Lessons learned from Hurricane Andrew: recommendations for care of the elderly in long-term care facilities.

We report on the experience of a 500-bed, long-term care facility in Miami, Fla, which provides housing and nursing care units for patients--ranging from those who are independently ambulatory to those who are acutely ill and feeble--in preparing for, during, and in the immediate aftermath of Hurricane Andrew, which struck on August 24, 1992. The problems encountered included a massive influx of evacuated elderly to the facility, facility isolation, loss of electrical power, loss of running water, special dietary needs, and limited professional staffing due to personal property losses or loss

Beyond Technology Adoption: Examining Need-Adoption Gaps Across Care Tasks in Taiwan's Long-Term Care Facilities.

This study examined need-adoption gaps across 21 care tasks in Taiwan's residential long-term care facilities using a questionnaire survey of 284 facilities that included open-ended questions. Administrative tasks generally showed high need and adoption, while direct care tasks often showed low need and adoption. The largest gaps involved safety monitoring, pressure-injury management, and transfers. Stronger technological infrastructure was associated with lower odds of safety-monitoring and transfer gaps, while external information and networking capacity was associated with lower odds of a p

Primary care-based interventions for post-stroke follow-up for long-term care: a systematic review.

BACKGROUND: Life after stroke is managed by primary care to prevent recurrent stroke and ensure that stroke survivors live well within the community. Unfortunately, stroke survivors do not always receive timely follow-up from clinical services, which leads to unaddressed and hidden unmet needs. It is not clear what follow-up interventions would be helpful for stroke survivors in the community once discharged from specialist services. AIM: The aim of this systematic review was to investigate current pathways and interventions used in primary and community care to support patients in their live

Long-Term Care as a Global Challenge: The Role of the World Health Organization.

Long-term care is today one of the most pressing issues in public policy. Its relevance is increasing everywhere but mostly, due to population aging, in low- and middle-income countries. Despite the advances in the past decades regarding the implementation of long-term care systems in several countries, the COVID-19 pandemic showed their fragility and called to rethink long-term care around the world. Issues like quality, workforce shortages, and financial sustainability are key in countries with systems in place and for those who are embarked in the task of designing and establishing a long-t

The Impact of Hurricane Maria on Puerto Rico's Long-Term Care Settings.

In September 2017, Hurricane Maria struck Puerto Rico, devastating the island for years to come. Puerto Rico has a rapidly aging population and does not have the long-term care (LTC) infrastructure of the United States mainland. This research describes experiences of individuals working in, residing in, and providing disaster response to LTC settings in Puerto Rico during and after Hurricane Maria. From January to March 2019, N = 20 in-person interviews were conducted with nursing home staff in three Puerto Rican nursing homes; in-home caregivers; and individuals providing disaster response. T

A mathematical model of Clostridioides difficile transmission in long-term care facilities.

Clostridioides difficile, also known as C. difficile, is a prevalent cause of infectious diarrhea in United States healthcare facilities. Spread through the fecal-oral route and often through contact with spores on contaminated surfaces, C. difficile can cause severe diarrhea, stomach pain, and colitis. Most individuals can mount an effective immune response, but older populations, immunocompromised individuals, and those taking antibiotics have a higher risk of being colonized by C. difficile. While extensive research has been conducted in hospital-based settings to improve understanding of t

Call the experts: identifying stakeholders in the long-term care of youth with hypospadias.

Long-term follow-up for individuals with hypospadias remains a critical area of need, yet evidence-based guidelines for such follow-up are lacking, and the role of involvement of relevant experts is not yet established. Using our hypospadias-specific health-related quality of life conceptual framework and a subsequent qualitative study of prepubertal males and parents of males with hypospadias, we identified potential priorities for long-term follow-up of youth with hypospadias. Using thematic codes from our patient and parent interviews, we searched PubMed for relevant articles and identified

Identifying predictors of cognitive decline in long-term care: a scoping review.

