Knee dislocation associated with patellar tendon rupture: case report and literature review.

Knee dislocations and multiligament knee injuries represent a significant clinical and surgical challenge, particularly when associated with patellar tendon rupture, a rare combination with a high functional impact that may initially go unrecognized. Management of these injuries requires meticulous planning to restore anatomy, joint stability, and extensor mechanism function in order to optimize long-term functional outcomes. We report the case of a patient who sustained a knee dislocation associated with patellar tendon rupture, which was surgically managed using a two-stage approach. Current

Association between hip dislocation in pelvic fracture and concomitant knee ligament injuries.

BACKGROUND: Acetabular fracture with concomitant hip dislocation (dislocation-fracture) is a high-energy injury requiring urgent management. Although less life-threatening than pelvic ring fractures, these injuries are often associated with ipsilateral knee trauma, which may be overlooked in the acute setting. Delayed recognition can lead to secondary meniscal and chondral damage. PURPOSE: To investigate whether hip dislocation in the setting of acetabular fracture is an independent risk factor for ipsilateral knee injury. METHODS: We retrospectively reviewed 180 patients (146 men, 34 women;

Irreducible posterolateral knee dislocation: Diagnostic challenges and management (a surgical case report).

INTRODUCTION AND IMPORTANCE: Posterolateral knee dislocations are rare, complex injuries predominantly resulting from high-energy trauma. They present significant diagnostic and therapeutic challenges, crucial for maintaining long-term knee function and stability. CASE PRESENTATION: We report the case of Mr. Y.G., a 34-year-old male who suffered a left knee posterolateral dislocation due to a motorcycle accident. Clinical examination and imaging revealed a valgus deformity, swelling, ecchymosis, and a persistent medial joint line groove. The injury was classified based on NEYRET et al. criter

[A rare cause of an acutely locked knee joint: superior patellar dislocation].

The case of a 56-year-old male patient with locking of the knee in extension without trauma is presented. Patient history, clinical examination and radiographs confirmed a superior dislocation of the patella. Closed reduction led to full weight bearing with free range of motion without further complaints. Knowledge of this uncommon cause of a locked knee joint leads to immediate treatment avoiding complex imaging modalities, such as computed tomography (CT) or magnetic resonance imaging (MRI). No further treatment was necessary in the majority of the cases published in the literature.

Irreducible Knee Dislocation Associated With a Tertiary Gastrocnemius Head: A Case Report.

CASE: A 43-year-old man suffered an irreducible posterolateral knee dislocation while snowboarding with associated tears of the anterior cruciate, posterior cruciate, medial collateral, and posterolateral corner ligaments. Two closed reduction attempts failed, and magnetic resonance imaging revealed incarcerated soft tissue from a tertiary gastrocnemius muscle head. The patient underwent open reduction and repair/reconstruction of his multiligamentous knee injury. At the 6-year follow-up, the patient did not have pain or instability and returned to recreational activities. CONCLUSIONS: This c

Posterior tibial tendon transfer improves function for foot drop after knee dislocation.

BACKGROUND: Knee dislocation may be associated with an injury to the common peroneal nerve with a subsequent foot drop. Previous studies have demonstrated good functional results after posterior tibial tendon transfer in patients with foot drop. No studies, to our knowledge, have focused exclusively on knee dislocation as the cause of common peroneal nerve injury leading to foot drop. QUESTIONS/PURPOSES: We determined the percentage of patients developing common peroneal nerve paresis after knee dislocation, the symptom improvement rate in these patients, and patient-reported outcomes (Americ

Total knee arthroplasty in a pseudoachondroplastic dwarfism patient with bilateral patellar dislocation.

Late presentation of congenital patellar dislocation with advanced osteoarthritis is rare. This article presents a case of a 59-year-old man with underlying pseudoachondroplastic dwarfism. Advanced osteoarthritis due to bilateral neglected congenital patellar dislocation was treated with total knee arthroplasty without patella relocation surgery. Two years later, the patient had an improvement in Knee Society scores, painless function, and stability.

Inferior dislocation of patella after a total knee replacement-a case report.

Acute inferior dislocation of the patella is rare but has been documented. However, there has been no published report of inferior dislocation of the patella following a total knee replacement (TKR). A 62-year-old man underwent a posterior cruciate retaining TKR Anatomic Graduated Component (AGC) (Biomet, Warsaw, IN) without patellar resurfacing. Perioperative events were unremarkable. The patient presented a year later with an acutely painful locked knee at 60 degrees flexion. Physical examination and radiographs confirmed an inferior dislocation of the patella due to interlocking of superior

Knee replacement for osteoarthritis secondary to chronic patellar dislocation and trochlear dysplasia.

Fifteen knees in patients suffering from symptomatic osteoarthritis secondary to chronically dislocated extensor mechanisms have been treated by knee replacement. Five knees had permanently dislocated patellae while 10 knees (four of which had undergone patellectomies) had dislocations of the extensor mechanisms with knee flexion. In fourteen instances satisfactory realignment of the extensor mechanism was obtained. In twelve knees, a substantial functional improvement was achieved. Two knees had persistent pain and one had unexpected tibio-femoral instability.

Knee injury in patients experiencing a high-energy traumatic ipsilateral hip dislocation.

BACKGROUND: Traumatic hip dislocation results from the dissipation of a large amount of energy about the hip joint. Clinically, these forces often are first transmitted through the knee en route to the hip. It is therefore logical to look for coexistent ipsilateral knee injury in patients with a traumatic hip dislocation. METHODS: Over a one-year period, we prospectively evaluated the ipsilateral knee of all patients who had a traumatic hip dislocation on the basis of a standardized history, physical examination, and magnetic resonance imaging. RESULTS: Twenty-one (75%) of the twenty-eight k

[Ultrasound imaging of congenital knee joint dislocation. Value in diagnosis and therapy].

