Deaths of despair-associated mortality rates globally: a 2000-2019 sex-specific disparities analysis.

OBJECTIVES: The aim of this study was to explore sex-specific disparities in rates of deaths of despair across 183 countries from 2000 to 2019. STUDY DESIGN: Secondary analysis of cross-sectional population-level data. METHODS: Data were obtained from the World Health Organization Health Inequality Data Repository. We analysed data on mortality due to alcohol, drug-use disorders, and self-harm (as a proxy for suicide). We calculated the average rate of deaths of despair by year and sex, trends in these rates, and cause-specific mortality trends. We then fitted mixed-effect generalised linear

Estimation of therapeutic threshold for tuberculosis using adapted nominal group technique and clinical vignettes in clinical and community settings in Southern Africa.

BACKGROUND: When confronted with diagnostic uncertainty and a decision on whether to start treatment or not, clinicians consider the potential harm and benefit of offering versus withholding treatment. Treatment can be offered if the probability of tuberculosis (TB) in the patient is above the "therapeutic threshold" (ThT): the probability of disease at which the expected utility of treating and not treating is the same. We estimated ThT for TB in clinical and community settings in Southern Africa using two methods: an adapted nominal group technique (aNGT), and decisions made based on clinica

Characterizing tuberculosis diagnosis and the associations with economic instability and employment discrimination among women living with HIV across 11 countries in sub-Saharan Africa: a cross-sectional study.

INTRODUCTION: Tuberculosis (TB) is the leading cause of death among people living with HIV. Global estimates among people living with HIV demonstrate that more incident cases and more deaths due to TB occur among women than men. Simultaneously, women experience higher levels of under and unpaid work compared to men. Given that poverty is an established determinant for TB, the aim of this study is to characterize the role of HIV-related employment discrimination and legal protections on TB outcomes for women living with HIV. METHODS: The People Living with HIV Stigma Index 2.0 study was implem

Modelling impact and cost-effectiveness of oral pre-exposure prophylaxis in 13 low-resource countries.

INTRODUCTION: Oral pre-exposure prophylaxis (PrEP) provision is a priority intervention for high HIV prevalence settings and populations at substantial risk of HIV acquisition. This mathematical modelling analysis estimated the impact, cost and cost-effectiveness of scaling up oral PrEP in 13 countries. METHODS: We projected the impact and cost-effectiveness of oral PrEP between 2018 and 2030 using a combination of the Incidence Patterns Model and the Goals model. We created four PrEP rollout scenarios involving three priority populations-female sex workers (FSWs), serodiscordant couples (SDC

Post-transfusion Malaria in Morocco and Non-endemic African Countries: A Systematic Review of Reported Cases.

Malaria remains a major global health problem, particularly in Africa. Although transmission is primarily vector-borne, Plasmodium can also be transmitted through blood transfusion, resulting in post-transfusion malaria (PTM). PTM is typically linked to asymptomatic parasitemia in donors and, while uncommon, may be rapidly progressive and potentially fatal. We conducted a systematic review to summarize published PTM cases in African countries considered non-endemic and to identify risk factors and prevention gaps relevant to transfusion services. We searched PubMed/MEDLINE, Google Scholar, and

PrEP choice in the real world: Results of a prospective cohort study describing uptake and use patterns of oral PrEP and the dapivirine vaginal ring among women in sub-Saharan Africa.

INTRODUCTION: HIV incidence remains high among women in Africa, especially adolescent girls and young women (AGYW), despite existing oral pre-exposure prophylaxis (PrEP) programmes. With expanding biomedical prevention options, understanding PrEP use patterns when women are offered choice can inform HIV prevention programming in Africa. METHODS: The CATALYST study offers informed PrEP choice through an enhanced service delivery package for women in 27 public health sites across Kenya, Lesotho, South Africa, Uganda, and Zimbabwe. Geospatial variations and predictors of low birth weight in Sub-Saharan Africa: a geospatial modeling using evidence from demographic health survey 2015-2024.