BackgroundCognitive impairment can cause social, emotional, and financial burdens on individuals, caregivers, and healthcare providers. This is especially important in settings such as long-term care (LTC) homes which largely consist of vulnerable older adults. Thus, the objective of this study is to review and summarize current research examining risk factors of cognitive decline in older adults within LTC.MethodsThis scoping review includes primary observational research studies assessing within-person change in cognition over time in LTC or equivalent settings in high resource countries. A Nationwide Survey of Dementia Prevalence in Long-Term Care Facilities in Taiwan.

BACKGROUND: As the average life expectancy of global citizens has increased, the prevalence of dementia has increased rapidly. The number of patients with dementia has increased by 6.7 times, reaching 300,000 in the past three decades in Taiwan. To realize the latest actual situation, the need for institutional care for elderly patients with dementia, and also a reference basis for government agencies to formulate dementia-related care policies, we investigated the institutional prevalence of dementia. METHODS: We randomly sampled 299 out of the 1607 registered long-term care facilities inclu

Frontiers in Clinical Practice of Long-Term Care of Chronic Ventilatory Failure.

Home mechanical ventilation (HMV) is an effective long-term treatment for chronic hypercapnic respiratory failure. In addition to the established practice of providing HMV for the treatment of chronic ventilatory failure in slowly progressive neuromuscular and chest wall disease, there is accumulating evidence for improvement of quality of life and prolongation of survival by HMV in highly prevalent diseases like chronic obstructive pulmonary disease and ever-increasing obesity hypoventilation syndrome as well as rapidly progressive neuromuscular disease. The key concepts for successful HMV ar

Infected with Scabies Again? Focus in Management in Long-Term Care Facilities.

Scabies is a significant public health condition in long-term care facilities, plaguing even developed countries. Although treatments are available, eradication and control of scabies cases still remain a challenge due to delays in diagnosis and difficulties in maintaining preventive and surveillance measures. Prompt treatment of patients and their contacts that are affected, along with concomitant education of health staff and family members, are paramount. Environmental disinfestation is also a concern.

Shortcomings of prosthodontic rehabilitation of patients living in long-term care facilities.

Removable dentures are a non-invasive, cost-effective prosthodontic solution for the reduced dentition. Their intended purpose is the rehabilitation of harmonious oral function and aesthetics on a long-term basis. The prevalence of removable dentures among patients of advanced age is high and the quality of the dentures is often poor. The aim of this study was to find the most important shortcomings of removable dentures and address the main targets for improving the quality of prosthodontic rehabilitation. The records from dental check-ups in Austrian residential homes were analysed retrospec

The Microbiome and Osteosarcopenic Obesity in Older Individuals in Long-Term Care Facilities.

Increasing evidence points to a role of altered microbiota on inflammation, obesity, and other chronic conditions. This commentary addresses the connection between osteosarcopenic obesity syndrome, an impairment in bone, muscle, and adipose tissues that occurs concurrently, with the altered microbiota in elderly individuals, particularly those living in long-term care facilities. As elderly move to long-term care facilities, they experience changes in gut bacteria that might exasperate the underlying conditions such as osteosarcopenic obesity. These individuals have exponentially higher osteop

Long-term burden of care and radiation exposure in survivors of esophageal atresia.

BACKGROUND: Patients with esophageal atresia with or without tracheoesophageal fistula (EA/TEF) historically have had a high risk of neonatal mortality but the majority of patients are now expected to live into adulthood. However, the long-term burden of care among recent EA/TEF survivors has not been documented. METHODS: A single-institution retrospective review of newborns with EA/TEF treated from 2001-2005 was conducted, including initial and total hospitalization length of stay, and number of clinic visits and procedures requiring general anesthesia in the first three years of life. [Preoperational study for the improvement of hygiene conditions in long-term care facilities].

Hygiene is becoming more and more important in long-term care facilities. Long-term care facilities are subject to monitoring by the Public Health Service (PHS) and other authorities. For the PHS in Bavaria the Bavarian Health and Food Safety Authority (Landesamt für Gesundheit und Lebensmittelsicherheit, LGL) published a hygiene monitoring concept and there exists an inspection guide developed by a specialist department for nursing homes and institutions for the handicapped (Fachstelle für Pflege und Behinderteneinrichtungen, FQA). Because inspections are performed in multiprofessional teams,
