in the intensive care unit (ICU). Since that time this treatment has undergone a remarkable technical and conceptual evolution. It is now available in most tertiary ICUs around the world and has almost completely replaced intermittent haemodialysis (IHD) in some countries. Specially made machines are now available, and venovenous therapies that use blood pumps have replaced simpler techniques. Although, it remains controversial whether CRRT decreases mortality when

Ultrasonography plays an increasingly important role in the evaluation of the vascular system. It is especially useful for the intensive care patient because of its ability to be performed at the patient's bedside, as well as the relatively high frequency of vascular complications in the ICU. Ultrasonography is the method of choice for evaluation of deep vein thromboembolic disease of the lower extremity in all patients, demonstrating excellent sensitivity and specificity for this condition. It also should be the initial method of evaluating upper extremity deep vein thrombosis. It may be limi

The development of acute cholecystitis in the ICU is now a well-recognized complication of many acute illnesses that precipitate ICU admission and may also result as a complication of the subsequent treatment. The etiology of the disease remains obscure and, unlike acute cholecystitis outside the ICU setting, most cases are acalculous and not associated with gallstones. The disease may often go unrecognized due to the complexity of the patient's medical and surgical problems. Clinical examination is often unhelpful, as many patients are receiving mechanical ventilation and have decreased menta

There were 70 admissions from a regional paediatric oncology centre to the intensive care unit over a six and a half year period. Patients were divided into those with systemic infections (n = 19), respiratory infections (n = 15), metabolic effects (n = 9), tumour mass effects (n = 10), neurological complications (n = 8), and others (n = 9). The overall survival was 51%. Patients admitted with metabolic or tumour mass related effects had the best prognosis with a survival of 84%. If dialysis is required in this group of patients then continuous arteriovenous haemofiltration is recommended. Pat

The cardiopulmonary support system is an extracorporeal device that allows for rapid cardiopulmonary support of the critically ill patient in the intensive care unit. It provides immediate and complete support of cardiac and pulmonary functions to maintain perfusion to vital organs in patients who are severely physiologically compromised (eg, in cardiogenic shock, adult respiratory distress syndrome or pulmonary edema). Successful cardiopulmonary support requires systemic anticoagulation, percutaneous venous and arterial cannulation and careful monitoring by the critical care team to maintain

A retrospective study of 17 babies admitted to the neonatal intensive care unit of the Royal Maternity Hospital, Belfast, was undertaken to determine the causes and prognosis of conjugated hyperbilirubinaemia (direct fraction greater than 20% of total) over a five year period. Mean gestational age was 29 weeks and mean birth weight was 1,240g with a 2:1 male preponderance. All babies had a complicated clinical course involving prolonged periods of parenteral nutrition and many episodes of sepsis. Liver damage was not found to be a contributory factor to death in any baby who died before the ag

Antimicrobial resistance remains a major clinical problem in the intensive care unit despite the introduction of potent new antibiotics and the application of infection control measures. Impaired antibiotic permeability, antibiotic inactivation and alteration of antibiotic target sites, typically in concert, are mechanisms by which this resistance occurs. Resistance traits may be chromosomally mediated, in which case they may be expressed constitutively or inducibly, or may be mediated by plasmids and/or transposons that may confer resistance to multiple drugs and facilitate spread to other or

From 1986 to 1988 84 patients with tuberculosis were treated in the Intensive Care Unit of the Thorax Clinic Heidelberg (2.8% of total admissions). Only 18% (n = 15) of the cases were referred in the course of conservative treatment while 82% (n = 69) were admitted after thoracic surgery. In 49 of these patients (71%) the diagnosis of tuberculosis could be confirmed only postoperatively by histological examination. This diagnosis should be considered in any ICU patient belonging to the typical groups at risk or showing clinical signs of tuberculosis, e.g. weight loss, chronic productive cough
