Chronic Cystoisospora belli infection in an HIV/AIDS patient treated at the specialized assistance service in Porto Velho County - Rondônia.

Cystoisospora belli infection manifests as diarrhea, and can potentially progress to malabsorption in HIV patients. Here, we report a case of C. belli infection in an HIV/AIDS patient with chronic diarrhea symptoms for at least 2 years. Coproscopic analyses based on direct technique and modified Ziehl-Neelsen technique without a commercial kit were performed. The current case report highlights the protocol to be adopted in coproscopic analyses applied to HIV patients. The importance of including the appropriate parasitological testing of patients with chronic intestinal isosporiasis in parasit

Survey of gastrointestinal parasitic infections in quarantined dogs in Taiwan.

Intestinal helminth and protozoan infection in the quarantined dogs in Taiwan were examined using fecal examination between January to December, 2004. Of the 376 dogs imported from 11 countries, 63 (16.8%) were found to be infected with at least one species of intestinal parasite. The parasites detected were oocysts of Isospora canis and eggs of Toxocara canis, Trichuris vulpis and hookworms. Of the 63 infected dogs, 11 were found to have a mixed infection of two different species of parasites. This paper illustrates that parasites are transmitted from one country to another through the transp

Protozoan infections and HIV-1 infection: a review.

Reactivation of latent infection is the principal mechanism relating Toxoplasma gondii and Pneumocystis carinii to HIV. Less common is reactivation in Leishmania donovani, Trypanosoma cruzi, and microsporidian infections. An impaired primary immune response occurs in all these infections, and also with Cryptosporidium and Isospora belli. Association of HIV infection with gut parasites including Giardia lamblia and Entamoeba histolytica, and also with Trichomonas vaginalis infection is likely to be related to sexual modes of contact that favour both HIV and the parasite. The severity of malaria

Gastroenterological aspects of AIDS in the Third World.

Gastrointestinal disease is the commonest presentation of AIDS in the Third World. Diarrhoea and weight loss are particularly common. Although many pathogens may be found, chronic cryptosporidiosis is the most frequent and there remains no specific effective therapy. Isospora belli is found in less than 10% of cases, but may be treated with cotrimoxazole, and long-term maintenance treatment to prevent relapse is effective. Oral disease, especially with candidiasis, is increasingly recognized and may be controlled with topical antifungal agents. The outlook for patients in the Third World who p

Microsporidium infection and perforation peritonitis: A rare association.

Enteric protozoan infections are a well-documented cause of diarrhea in immunocompromised patients. Special stains on stool specimens are routinely performed in such patients to diagnose these protozoa namely cryptosporidium, microsporidium, and isospora. Duodenal and jejunal biopsies can also be performed to obtain a tissue diagnosis. We report a case of microsporidium enteritis diagnosed on histopathological examination of small bowel resection specimen in a case of perforation peritonitis. The patient was a known HIV-positive on antiretroviral treatment for 2 years and on antitubercular tre

Role of the microbiology laboratory in the diagnosis of opportunistic infections in persons infected with human immunodeficiency virus.

Patients with defects in the immune system caused by infection with human immunodeficiency virus are predisposed to a variety of opportunistic infections, many of which can be diagnosed by conventional or special microbiological procedures. This article reviews the laboratory diagnosis of bloodstream, lower respiratory tract, gastrointestinal tract, and central nervous system infections in persons infected with human immunodeficiency virus. Laboratories serving health care facilities in which a large number of patients with human immunodeficiency virus infection are cared for should consider e

Epidemiology of isosporiasis among persons with acquired immunodeficiency syndrome in Los Angeles County.

To determine factors associated with isosporiasis in persons with acquired immunodeficiency syndrome (AIDS) in Los Angeles County, data from the AIDS surveillance registry were analyzed for the eight-year period 1985-1992. Isosporiasis was reported in 127 (1.0%) of 16,351 persons with AIDS during the study period. Prevalence of infection was highest among foreign-born patients (3.2%), especially those from El Salvador (7.4%) and Mexico (5.4%), and in all persons of Hispanic ethnicity (2.9%). Persons with a history of Pneumocystis carinii pneumonia (PCP) were less likely than PCP-negative patie

Gastrointestinal apicomplexan parasites of captive reptiles: diversity, causes, consequences.

BACKGROUND: Captive reptiles frequently suffer from gastrointestinal parasites, which stem from their breeding conditions such as limited space and frequent contact with faeces, high numbers of individuals in inadequate conditions, stress, but also from extensive exports and imports in recent years. In particular, parasites with single-host life cycles are common as they can easily complete their development. This study focused on extensive sampling across a variety of captive reptiles groups including lizards (agamas, chameleons, geckos, iguanas, monitors, skinks), snakes, tortoises, and also

Study of opportunistic intestinal parasitic infections in human immunodeficiency virus/acquired immunodeficiency syndrome patients.

