Lower extremity manifestations of Vibrio vulnificus infection.

Vibrio vulnificus is a potentially lethal marine bacterium that has not been previously described in podiatric literature. A review of the microorganism's characteristics, susceptible patient population, and lower extremity manifestations of infection is presented. V. vulnificus is found as part of the normal flora of the Gulf of Mexico, Atlantic, and Pacific coastal waters and is often isolated from the filter feeding shellfish of these regions. Its pathogenicity is generally reserved for the immunocompromised host, and is specifically related to disease states which exhibit high serum iron l

Arterial injuries associated with lower-extremity fractures.

The records of 110 patients with arterial injuries of the lower extremities associated with bone fractures, and managed in the authors' vascular surgery clinic, were reviewed. The majority (72%) were young men; 66 had blunt and 44 penetrating arterial injuries. A total of 92 patients were treated by saphenous vein interposition grafting, 12 with composite grafts and six with polytetrafluoroethylene grafts. The limb salvage rate was 93%. All eight amputations were in neglected cases with arterial injury at the popliteal or infrapopliteal level and associated with muscle loss.

Accuracy of lower extremity arterial duplex mapping.

We performed lower extremity arterial duplex mapping from the aortic bifurcation to the ankle in 150 consecutive patients evaluated for aortic and lower extremity arterial reconstruction and compared lower extremity arterial duplex mapping in a blinded fashion to angiography. On the basis of history, physical examination, and four-cuff segmental Doppler pressures individual lower extremities were classified as normal, isolated aortoiliac disease, infrainguinal disease, and multilevel inflow and outflow disease. For vessels proximal to the tibial arteries, lower extremity arterial duplex mappin

Bipedicled fasciocutaneous flaps in the lower extremity.

It is well known that a bipedicled skin flap permits survival of longer flaps due to the secondary recruitment of vascularity. Inclusion of the deep fascia with such a flap, obeying the principles of the single-pedicled fasciocutaneous flap, provides even greater security for the immediate transposition of yet larger or riskier flaps without the need for delay maneuvers. This variation is especially valuable for the management of difficult wounds encountered in the lower extremity when no other local options may be available. Thirteen local bipedicled fasciocutaneous flaps including both verti

Amputation of the lower extremity: an overview.

Amputation of the lower extremity may be traumatic or a necessary procedure to prevent further morbidity and possibly death in a patient. Advances have been made in the area of lower extremity amputation surgery and subsequent management. Prosthetic design has resulted in more lightweight, better fitting prostheses that more closely resemble the workings of a normal leg and foot. This results in a decrease in energy expenditure with walking and allows an increased activity level for amputees. This article addresses factors concerning phantom limb sensations and pain, prosthesis use and nursing

Soft-tissue infection in lower-extremity trauma.

Soft-tissue infection after lower-extremity trauma has not been studied in detail in light of recent data on the biology of infection. This article examines specific problems in lower-extremity trauma that allow the wound to become susceptible to wound infection. It also illustrates the various principles of wound management in lower-extremity trauma that serve to prevent infection. Two case examples are used to illustrate principles of management. Other wound problems in lower-extremity trauma are also discussed, such as rabies, necrotizing soft-tissue infection, tetanus, and diabetic foot in

Nerve compression syndromes of the lower extremity.

Nerve compression syndromes of the lower extremity present a challenge in differential diagnosis. Compression of the common peroneal nerve occurs relatively frequently; compression of the sciatic nerve occurs infrequently. The pattern of weakness helps distinguish lumbar root entrapment from peripheral compression syndromes. Compression syndromes must also be differentiated from diabetic, alcoholic or vasculitic neuropathy. Recovery correlates with the degree and duration of nerve injury. Thus, early diagnosis and treatment are important.

Free tissue transfer to the lower extremity.

The results of 100 free microvascular transfers to the lower limb are presented. These show a high success rate with successful salvage of 95 limbs. The major complications were infection and delayed healing, osteomyelitis, and non-union. Early skin cover, within 14 days of injury, reduced the incidence of complications by half. Aggressive debridement and provision of early skin cover should be the aim of management of tibial trauma with tissue loss.

Scurvy: bilateral lower extremity ecchymoses and paraparesis.

A 46-year-old man presented with two weeks of progressive paraparesis and large confluent ecchymoses of both thighs. There was a history of poor dietary intake and daily alcohol use. He had had similar problems each of the past two winters. In 1979 he required hospitalization for "sciatica." In 1980 he was bedridden for two months because of lower extremity weakness. Examination revealed poorly fitting dentures, large confluent ecchymoses of both thighs, perifollicular hemorrhages, and a low serum ascorbic acid concentration. Oral ascorbic acid was begun in the hospital and all clinical sympto

Assessment of a lower extremity training program.

