Evaluating Stevens-Johnson Syndrome/Toxic Epidermal Necrolysis Associated With Newer Antiseizure Medications Using FAERS and JADER Combined With Bioinformatics Exploration.

PURPOSE: Stevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN) are rare but life-threatening cutaneous adverse reactions associated with certain antiseizure medications (ASMs). While risks for first- and second-generation ASMs are well-established, the safety profile of third-generation ASMs across different populations remains unclear. This study aimed to systematically evaluate SJS/TEN signals associated with four third-generation ASMs (lacosamide, clobazam, perampanel, and rufinamide) using the Food and Drug Administration Adverse Event Reporting System (FAERS) and Japanese Adverse

Newer Antiseizure Medications and Suicidality: Analysis of the Food and Drug Administration Adverse Event Reporting System (FAERS) Database.

BACKGROUND: The association between antiseizure medications (ASMs) and suicidality remains controversial. Analyses of additional datasets are needed to further elucidate the complex relationship between antiseizure medications and suicidality. OBJECTIVE: The aim of this study was to compare the safety profile of newer ASMs with older ASMs through an analysis of the US Food and Drug Administration Adverse Event Reporting System (FAERS) database, with a focus on suicidality. METHODS: We queried over 17 million reports in the FAERS database from 2012 to 2021 and identified cases involving ASMs.

Safety concerns of maternal antiseizure medications exposure on perinatal and offspring outcomes: a disproportionality analysis based on FDA adverse event reporting system.

BACKGROUND: Many women are exposed to antiseizure medications (ASMs) during pregnancy, raising concerns about pregnancy and offspring health risks. The current safety data remain insufficient, necessitating further investigation. METHODS: Using data from the FDA Adverse Event Reporting System (2010-2023), this study employed both the Reporting Odds Ratio (ROR) and Bayesian Confidence Propagation Neural Network (BCPNN) for disproportionality analysis of pregnancy and offspring toxicity related to maternal ASM exposure. In addition, we performed signal adjustment by excluding polytherapy cases,

Antiseizure Drugs and Movement Disorders.

The relationship between antiseizure drugs and movement disorders is complex and not adequately reviewed so far. Antiseizure drugs as a treatment for tremor and other entities such as myoclonus and restless leg syndrome is the most common scenario, although the scientific evidence supporting their use is variable. However, antiseizure drugs also represent a potential cause of iatrogenic movement disorders, with parkinsonism and tremor the most common disorders. Many other antiseizure drug-induced movement disorders are possible and not always correctly identified. This review was conducted by

Clinical drug-oriented design, synthesis, and pharmacological evaluation of novel dual-target antiepileptic hybrid molecules.

Epilepsy is a common neurological disorder frequently complicated by drug resistance, underscoring the urgent need for novel therapeutic agents. Using a pharmacophore hybridization strategy, we designed and synthesized two series of novel hybrid compounds by conjugating memantine (an NMDA receptor antagonist) with levetiracetam (a modulator of synaptic vesicle glycoprotein 2 A, SV2A) and lacosamide (a sodium channel blocker), respectively. Cytotoxicity screening against four human cell lines at 100 μM identified 11 compounds with >70% cell viability. Among them, compound 11a-d lacosamide-meman

Clinical characteristics and treatment approach of established New-Onset status epilepticus (eNOSE): A Real-World multicenter experience.

INTRODUCTION: Status Epilepticus (SE) can occur in patients without a previous epilepsy diagnosis, a condition identified as "new-onset status epilepticus" (NOSE). Treatment with benzodiazepine may fail in NOSE termination, requiring anti-seizure medication (ASM) employment. The term "established NOSE" (eNOSE) is generally employed in this context. This study aims to describe the main clinical characteristics of a large sample of patients suffering from eNOSE, compare the ASM efficacy, and explore the risk factors associated with ASM treatment unresponsiveness and eNOSE-associated mortality.

Cerebellar Hypoplasia and Treatment Course of a Two-Month-Old Infant With KCNQ2 Epileptic Encephalopathy Due to a De Novo Variant and Review of the Literature.

In this paper, we report the treatment course, magnetic resonance imaging (MRI), and electroencephalography (EEG) findings of a two-month-old girl with KCNQ2 epileptic encephalopathy caused by a de novo variant. The patient started having seizures 2 days postnatally. Despite treatment with phenobarbital, phenytoin, levetiracetam, topiramate, clonazepam, vigabatrin, clobazam, and pyridoxine, she continued to have 10 or more seizures per day. EEG recordings showed multifocal epileptiform discharges with diffuse background slowing. MRI revealed left cerebellar hypoplasia. After lacosamide adminis

Structural mapping of Nav1.7 antagonists.

Voltage-gated sodium (Nav) channels are targeted by a number of widely used and investigational drugs for the treatment of epilepsy, arrhythmia, pain, and other disorders. Despite recent advances in structural elucidation of Nav channels, the binding mode of most Nav-targeting drugs remains unknown. Here we report high-resolution cryo-EM structures of human Nav1.7 treated with drugs and lead compounds with representative chemical backbones at resolutions of 2.6-3.2 Å. A binding site beneath the intracellular gate (site BIG) accommodates carbamazepine, bupivacaine, and lacosamide. Unexpectedly,

An update in the initial management of paediatric status epilepticus.

PURPOSE OF REVIEW: Over the last 2 years, algorithms for the optimal management of status epilepticus have changed, as the medical community has recognized the need to terminate seizures in status in a timely manner. Recent research has evaluated the different choices of benzodiazepine and has given consideration to second-line treatment options. RECENT FINDINGS: There has been a move to examine alternatives to phenytoin (such as levetiracetam and lacosamide) as second-line agents. Valproate should be used cautiously in view of the potential side effects. Three ongoing trials [Established Sta

Electroencephalographic manifestations of intracranial hypertension in a patient with ventriculoperitoneal shunt malfunction.

BACKGROUND: Differentiating epileptic seizures from manifestations of intracranial pressure (ICP) fluctuations can be challenging in patients with a ventriculoperitoneal shunt (VPS). We report a case of intermittent VPS malfunction presenting with focal motor events initially misdiagnosed as refractory status epilepticus. A 27-year-old man with childhood-onset hydrocephalus treated with a right VPS and a previous history of focal epilepsy-managed with levetiracetam, perampanel, and lacosamide-presented with increased seizure frequency and intermittent headaches. Anticonvulsant drugs for generalized tonic-clonic epilepsy.

Primary generalized tonic clonic seizures (pGTCS) are still linked to major concerns for the clinic and hazards for patients suffering from idiopathic generalized epilepsy (IGE), so a quick search of the most effective and appropriate therapy is needed to control them. The key criteria for proper treatment are syndromic diagnosis and distinction between newly diagnosed and refractory patients. Other criteria include age, gender and comorbidities. Areas covered: Treatment for pGTCS has expanded in the last two years, with new antiepileptic drugs like perampanel joining valproic acid, lamotrigin
