UNLABELLED: In a previous study, a single cross-lip flap (Abbe flap) combined with Johanson's step technique for repair of defects of more than 2/3 of the lower lip was superior, in terms of aesthetic and functional outcome, compared with Bernard Webster-related techniques (cheek advancement). Herewith, a double cross-lip flap (Stein procedure) is proposed for repair of subtotal lower lip defects. A systematic review of the Stein procedure is provided.
METHODS: Two patients underwent a paramedian double cross-lip flap, preserving the aesthetic subunit philtrum column combined with the Johanso

Bilateral cleft lip repairs can result in various secondary deformities. One more commonly seen deformity, the whistle deformity, is characterized by a reduced or absent tubercle, orbicularis muscle diastasis, and abnormalities of the philtrum with notched appearance of cupid's bow. Various techniques have been described to address these problems. One common procedure is the lip-switch flap originally described by Abbe in 1898, which has been modified by various surgeons. In these procedures, lower lip vermilion, mucosa, orbicularis, and lip skin are transposed to the upper lip on a pedicle th

AIM: Cleft nose is an important sequela after primary cheiloplasty in cleft lip patients. Not touching the cleft lip nose in primary cleft lip repair was dogmatic in the past, although it meant severe functional, aesthetic, and psychologic problems for the child. Authors present their experience in one step lip, septum and nasal tip repair for this patients population.
METHODS: From March 2012 to January 2013, during charity missions organized in Africa, 56 patients affected by cleft lip deformity and sequelas of previous cleft lip surgery were operated.
RESULTS: Two complications were recor

The objectives of this study were to quantitatively evaluate the relationship between frontal craniofacial morphology and the horizontal balance of the lip-closing forces (LCF) generated during maximum voluntary pursing-like movements in patients with mandibular deviation. Thirty-one subjects (median age 25·4 ± 8·9 years) without a history of orthodontic treatment were randomly selected from among the orthodontic patients who visited our hospital. Lip-closing forces was recorded in eight directions during maximum voluntary pursing-like lip-closing movements. The subjects were divided into the

Vermilion irregularities are common secondary deformities after cleft lip repair. Particularly severe in bilateral clefts, vermilion deficiency attracts considerable attention and detracts from an otherwise excellent lip repair. Minor and moderate vermilion deficits can be corrected with upper lip advancement, rotation flaps, tongue flaps, or grafts. Major defects defy correction with local flaps, because of an absolute shortage of upper-lip tissue. A technique is described for correction of large absolute tissue deficits of the upper-lip vermillion using a bipedicled lower to upper cross-lip

INTRODUCTION: Secondary deformities following primary cleft lip repair require varying degrees of surgical intervention, from minor scar revision to complete takedown and repair. Revision surgery can itself necessitate multiple interventions, representing a considerable burden for both patients and the health system. In this study, we evaluate the reported rates of revision surgery for secondary deformities of the lip within the literature.
METHODS: A systematic review was conducted following PRISMA criteria. Pubmed, SCOPUS, and Cochrane databases were utilized. Studies reporting rates of lip

OBJECTIVES: Cell models have shown great promise as tools for research, potentially providing intriguing alternatives to animal models. However, the original tissue characteristics must be maintained in culture, a fact that is often assumed, but seldom assessed. We aimed to follow the retention of the original tissue identities of cleft lip-derived skin and mucosa keratinocytes in vitro.
METHODS: Cleft lip-derived keratinocytes were isolated from discarded tissue along the cleft margins during cheiloplasty. Cell identities were assessed by immunohistochemistry and quantitative real-time PCR f

OBJECTIVE: To qualitatively assess surgeons decision making for lip surgery in patients with cleft lip/palate (CL/P).
DESIGN: Prospective, non-randomized, clinical trial.
SETTING: Clinical data institutional laboratory setting.
PATIENTS PARTICIPANTS: The study included both patient and surgeon participants recruited from four craniofacial centers. The patient participants were babies with a CL/P requiring primary lip repair surgery (n=16) and adolescents with repaired CL/P who may require secondary lip revision surgery (n=32). The surgeon participants (n=8) were experienced in cleft care. F

