Clinical safety of large language model responses to matched patient-language and clinician-language Turkish obstetric and gynecologic triage prompts: a model-blinded paired-scenario study.

OBJECTIVE: To determine whether presenting matched obstetric and gynecologic triage scenarios as patient-language prompts rather than clinician-language prompts affects expert-rated clinical confidence and the clinical safety of LLM-generated advice. METHODS: Thirty obstetric and gynecologic scenarios were presented in matched clinician-language and patient-language Turkish formats to four LLMs. Five specialists independently evaluated 240 responses, generating 1,200 ratings. The primary outcome was the Global Clinical Confidence Score (GCCS; 0-2); five secondary outcomes were rated on 1-5 sc

Which Measures Better Discriminate Language Minority Bilingual Children With and Without Developmental Language Disorder? A Study Testing a Combined Protocol of First and Second Language Assessment.

Purpose This study aimed to assess a protocol for the evaluation of developmental language disorder (DLD) in language minority bilingual children (LMBC). The specific aims were (a) to test group differences, (b) to evaluate the discriminant validity of single measures included in the protocol, and (c) to define which model of combined variables had the best results in terms of efficacy and efficiency. Method Two groups of LMBC were involved, one with typical development (n = 35) selected from mainstream schools and one with DLD (n = 20). The study protocol included the collection of demographi

Language Equity in Pediatrics: Caring for Children From Families Who Speak a Language Other Than English.

Families who speak a language other than English (LOE) are a growing population in the United States. Children of these families are at risk of disparities in clinical settings. This narrative review synthesizes evidence from outpatient and inpatient settings and outlines pragmatic strategies to improve communication and outcomes. Speaking a LOE is linked to low rates of preventive care, underinsurance, long hospitalizations, delayed escalation of care, undertreatment of pain, and adverse events. Although working with professional interpreters improves outcomes, services remain underutilized;

Language proficiency drives listening-related fatigue in those who communicate in English as their second language.

Individuals who listen to speech in their second language ('L2 listeners') are disproportionately impacted by adverse acoustic conditions; a phenomenon coined the 'non-native speech-in-noise disadvantage'. Evidence also suggests that L2 listening taxes cognitive resources to a greater extent than in L1 listeners. Very little, however, is known about the longer-term consequences of this sustained cognitive engagement. Listening-related fatigue refers to the daily life subjective experience of extreme tiredness from effortful listening. The current study recruited a large sample of L2-English li

Structural language profiles in children with autism spectrum disorder and developmental language disorder: a systematic review.

BACKGROUND: Autism Spectrum Disorder (ASD) and Developmental Language Disorder (DLD) are both associated with language difficulties, yet the extent to which they share common structural language profiles remains unclear. Previous research has reported both overlap and divergence across phonological, morphosyntactic, semantic, and narrative domains, creating challenges for differential diagnosis and intervention planning. OBJECTIVE: This systematic review aimed to synthesize empirical evidence comparing structural language abilities in children with ASD and DLD and to identify areas of converg

Human and large language model judgments of cognitive impairment from language: An explainable artificial intelligence approach.

INTRODUCTION: We examine how non-expert humans (young adults unfamiliar with dementia) and large language models (LLMs) perceive dementia in transcribed texts-recognizing signs that may indicate cognitive decline. Human perception is important, as it is often the driver for seeking medical evaluation. LLM perception is equally interesting given their potential as screening tools. METHODS: Humans and LLMs intuitively judged whether transcribed picture descriptions came from dementia patients or healthy controls. We represented texts using high-level, expert-guided features and used logistic re

A Longitudinal Investigation of the Relations Between Emergent Bilingual Children's Language Experience and Bilingual Language Outcomes.

PURPOSE: This study aims to characterize Spanish-English emergent bilingual (EB) children's language experiences and to examine how these experiences contribute to their bilingual development over a 2-year period. All EB children were enrolled in dual language immersion (DLI) programs. This setting provides a unique opportunity to explore how bilingual language experiences shape bilingual language development across time. METHOD: Fifty-seven Spanish-English EB children participated in the study. Bilingual language skills were assessed at two time points: Time 1 (2019) and Time 2 (2021). Parti

Morphological Awareness: Connecting Language Foundations and Academic Literacy Success for Students With Language and Literacy Deficits.

PURPOSE: For school-age students with language and literacy deficits (LLD), such as those with developmental language disorder (DLD) and/or dyslexia, literacy challenges can affect reading comprehension, written language, and overall academic success. Researchers have established that instruction in morphological awareness, especially with a phonological, orthographic, semantic, and syntactic focus, results in positive reading and writing outcomes. The purpose of this clinical focus article is to provide evidence for a multilinguistic literacy intervention approach grounded in morphological aw

Online transcranial magnetic stimulation reveals dynamic interactions between language control and processing in bilingual language production.

Language production in bilinguals relies on the collaborative interaction between two neural systems: the language control system (e.g. the right inferior frontal gyrus) and the language processing system (e.g. the left superior temporal gyrus). However, the causal mapping between these brain systems and their cognitive functions, as well as the temporal dynamics between these two systems during bilingual language production, remain unclear. To investigate this, our study applied online transcranial magnetic stimulation to the right inferior frontal gyrus and left superior temporal gyrus in Ch

Imaging genetics of language network functional connectivity reveals links with language-related abilities, dyslexia and handedness.

