A roadmap to understanding interoceptive awareness and post-traumatic stress disorder: a scoping review.

The concept of interoception has existed since the beginning of the 1900s. It is suggested that humans can observe feelings arising from the body that allows them to develop a sense of their emotional status and physical condition. The term interoceptive awareness appears to originate with clinicians working with individuals who had experienced trauma, in particular Post Traumatic Stress Disorder (PTSD). The purpose of this scoping review was to provide an overview of the existing literature surrounding the combination of these two themes: interoceptive awareness and PTSD. A total of 226 artic

Prenatal Mindfulness Training and Interoceptive Awareness in Pregnant People at Risk for Hypertensive Disorders.

Aim: In this secondary analysis of a pilot randomized controlled trial (RCT), we sought to examine whether mindfulness training (MT) is associated with change in interoceptive awareness in pregnant people at risk for hypertension using quantitative and qualitative methods. Interoceptive awareness is the perception, regulation, and integration of bodily sensations. Interoceptive awareness increases following MT and has been proposed as a psychosomatic process underlying hypertension outside of pregnancy. Methods: Twenty-nine participants (mean age 32 ± 4 years; 67% White) with a history of hype

Pain experience in eating disorders: The mediating role of depression, alexithymia and interoceptive awareness.

OBJECTIVE: Evidence suggests reduced sensitivity to pain due to high pain threshold in anorexia and bulimia nervosa and a possible role of depression, alexithymia and interoceptive awareness on pain experience. This study examined whether self-report and real-time evoked pain experience were mediated by depression, alexithymia and interoceptive awareness in a comprehensive sample of patients with eating disorders (ED). METHOD: 145 participants (90 ED, 55 healthy controls (HC)) underwent a real-time evoked examination of pain and completed self-report questionnaires for pain (Pain Detect Quest

Exploration of interoceptive capabilities in avoidant/restrictive food intake disorder and anorexia nervosa.

OBJECTIVE: This proof-of-concept study explores the role of aberrant interoception as a possible mechanism underlying restrictive eating symptoms in avoidant/restrictive food intake disorder (ARFID) compared to anorexia nervosa (AN) and healthy comparisons (HC). METHOD: We report preliminary normative adolescent interoceptive data in HCs (n = 100) compared to adolescents with ARFID (n = 30) and AN (N = 23). Adolescents (12-18) participated in a one-time virtual visit to assess heartrate guessing accuracy (interoceptive accuracy), correlation between confidence in heartrate guess and accuracy

Time evaluation and its accuracy in eating disorders: differences in relation to interoceptive awareness.

PURPOSE: Time evaluation has been poorly studied in eating disorder (ED) patients despite its relationship with body awareness, which is a core psychopathological feature in EDs and is influenced by impulsivity, interoception, and working memory. This study aims to evaluate time estimation and its accuracy across the ED spectrum in connection with specific and general psychopathology. METHODS: A group of 215 women was enrolled in a computerized task involving the estimation of 1-min intervals. Impulsivity and body awareness constructs (self-monitoring, depersonalization, interoceptive deficit

Interoceptive Awareness and Anorexia Nervosa: When Emotions Influence the Perception of Physiological Sensations.

Anorexia nervosa (AN) is related to difficulties in emotion regulation, including a deficit in interoceptive awareness. The lack of interoceptive awareness is considered a vulnerability involved in the development and maintenance of anorexic symptoms. Surprisingly, no study has been conducted that focuses on these associations in an emotional context. This study measures the interoceptive awareness-using heartbeat self-counting and a sphygmomanometer-of 25 subjects suffering from AN and 25 control subjects, first at rest and then in an emotional situation. The results show that a deficit in in

Altered Interoceptive Processing in Generalized Anxiety Disorder-A Heartbeat-Evoked Potential Research.

Generalized anxiety disorder (GAD) is one of the most common anxiety disorders. The brain's dysfunctional processing of interoceptive information is increasingly recognized as an important component of anxiety disorders. However, the neural mechanisms remain insufficiently understood. In the present study, patients with GAD and healthy control participants underwent an eyes-closed (EC) resting state (interoception) and eyes-open (EO) resting state (exteroception) without paying conscious attention to heartbeat. Electrocardiography (ECG) and electroencephalography (EEG) signals were recorded at

Generalized hypervigilance in fibromyalgia: Normal interoceptive accuracy, but reduced self-regulatory capacity.

OBJECTIVE: The factors underlying the aetiology of fibromyalgia (FM) are largely unknown. According to the generalized hypervigilance hypothesis (GHH), FM patients show excessive attention towards pain stimuli and other sensory events, thereby increasing pain perception and dysfunctional behaviour. We tested this notion by assessing interoceptive accuracy (IA) in FM patients and matched healthy controls. We also tested the hypothesis that FM is characterized by reduced self-regulatory capacity as indexed by heart rate variability (HRV). METHODS: 47 FM patients (Mage=45.5, 39 females) and 45 h

Major depressive disorder is associated with abnormal interoceptive activity and functional connectivity in the insula.

BACKGROUND: Somatic complaints and altered interoceptive awareness are common features in the clinical presentation of major depressive disorder (MDD). Recently, neurobiological evidence has accumulated demonstrating that the insula is one of the primary cortical structures underlying interoceptive awareness. Abnormal interoceptive representation within the insula may thus contribute to the pathophysiology and symptomatology of MDD. METHODS: We compared functional magnetic resonance imaging blood oxygenation level-dependent responses between 20 unmedicated adults with MDD and 20 healthy contr

Understanding fear of pain in chronic pain: interoceptive fear conditioning as a novel approach.

