Extreme ST-segment elevations in seemingly no significant angiographic coronary artery abnormalities: a case report.

BACKGROUND: Obstructive coronary artery disease is found in approximately 97% of patients presenting with ST-elevation myocardial infarction and 92% of patients with non ST-elevation myocardial infarction (Bainey KR, Welsh RC, Alemayehu W, Westerhout CM, Traboulsi D, Anderson T, et al. Int J Cardiol 264: 12-17, 2018). Recent studies showed that myocardial infarction without obstructive coronary atherosclerosis (MINOCA) is also associated with a long-term risk of adverse events (Bainey KR, Welsh RC, Alemayehu W, Westerhout CM, Traboulsi D, Anderson T, et al. Int J Cardiol 264: 12-17, 2018).. Th

Telomere-telomerase system status in patients with acute myocardial infarction with ST-segment elevation - relationship with oxidative stress.

INTRODUCTION: Telomeres are protective chromosomal ends. Short telomeres are a proven biomarker of biological aging. We aimed to find an association of telomere length and telomerase activity in circulating leukocytes and thromboaspirates of patients with acute myocardial infarction. Furthermore, association of the telomere-telomerase system with oxidative stress markers (as common risk factors for coronary artery disease (CAD)) was tested. MATERIAL AND METHODS: Patients were selected from the patients admitted to the intensive care unit with acute myocardial infarction with ST-segment elevat

Medical Therapy for Secondary Prevention and Long-Term Outcome in Patients With Myocardial Infarction With Nonobstructive Coronary Artery Disease.

BACKGROUND: Myocardial infarction with nonobstructive coronary arteries (MINOCA) occurs in 5% to 10% of all patients with myocardial infarction. Clinical trials of secondary prevention treatment in MINOCA patients are lacking. Therefore, the aim of this study was to examine the associations between treatment with statins, renin-angiotensin system blockers, β-blockers, dual antiplatelet therapy, and long-term cardiovascular events. METHODS: This is an observational study of MINOCA patients recorded in the SWEDEHEART registry (the Swedish Web-system for Enhancement and Development of Evidence-b

Cancer and Myocardial Infarction in Women.

Women who present with myocardial infarction (MI) are more likely to be diagnosed with nonobstructive coronary arteries (MINOCAs), spontaneous coronary artery dissection (SCAD), and takotsubo syndrome (TS) than men. Malignancy may predispose to MI and TS through shared risk factors and inflammatory mediators. This study aimed to determine the prevalence of cancer in women presenting with clinical syndrome of MI and the association between cancer and mechanism of MI presentation. Among 520 women with MI who underwent coronary angiography at NYU Langone Health from March 2016 to March 2020 or Se

A rare cause of myocardial infarction with non-obstructive coronary arteries-case report of ST-segment elevation myocardial infarction caused by a mediastinal mass.

INTRODUCTION: ST-segment elevation myocardial infarction (STEMI) is attributable to an occluded coronary artery in almost 90% of patients. Accordingly, restoration of coronary perfusion as early as possible, preferably with primary percutaneous coronary intervention, is the recommended treatment by the European Society of Cardiology, to maximise myocardial salvage. However, not all cases of STEMI are because of coronary artery occlusion. ST-segment elevation myocardial infarction that occurs in the absence of obstructive coronary artery disease on angiography has been termed myocardial infarct

[Myocardial infarction with healthy coronaries].

No significant coronary artery lesion is seen on an emergent coronary angiogram in 5 to 10% of patients with myocardial infarction. These patients are most often female, younger and with less risk factors for atherosclerotic disease. Causes of myocardial infarction with no significant coronary artery stenosis (MINOCA) include plaque rupture, coronary artery spasm and coronary microvascular lesions. MINOCA is diagnosed using clinical (prolonged chest pain) ECG and biological (rise in troponin) data. A coronary angiogram is mandatory to confirm the absence of significant coronary artery lesion.

STEMI: rare initial presentation of a mediastinal mass supplied by the RCA.

In the setting of severe septic shock, a 70-year-old woman had an ST segment myocardial infarction with ST elevations in the inferolateral leads. On cardiac catheterisation, no obstructive pathology was noted. Chest imaging revealed a large mediastinal mass measuring 8.5×6.5×7.5 cm in the visceral compartment of the mediastinum, with contrast enhancement from the right coronary artery (RCA). A biopsy was preformed and cytology was consistent with a well-differentiated neuroendocrine neoplasm. On review of the cardiac catherisation, it was noted that the mass was deriving blood supply from the

Spectral cardiac CT in type 1 Kounis syndrome presenting as myocardial infarction with non-obstructive coronary arteries following anaphylactic shock.

Myocardial infarction with non-obstructive coronary arteries MINOCA poses a diagnostic challenge, particularly in complex clinical settings. We report the case of a 62-year-old woman who developed acute myocardial injury during severe anaphylactic shock following a Hymenoptera sting. Although she did not experience chest pain, electrocardiography revealed ST-segment elevation and cardiac biomarkers were markedly elevated. Coronary computed tomography angiography CCTA excluded obstructive coronary artery disease. Notably, delayed dual-energy spectral imaging demonstrated myocardial late iodine

Hyperuricemia as a prognostic marker for long-term outcomes in patients with myocardial infarction with nonobstructive coronary arteries.