The response to a lower extremity program has been studied in workmen undergoing progressive rehabilitation following injury to the lower leg or ankle region. When first seen, the isometric strength of the injured limb was substantially reduced relative to its counterpart, and aerobic power was also poor, relative to normal standards. Substantial gains of isometric strength occurred over a four- to six-week period of therapy. Measurements above the site of injury suggested a 13% gain of strength, and measurements below the injury indicated a gain of 34%. On the other hand, there were minimal c

Lower extremity arterial injury patterns and reconstructive outcomes in patients with severe lower extremity trauma: a 26-year review.

BACKGROUND: Management of severe traumatic lower extremity injuries remains a considerable challenge. Free tissue transfer is now a standard part of reconstruction for Gustilo IIIB and IIIC injuries. There is limited information on arterial injury patterns in this population. We undertook a review of our experience to gain insight on vascular injury patterns and surgical outcomes. STUDY DESIGN: A 26-year retrospective analysis was performed of all lower extremity Gustilo IIIB and IIIC injuries requiring microvascular reconstruction at New York University Medical Center. Patient demographics,

The Ankle-Pelvic Angle (APA) and Global Lower Extremity Angle (GLA): Summary Measurements of Pelvic and Lower Extremity Compensation.

BACKGROUND: Adult sagittal spinal deformity (SSD) leads to the recruitment of compensatory mechanisms to maintain standing balance. After regional spinal compensation is exhausted, lower extremity compensation is recruited. Knee flexion, ankle flexion, and sacrofemoral angle increase to drive pelvic shift posterior and increase pelvic tilt. We aim to describe 2 summary angles termed ankle-pelvic angle (APA) and global lower extremity angle (GLA) that incorporate all aspects of lower extremity and pelvic compensation in a comprehensive measurement that can simplify radiographic analysis. A preliminary multifactorial approach describing the relationships among lower extremity alignment, hip muscle activation, and lower extremity joint excursion.

CONTEXT: Multiple factors have been suggested to increase the risk of faulty dynamic alignments that predict noncontact anterior cruciate ligament injury. Few researchers have examined this relationship using an integrated, multifactorial approach. OBJECTIVE: To describe the relationship among static lower extremity alignment (LEA), hip muscle activation, and hip and knee motion during a single-leg squat. DESIGN: Descriptive laboratory study. SETTING: Research laboratory. PATIENTS OR OTHER PARTICIPANTS: Thirty men (age= 23.9± 3.6 years, height =178.5± 9.9 cm, mass= 82.0± 14.1 kg) and 30 wo

[Non-Unions of the lower extremities].

Non-Union is still a frequent and severe complication in fracture treatment and represents a major challenge for orthopedic trauma surgery. Due to chronic pain and loss of mobility, pseudarthrosis can lead to a considerable impairment of the individual quality of life and the often long duration of treatment represents a high socioeconomic burden. The lower extremities are most frequently affected due to the high biomechanical stress. A multimodal concept combining diagnostics and treatment is necessary to adequately address this complex clinical condition.

Fibromuscular dysplasia of the lower extremities.

Fibromuscular dysplasia (FMD) is a nonatherosclerotic, non-inflammatory vascular disease that mainly affects the renal and internal carotid arteries. Involvement of other sites, including arteries of the extremities, is uncommon, and only a few histologically confirmed cases have been reported. FMD of the arteries of the extremities can result in ischemia requiring surgical or endovascular reconstruction. In the present report, two cases of FMD are described: one case of femoropopliteal artery occlusive disease, and one case of nonsymptomatic progression of external iliac artery dissection, bo

CT angiography of the lower extremities.

CT angiography (CTA) of the lower extremities has evolved into a robust noninvasive angiographic technique with the advent of 16 and 64 multidetector computed tomographic systems and advances in system design. CTA has displaced conventional catheter arteriography in a large range of applications and is predominantly used in the evaluation of atherosclerotic peripheral arterial occlusive disease in symptomatic patients who are candidates for intervention. Other disease entities including atheroembolism and thromboembolism, aneurysmal disease, and arteritides including Buerger disease and Takaya

Soccer injury in the lower extremities.

Information about soccer injuries is required to develop prevention and rehabilitation programmes. Most soccer injuries occur in the lower extremities. This type of injury is reviewed here. Definitions of injury, injury rate, injury percentage, mechanism of injury, anatomical region of injury, type of injury, and severity of injury are summarised. In each section, a description and summary of the data are provided. Finally, the limitations of the studies and suggestions to improve the investigation of soccer injuries are provided.
