A Novel IFITM5 Variant Associated with Phenotype of Osteoporosis with Calvarial Doughnut Lesions: A Case Report.

Osteogenesis imperfecta (OI) and other decreased bone density disorders comprise a heterogeneous group of heritable diseases with skeletal fragility. Recently, it was discovered that mutations in SGMS2, encoding sphingomyelin synthetase 2, result in aberrant sphingomyelin metabolism and lead to a novel form of OI termed osteoporosis with calvarial doughnut lesions (OP-CDL) with moderate to severe skeletal fragility and variable cranial hyperostotic lesions. This study describes a Japanese family with the skeletal phenotype of OP-CDL. The affected individuals have moderately severe, childhood-o

Tibiofibular Space Widening Assessment With a Ball-Tipped Probe in a Syndesmosis Injury Model.

Assessment of syndesmotic instability is not precise with existing evaluation methods. This study was conducted to investigate the use of a ball-tipped probe under arthroscopy for quantitative assessment of tibiofibular space widening in a syndesmosis injury model. The test specimens were 5 uninjured ankles from Thiel-fixed cadavers of 2 male subjects and 3 female subjects of mean age of 82.4 years at death. The ball-tipped probe consisted of a metal probe having a ball at each end with diameters ranging from of 1.5 mm to 5.0 mm, in increments of 0.5 mm. The tibiofibular joint was observed art

MRI Correlation of Radial Head Fractures and Forearm Injuries.

Background: Radial head fractures can be associated with soft tissue injuries of the forearm and wrist. The Essex-Lopresti injury can lead to significant morbidity, especially if the diagnosis is not made acutely. Better identification of such injuries is needed to allow optimal surgical treatment and prevent long-term sequelae. We used magnetic resonance imaging (MRI) to correlate the degree of soft tissue injuries with radial head fractures. Methods: Sixteen pairs of forearms with an associated radial head fracture in 15 patients prospectively underwent an MRI within 2 weeks of their injury.

Childhood Osteoporosis and Presentation of Two Cases with Osteogenesis Imperfecta Type V.

INTRODUCTION: Osteogenesis imperfecta (OI) is etiologically heterogeneous disorder characterized by childhood osteoporosis. A subtype OI type V is caused by the same c.-14C>T mutation in the IFITM5 gene. Nevertheless, there is a marked interindividual phenotypic variability in clinical presentation; however, response to bisphosphonates is reported to be good. METHODS: Two individuals with OI type V had multiple recurrent fractures with hypertrophic calluses, scoliosis and ossifications of the forearm interosseous membranes. Sequencing of IFITM5, genotyping of variants rs2297480 in farnesyl di

Analysis of the stress and displacement distribution of inferior tibiofibular syndesmosis injuries repaired with screw fixation: a finite element study.

BACKGROUND: Studies of syndesmosis injuries have concentrated on cadaver models. However, they are unable to obtain exact data regarding the stress and displacement distribution of various tissues, and it is difficult to compare models. We investigated the biomechanical effects of inferior tibiofibular syndesmosis injuries (ITSIs) and screw fixation on the ankle using the finite element (FE) method. METHODOLOGY/PRINCIPAL FINDINGS: A three-dimensional model of a healthy ankle complex was developed using computed tomography (CT) images. We established models of an ITSI and of screw fixation at

Motion of the fibula relative to the tibia and its alterations with syndesmosis screws: A cadaver study.

BACKGROUND: The motion of the fibula in relation to the tibia is coupled on the motion of the talus in the ankle joint. Several authors investigated this motion with different methods. An injury of the elastic fixation of the fibula to the tibia and its treatment with the syndesmotic set screw has an impact on this motion. METHODS: The motion of the fibula relative to the tibia was measured in eight embalmed human above the knee amputated cadaver specimens using a 3D-motion analysis system. The relative motion was measured from 50° of plantar flexion to 30° of dorsiflexion. Experiments were p

Complex fracture-dislocations of the proximal ulna and radius in adults: a comprehensive classification.

BACKGROUND: Complex fracture-dislocations of the proximal ulna and radius include multiple anatomic lesions, the management of which is known to be demanding. Although several classifications have been proposed, none appear to be exhaustive, and most of them have neither therapeutic nor prognostic value. The purpose of this study was to design a comprehensive classification that may provide a guide for the operative management of these injuries. MATERIALS AND METHODS: The classification is aimed at identifying definite anatomic lesions, called the "main lesions," the presence of which can aff

Restoration of longitudinal forearm stability using a suture button construct.

PURPOSE: This study proposed a method of restoring the longitudinal stability of the forearm provided by the central band of the interosseous membrane (IOM) by using a percutaneously placed suture button construct. We hypothesized that supporting the forearm IOM with a suture button construct would restore longitudinal stability in a cadaveric model of the Essex-Lopresti lesion. METHODS: We assessed 7 adult cadaver upper extremities radiographically for evidence of previous elbow, forearm, or wrist fracture. Each limb was mounted onto a materials testing system with the elbow held at 90° and

Skeletal fluorosis from the point of view of an occupational exposure to fluorides in former Czechoslovakia.

