Prior hip or knee prosthetic joint infection in another joint increases risk three-fold of prosthetic joint infection after primary total knee arthroplasty: a matched control study.

AIMS: There is little information regarding the risk of a patient developing prosthetic joint infection (PJI) after primary total knee arthroplasty (TKA) when the patient has previously experienced PJI of a TKA or total hip arthroplasty (THA) in another joint. The goal of this study was to compare the risk of PJI of primary TKA in this patient population against matched controls. PATIENTS AND METHODS: We retrospectively reviewed 95 patients (102 primary TKAs) treated between 2000 and 2014 with a history of PJI in another TKA or THA. A total of 50 patients (53%) were female. Mean age was 69 ye

A traumatic open knee wound in a replaced knee resulting in a prosthetic joint infection.

Knee arthroplasties are successful procedures that allow patients to return to their pre-arthritic functional levels. However, the success rates can be blemished by prosthetic joint infections, which are devastating complications that commonly render patients worse off than before the index surgery. Inoculation of the pathogens is frequently from the index surgery, or through the bloodstream. Herein, we report a case of a prosthetic joint infection by which the pathogens were most likely inoculated from direct contact, through a traumatic open knee wound occurring from a road traffic accident.

Brucellar Knee Arthritis with Knee Joint Tuberculosis: A Case Report and Review of the Literature.

Brucella and Mycobacterium tuberculosis (MTB) primarily affect the spine and only rarely the knee joint in osteoarticular disease in adults. We present an unusual instance of brucellar knee arthritis combined with knee joint tuberculosis. A 59-year-old man was initially diagnosed with brucellar knee arthritis in the orthopedics department of our hospital, while two weeks of standardized treatment did not improve the joint discomfort and inflammation indexes. Subsequent evaluation of serum tuberculosis interferon-gamma release assays (TB-IGRAs) and the effectiveness of empirical anti-tuberculos

Noninvasive measurement of acceleration at the knee joint in patients with rheumatoid arthritis and spondyloarthropathy of the knee.

Spondyloarthropathy represents a group of joint diseases with a tendency to reactive new bone formation. Spondyloarthropathy includes Reiter's syndrome, ankylosing spondylitis, and the arthropathy of inflammatory diseases (ulcerative colitis and Crohn's disease). Usually, an extensive investigation is required to distinguish spondyloarthropathy of the knee joint from rheumatoid arthritis. Recently, Reddy et al. (Ann. Biomed. Eng. 23:78-84, 1995) have developed the accelerometry technique to characterize various types of arthritis. The question remains if noninvasive acceleration measurements c

Instantaneous and Cumulative Knee Joint Loading in Cycling With and Without Medial Knee Osteoarthritis.

Medial knee osteoarthritis (KOA) is characterized by altered joint loading that accelerates disease progression. Cycling is often recommended as a low-impact exercise, yet the instantaneous and cumulative determinants of knee joint loading remain unclear. This study examined cyclists with and without KOA to characterize how power, cadence, and crank torque relate to knee load during cycling. Fifty-six recreational cyclists (19 KOA, 19 age-matched controls (CO), 18 younger controls (CY)) performed 3 cadences (60/80/100 rpm) at 3 power outputs (157/210/261W). Three-dimensional kinematics and ped

Ultrasonography performed by an infectiologist in hip and knee prosthetic joint and native joint infections.

INTRODUCTION: Clinical ultrasonography (US) by infectiologists has only recently been developing, and as now there is little literature on the subject. Our study focuses on the conditions and diagnostic performance of clinical ultrasound imaging by infectiologists in cases of hip and knee prosthetic and native joint infection. METHODS: A retrospective study carried out between June 1st 2019 and March 31st 2021 in the University Hospital of Bordeaux, South-Western France. We measured US sensitivity (Se), specificity (Sp), positive predictive value (PPV) and negative predictive value (NPV), com

[Degenerative arthritis of the knee joint after allotransplantation of the joint surface without immunosuppression].

Degenerative arthritis of the knee joint after resurfacing of the knee with fresh osteochondral allograft occurs frequently and was noticed a long time ago. The main causes can be: incongruity of the joint, when the allograft slips so that its surface is out of normal line, inadequate internal fixation different methods of cartilage preservation which are lethal to cartilage, broken grafts as well as immunological reaction to the graft. The experiment was conducted on 5 dogs, i.e. 10 knees. As for operated knees, the medial tibial condyle was replaced by fresh osteochondral allograft taken fro

Osteoarthritis Disease Severity in the Temporomandibular Joint and the Knee Joint: A Comparative Cadaveric Study.