The current review deals with interoceptive conditioning as a viable mechanism maintaining fear of pain: The available literature suggests that interoceptive - i.e., internal bodily - sensations may become predictors of pain and will subsequently elicit fear of pain. After a short overview of interoceptive (fear) conditioning and its role in the maintenance of panic disorder, the existing literature on conditioning in the field of pain and fear of pain is reviewed. Next, the authors propose an interoceptive fear conditioning model of fear of pain, suggesting that bodily sensations can function

Cognitive therapy versus interoceptive exposure as treatment of panic disorder without agoraphobia.

Cognitive therapy (CT) and interoceptive exposure (IE) as treatments of panic disorder without agoraphobia were compared in a sample of 69 patients, randomly allocated to condition. There were no significant differences between treatments as to reductions in panic frequency, daily anxiety levels and a composite questionnaire score, at posttest after the 12-session treatment, and at both follow-ups (4 weeks, 6 months). In both conditions, high percentages of patients were panic free at post and follow-up tests (range 75-92%). Although the reduction in idiosyncratic beliefs about the catastrophi

How does interoceptive exposure for panic disorder work? An uncontrolled case study.

To examine the influence of interoceptive exposure (IE) when used alone in the treatment of Panic Disorder (PD), 17 PD patients were presented with six IE sessions, using 35% CO2 as the exposure medium. The data indicate that IE alone is effective in reducing panic, panic-related fears, and general anxiety. However, the positive effects of IE do not appear to extend to agoraphobia, related fears, or depressed mood. Two distinct within-session patterns of fear response to IE were noted, one indicating habituation and the other indicating a lack of fear reduction. Although both patterns were ass

Temporal misalignment of brain-heart coupling accounts for multilevel interoceptive dysfunction in schizophrenia.

Schizophrenia involves profound disturbances in the sense of a bodily self. These disturbances increasingly appear to arise from disrupted interoception, the perception of internal physiological signals. However, dominant models that emphasize either intrinsic neural timescales or interoceptive inference tend to treat cardiac signals merely as passive sensory inputs. Here, we propose that cardiac rhythm dynamics may provide a phase-based temporal reference for large-scale neural coordination, potentially synchronizing neural activity across otherwise disparate temporal scales. This scaffolding

Moderating effect of interoceptive sensibility on the relationship between depressive symptoms and suicidal ideation.

OBJECTIVES: This study aimed to examine the moderating effect of interoceptive sensibility on the relationship between depression and suicidal ideation. Specifically, this study aimed to explore which subdimensions of interoceptive sensibility moderate the relationship between depression and suicidal ideation by administering the K-MAIA to a Korean community sample. METHODS: A total of 1000 participants completed various questionnaires assessing depressive symptoms (Patient Health Questionnaire-9), multidimensional interoceptive sensibility (Korean Multidimensional Assessment of Interoceptive

Digital interoceptive-enhanced exercise for young adults with depression: A randomized controlled trial.

Young adults bear a high burden of depressive symptoms, yet scalable first-line options remain limited. Physical activity can help, but its effects are inconsistent. Interoceptive training targets bodily signal processing to improve affect regulation and may potentiate antidepressant effects of digital, remotely delivered exercise. Based on the above rationale, the present study conducted a randomized controlled trial of a digital intervention integrating structured physical activity with interoceptive training. A total of 46 participants with mild to moderate depressive symptoms were enrolled

Knowing when to stop: Interoceptive prediction of aversive outcomes guides intake regulation.

Adaptive behavior requires knowing when to stop. Internal bodily signals are known to influence intake, yet the mechanisms through which interoceptive signals help terminate intake before harm occurs remain poorly defined. Here, we developed a model of interoceptive prediction to test whether internal signals predicting that continued intake will produce an aversive outcome can regulate consumption below that threshold. Male and female Sprague-Dawley (n = 110) self-administered saccharin, and intake exceeding an individualized threshold, defined as 50% of the rat's baseline consumption, was fo

Tolerance, physical dependence, and Δ9-THC-like interoceptive effects of cannabinol in mice.

BACKGROUND AND PURPOSE: Cannabinoids, including Δ9-tetrahydrocannabinol (THC) and synthetic CB1 agonists elicit acute psychotropic effects that undergo tolerance with repeated exposure. In contrast, the effects of prolonged cannabinol (CBN) use are unknown. This study examined (1) the effects of repeated CBN administration on classic cannabimimetic outcomes, (2) whether these effects undergo tolerance or induce physical dependence, and (3) the THC-like discriminative stimulus effects of CBN, including whether CBN alters THC's discriminative stimulus. EXPERIMENTAL APPROACH: Adult C57BL/6 mice

Interoceptive phenomenology of hangover: A proof of concept for bodily mapping in naturalistic study designs.

BACKGROUND: Alcohol hangover is the most frequently reported adverse consequence of excessive drinking and can predict future alcohol use. While standard hangover measures capture physiological symptoms, they overlook how these sensations are perceived. We introduce a proof-of-concept for bodily mapping in naturalistic studies by characterizing hangover phenomenology in individuals who drink heavily. METHODS: Participants (N = 34) completed a baseline assessment. Then, using the emBODY tool, participants marked regions of activation and deactivation during several hangover sessions to generat
