Adult Intestinal Malrotation With Congenital Transverse Meso-Colic Internal Hernia: An Infrequent Cause of Small Bowel Obstruction.

Adult intestinal malrotation along with congenital transverse-mesocolic internal hernia causing small bowel obstruction is extremely rare. Most of these patients don't have any obvious clinical symptoms. Only a few cases have been documented in the English literature. We present the unique case of a 43-year-old male without any prior surgical history who presented with nonspecific abdominal pain and was diagnosed with malrotation of the small intestine by computed tomography (CT) scan and underwent exploratory laparotomy found to have internal herniation through the transverse-mesocolon. The p

Ileal Obstruction Secondary to Internal Hernia Under the External Iliac Artery: A Case Report and Literature Review.

Bowel obstruction is one of the most common causes of surgical admission. Most of these patients are managed with non-operative management, often resolving 24 to 48 hours after admission. If conservative management fails in patients with adhesional bowel obstruction, surgery is usually the only other option. Surgery often involves the division of adhesions and resection of the non-viable intestine. Occasionally, unexpected findings require quick but safe interventions, including discussions with other specialities. This report presents a patient who had previously undergone robotic radical cys

Internal Hernia Through a Congenital Defect in Broad Ligament: A Rare and Elusive Cause of Intestinal Obstruction.

Internal hernias through a defect in the broad ligament is an uncommon cause of intestinal obstruction and most often the finding is intra-operative. What makes it rare is when the hernia occurs through a congenital defect in the broad ligament. We present the case of a 48-year-old female patient operated on an emergency basis for intestinal obstruction and intra-operatively the cause was identified to be a herniated ileal loop through a defect in the broad ligament. There was no history of any prior abdominal surgery or any interventions and all of her children were born by normal vaginal del

[A Case of Immediately-Treated Internal Hernia Occurring during Chemotherapy for Liver Metastasis after Gastric GIST Resection].

A 71-year-old man underwent total gastrectomy with Roux-en-Y reconstruction for gastric GIST in October 2017. Liver metastasis was identified in June 2019, and chemotherapy with imatinib was started in July. In December, the patient presented with acute upper abdominal pain and back pain. Abdominal contrast-enhanced CT showed that the jejunum extending from the duodenal stump was dilated. In addition, part of the jejunum had a poor wall contrast effect, with ascites also found surrounding it. We suspected a strangulated ileus and immediately performed emergency surgery. We found an internal he

Strangulated Internal Hernia Beneath the Obturator Nerve After Laparoscopic Radical Cystectomy With Extended Pelvic Lymph Node Dissection.

Internal hernia beneath the vascular structures after pelvic lymphadenectomy is a rare condition. Herein, we report a case of a strangulated internal hernia beneath the obturator nerve 38 months after laparoscopic radical cystectomy with extended pelvic lymphadenectomy. Computed tomography revealed dilated small bowels and a closed loop in the pelvis. The emergency laparotomy was performed, and a strangulated internal hernia beneath the obturator nerve was observed. It is necessary to consider the possibility of internal hernia beneath the vascular structure, including the obturator nerve, aft

Internal hernia from the interureteric space after robot-assisted radical cystectomy and urinary diversion: Two case reports.

RATIONALE: Radical cystectomy and urinary diversion remains the definite management for muscle invasive bladder urothelial cancer. Internal herniation caused by ureteral adhesion is an extremely rare complication after the procedure. To the best of our knowledge, this is the first case report of small bowel obstruction and internal herniation occurring between bilateral ureters and urinary diversion after robot-assisted radical cystectomy (RARC). PATIENT CONCERNS: A 64-year-old woman presented with symptom of small bowel obstruction such as nausea, vomiting, and abdominal fullness after RARC

Use of Bioabsorbable Tissue Reinforcement Reduces Incidence of Internal Hernia in Roux-en-Y Gastric Bypass Patients.

Internal hernias are one of the most devastating late, postsurgical complications associated with laparoscopic Roux-en-Y gastric bypass (LRYGB). The objective of this study was to determine whether placement of a bioabsorbable tissue matrix in soft tissue defects after gastric bypass resulted in a lower incidence of internal hernia development. Prospective database was used to identify all patients who underwent LRYGB between January 2002 and January 2016. These patients were then retrospectively reviewed to determine the development of internal hernia. Before 2009, the retro-Roux defect was l

Favorable internal hernia rate achieved using retrocolic, retrogastric alimentary limb in laparoscopic Roux-en-Y gastric bypass.

BACKGROUND: The present study was performed at a tertiary care university hospital. The present study examined the incidence of internal hernia (IH) in our series of laparoscopic Roux-en-Y gastric bypass (LRYGB) with retrocolic, retrogastric routing of the alimentary limb accompanied by routine secure closure of all mesenteric defects. METHODS: During a 4-year period, 847 patients underwent LRYGB. Our operative technique included retrocolic, retrogastric placement of the alimentary limb. The enteroenterostomy mesenteric defect, mesocolic defect, and Petersen defect were routinely closed in ru

Small-bowel volvulus in late pregnancy due to internal hernia after laparoscopic Roux-en-Y gastric bypass.

Internal hernias are a specific cause of acute abdominal pain and are a well-known complication after laparoscopic Roux-en-Y gastric bypass for morbid obesity. Although internal hernias are a rare cause of intestinal obstruction, they may evolve towards serious complications, such as extensive bowel ischemia and gangrene, with the need for bowel resection and sometimes for a challenging reconstruction of intestinal continuity. The antecolic position of the Roux limb is associated with a decrease in the incidence of small-bowel obstruction and internal hernias. The best prevention of the format

Internal hernia with strangulation through a mesenteric defect after laparoscopy-assisted transverse colectomy: report of a case.

A 58-year-old woman underwent laparoscopy-assisted transverse colectomy for transverse colon cancer. On postoperative day 7, she experienced sudden abdominal pain accompanied by vomiting and fever. Computed tomography showed a small bowel obstruction caused by an internal hernia. Laparotomy revealed an internal hernia through the mesenteric defect at the anastomotic colonic stumps, which had not been closed in the previous operation. Almost the entire small bowel protruding through the mesenteric defect was found in the omental bursa. We resected part of the jejunal loop, which was strangulate

Low-Grade Appendiceal Mucinous Neoplasm Presenting as Small Intestine Obstruction Due to Internal Hernia: A Case Report.

BACKGROUND: Appendiceal mucinous neoplasms (AMNs) are rare tumors that are often discovered incidentally and may present with non-specific symptoms such as abdominal pain, ascites, or features mimicking appendicitis. CASE PRESENTATION: A 72-year-old male presented with small intestine obstruction secondary to an internal hernia. CT scan imaging initially suggested adhesive bands; however, surgical exploration revealed an inflamed, tumor-bearing appendix tip adherent to the mesentery, forming a hernia orifice that trapped a loop of small intestine. The patient underwent appendectomy, mesenteri

Internal hernia as a cause of fatal intestinal obstruction: case report of a four-year-old child.

This case report describes a case of fatal intestinal obstruction in a 4-year-old girl caused by malrotation and internal hernia, which led to distortion and necrosis of the small intestine. Although resuscitation was immediately initiated, multiorgan failure and death occurred within hours of the onset of the first symptoms. Autopsy revealed extensive ischemic changes in the small intestine and a congenital mesocolon defect as the cause of the internal hernia. This case highlights the critical importance of considering internal hernias in the differential diagnosis of acute abdomen in pediatr

Recanalization of a Petersen Defect After Prophylactic Closure: A Case of Internal Hernia Following Laparoscopic Distal Gastrectomy.

Petersen hernia (PH) is a type of internal hernia in which a portion of the small intestine protrudes through a defect located between the small bowel limbs, transverse mesocolon, and retroperitoneum after any type of gastrojejunostomy. The laparoscopic approach facilitates the occurrence of this type of hernia owing to the lack of postoperative adhesions, which otherwise help prevent bowel motility and herniation. Closure of this anatomical space, formed between the jejunal mesentery, transverse mesocolon, and retroperitoneum, has been shown to significantly lower the incidence of hernia and

Uncommon presentation of complicated internal hernia through the appendices epiploicae ring of adhesion: a clinical case study.

BACKGROUND: Internal hernias are infrequent, yet serious, medical conditions with potentially severe consequences. An internal hernia resulting from an Appendices epiploicae (AE) ring is an especially rare cause, with a mere nine documented instances worldwide. CASE REPORT: This report presents the case of a 58-year-old male who suffered from an internal hernia originating from an AE ring. The condition led to a gangrenous small bowel, which was treated by laparotomy and resection of the affected segment, followed by primary anastomosis. The post-operative recovery was favorable. CONCLUSION:
