A Cost-Effective Strategy for Mandibular Reconstruction: Fibula Flap With Existing Titanium Plates.

With the rapid advancement of digital and microsurgical techniques, immediate autologous bone grafting following resection of benign and malignant mandibular tumors has become a standard treatment. However, some patients are unable to undergo immediate autologous bone grafting due to various reasons. In such cases, titanium plate implantation (or combined with soft tissue free flap such as anterolateral thigh flap) serves as an alternative to prevent airway compression and maintain facial shape and function. This article reports a case of a patient, referred to as patient 1, who experienced a

Pediatric Mandibular Reconstruction With Free Serratus Anterior-Rib Composite Flap: A Case Report.

Reconstruction of children's mandibles after mandibular resection for benign or malignant tumors is challenging. Microvascular flap reconstruction is a common treatment option for restoring mandibular continuity after the resection of oral cavity neoplasms. We presented 2 cases of childhood mandibular reconstruction after tumor wide excision and segmental mandibulectomy, one for malignant cancer and one for benign ossifying fibroma, with serratus-rib composite free flap. All 2 patients had a favorable facial profile, functional outcome, and dental occlusion at the last follow-up. The developmen

Biomechanical analysis of mandibular defect reconstruction based on a new base-fixation system.

Buccal titanium plate fixation is a common method for repairing mandibular defects. However, the method presents certain problems: the requirement of a large volume of titanium plate, a large number of fixation screws, a lengthy duration of the surgical operation, and exposure of the fixation plate which easily causes wound infection. In this study, a new base-fixation system was designed. Mandibular reconstruction was performed using the three-dimensional reconstruction package Mimics. In order to compare the newly designed base-fixation system and the common buccal-fixation system, the stres

Biomechanical validation of structural optimized patient-specific mandibular reconstruction plate orienting additive manufacturing.

BACKGROUND AND OBJECTIVE: Owing to the unexpected in vivo fracture failure of the original design, structural optimized patient-specific mandibular reconstruction plates (PSMRPs) were created to boost the biomechanical performance of bridging segmental bony defect in the mandibular reconstruction after tumor resection. This work aimed to validate the biomechanical benefit of the structural optimized PSMRPs relative to the original design and compare the biomechanical performance between PSMRP1 with generic contour customization and PSMRP2 with a tangent arc upper margin in mandibular angle reg

Reconstruction of Anterior Mandibular Defect Using Submental Island Flap Pedicled With Mental Artery.

Gingival carcinoma is a common malignant tumor occurring in the anterior area of the mandible, which can be derived from the epithelium of gingival mucosa. Surgical extended resection is the main treatment of gingival cancer, which can lead to anterior mandibular defect including mouth floor and mandible and mucosa of lower lip. According to the size of the defect, the common repair method is free musculocutaneous flap with vascular pedicle or pedicle flap. We present a method of repairing mandibular anterior tooth defect with an island flap pedicled with the mental artery.

Objectifying growth of vascularized bone transfers after mandibular reconstruction in the pediatric population.

BACKGROUND: Autologous vascularized bone transfer is the preferred strategy for the reconstruction of mandibular defects in a pediatric population. The principal argument is the theoretical postoperative growth potential of the neomandible, which uses vascularized donor tissues. OBJECTIVES: The purpose of this study was to objectify the veritable growth potential of vascularized bone transfers in children. METHODS: A literature search was performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, revealing 57 patients younger than or equal to 18

Twelve-year experience in mandibular reconstruction using osteo-muscular dorsal scapular pedicled flaps.

Primary mandibular reconstruction after tumor removal or osteoradionecrosis treatment is a standard procedure. The most common reconstruction techniques are fibula, scapula, and iliac crest free flaps. Nevertheless, all patients are not eligible for microsurgery. In this study, we assess 12 years of mandibular reconstruction using an osteo-muscular dorsal scapular pedicled flap (OMDS). We included 40 patients operated on using an OMDS flap. We collected parameters such as length of hospital stay, recurrence risk, and need for secondary flap for oral cutaneous fistula (OCF) treatment. Flap bone

Expanding the Utilization of the Osteocutaneous Radial Forearm Free Flap beyond Mandibular Reconstruction.

Background The osteocutaneous radial forearm free flap (OCRFFF) for mandibular reconstruction has been well described. Despite this flap's utility in the repair of such defects, the indications for the OCRFFF have continued to expand in recent years. The advantages of the OCRFFF allow for a high degree of versatility in the reconstruction of the various anatomical and aesthetic units of the head and neck. In this review, the authors aim to explore the successful utilization of the OCRFFF beyond the reconstruction of composite mandibular defects. Methods A retrospective chart review was perform

Mandibular reconstruction with a bioactive-coated cementless Ti6Al4V modular endoprosthesis in Macaca fascicularis.

The titanium mandibular modular endoprosthesis fixed with polymethylmethacrylate cement in the medullary space of the mandible has been introduced in previous studies. However, the internal parts of these devices have been found to be prone to loosening and wound dehiscence. The current study introduces a newly designed bioactive-coated cementless modular mandibular endoprosthesis, which was used for reconstruction in Macaca fascicularis. The devices were coated with hydroxyapatite (HA) and hydroxyapatite/bioglass (HA/BG) and implanted in unilateral mandibular segmental defects in nine monkeys

[Long-term results of mandibular reconstruction with avascular spongiosa transfer and titanium mesh].

Severe and mild complications could only be detected in damaged transplant bed (radical operation and radiation exposure, traumatic lesions) in osteoplastic reconstruction of the mandible with avascular cancellous bone transfer using a titanium carrier. Altogether, we rate this method as more favorable compared to an avascular block transplant because of advantages in handling the system in the surgical technique, contouring of the problem zones in the chin and mandibular angle region, more simple transplant extraction and greater resistance to infections. Owing to the high osteogenic potency

Histological evaluation of free revascularized iliac crest bone after transplantation for mandibular reconstruction.

Differing views on the nutritional status of revascularized bone in animal studies and in human transfer have been expressed. Normal viability is reported as well as single cell necrosis and devitalized areas and, finally, necrosis of the complete graft. From 19 patients with revascularized bone transfer into injured recipient tissues, we obtained 32 drill biopsies out of the centre of the graft and near the mandibular stump during the removal of the osteosynthesis plates. The histological preparations were examined for bone viability. In 58% of all patients, there occurred partial necroses in