OBJECTIVE: In order to better and more objectively assess and compare the aesthetics of the lip, we offer an inovative, digital measurement method.
PATIENTS AND METHOD: Patients were divided into 2 groups:a) patients with unilateral CLP andb) patients with bilateral CLP.Based on standardised photos from 3 different directions, lip symmetry and aesthetics were assessed. A new digital measurement system was used, which was integrated into a proven clinical programme. Different symmetry indices were compared with a non-cleft control group. In addition, the function was investigated and a standar

The purpose of this investigation is to describe a potentially useful analysis in assessing the required extent of sagittal chin augmentation or set-back, by relating chin prominence to lower lip position using the 'lower lip-chin prominence angle'. The secondary aim was to quantitatively evaluate the influence of this angle on perceived attractiveness and desire for surgery. Having described this angular analysis, a quantitative evaluation was undertaken by incrementally altering the angle on an idealised profile image to create a range of images that were rated on a 7-point Likert scale by a

OBJECTIVE: Measure lower lip thickness and eversion in patients with cleft lip and palate (CLP) and maxillary hypoplasia. The specific aims were to (1) compare lower lip thickness/eversion in patients with CLP to noncleft controls with maxillary hypoplasia, (2) determine differences between patients with unilateral CLP (UCLP) and bilateral CLP (BCLP), and (3) document changes in the lower lip that occur with Le Fort I advancement.
DESIGN: Retrospective case-control study.
SETTING: Tertiary care center.
PATIENTS/PARTICIPANTS: Patients with available pre- and postoperative CT scans and 2D lat

BACKGROUND: There is no accepted protocol for inpatient versus ambulatory cleft lip surgery. The aim of this study was to review the safety of outpatient repair and develop guidelines.
METHODS: A retrospective review of patients younger than 2 years undergoing primary cleft lip repair from 2008 to 2015 at six centers was performed. Patients were divided into two groups: predominantly ambulatory (discharged or admitted for specific concerns) and inpatient (admitted due to surgeon's preference). The impact of independent variables on admission, emergency department visits, and readmission withi

OBJECTIVE: Maxillary growth in patients with cleft lip and palate is highly variable. The authors' aim was to investigate associations between preoperative cleft lip measurements and maxillary growth determined cephalometrically in patients with complete unilateral cleft lip and palate (cUCLP).
DESIGN: Retrospective cross-sectional study.
PATIENTS: Children with cUCLP.
METHODS: Preoperative cleft lip measurements were made at the time of primary cheiloplasty and available for each patient. Maxillary growth was evaluated on lateral cephalometric radiographs taken prior to any orthodontic tre

OBJECTIVES: The upper lip's musculature comprises several muscle fiber groups with different spatial orientations for various lip movements. In the past, it has not been possible to describe these various groups and their motor units in detail with traditional methods of electromyography (EMG). The purpose of this investigation was to create a viable method for studying the architecture of upper-lip muscle fibers in patients with surgically closed cleft lip by refining the design of a flexible electrode array for multichannel EMG.
METHODS: Calibrated perioral photographs of 36 adult subjects

Congenital vascular anomalies most notably hemangiomas involving the lips, especially those which fail to regress, pose a difficult problem both for the surgeon and the patient. These lesions not only discolour the skin but may also distort the shape of the lip. When nonsurgical modalities fail to treat these lesions, surgery is chosen as the next viable step. In this article, we propose a well planned sequential surgical procedure for unilateral lesions. This approach in comparison with previously used procedures produces noticeably better results and fewer complications. During a period of 4

We describe the use of gas chromatography with mass spectrometry (GC-MS) in the investigation of a case of allergic contact dermatitis of the lip margins caused by a lip liner. Patch testing identified sensitization to para-tertiary-butylphenol-formaldehyde resin (PTBP-FR), a resin glue frequently used in leather work but rarely found in cosmetics. Investigation specified the para-tertiary-butylphenol (PTBP) component of the resin as the hapten responsible for the allergic contact dermatitis, as well as for associated depigmentation.

The cleft abnormality is the cause of underdevelopment and subsequent loss of function. Primary cleft surgery and surgery to correct the secondary deformities of previous non-functional repair should aim to restore normal anatomy and physiology, with an emphasis on muscle reconstruction of the lip and soft palate if normal facial development is to be encouraged.