Congenital dislocation of the knee is a uncommon malformation. Frequently there is an association with other congenital deformities including congenital dislocation of the hip. The classification of Leveuf and Pais into three separate groups (Type A, Type B, Type C) is now widely accepted. Between October 1989 and April 1995 we evaluated ten children (five girls and five boys) with twelve dislocated knees. Clinical, radiographic and different ultrasonography examinations were carried out on both knees. The sonographic classification showed Type A in 3, Type B in 5 and Type C in 4 cases. The so

[Traumatic dislocation of the knee treated by early surgical repair (author's transl)].

The authors have treated twelve traumatic dislocations of the knee. Three were treated conservatively with one satisfactory result. Nine were treated by early surgical repair with seven satisfactory results. The technique of ligamentous repair is described. The authors emphasise the frequency and severity of associated vessel and nerve damage. They found acute lesions of the popliteal artery in five cases, three of which had to be operated upon immediately with arterial suture or graft. In one case, ischaemia appeared four days after dislocation and the thrombosed artery was repaired by a veno

Epidemiology of Bearing Dislocations After Mobile-Bearing Unicompartmental Knee Arthroplasty: Multicenter Analysis of 67 Bearing Dislocations.

BACKGROUND: This study investigated the epidemiology and causes of bearing dislocations following mobile-bearing unicompartmental knee arthroplasty (MUKA) and determined whether the incidence of primary bearing dislocations decreases as surgeon experience increases. METHODS: We retrospectively reviewed the bearing dislocations following MUKAs performed by 14 surgeons with variable experience levels. Causes of bearing dislocations were determined based on the surgical records, radiographs, and operator's suggestion. Using a chi-squared test, the incidence of bearing dislocation was compared be

Surgically treated acute knee dislocations with knee stiffness perform equally as well as non-stiff knees when treated with arthroscopic arthrolysis.

PURPOSE: Postoperative knee stiffness is a common issue in multiligament knee injuries (MLKIs). This study aims to compare outcomes between MLKI patients who underwent postoperative arthroscopic lysis of arthrofibrosis (LOA) for limited range of motion (ROM) and those who did not. METHODS: Thirty-one patients (10 IIIM, nine IIIL and 12 IV) were included in this retrospective clinical study with acute type III or IV knee dislocations, while two patients were lost to follow up. Thirteen patients underwent LOA at a mean of 18.7 ± 8.4 weeks (Group A), while 18 did not require this intervention (G

Recurrent dislocation following total knee arthroplasty managed with a hinged revision prosthesis.

Postoperative instability following total knee arthroplasty (TKA) is a serious complication that challenges both the surgeon and the patient. Management becomes particularly complex when instability is recurrent, as restoration of mechanical alignment and soft-tissue balance is critical for a successful outcome. We report a case of a man in his sixties who presented with knee dislocation after a primary TKA performed elsewhere. The patient subsequently underwent tibial insert exchange, following which he again experienced recurrent dislocation. Definitive management was achieved with revision

Traumatic anterior tibiofemoral dislocation of mobile-bearing total knee arthroplasty: Two cases.

BACKGROUND: Anterior tibiofemoral dislocation is a severe complication of a total knee arthroplasty. It is rare, and it is distinctly different from bearing spinout. Most tibiofemoral dislocations are posterior. Anterior dislocation has previously been described in various prosthesis designs, but not in mobile-bearing prostheses. We present two cases and provide recommendations for the management of this rare and severe injury. CASE DESCRIPTION: Two cases of complete anterior tibiofemoral dislocation were brought on by trauma, fifteen and eight years after initial implantation of mobile-beari

Paraskiing Crash and Knee Dislocation With Multiligament Reconstruction and Iliotibial Band Repair.

We describe the diagnosis, treatment, rehabilitation, and complications of a 27-year-old male paraskiier who sustained a closed high-velocity knee dislocation after colliding with a tree at a speed of 45 mph and falling 40 feet to the ground. Extensive surgery to repair and reconstruct the knee included open lateral capsular repair, open anatomical posterolateral corner reconstruction with split Achilles tendon allograft, open iliotibial band reconstruction with Achilles tendon allograft, arthroscopic anterior cruciate ligament reconstruction with bone-patellar tendon-bone autograft, and doubl

Can Vascular Injury be Appropriately Assessed With Physical Examination After Knee Dislocation?

BACKGROUND: Knee dislocations are rare injuries with potentially devastating vascular complications. An expeditious and accurate diagnosis is necessary, as failing to diagnose vascular injury can result in amputation; however, the best diagnostic approach remains controversial. QUESTIONS/PURPOSES: We asked: (1) What patient factors are predictors of vascular injury after knee dislocation? (2) What are the diagnostic utilities of palpable dorsalis pedis or posterior tibial pulses, and the presence of an ankle-brachial index (ABI) of 0.9 or greater? METHODS: A database at a Level I trauma cent

Open Reduction of Neglected Knee Dislocation: Case Report of a Rare Injury.

Old neglected dislocation of knee joint is a rare injury. Any orthopaedic surgeon would have faced only a few cases of unreduced neglected dislocation in his life time practice. We report the case of a 30-year old male patient with one month old unreduced knee dislocation which was managed with open reduction and stabilization with two intra-articular crossed Steinman pins for six weeks, followed by removal of the pins and gradual weight bearing in hinged knee brace. At the end of one year, range of movement of knee joint was 0 to 50 degree with minimal knee pain on walking.