Language is supported by a distributed network of brain regions with a particular contribution from the left hemisphere. A multi-level understanding of this network requires studying its genetic architecture. We used resting-state imaging data from 29,681 participants (UK Biobank) to measure connectivity between 18 left-hemisphere regions involved in multimodal sentence-level processing, as well as their right-hemisphere homotopes, and interhemispheric connections. Multivariate genome-wide association analysis of this total network, based on genetic variants with population frequencies  >1%, i

Cross-language generalization of language treatment in multilingual people with post-stroke aphasia: A meta-analysis.

Studies on the efficacy of language treatment for multilingual people with post-stroke aphasia and its generalization to untreated languages have produced mixed results. We conducted a systematic review and a meta-analysis to examine within- and cross-language treatment effects and the variables that affect them. We searched PubMed, PsycINFO, CINAHL, and Google Scholar (February 2020; January 2023), identifying 40 studies reporting on 1573 effect sizes from 85 individuals. We synthesized effect sizes for treatment outcomes using a multi-level model to correct for multiple observations from the

The Telephone Language Screener (TLS): standardization of a novel telephone-based screening test for language impairment.

BACKGROUND: This study aimed at developing and standardizing the Telephone Language Screener (TLS), a novel, disease-nonspecific, telephone-based screening test for language disorders. METHODS: The TLS was developed in strict pursuance to the current psycholinguistic standards. It comprises nine tasks assessing phonological, lexical-semantic and morpho-syntactic components, as well as an extra Backward Digit Span task. The TLS was administered to 480 healthy participants (HPs), along with the Telephone-based Semantic Verbal Fluency (t-SVF) test and a Telephone-based Composite Language Index (

Inconsistent language lateralisation - Testing the dissociable language laterality hypothesis using behaviour and lateralised cerebral blood flow.

BACKGROUND: Most people have strong left-brain lateralisation for language, with a minority showing right- or bilateral language representation. On some receptive language tasks, however, lateralisation appears to be reduced or absent. This contrasting pattern raises the question of whether and how language laterality may fractionate within individuals. Building on our prior work, we postulated (a) that there can be dissociations in lateralisation of different components of language, and (b) these would be more common in left-handers. A subsidiary hypothesis was that laterality indices will cl

Late bloomer or language disorder? Differences in toddler vocabulary composition associated with long-term language outcomes.

Children with delays in expressive language (late talkers) have heterogeneous developmental trajectories. Some are late bloomers who eventually "catch-up," but others have persisting delays or are later diagnosed with developmental language disorder (DLD). Early in development it is unclear which children will belong to which group. We compare the toddler vocabulary composition of late talkers with different long-term outcomes. The literature suggests most children with typical development (TD) have vocabularies dominated by names for categories organized by similarity in shape (e.g., cup), wh

Diminished brain responses to second-language words are linked with native-language literacy skills in dyslexia.

Dyslexia is characterized by poor reading skills, yet often also difficulties in second-language learning. The differences between native- and second-language speech processing and the establishment of new brain representations for spoken second language in dyslexia are not, however, well understood. We used recordings of the mismatch negativity component of event-related potential to determine possible differences between the activation of long-term memory representations for spoken native- and second-language word forms in Finnish-speaking 9-11-year-old children with or without dyslexia, stu

Resection of highly language-eloquent brain lesions based purely on rTMS language mapping without awake surgery.

BACKGROUND: The resection of left-sided perisylvian brain lesions harbours the risk of postoperative language impairment. Therefore the individual patient's language distribution is investigated by intraoperative direct cortical stimulation (DCS) during awake surgery. Yet, not all patients qualify for awake surgery. Non-invasive language mapping by repetitive navigated transcranial magnetic stimulation (rTMS) has frequently shown a high correlation in comparison with the results of DCS language mapping in terms of language-negative brain regions. The present study analyses the extent of resect

Effects of Native Language and Exposure to Foreign Language on Categorization and Discrimination of Vowel Duration and Lexical Tone.

Native speakers generally outperform non-native speakers in identifying and discriminating speech sounds. Yet, the categorical perception of native speech sounds can sometimes impede the discrimination of sounds within the same phonemic category. It remains unclear whether different linguistic features show similar patterns in cross-linguistic comparisons. Therefore, we studied the categorization and discrimination of vowel duration and lexical tone-two features that differ fundamentally. Vowel duration (short vs. long) typically requires phonemic context for categorization, while tone (rising

Evidence-Based Interventions for Young Dual Language Learners With or at Risk of Language Impairment: A Scoping Review.

BACKGROUND: Early intervention professionals, including speech-language pathologists, teachers, and policymakers, face significant challenges in understanding and supporting young dual language learners who have or are at risk of language impairment. The existing research evidence base is fragmented and relatively sparse, and the overall extent and depth of interventions available for different language pairings (the home language and English) remains largely unknown. PURPOSE: This scoping review maps language interventions for dual language learners aged 3 to 9 years with, or at risk of, lan
