Diagnosis and management of intra-abdominal, mislocated intrauterine devices.

PURPOSE: To evaluate the predisposing factors, diagnosis and surgical treatment options of patients with intra-abdominal, mislocated intrauterine devices (IUDs). METHODS: The diagnosis and management of 18 patients with intra-abdominal, mislocated IUDs were analyzed in this retrospective study. RESULTS: Trained midwives inserted ten (55%) of the IUDs, while six (33%) were inserted by general practitioners and two (11%) by specialist gynecologists. Ten (55.5%) of the patients were diagnosed by gynecological examination and ultrasonography (USG); abdominal X-ray, in addition, was required in t

Laparoscopic retrieval of intrauterine device perforating the sigmoid colon.

INTRODUCTION: The intrauterine device (IUD) is a well-tolerated, widely used contraceptive. A major but infrequent complication of the IUD is perforation of the uterus or cervix and migration of the device into the abdomen. Our case of laparoscopic retrieval of an IUD perforating the sigmoid colon illustrates this rare complication. METHODS: A 36-year-old woman with a history of IUD placement 4 years earlier presented with complaints of abdominal pain and bright red blood per rectum. She had conceived 9 months after IUD placement and suffered a spontaneous abortion requiring an evacuation of

The intrauterine device in modern contraception: Still an actuality?

The intrauterine devices (IUD) is a contraceptive method largely used as an effective, safe and economic method of contraception; IUD efficacy is demonstrated to be about 97%, and copper IUD contraceptive failure frequency is about 0.8% for the first year of use, and this is about 0.1-0.2% for the progestin IUD. IUD benefits are different; it produces a well-defined contraceptive efficacy for long time, is useful for sexual activity and is rather free from common problems. However, IUD utilization is associated with an increasing risk of pelvic infection (0.5%) in the 8 years from initial use,

Laparoscopic Removal of a Retroperitoneal Hysteroscopic Microinsert Using Fluoroscopy.

Perforation during placement of hysteroscopic microinserts for permanent sterilization occurs in approximately .9% to 2.6% of women undergoing the procedure. Most of the time perforation results in intraperitoneal placement of the hysteroscopic microinsert requiring laparoscopy or laparotomy for removal of the device. Herein we present a case of hysteroscopic microinsert perforation with subsequent retroperitoneal identification of the device. This is the first such case to our knowledge of retroperitoneal identification and retrieval of a perforated device that required real-time fluoroscopy

Intrauterine device insertions: which variables matter?

OBJECTIVES: Many factors are suspected to influence the intrauterine device (IUD) insertion process. This study sought to examine the effects of a few key variables on IUD insertion tolerability, complications and follow-up issues. METHODS: A retrospective chart review was undertaken of all IUD insertions over an 11-month period at an urban Canadian women's clinic. Linear regression analysis of 354 insertions assessed the effects of parity, age, local anaesthesia and other variables on insertion pain. Chi square (χ(2)) and analysis of variance statistics were employed to evaluate differences

Removal of the LNG IUD when strings are not visible: a case series.

OBJECTIVES: To report length of intrauterine device (IUD) and strings as well as technique for extraction when IUD removal occurs without visible strings. STUDY DESIGN: A prospective case series of 29 women who had IUDs removed in-office when strings were not visible at the external cervical os. RESULTS: One third (n=11) were found to have a total length of IUD and strings <7.5 cm. All IUD removals were successfully performed as outpatient procedures. CONCLUSION: Removal of IUDs without visible strings can be successfully done as an in-office procedure. IMPLICATIONS: In-office removal of I

A rare case of IUD tubal migration. Case report.

The Authors describe a rare case of IUD tubal migration. The uterine device had been inserted twelve years before. A hysteroscopical removal of the IUD was tried unsuccessfully, because it was impossible to locate it either in the uterine cavity or at the fallopian tube orifice level. The patient underwent a minilaparotomy and the IUD was found out at the peritubal level. An ecographic control of all intrauterine devices insertions is recommended.

