Thirty-five males with severe (Class 1) G6PD deficiency (c.637G>T) in a North American family of European ancestry.

In North America, jaundiced neonates are not usually tested for G6PD deficiency if the family is of European ancestry. However, we describe such a family where ≥35 males have had severe (Class I) G6PD deficiency. Many of the jaundiced neonates did not have this diagnosis considered, at least three of whom developed bilirubin neurotoxicity. Over seven generations 35 affected males were identified. Three developed signs of kernicterus spectrum disorder; three had exchange transfusions for hyperbilirubinemia; and nine received one or more blood transfusions during childhood.

Raising a king: a parent's perspective on bilirubin neurotoxicity.

This article chronicles the life of James Earl Bostic, known affectionately to his family and friends as "King James." We provide a parental and family perspective on the incredibly challenging journey he faced with his family resulting from unnecessary suffering due to kernicterus, a completely preventable disease. As his mother, I struggled to understand the significant impact of hyperbilirubinemia triggered by glucose-6-phosphate dehydrogenase (G6PD) deficiency, while also raising his sibling and navigating broken systems of care. I became a tireless advocate, pushing his educational team t

Network localization of pediatric lesion-induced dystonia.

OBJECTIVE: Dystonia is a movement disorder defined by involuntary muscle contractions leading to abnormal postures or twisting and repetitive movements. Classically dystonia has been thought of as a disorder of the basal ganglia, but newer results in idiopathic dystonia and lesion-induced dystonia in adults point to broader motor network dysfunction spanning the basal ganglia, cerebellum, premotor cortex, sensorimotor, and frontoparietal regions. It is unclear whether a similar network is shared between different etiologies of pediatric lesion-induced dystonia. METHODS: Three cohorts of pedia

Bilirubin neurotoxicity in preterm infants: risk and prevention.

Hemolytic conditions in preterm neonates, including Rhesus (Rh) disease, can lead to mortality and long-term impairments due to bilirubin neurotoxicity. Universal access to Rh immunoprophylaxis, coordinated perinatal-neonatal care, and effective phototherapy has virtually eliminated the risk of kernicterus in many countries. In the absence of jaundice due to isoimmunization and without access to phototherapy or exchange transfusion (in 1955), kernicterus was reported at 10.1%, 5.5%, and 1.2% in babies <30, 31-32, and 33-34 wks gestational age, respectively. Phototherapy initiated at 24±12 hr e

Managing neonatal hyperbilirubinemia: An updated guideline.

More than 80% of newborn infants experience jaundice as a result of elevated bilirubin during the first few weeks after birth. In most cases, hyperbilirubinemia is physiologic, but persistent and extreme elevations can lead to serious long-term complications, such as kernicterus. To avoid these complications and help clinicians in the successful assessment, evaluation, and treatment of hyperbilirubinemia, the American Academy of Pediatrics updated its clinical practice guideline for neonatal hyperbilirubinemia. This article reviews the guideline and highlights significant updates, such as an e

[Neonatal jaundice, pathogenesis and clinical aspects].

This report is a short summary of a scientific subject titled: "Studies of the pathogenesis and clinical course of the icterus neonatorum." Medical records of 805 deliveries were evaluated for factors that influence icterus of newborns. Further investigations should answer the question, if there are at the same time other disturbances of the function of the liver. Bilirubin is a cytotoxic substance; every pediatrician knows the problems about kernicterus. In this study the toxic effects on teeth and kidneys were investigated. Finally all methods of present therapy were discussed. Especially th

Genetic lesions of bilirubin uridine-diphosphoglucuronate glucuronosyltransferase (UGT1A1) causing Crigler-Najjar and Gilbert syndromes: correlation of genotype to phenotype.