BACKGROUND: Hyperuricemia (HUA) has been proved as a predictor of worse outcomes in patients with coronary artery disease. Here, we investigated the prognostic value of HUA in a distinct population with myocardial infarction with nonobstructive coronary arteries (MINOCA). METHODS: A total of 1179 MINOCA patients were enrolled and divided into HUA and non-HUA groups. HUA was defined as a serum uric acid level ≥ 420 μmol/L in men or ≥ 357 μmol/L in women. The primary study endpoint was a composite of major adverse cardiovascular events (MACE), including all-cause death, nonfatal MI, nonfatal st

Takotsubo syndrome after receiving the COVID-19 vaccine.

The coronavirus disease (COVID)-19 pandemic has affected millions worldwide with prevention efforts culminating in the development of a vaccine. An mRNA vaccine, developed by Moderna (Cambridge, MA, USA), mounts an immunologic response leading to antibody neutralization. Commonly reported vaccine side effects include myalgia, fever, and chills, with low reported rates of cardiovascular events. This case demonstrates the development of takotsubo syndrome (TTS) after administration of the COVID-19 vaccine. A 73-year-old woman with recently diagnosed myocardial infarction with no obstructive coro

Stress Cardiac MRI in Women With Myocardial Infarction and Nonobstructive Coronary Artery Disease.

BACKGROUND: In a prospective study, cardiac MRI (CMR) and intravascular ultrasound were performed in women with myocardial infarction (MI) and nonobstructive coronary artery disease (MINOCA). Forty participants underwent adenosine-stress CMR (sCMR). HYPOTHESIS: Abnormal perfusion may co-localize with ischemic late gadolinium enhancement (LGE) and T2-weighted signal hyperintensity (T2+), suggesting microvascular dysfunction contributed to MI. METHODS: Qualitative perfusion analysis was performed by 2 independent readers. Abnormal myocardial perfusion reserve index (MPRI) was defined as global

A Stepwise Diagnostic Approach to Transmural Myocardial Infarction With Non-obstructive Coronary Arteries With Microvascular Obstruction Highlighting the Role of Cardiac MRI in Suspected Thrombolysis.

Myocardial infarction with non-obstructive coronary arteries (MINOCA) presents a diagnostic challenge due to its heterogeneous etiologies and the absence of obstructive coronary lesions. We report the case of a 74-year-old male with hypertension, prior deep vein thrombosis, and a history suggestive of thrombophilia under evaluation, on chronic warfarin therapy, who presented with chest pain, ST elevation, and elevated troponins. Coronary angiography revealed no obstructive coronary artery disease. Cardiac magnetic resonance imaging (MRI) demonstrated an acute transmural myocardial infarction w

Comparison of Patients With Nonobstructive Coronary Artery Disease With Versus Without Myocardial Infarction (from the VA Clinical Assessment Reporting and Tracking [CART] Program).

Comparisons of the outcomes of patients with myocardial infarction with nonobstructive coronary artery disease (MINOCA) and patients with nonobstructive coronary artery disease (CAD) without myocardial infarction (MI) are limited. Here we compare the outcomes of patients with MINOCA and patients with nonobstructive CAD without MI and assess the influence of medical therapy on outcomes in these patients. Veterans who underwent coronary angiography between 2008 to 2017 with nonobstructive CAD were divided into those with or without pre-procedural troponin elevation. Patients with prior revascula

Clinical, angiographic and echocardiographic correlates of epicardial and microvascular spasm in patients with myocardial ischaemia and non-obstructive coronary arteries.

BACKGROUND: Coronary vasomotor dysfunction represents an important mechanism responsible for myocardial ischaemia in patients with non-obstructive coronary artery disease (CAD). The use of invasive provocative tests allows identifying patients with epicardial or microvascular spasm. Of note, clinical characteristics associated with the occurrence of epicardial or microvascular spasm have still not completely clarified. Coronary Inflammation and Cardiovascular Events in Patients Without Obstructive Coronary Artery Disease.

PURPOSE OF REVIEW: This review evaluates the role of vascular inflammation in patients who develop myocardial infarction with non-obstructive coronary arteries (MINOCA). It also introduces pericoronary adipose tissue (PCAT) and epicardial adipose tissue (EAT) as possible biomarkers for risk prediction in patients with non-obstructive coronary artery disease (CAD). RECENT FINDINGS: PCAT and EAT contribute to the development and progression of coronary artery inflammation and plaque vulnerability. Coronary computed tomography angiography (CCTA) can detect localized areas of inflammation through

Effects of Impella support level on coronary blood flow velocity measured by transthoracic Doppler echocardiography.

UNLABELLED: Previous invasive studies on the Impella device (Abiomed, Danvers, MA, USA) have reported that its effect increases coronary artery perfusion. However, non-invasive evaluation using transthoracic Doppler echocardiography (TTDE) remains limited. We assessed the effects of different Impella support levels on coronary blood flow velocity using TTDE in four patients: two with ST-elevation myocardial infarction (STEMI), one with acute myocarditis, and one with myocardial infarction with non-obstructive coronary arteries (MINOCA) due to coronary spasm.
