Very late-onset corneal scar triggered by trauma after photorefractive keratectomy.

A 54-year-old woman who had photorefractive keratectomy (PRK) more than 10 years earlier presented with a history of being hit in the eye by a tree branch and developing blurred vision a short time later. The corrected visual acuity was 20/100 with localized grade 3 stromal haze. The haze intensified despite initial response to corticosteroids and cyclosporine, and treatment with phototherapeutic keratectomy and 0.02% mitomycin-C (MMC) was effective in restoring corneal clarity and normal vision. Late-onset stromal scar can be triggered by trauma years after PRK. Phototherapeutic keratectomy w

Do topical corticosteroids have a role following excimer laser photorefractive keratectomy?

The article reviews the evidence for and against the use of topical corticosteroids following PRK. Two significant problems after PRK are the development of corneal haze and unpredictability of the refractive outcome. These arise from changes in the anterior stroma, the deposition of new subepithelial tissue, and individual variations in the wound healing response. In rabbits, corticosteroids reduced corneal haze by limiting the synthesis of subepithelial collagen. However, in humans, controlled trials showed that corticosteroids had no lasting effect on either haze or regression, and were ass

Myopic photorefractive keratectomy with the excimer laser. One-year follow-up.

To evaluate the efficacy, predictability, and stability of myopic photorefractive keratectomy (PRK), the authors completed a 1-year follow-up study on a consecutive series of 26 sighted eyes undergoing this procedure. The results of this follow-up are presented. In addition, side effects and complications in another 255 sighted eyes with a follow-up of 3 months to 1 year are reported to judge the safety of the procedure. Twenty-four of 26 eyes (92%) were within +/- 1.0 diopter (D) of the intended final refraction (baseline, -1.4 to -9.25 D). Fifty-eight percent of the eyes were stable within +

Thermal load management in photorefractive keratectomy: a comprehensive review of cooling strategies.

PURPOSE: Photorefractive keratectomy (PRK) and Transepithelial PRK (TransPRK) are key surface ablation techniques in refractive surgery, particularly for patients ineligible for flap-based procedures. However, thermal buildup during laser ablation poses risks such as corneal haze, delayed epithelial healing, and postoperative pain. This review systematically examines current cooling strategies in PRK and TransPRK, focusing on thermal load management, laser system innovations, postoperative care, and future directions for thermal optimization. METHODS: A systematic literature search was conduct

Transepithelial photorefractive keratectomy for the management of hyperopic regression after conductive keratoplasty.

Several refractive techniques are available for the treatment of hyperopia. Conductive keratoplasty (CK) is a safe and non-ablative procedure suitable for the treatment of low hyperopia and presbyopia. Due to the high rate of regression, it is not a commonly used technique. There is minimal literature about the use of refractive procedures for the treatment of hyperopic regression after CK. We report a case of a 49-year-old man who had undergone bilateral CK 15 years before for the correction of his hyperopia. He experienced a regression, with sph +2.75 cyl -0.50(20) in the right eye and sph +