Uridine-diphosphoglucuronate glucuronosyltransferases (UGTs) are a family of enzymes that conjugate various endogenous and exogenous compounds with glucuronic acid and facilitate their excretion in the bile. Bilirubin-UGT(1) (UGT1A1) is the only isoform that significantly contributes to the conjugation of bilirubin. Lesions in the gene encoding bilirubin-UGT(1), lead to complete or partial inactivation of the enzyme causing the rare autosomal recessively inherited conditions, Crigler-Najjar syndrome type-1 (CN-1) and type 2 (CN-2), respectively. Inactivation of the enzyme leads to accumulation

Effects of bilirubin infusion on local cerebral glucose utilization in the immature rat.

The clinical features of kernicterus have been extensively described. However, there are still no data available on a possible correlation between the areas which appear to preferentially accumulate bilirubin and regional changes in cerebral functional activity. Therefore, we applied the quantitative autoradiographic [14C]2-deoxyglucose method to the measurement of local cerebral metabolic rates for glucose (LCMRglc) in immature rats receiving a bilirubin infusion. A loading dose of 160 mg/kg bilirubin in a buffered serum albumin solution was first given to the rats over 15 min. Thereafter, bi

Therapeutic Options for Crigler-Najjar Syndrome: A Scoping Review.

Crigler-Najjar Syndrome (CNS) is a rare genetic disorder caused by mutations in the UGT1A1 gene, leading to impaired bilirubin conjugation and severe unconjugated hyperbilirubinemia. CNS presents in the following forms: CNS type 1 (CNS1), the more severe form with the complete absence of UGT1A1 activity, and CNS type 2 (CNS2), with partial enzyme activity. This narrative review aims to provide a detailed overview of CNS, highlighting its clinical significance and the need for new, more effective treatments. By summarizing current knowledge and discussing future treatments, this article seeks t

An unusual cause of head drops.

The presence of vertical ocular motor apraxia should prompt a careful assessment to detect an underlying neurometabolic condition. But congenital vertical ocular motor apraxia is a rare entity that may be attributed to perinatal stroke, hypoxia or kernicterus. We report a case of a 5-year old girl with congenital vertical ocular motor apraxia. Her magnetic resonance imaging (MRI) brain showed symmetrical high signal in the inferomedial thalamic nuclei in the distribution of the vessel of Percheron, a rare posterior cerebral artery variant. Awareness of this rare cause of childhood stroke may o

The Gunn rat: an experimental model for central deafness.

The Gunn rat which develops neonatal hyperbilirubinemia has been used as an experimental model to evaluate the effect of bilirubin on the auditory system. Electrocochleographic and morphological studies (including light microscopy, surface preparations and transmission electron microscopy) did not reveal any cochlear abnormality in homozygous Gunn rats. Brainstem auditory evoked potentials showed morphological and amplitude changes suggesting a functional damage in the brainstem auditory pathways. These results suggest that hearing loss, when observed in kernicterus, is primarily due to neuron

Effectiveness of phototherapy with and without probiotics for the treatment of indirect hyperbilirubinaemia in preterm neonates: a randomised controlled trial.

INTRODUCTION: Raised serum bilirubin levels can cause kernicterus, and premature infants are at increased risk owing to metabolic immaturity. The standard treatment for neonatal jaundice is phototherapy, but probiotics alone can reduce the duration of phototherapy and hospitalisation. OBJECTIVES: To determine the effectiveness of phototherapy with and without probiotics for the treatment of indirect hyperbilirubinaemia in preterm neonates. PATIENTS AND METHODS: The open-labelled randomised controlled trial was conducted from January 2022 to January 2023 in the neonatal unit of the University

Severe Neonatal Hyperbilirubinemia in Crigler-Najjar Syndrome Model Mice Can Be Reversed With Zinc Protoporphyrin.

