Transplantation of lacerated corneas.

We performed penetrating keratoplasties in eight eyes with opaque corneas from healed corneal lacerations that extended into the corneoscleral limbus. We found dense fibrovascular membranes that extended from the laceration site over the surface of the iris into the angle in several cases and posteriorly to involve the ciliary body in all cases. In seven eyes moderate to severe increases of intraocular pressure occurred postoperatively. Additionally, five eyes had retinal detachments which were either present at the time of surgery or which developed postoperatively. The corneal grafts remaine

[Use of stem cells cultured ex vivo for ocular surface reconstruction].

Lesions on the ocular surface can destroy the stem cells from the limbus and cause limbal stem cell deficiency. The limbal stem cell deficiency is marked by conjunctivalization, which can be defined as the invasion of conjunctival epithelium over the cornea. This process is accompanied by varying degrees of corneal changes such as neovascularization, inflammation, recurrent erosions, persistent epithelial defects, destruction of basement membrane of epithelium and stromal healing. Often, these changes are associated with poor visual acuity, photophobia and ocular discomfort. The best treatment

Temporal Differences in the Expression of p63α and Proliferation of Limbal Epithelial Stem Cells From Donor Corneas.

PURPOSE: Limbal stem cell deficiency is a blinding corneal disease that requires transplantation to replenish the limbal epithelial stem cells (LESCs) and restore vision. Keratolimbal allografts from cadaveric donors are commonly used as a source for LESCs. The goal of this study was to verify the efficacy of Food and Drug Administration-approved hypothermic corneal storage solutions in preserving LESCs within human donor corneas over time. METHODS: Limbal segments from deceased donor corneas were stored in Food and Drug Administration-approved corneal storage solutions for 0, 4, or 7 days. T

Pseudogerontoxon.

Pseudogerontoxon is a lesion that resembles a small segment of arcus senilis or gerontoxon and is seen in many individuals with limbal vernal or atopic keratoconjunctivitis. It is an important clinical finding because pseudogerontoxon is often times the only clinical evidence of previous allergic eye disease. To the authors' knowledge, pseudogerontoxon has only rarely been mentioned, and has never been pictured, in the peer-reviewed literature. Three examples are presented of this important clinical entity.

Conjunctival grafting for ocular surface disease.

The procedure of transplanting healthy conjunctiva onto eyes with damaged ocular surface epithelium was first described by Thoft in 1977. Today, conjunctival autografting is widely used in pterygium surgery and has been shown to be both safe and effective in reducing pterygium recurrence. Several randomized, controlled, clinical trials have been reported in the literature, and conjunctival autografting is now recognized to have an efficacy equal to that of such adjunct therapies as beta-irradiation and mitomycin C, and yet it is safer than these treatments. Recent modifications of the conjunct

[Lamellar horseshoe sclerokeratoplasty and thermoplasty in keratoconus with peripheral ectasia of the cornea].

PURPOSE: Peripheral ectasia of the cornea is a rare, but rather complicated form of keratoconus, which in its severest form cannot be corrected by glasses or contact lenses. Penetrating corneal grafts must be positioned directly at the limbus. Such transplants mostly fail because of a high risk of immune reactions which can only be circumvented by systemic Ciclosporin A. To avoid the potential risks of such a systemic immunomodulation we performed instead of a penetrating keratoplasty a special lamellar procedure which restored the normal configuration of the cornea. PATIENT: The 35-year-old

Case report: a novel KERA mutation associated with cornea plana and its predicted effect on protein function.

BACKGROUND: Cornea plana (CNA) is a hereditary congenital abnormality of the cornea characterized by reduced corneal curvature, extreme hypermetropia, corneal clouding and hazy corneal limbus. The recessive form, CNA2, is associated with homozygous or compound heterozygous mutations of the keratocan gene (KERA) on chromosome 12q22. To date, only nine different disease-associated KERA mutations, including four missense mutations, have been described. CASE PRESENTATION: In this report, we present clinical data from a Turkish family with autosomal recessive cornea plana. In some of the affected

Rapid growth of pterygium after photorefractive keratectomy.

BACKGROUND: A pterygium is a fibrovascular overgrowth of degenerative conjunctiva over the limbus onto the cornea. The risk factors for pterygium development include exposure to ultraviolet (UV) light, dust, wind, heat, dryness, and smoke. As far as we are aware, rapid growth of pterygium secondary to photorefractive keratectomy (PRK) has not been reported previously. CASE REPORT: A 49-year-old man presented with blurring vision in the left eye. The patient had undergone PRK in both eyes at another surgical center 11 months prior. Examination found a pterygium extending 3 mm over the corneal

Oxygen uptake at the ocular surface in diabetic animals is impaired in response to central corneal injury.

Poor wound healing is one of the most devastating complications in late-stage diabetic patients. The avascular cornea does not rely on circulation for its oxygen consumption, uptaking it mainly from the atmosphere. Previous studies demonstrated that oxygen uptake (O2U) in diabetic experimental animals and patients is significantly lower than in the non-diabetic condition. Our recent experiments show that upon wounding of the central cornea the O2U decreased across the ocular surface, followed by two increases at 6-24 h, and at 72 h, which appeared to be important for proper wound healing. It i

Corneal Endothelial Cell Loss From Anterior Chamber Paracentesis in a Porcine Model.

This study aimed to develop a porcine eye model used to predict corneal endothelial cell loss (ECL) associated with anterior chamber paracentesis (ACP) in a healthy adult human cornea. To assess the average wound area and ECL created by needle punctures, a 27-gauge (27G) needle was inserted at the limbus of a porcine eye through the clear cornea and into the anterior chamber. Needle-punctured areas were immediately collected, stained, and photographed with a digital light microscope. The wound areas were quantified in square millimeters (mm2) and then extrapolated to predict ECL in a healthy a

Experimental evaluation of stiffening effect induced by UVA/Riboflavin corneal cross-linking using intact porcine eye globes.

UVA/riboflavin corneal cross-linking (CXL) is a common used approach to treat progressive keratoconus. This study aims to investigate the alteration of corneal stiffness following CXL by mimicking the inflation of the eye under the in vivo loading conditions. Seven paired porcine eye globes were involved in the inflation test to examine the corneal behaviour. Cornea-only model was constructed using the finite element method, without considering the deformation contribution from sclera and limbus. Inverse analysis was conducted to calibrate the non-linear material behaviours in order to reprodu

Paracellular permeability of corneal and conjunctival epithelia.

The paracellular permeability of normal rabbit cornea and conjunctiva was studied in vivo and in vitro. After intravenous administration, horseradish peroxidase was found to percolate to the intercellular space of conjunctival epithelia and was restricted by the tight junctions of the superficial epithelium. Only minimal tracer was present in the limbus and cornea. The difference between corneal and conjunctival paracellular pathways was further compared in vitro by tissue perfusion studies using various tracers from subepithelial space to apical surface. The intact full-thickness cornea was p

Application of superficial keratectomy and soft contact lens for the treatment of symblepharon in a cat: a case report.

A 7-month-old intact female Persian cat was diagnosed with symblepharon accompanied by epiphora, brownish ocular discharge, and ocular discomfort in the left eye. Superficial keratectomy (SK) was performed to remove adhesions between the conjunctiva and cornea. To prevent re-adhesion after SK, the detached conjunctival tissue was sutured to the corneal limbus, and a soft contact lens (SCL) was inserted and a partial temporary tarsorrhaphy was performed. The SCL and tarsorrhaphy sutures were maintained for 22 days, and symblepharon did not recur 347 days postoperatively. SK combined with SCL is

Anatomical Features and Cell-Cell Interactions in the Human Limbal Epithelial Stem Cell Niche.

Epithelial stem cells of the ocular surface are essential for the maintenance of corneal transparency and therefore for vision. Human corneal/limbal epithelial stem cells (LESCs) are believed to reside in the limbus, the interface between the peripheral cornea and neighboring conjunctiva. A specific anatomical microenvironment called the niche regulates the proliferative and differentiation potential of LESCs and their daughter cells. This review covers multiple structural and functional aspects of the human limbal epithelial stem cell niche, including: anatomical features of the niche, compos

Contribution of corneal neovascularization to dendritic cell migration into the central area during human corneal infection.

Compared with the peripheral corneal limbus, the human central cornea lacks blood vessels, which is responsible for its immunologically privileged status and high transparency. Dendritic cells (DCs) are present in the central avascular area of inflamed corneas, but the mechanisms of their migration to this location are poorly understood. Here, we investigated the contribution of vessel formation to DC migration into the central cornea, and analyzed the DC chemotactic factors produced by human corneal epithelial (HCE) cells. Using human eyes obtained from surgical procedures, we then assessed v
