Capsular hypesthetic ataxic hemiparesis.

Twenty-three patients with hypesthetic ataxic hemiparesis underwent computed tomography or magnetic resonance imaging. Twenty-two patients had infarcts of lacunar or slightly larger size in the contralateral posterior limb of the internal capsule. In 15 patients the infarct extended superiorly into the adjacent paraventricular region, and in seven it extended into the lateral thalmus. In eight patients the infarct was limited to the posterior limb of the internal capsule, and in only two patients was an ipsilateral to capsular pontine lacune found. Despite a location similar to that of pure mo

Retrosigmoid Craniotomy for Resection of an Epidermoid Cyst of the Posterior Fossa.

This video illustrates the case of a 51-year-old woman who presented with sudden-onset headache, vertigo, and nausea. Imaging revealed an epidermoid cyst of the posterior fossa with mass effect upon the brainstem and displacement of the basilar artery. This lesion was approached using a left-sided keyhole retrosigmoid craniotomy with monitoring of the cranial nerves. This video illustrates the technique of internal debulking of the cyst contents with minimal manipulation of the cyst capsule, which is often densely adherent to the brainstem, cranial nerves, and vessels in the posterior fossa. Whole-Genome Sequencing for Characterization of Capsule Locus and Prediction of Serogroup of Invasive Meningococcal Isolates.

Invasive meningococcal disease is mainly caused by Neisseria meningitidis serogroups A, B, C, X, W, and Y. The serogroup is typically determined by slide agglutination serogrouping (SASG) and real-time PCR (RT-PCR). We describe a whole-genome sequencing (WGS)-based method to characterize the capsule polysaccharide synthesis (cps) locus, classify N. meningitidis serogroups, and identify mechanisms for nongroupability using 453 isolates from a global strain collection. We identified novel genomic organizations within functional cps loci, consisting of insertion sequence (IS) elements in unique p

Simultaneous occurrence of aneurysmal subarachnoid hemorrhage and remote intracerebral hemorrhage--case report.

A 77-year-old female presented with a rare aneurysmal subarachnoid hemorrhage accompanied by a remote hypertensive intracerebral hemorrhage. With a past history of hypertension, she suddenly developed right hemiparesis followed by delayed loss of consciousness. Left carotid angiography demonstrated a left internal carotid-posterior communicating artery aneurysm. The intracerebral hematoma was located in the posterior limb of the internal capsule ipsilateral to the ruptured aneurysm. The aneurysm was clipped with a fenestrated clip 3 weeks after the onset. The rise in blood pressure at the onse

Differential assessment of frontally-mediated behaviors between self- and informant-report in patients with obsessive-compulsive disorder following gamma ventral capsulotomy.

Obsessive-Compulsive Disorder (OCD) is a debilitating disorder causing marked distress and functional impairment. While advances in behavioral and pharmacotherapies have been effective for a majority of patients with OCD, 10-30% remain treatment refractory and severely impaired. For a subset of treatment-resistant individuals with the most severe and disabling (intractable) illness, gamma ventral capsulotomy (GVC) appears effective in reducing OCD symptoms and functional impairment. However, the effects of the ventral internal capsule lesion via GVC surgery on executive function in everyday li

Neuroblastoma in neonates: a case report and literature review.

Neuroblastoma in neonates is a rare solid tumor. This report presents two rare cases of cervical neuroblastoma, both characterized by stridor and coughing during feeding due to tumor compression. The first case of neuroblastoma presented with an intact capsule, allowing for complete surgical resection with preservation of the cervical vasculature and nerves. In contrast, although preoperative MRI indicated an intact capsule in the second case, the intraoperative findings revealed no discernible capsule formation and involvement of both the internal and external carotid arteries. None of the tw

Cochlear-Internal Canal and Cochlear-Facial Dehiscence: A Novel Entity.

Different types of otic capsule dehiscence restricted to the cochlea have been described. Here we describe the case of a patient with a cochlear-internal auditory canal dehiscence associated with a cochlear-facial dehiscence not reported before. A 53-year-old patient with severe to profound sensorineural hearing loss due to bilateral Meniere's disease underwent a cochlear implant surgery on the right ear. Preoperative brain magnetic resonance imaging findings were reported to be normal; during surgery, a cerebrospinal fluid gusher occurred at the time of round window opening. Postoperative com

[The image of prostatic adenoma and cancer in transrectal sonography].

The transrectal ultrasonography of the prostate is introduced. The sonographic appearance of the adenoma and carcinoma in 357 histologically proven cases is evaluated. Shape, symmetry and definition of the capsule are more reliable differential criteria than the internal echogenicity. The sensitivity is 96.6% for adenoma and 84.2% for carcinoma, whereas the sensitivity of palpation is 91.7% and 77.5% respectively. 6 early cancers are detected. Since infiltration of the capsule and the seminal vesicles can be seen, staging of carcinoma is considerably improved. Accurate measurement of the prost

SCMAS: an IMU-based 6-DOF closed-loop permanent magnet actuation system for magnet agent.

Over the past two decades, the method of controlling magnetic capsule robots for targeted drug delivery in gastrointestinal (GI) environments using external magnetic fields has been frequently discussed. In typical targeted drug delivery tasks, capsule robots must not only possess the ability to navigate across large obstacles but also adjust their orientation upon reaching the target site. However, most reported magnetic actuation systems struggle to simultaneously fulfill these two requirements. In this paper, we present a three-degree-of-freedom (3-DOF) semi-open closed-loop magnetic actuat

Biodegradable Defined Shaped Printed Polymer Microcapsules for Drug Delivery.

This work describes the preparation and characterization of printed biodegradable polymer (polylactic acid) capsules made in two different shapes: pyramid and rectangular capsules about 1 and 11 μm in size. Obtained core-shell capsules are described in terms of their morphology, loading efficiency, cargo release profile, cell cytotoxicity, and cell uptake. Both types of capsules showed monodisperse size and shape distribution and were found to provide sufficient stability to encapsulate small water-soluble molecules and to retain them for several days and ability for intracellular delivery. Isolated intracranial fibromuscular dysplasia presents as stroke in a 19-year-old female.

Isolated intracranial fibromuscular dysplasia is rare and may present with cerebrovascular events. It should be considered as etiology of stroke in otherwise healthy young patients. Though diagnosis is often challenging, characteristic morphologies may be revealed on magnetic resonance and catheter angiography. Cephalocervical fibromuscular dysplasia typically involves the extracranial portion of the internal carotid artery (nearly 95%). This rare case demonstrates isolated intracranial fibromuscular dysplasia in a 19-year-old female with left caudate and genu of internal capsule stroke.

Longitudinal tract-based spatial statistics analysis of white matter diffusivity changes and cognitive decline during the transition from MCI to Alzheimer's disease.

Longitudinal studies that analyze the changes in the axial diffusivity (AxD) and radial diffusivity (RD) values over time can elucidate the progression of white matter damage and its causal relationship with cognitive decline. This study aimed to investigate the longitudinal changes in white matter integrity based on AxD and RD and their association with cognitive decline in patients with mild cognitive impairment (MCI) that progressed to Alzheimer's disease (AD). Eighteen participants diagnosed with MCI at baseline and AD at the follow-up examination were selected from the AD Neuroimaging Ini