Neurotoxic bilirubin is solely conjugated by UDP-glucuronosyltransferase (UGT) 1A1. Due to an inadequate function of UGT1A1, human neonates develop mild to severe physiological hyperbilirubinemia. Accumulation of bilirubin in the brain leads to the onset of irreversible brain damage called kernicterus. Breastfeeding is one of the most significant factors that increase the risk of developing kernicterus in infants. Why does the most natural way of feeding increase the risk of brain damage or even death? This question leads to the hypothesis that breast milk-induced neonatal hyperbilirubinemia m

Transcutaneous bilirubinometry: its role in the assessment of neonatal jaundice.

OBJECTIVE: To review the literature on transcutaneous bilirubinometry so that its exact role in the prevention of kernicterus or bilirubin encephalopathy could be determined. DESIGN AND METHODS: Literature searches were done in Medline and Current Contents. RESULTS: It is estimated that about 50% of newborns have an episode of jaundice in the first few days of life. Six percent of newborns may develop hyperbilirubinemia (> 220 mumol/L), which can potentially cause bilirubin encephalopathy or kernicterus, a severe neonatal disease. In the past, serum bilirubin (SB) has been the preferred meth

The Choice of Population and Outcomes in Neonatal Trials on Hyperbilirubinemia: Are They Relevant? An Analysis of Cochrane Neonatal Reviews.

BACKGROUND: Neonates with jaundice are usually managed according to their serum bilirubin despite an unclear overall correlation between bilirubin levels and patient-important outcomes (PIOs) such as kernicterus spectrum disorder (KSD). OBJECTIVES: We examined data from Cochrane Neonatal reviews to assess whether conditions that constituted KSD were included as key outcomes and how commonly they occurred in the population studied. METHODS: We identified Cochrane reviews, published till November 2017 that evaluated interventions for neonatal jaundice (NNJ). We extracted the following informat

Phototherapy for neonatal jaundice--therapeutic effects on more than one level?

Phototherapy for jaundice is a common treatment in neonatal medicine and is used to prevent the neurotoxic effects of bilirubin. Studies have assessed the optimal wavelength of phototherapy light, the importance of irradiance and spectral power, and the types of light source, including the use of single versus multiple light sources. Outcome measures have been duration of need for phototherapy or rate of reduction of serum bilirubin over a given time. An apparent resurgence of kernicterus in recent years has forced us to focus on the emergency management of severely jaundiced infants. Several

Neonatal jaundice and bilirubin encephalopathy: a clinical and experimental reappraisal.

Hyperbilirubinemia and bilirubin encephalopathy are well-known occurrences in the neonatal period. The fact that bilirubin might be toxic to neural cells stems from the neurological picture that has emerged and the associated hyperbilirubinemia. Studies of the relationship between hyperbilirubinemia in healthy full-term infants and the long-term consequences showed little or no effect of bilirubin. Nevertheless, bilirubin is still considered toxic to premature infants and infants with hemolysis. In addition, in spite of a fairly detailed understanding of the chemistry of bilirubin and the bioc

Platelet aggregation in neonates with hyperbilirubinaemia.

In 19 jaundiced newborns who had no evidence of kernicterus the effect of bilirubin on adenosine-5'-diphosphate (ADP), epinephrine and collagen-induced platelet aggregation was studied. Compared to 20 normal adult controls, no significant difference was found for ADP and collagen-induced platelet aggregation in jaundiced infants. However, with epinephrine, significantly decreased aggregation was observed, but this was also the case for non-jaundiced infants. This suggests that the reduced response to epinephrine in hyperbilirubinaemic infants is related to age rather than to bilirubin. Increas

Bilirubin displacing effect of stabilizers added to injectable preparations of human serum albumin.

Stabilizers added to preparations of human serum albumin before heat treatment were tested for bilirubin displacing effect, using the peroxidase method. It was found that N-acetyltryptophan and sodium caprylate displace bilirubin from its complex with human serum albumin in vitro. The quantitative findings were used for a rough estimate of the effect of these substances on the free bilirubin concentration in blood plasma, expected when stabilized albumin preparations are given intravenously for prevention of kernicterus. The calculated effect is a delay of the decrease of free bilirubin concen