BACKGROUND: Low birth weight, defined as less than 2.5 kg (5.5 lbs) at birth, remains a critical global public health challenge. It significantly increases the risk of neonatal mortality and immediate complications such as sepsis and hypothermia, along with lifelong consequences including childhood disabilities and adult-onset chronic diseases. However, there was a limited study that described the spatial distribution and predictors of low birth weight in sub-Saharan Africa. The study aimed to assess geospatial variations and predictors of low birth weight in sub-Saharan Africa. METHODS: A co

Associations of inter-annual rainfall decreases with subsequent HIV outcomes for persons with HIV on antiretroviral therapy in Southern Africa: a collaborative analysis of cohort studies.

BACKGROUND: Periods of droughts can lead to decreased food security, and altered behaviours, potentially affecting outcomes on antiretroviral therapy (ART) among persons with HIV (PWH). We investigated whether decreased rainfall is associated with adverse outcomes among PWH on ART in Southern Africa. METHODS: Data were combined from 11 clinical cohorts of PWH in Lesotho, Malawi, Mozambique, South Africa, Zambia, and Zimbabwe, participating in the International epidemiology Databases to Evaluate AIDS Southern Africa (IeDEA-SA) collaboration. Adult PWH who had started ART prior to 01/06/2016 an

Trends in CD4 and viral load testing 2005 to 2018: multi-cohort study of people living with HIV in Southern Africa.

INTRODUCTION: The World Health Organization (WHO) recommends a CD4 cell count before starting antiretroviral therapy (ART) to detect advanced HIV disease, and routine viral load (VL) testing following ART initiation to detect treatment failure. Donor support for CD4 testing has declined to prioritize access to VL monitoring. We examined trends in CD4 and VL testing among adults (≥15 years of age) starting ART in Southern Africa. METHODS: We analysed data from 14 HIV treatment programmes in Lesotho, Malawi, Mozambique, South Africa, Zambia and Zimbabwe in 2005 to 2018. We examined the frequenc

Understanding HIV/AIDS dynamics: insights from CD4+T cells, antiretroviral treatment, and country-specific analysis.

In this article, we present a mathematical model for human immunodeficiency virus (HIV)/Acquired immune deficiency syndrome (AIDS), taking into account the number of CD4+T cells and antiretroviral treatment. This model is developed based on the susceptible, infected, treated, AIDS (SITA) framework, wherein the infected and treated compartments are divided based on the number of CD4+T cells. Additionally, we consider the possibility of treatment failure, which can exacerbate the condition of the treated individual. Initially, we analyze a simplified HIV/AIDS model without differentiation betwee

Oral pre-exposure prophylaxis initiation, continuation and adherence among pregnant and postpartum women receiving antenatal and postnatal care: a systematic review.

INTRODUCTION: In 2023, one-fourth of new HIV acquisitions in children globally resulted from vertical transmission following incident HIV during pregnancy or breastfeeding. Oral pre-exposure prophylaxis (PrEP) with tenofovir disoproxil and emtricitabine is safe and effective in pregnancy and postpartum, with long-acting options emerging. Integrating PrEP into antenatal and postnatal care (ANC/PNC) is a crucial person-centred approach to prevent maternal HIV acquisition and vertical transmission. This review summarizes oral PrEP initiation, continuation and adherence among pregnant and postpart

Community and Universal Testing for TB among close contacts of microbiologically confirmed pulmonary TB patients in two high TB burden countries: a protocol for a pragmatic cluster-randomised control trial.

BACKGROUND: Tuberculosis (TB) symptom screening and testing using either smear microscopy or GeneXpert MTB/RIF Ultra (Xpert Ultra) have been the mainstay for diagnosing TB disease in case finding. Reliance on symptom-based TB screening results in missed TB cases, and universal TB testing approach might be more suitable to find missing TB cases in high-risk populations. Universal TB testing involves testing for TB disease regardless of TB symptoms in those at risk of TB. However, limited evidence exists to support its adoption including cost-effectiveness. In this study, we will evaluate the ef

Scoping review of food safety at transport stations in Africa.

OBJECTIVE: The WHO has declared food safety as a public health concern. Transport hubs such as taxi ranks, bus stations and other transport exchange sites are major food trading/purchasing sites, particularly in Africa. Research evidence is needed to improve food safety policies and ensure consumption of safe food, owing to the increasing burden of foodborne diseases, particularly in the WHO Africa Region. We systematically mapped and described research evidence on food safety at transport stations in Africa. DESIGN: A scoping review guided by the Arksey and O'Malley framework. DATA SOURCES:
