INTRODUCTION: Sexual function following genital sexual reassignment surgery (SRS) is an important outcome for many transsexuals, affecting the choice of surgical technique, satisfaction with surgery, and quality of life. However, compared to other outcome measures, little clinical and research attention has been given to sexual functioning following SRS.
AIM: To discuss the potential impact of cross-sex hormone therapy and SRS on sexual function and to summarize the published empirical research on postsurgical sexual functioning in male-to-female (MtF) and female-to-male (FtM) transsexuals.

The authors from the Florence Forensic Department present a case that demonstrates the paradigms attached to accidental deaths while performing autoerotic maneuvers. The incidents of such practices are underestimated and are only the tip of the iceberg since they do not represent the cases that are never reported due to successful practice. After analyzing the statistic data, the authors describe the case and discuss about the element that prove the accidental nature of the death and the importance of the correct application of forensic methodology at the scene and in the mortuary.

A 36-year-old man was hospitalized for self-insertion of a metallic body into the urethra. The self-inserted foreign body (SFB) was removed by endoscopic means; psychiatric evaluation of the patient showed a disturbed schizoid personality. After a review of the current literature, including clinical features, diagnostic manoeuvres and methods of treatment of SFB, the authors suggest that a psychiatric evaluation is always recommended in order to discover mental underlying disorders, thus reducing the risk of recurrence.

In patients treated for prostatic carcinoma with oestrogen, orchiectomy or external radiation, serum concentrations of testosterone, LH, FSH, prolactin, TeBG and oestradiol-17 beta as well as changes in sexual behaviour and mental mood were studied. OBJECTIVE: To report two cases of urethral foreign body. Although this condition is not uncommon, few cases are reported in the literature.
METHODS/RESULTS: Two cases of urethral foreign body are described: a 74-year-old male with mental disorder who presented with acute urinary retention arising from a fragment of a metal tube in the bulbous urethra and a 34-year-old female with intense micturition syndrome arising from a calcified electrical cable located in the urethra. This article argues that that there are two important reasons why many potential donors refrain from donating and why many donors value not being outed as a sperm donor. Kleine-Levin syndrome is characterized by periodic hypersomnia, hyperphagia, sexual disinhibitions and behavioral disturbances. The prognosis is generally benign, with normal cognitive and social functions after the episodes. We describe a typical case of Kleine-Levin syndrome associated with apparent academic decline, neuropsychological sequelae and personality alterations after the second episode of the illness. Further research in the natural history of Kleine-Levin syndrome is needed, for example, to determine whether early intervention would improve long-term prognosis.

Abnormal sexual behavior occurs in 30-40% of patients with Tourette's syndrome (TS). Two patients with TS who exhibited distressing abnormal sexual behavior experienced amelioration of symptoms with administration of the oral opiate receptor antagonist naltrexone (TrexanR). Conventional anti-TS drugs including haloperidol and clonidine were ineffective. Abnormal sexual behavior may be another feature of the disease responding to opiate blockers.