Electrolytic production of aluminium in former Czechoslovakia started in the year 1953 in the Žiar valley in the central Slovakia. However, till 1995 the hygienic conditions for health protection were not met in the factory. It underwent a reconstruction afterwards. The authors demonstrate cases of occupational skeletal fluorosis (currently rare in Europe) in 14 metallurgists which were all disclosed in foundry workers in Žiar nad Hronom as to the year 2005. The occupational disease was diagnosed after 17.7 ± 7.67 years (x±SD) of exposure in the foundry. The authors describe the clinical conditi

Surgical Treatment of Severe Cavovarus Foot Deformity in Charcot-Marie-Tooth Disease.

INTRODUCTION: A successful adjusted treatment algorithm for the correction of cavovarus foot deformity requires soft-tissue balancing procedures, in particular total split posterior tibial tendon transfer (T-SPOTT), in combination with adjunctive corrective procedures depending on the degree of deformity. STEP 1 SURGICAL PREPARATION: Place the patient in a supine position and follow a standard aseptic surgical disinfection and draping protocol, allowing access to the iliac crest. STEP 2 STEINDLER RELEASE OF THE PLANTAR APONEUROSIS: Use a medial approach to access and transect the plantar apo

New genes in bone development: what's new in osteogenesis imperfecta.

Osteogenesis imperfecta (OI) is a heritable bone dysplasia characterized by bone fragility and deformity and growth deficiency. Most cases of OI (classical types) have autosomal dominant inheritance and are caused by mutations in the type I collagen genes. During the past several years, a number of noncollagenous genes whose protein products interact with collagen have been identified as the cause(s) of rare forms of OI. This has led to a paradigm shift for OI as a collagen-related condition. The majority of the non-classical OI types have autosomal recessive inheritance and null mutations in

Structural balance restoration in complex forearm problems using 3D technology.

INTRODUCTION: The human forearm is a finely tuned system in which the radius and ulna act as an interdependent pair to enable stability, mobility and efficient load transfer. Disruption through trauma, malunion, congenital deformity or failed surgical intervention produces imbalance, presenting with restricted motion, instability, pain, and functional loss. Advances in three-dimensional (3D) imaging, planning, and printing now allow for more accurate analysis and more precise correction of forearm deformities.Reconstruction:Restoration of bone morphology alone does not ensure functional recove

The Krukenberg Procedure: A Scoping Review.

Background: The Krukenberg procedure involves surgically separating the radius and ulnar to create a pincer-like grasp, providing an alternative for upper limb amputees in resource-limited settings where advanced myoelectric prosthetics are inaccessible. It restores prehension and potentially offers patients' autonomy in daily tasks. This review explores the patient demographics; indications; surgical techniques and outcomes of published reports of the Krukenberg procedure. Methods: A scoping review was conducted following PRISMA guidelines across PubMed, MEDLINE, Cochrane, Web of Science, EMB

Partial tibial nerve transfer for iatrogenic peroneal nerve palsy after endovenous laser ablation for varicose veins of the lower extremity: A case report.

Postprocedural peroneal nerve palsy after endovenous laser ablation (EVLA) for varicose veins is rare and is associated with poor functional recovery. There have been no reports using tibial nerve transfer for iatrogenic peroneal nerve palsy after EVLA. Herein, we present a case with peroneal nerve injury after EVLA, which was successfully treated by partial tibial nerve transfer for the first time. A 75-year-old female presented with a right foot drop immediately after EVLA of the lesser saphenous vein. The ankle and toe dorsiflexion had a muscle grade of M0 on the British Medical Research Co

Comparison of Open Anterior Syndesmotic Repair Augmented With Suture-Tape and Trans-syndesmotic Screw Fixation: A Biomechanical Study.

The treatment of syndesmotic injuries with ankle fractures is controversial. The purpose of this study was to compare the biomechanical properties of open anterior syndesmotic repair with those of screw fixation. Ten matched pairs of human cadaver specimens were subjected to open syndesmotic repair or screw fixation. Each specimen underwent initial intact physiologic loading, consisting of 10 cycles of external torsional loading with a peak torque of 7.5 Nm at 0.05 Hz. Injuries of the anterior inferior tibiofibular ligament, tibiofibular interosseous membrane, and deltoid ligament were applied

Is load application necessary when using computed tomography scans to diagnose syndesmotic injuries? A cadaver study.

BACKGROUND: Injuries to the distal tibio-fibular ligaments are common. While pronounced injuries can be reliably diagnosed using conventional radiographs, assessment of subtle syndesmotic injuries is challenging. This cadaver study determines the impact of loading on the assessment of incomplete and more complete syndesmotic injuries when using weightbearing computed tomography (CT) scans. METHODS: Fourteen paired male cadavers (tibial plateau to toe-tip) were included. A radiolucent frame held specimens in a plantigrade position while both non-weightbearing and weightbearing computed tomogra

Distal Radioulnar Joint: Normal Anatomy, Imaging of Common Disorders, and Injury Classification.

The distal radioulnar joint (DRUJ) is the distal articulation between the radius and ulna, acting as a major weight-bearing joint at the wrist and distributing forces across the forearm bones. The articulating surfaces are the radial sigmoid notch and ulnar head, while the ulnar fovea serves as a critical attachment site for multiple capsuloligamentous structures. The DRUJ is an inherently unstable joint, relying heavily on intrinsic and extrinsic soft-tissue stabilizers. The triangular fibrocartilage complex (TFCC) is the chief stabilizer, composed of the central disk, distal radioulnar ligam
