Work-related injuries of doctors of chiropractic in the United States.

PURPOSE: This study was undertaken to determine the prevalence and types of work-related injuries among a nationwide sample of chiropractors and to identify factors associated with these injuries. METHODS: Using a survey, 1000 randomly selected doctors of chiropractic in the United States were asked to record their 3 most serious injuries. Details were obtained about the type of injury, area of the body affected, activity performed at that time, year of practice when the injury had occurred, if the doctor had needed to take time off from work, and what they had changed as a result of the inju

Chiropractic management of patients post-disc arthroplasty: eight case reports.

BackgroundWhen conservative therapies for low back pain (LBP) are not effective, elective surgery may be proposed to these patients. Over the last 20 years, a new technology, disc replacement, has become increasingly popular because it is believed to maintain or restore the integrity of spinal movement and minimize the side-effects compared to fusion. Although disc replacement may relieve a patient from pain and related disability, soreness and stiffness of the lumbopelvic region seem to be common aftermaths of the surgery. This prospective case series was undertaken to identify and describe p

Radiculomedullary complications of cervical spinal manipulation.

Spinal manipulation is commonly used by some therapists for the treatment of cervical pain. Flexion-extension of the cervical spine produces sliding movements of one vertebra over the one below it, which leads to physiological reduction in the antero-posterior diameter of the spinal canal. Spinal manipulation provokes movements that exceed the physiological limits of these articulations and thereby lead to a more significant reduction of the canal diameter. In patients with pre-existing stenosis of the canal or those with vertebral instability, these movements may cause (or aggravate) myelopat

From mannequins to humans - are manual therapy motor skills transferable? A mixed-methods study.

BACKGROUND: Manual therapy, including spinal manipulative therapy (SMT), is a core component of chiropractic education. Simulation tools such as the Human Analogue Mannequin (HAM®) have been developed and coupled with force-sensing technology to support safe and structured training of SMT force-time characteristics. However, it remains unclear whether motor skills acquired using these tools are transferable to real-world scenarios. METHODS: This sequential explanatory mixed-methods observational study investigated whether chiropractic students, proficient at applying a set of pre-specified SMT

Prevalence of headache consultations in Danish chiropractic practice: A cross-sectional study.

BackgroundHeadache is a leading cause of disability and a major public health problem worldwide. Although most patients with headaches are managed in primary care settings, including chiropractic clinics, there are few studies describing the prevalence of headache and headache diagnoses in primary care. The present study aimed to: (1) estimate the prevalence of headache consultations in chiropractic practice; (2) estimate the prevalence of different headache diagnoses used in chiropractic practice; and (3) determine the association between the prevalence of headache consultations and chiroprac

Veteran Response to Dosage in Chiropractic Therapy (VERDICT): Study Protocol of a Pragmatic Randomized Trial for Chronic Low Back Pain.

BACKGROUND: Low back pain is a leading cause of disability in veterans. Chiropractic care is a well-integrated, nonpharmacological therapy in Veterans Affairs health care facilities, where doctors of chiropractic provide therapeutic interventions focused on the management of low back pain and other musculoskeletal conditions. However, important knowledge gaps remain regarding the effectiveness of chiropractic care in terms of the number and frequency of treatment visits needed for optimal outcomes in veterans with low back pain. DESIGN: This pragmatic, parallel-group randomized trial at four

Cost comparison of two approaches to chiropractic care for patients with acute and sub-acute low Back pain care episodes: a cohort study.

BACKGROUND: Low back pain (LBP) imposes a costly burden upon patients, healthcare insurers, and society overall. Spinal manipulation as practiced by chiropractors has been found be cost-effective for treatment of LBP, but there is wide variation among chiropractors in their approach to clinical care, and the most cost-effective approach to chiropractic care is uncertain. To date, little has been published regarding the cost effectiveness of different approaches to chiropractic care. Thus, the current study presents a cost comparison between chiropractic approaches for patients with acute or su

The UQTR university-based chiropractic clinic cohort (UQTRChiCo): description and summary of an available data source for research collaborations.

BACKGROUND: Musculoskeletal disorders are the leading cause of disability in Canada, and chiropractic care is among the most commonly used healthcare services for their management. Despite this, little is known about the quality of care and adherence to clinical practice guidelines in chiropractic educational settings. This study aimed to describe the Université du Québec à Trois-Rivières university-based chiropractic clinic cohort (UQTRChiCo) and summarize its patient characteristics, care provided, and available outcome data. METHODS: We conducted a retrospective cohort study of patients wh

Utilization of chiropractic services in patients with osteoarthritis and spine pain at a publicly funded healthcare facility in Canada: A retrospective study.

BACKGROUND: Osteoarthritis (OA) is one of the most prevalent and disabling musculoskeletal diseases worldwide. There is preliminary evidence from experimental studies and consensus documents that chiropractic management may alleviate spine and/or extremity OA related pain in the short term. OBJECTIVE: This research explores the potential relationship of a pragmatic course of care, including soft tissue therapy, spinal manipulation, and other treatments commonly delivered by chiropractors, to spine and extremity pain in patients with OA. METHODS: A retrospective analysis of prospectively coll

Hallux rigidus.

Hallux rigidus is a common problem of the first metatarsophalangeal joint and is particularly common in the 31-69 year old age group. Loss of articular cartilage narrowing of joint space and formation of periarticular osteophytes are present and increase over time, often leading to palpable osteophyte formation. The authors suggest that a diagnosis of hallux rigidus be made if at least 4 of the following are present at the big toe: pain, stiffness, palpable exostosis, positive X-ray findings, positive axial grind test, occasional synovitis, decreased motion on motion palpation (particularly do

Reinnervation of the paralyzed diaphragm: application of nerve surgery techniques following unilateral phrenic nerve injury.

BACKGROUND: Unilateral phrenic nerve injury often results in symptomatic hemidiaphragm paralysis, and currently few treatment options exist. Reported etiologies include cardiac surgery, neck surgery, chiropractic manipulation, and interscalene nerve blocks. Although diaphragmatic plication has been an option for treatment, the ideal treatment would be restoration of function to the paralyzed hemidiaphragm. The application of peripheral nerve surgery techniques for phrenic nerve injuries has not been adequately evaluated. METHODS: Twelve patients presenting with long-term, symptomatic, unilate

The contemporary model of vertebral column joint dysfunction and impact of high-velocity, low-amplitude controlled vertebral thrusts on neuromuscular function.

PURPOSE: There is growing evidence that vertebral column function and dysfunction play a vital role in neuromuscular control. This invited review summarises the evidence about how vertebral column dysfunction, known as a central segmental motor control (CSMC) problem, alters neuromuscular function and how spinal adjustments (high-velocity, low-amplitude or HVLA thrusts directed at a CSMC problem) and spinal manipulation (HVLA thrusts directed at segments of the vertebral column that may not have clinical indicators of a CSMC problem) alters neuromuscular function. METHODS: The current review

Chiropractic care for foot drop due to peroneal nerve neuropathy.

Peroneal nerve palsy is the most common entrapment neuropathy in the lower extremity, and the presentation of foot drop is a frequent initial manifestation of this disorder. This condition can have a marked influence on the patient's activities of daily living, and is classified as 'Impairment of Body Structure.' Current literature provides little direction to its evaluation and management, and the importance to differentially diagnose the causes of foot drop. Therefore, the purpose of this case report is to describe the application of chiropractic manipulative therapy, for a 14-year-old femal