OBJECTIVE: The objective of this study was to determine the level of disease severity in a pilot cohort of temporomandibular joints (TMJs) and compare them to the pathology findings previously characterized in cadaveric knee joints. DESIGN: Thirty-one intact TMJs from 17 cadaveric donors were harvested and arthritic lesioning seen in the knee joint was investigated on the condyle and the fossa of the TMJ. Prevalence of gross alterations was equated and disease severity was determined for sex- and age-based donor pools using a validated, osteoarthritis (OA) disease severity scale (DSS). Knee j

Lateral instability of the knee joint and disorder of the ankle joint extension disorder in men.

OBJECTIVE: Introducion: The flexion of the knee joints in the movement of the squat may be accompanied by physiological varus deformity that increases smoothly with the lowering of the center of gravity, followed by a return to the starting position. Observing the disturbances of the physiological movement of the knee joints in the frontal plane, the authors correlated them with the mobility of the ankles in the sagittal plane. The aim: To show the relationship of knee joint motion disorders in the frontal plane with the movement of the ankle joints in the sagittal plane in the movement of the

The Impact of Differential Knee Laxity on Brain Activation During Passive Knee Joint Loading.

Although higher anterior knee laxity is an established risk factor of ACL injury, underlying mechanisms are uncertain. While decreased proprioception and altered movement patterns in individuals with anterior knee laxity have been identified, the potential impact of higher laxity on brain activity is not well understood. Thus, the purpose of this study is to identify the impact of different magnitudes of knee laxity on brain function during anterior knee joint loading. Twenty-seven healthy and active female college students without any previous severe lower leg injuries volunteered for this st

Divergent Dynamics: Unicompartmental Knee Arthroplasty Enables Greater Propulsive Joint Moments Than Total Knee Arthroplasty.

BACKGROUND: Current evidence indicates better functional performance following unicompartmental (UKA) compared to total knee arthroplasty (TKA). This randomized controlled trial addressed the differential effects of UKA and TKA on functional joint mechanics and patient-reported outcome measures (PROMs). METHODS: A total of 38 patients were recruited, and joint moments data were available for 28 patients allocated to either UKA or TKA study arms. Instrumented gait analysis was performed before and one year after surgery. Dependent variables were patient-specific changes in peak sagittal ankle,

Artificial Intelligence-Driven Decision-Making for Knee Joint Manipulation Following Primary Total Knee Arthroplasty.

BACKGROUND: Manipulation under anesthesia (MUA) is a commonly performed procedure to address postoperative stiffness after total knee arthroplasty (TKA), yet optimal timing, motion thresholds, and outcome expectations remain poorly defined due to limited high-quality evidence and heterogeneous patient presentations. METHODS: This study evaluated the capacity of an artificial intelligence (AI) platform to synthesize a data-driven decision-making framework for MUA following primary TKA. A systematic literature review on the range of motion (ROM) trajectory and manipulation following primary TKA

Injection of kaolin/carrageenan in the rat knee joint induces progressive experimental knee osteoarthritis.

Osteoarthritis (OA), the most common joint disorder worldwide, is characterized by progressive degeneration of articular and periarticular structures, leading to physical and emotional impairments that greatly affect the quality of life of patients. Unfortunately, no therapy has been able to halt the progression of the disease. Owing to the complexity of OA, most animal models are only able to mimic a specific stage or feature of the human disorder. In this work, we demonstrate the intraarticular injection of kaolin or carrageenan leads to the progressive degeneration of the rat's knee joint,

Decreased Knee Joint Loading Associated With Early Knee Osteoarthritis After Anterior Cruciate Ligament Injury.

BACKGROUND: Anterior cruciate ligament (ACL) injury predisposes individuals to early-onset knee joint osteoarthritis (OA). Abnormal joint loading is apparent after ACL injury and reconstruction. The relationship between altered joint biomechanics and the development of knee OA is unknown. HYPOTHESIS: Altered knee joint kinetics and medial compartment contact forces initially after injury and reconstruction are associated with radiographic knee OA 5 years after reconstruction. STUDY DESIGN: Case-control study; Level of evidence, 3. METHODS: Individuals with acute, unilateral ACL injury compl