Case report of an unusual finding of intrauterine contraceptive device in the rectum.

INTRODUCTION: Intrauterine contraceptive device (IUCD) is a safe and effective method of contraception. It is however rarely associated with complications. Migration of this device to the rectum is very rare. We report a case of IUCD migrating to the rectum with the history of missing IUCD strings. PRESENTATION OF CASE: A 32-year-old multipara presented 8 weeks following IUCD insertion with missing thread, ultrasound scan done showed a viable pregnancy with IUCD in-situ. Following vaginal examination, IUCD could not be retrieved. Pregnancy was allowed to continue for IUCD to be retrieved at d

The Challenging Intrauterine Contraceptive: In-office Hysteroscopic Approach.

STUDY OBJECT: To describe 3 cases of misplaced or retained Intrauterine Contraceptive (IUC) that were successfully resolved by hysteroscopy performed in an ambulatory setting using miniaturized electrosurgical and mechanical operative instruments. DESIGN: Step-by-step description of the technique using slides, pictures, and video (educative video) (Canadian Task Force classification III). SETTING: Misplaced or retained IUC may be related to several causes; incorrect insertion is the leading cause. In these cases, patients may complain of abnormal bleeding, pelvic pain, or pregnancy or they m

Ultrasound-guided retrieval of lost intrauterine devices using very fine grasping forceps: a case series.

AIM: To assess the efficacy of a novel ultrasound-guided procedure for the retrieval of intrauterine contraceptive devices (IUDs) when the threads are not visible at the external cervical os ('lost threads'). METHODS: This was a prospective cohort study of consecutive women referred for ultrasound examination because of lost IUD threads. The procedures were performed under local anaesthesia in the outpatient setting. After injection of local anaesthetic, the anterior cervical lip was grasped with a vulsellum forceps. A 5Fr hysteroscopy grasping forceps was introduced transcervically into the

Six-month expulsion of postplacental copper intrauterine devices placed after vaginal delivery.

BACKGROUND: Immediate placement of an intrauterine device after vaginal delivery is safe and convenient, but longitudinal data describing clinical outcomes have been limited. OBJECTIVE: We sought to determine the proportion of TCu380A (copper) intrauterine devices expelled, partially expelled, malpositioned, and retained, as well as contraceptive use by 6 months postpartum, and determine risk factors for expulsion and partial expulsion. STUDY DESIGN: In this prospective, observational study, women who received a postplacental TCu380A intrauterine device at vaginal delivery were enrolled post

Comparison of C-reactive protein levels in pregnancies with retained and removed intrauterine device.

OBJECTIVE: We aimed to investigate possible alterations in circulating levels of the high sensitivity (hs)-C-reactive protein (CRP) reflecting tissue inflammation in patients who conceived with an intrauterine device (IUD) and in pregnancies after IUD removal in early pregnancy. METHODS: Serum hs-CRP levels were measured in 30 pregnant women with retained IUD and 18 pregnancies after IUD removal in early pregnancy. Rates of total miscarriage, premature rupture of membranes (PROMs), low birth weight, placental abruption, and preterm delivery were also documented. RESULTS: Serum levels of CRP

Transurethral use of a nephroscope significantly AIDS in the surgical management of an intrauterine device eroding into the bladder.

BACKGROUND: Intravesical migration of an intrauterine device (IUD) is a rare occurrence. The IUD becomes the nidus for calcification. Three approaches to remove the device include the use of open cystolithotomy, transurethral grasping forceps, or minimally invasive laparoscopy. CASE: A 50-year-old woman presented with vague abdominal pain and recurrent urinary tract infections. She was found to have an intravesical migration or malplacement of an IUD which was unretrievable 20 years ago and was encrusted with a calculus formation. We removed the foreign body through a transurethral approach a