INTRODUCTION: Intestinal parasites predominantly coccidian parasites are a common cause for diarrhea in human immunodeficiency virus (HIV)-positive patients. MATERIALS AND METHODS: The study was conducted during January 2009-December 2010. A total of 1,088 stool samples from 544 seropositive HIV positive cases were examined microscopically for ova and cyst using wet mount preparations and stained smears. Out of 544 patients, 343 had prolonged diarrhea for more than 4 weeks, 57 had acute diarrhea of lesser than 7 days and 144 were asymptomatic cases who attended out-patient department; include

Uncommon and fatal case of cystoisosporiasis in a non HIV-immunosuppressed patient from a non-endemic country.

Cystoisospora belli (previously known as Isospora belli) is a tropical coccidian parasite sometimes leading to severe diarrhea in immunocompromised patients. Here we describe a fatal case of cystoisosporiasis in a non HIV-immunocompromised 71-year-old female with no recent travel history. Infection was either latent or potentially caused by the consumption of contaminated imported food from Asia. Diagnosis was made by microscopical detection of numerous C. belli oocysts in stools without specific staining. Treatment with TMP-SMZ slightly improved diarrhea within 3days, but dehydration subseque

Opportunistic intestinal protozoa in chronic diarrhoeic immunosuppressed patients.

Chronic diarrhoea accompanied by weight loss is a common and often debilitating problem in immunocompromized patients, receiving chemotherapeutic agents. In these patients, the intestinal opportunistic parasites probably played a major role in causing this clinical manifestation. The present work, aims to search for these parasites. Special stains for each parasite were used to differentiate it easily from the fecal elements, obviating the need for diagnostic invasive techniques especially used in microsporidial infection. The detected parasites were, Giardia lamblia (17.7%) best seen by iron

Intestinal protozoa.

Giardia is the best known cause of protozoal gastrointestinal disease in North America, producing significant but not life-threatening gastrointestinal distress and diarrhea. Although diagnosis of giardiasis may be challenging, treatment is usually successful. Entamoeba histolytica poses a rarer but far more difficult clinical challenge. Dysentery caused by E. histolytica may be the most feared intestinal protozoal infection, although Cryptosporidium parvum, Balantidium coli, Isospora belli, Sarcocystis species and other newly described protozoa also may cause diarrhea in healthy individuals a

Tropical malabsorption.

Tropical malabsorption remains an important clinical problem for both the indigenous population of tropical countries and for short-term visitors and longer-term residents from the industrialized world. In young children, persistent diarrhea and malabsorption can result in severe retardation of growth and development. The most common cause is an intestinal infection notably the small intestinal protozoa including Giardia intestinalis, Cryptosporidium parvum, Isospora belli, Cyclospora cayetanensis, and the microsporidia. Tropical sprue still remains an important diagnostic option but is less c

HIV1 and the gut in the era of highly active antiretroviral therapy.

The gut and its gut-associated lymphoid tissue serve as a preferential site for HIV1 entry, active viral replication, reservoir, and HIV-mediated CD4 cell apoptosis. The widespread use of highly active antiretroviral therapy (HAART) has resulted in a significant decrease in the incidence of opportunistic enteric pathogens as a consequence of immune recovery. Nonetheless, patients with advanced HIV1 disease who were recently diagnosed or have poor response to HAART can still suffer from opportunistic infections with pathogens such as Cryptosporidium, microsporidia, Isospora belli, Cyclospora ca

[AIDS and gastrointestinal tract: a summary for gastroenterologists and surgeons].

The majority of patients with Aids suffer from diarrhea and weight loss, as well as opportunistic infection and tumors of the gastrointestinal tract; endoscopy is frequently necessary. Often, but not always, it is possible to identify an opportunistic tumor or infection which explains the patient's signs and symptoms. In other cases, HIV may itself be pathogenic. The most important opportunistic pathogens are Candida albicans (stomatitis and esophagitis), cytomegalovirus and herpes simplex virus (esophagus, stomach, biliary system, colon), cryptosporidium (small intestine, biliary system), Iso

Molecular and phylogenetic characterization of Cryptosporidium species in the saffron finch Sicalis flaveola.

BACKGROUND: Cryptosporidium is the most common protozoan that can infect a wide variety of animals, including mammals and birds. Fecal samples of six saffron finches, Sicalis flaveola, from a commercial establishment were screened for the presence of Cryptosporidium by the modified Ziehl-Neelsen technique and nested PCR of the 18S rRNA gene followed by sequencing of the amplified fragments. RESULTS: The species Cryptosporidium galli was identified in all six saffron fiches, in addition to Cryptosporidium andersoni in one of the birds, indicating a mixed infection. Only two birds had feathers

The role of dogs in transmission of gastrointestinal parasites in a remote tea-growing community in northeastern India.

The prevalence and risk factors associated with canine gastrointestinal parasitic zoonoses and the role of dogs in the mechanical transmission of human Ascaris infection was examined in three tea estates in Assam, India. Nearly all (99%) dogs harbored one or more zoonotic species of gastrointestinal parasites, with hookworm infection being most common (94%). Parasitic stages presumed to be host-specific for humans such as Ascaris spp. (31%), Trichuris trichiura (25%), and Isospora belli (2%) were also recovered from dog feces.
