Vascular access care and treatment practices associated with outcomes of arteriovenous fistula: international comparisons from the Dialysis Outcomes and Practice Patterns Study.

BACKGROUND: Vascular access (VA) guidelines recommend the native arteriovenous fistula (AVF) as VA of first choice for chronic hemodialysis patients. AVF management is important in hemodialysis patient care. AVF survival is associated with various physical factors, but the effects of dialysis treatment factors upon AVF survival are still not clear. METHODS: Study patients were treated at 498 dialysis facilities participating in the Dialysis Outcomes and Practice Patterns Study (DOPPS) 2 or 3 (2002-2007). Analyses included 1,183 incident hemodialysis patients (on dialysis ≤7 days and using an

Evaluation of Clinical Meaningfulness of Fortasyn Connect in Terms of "Time Saved".

Assessment of meaningfulness in randomized clinical trials (RCTs) in Alzheimer's disease (AD) is challenging, particularly in early disease. Converting clinical outcomes to disease progression time allows assessment of treatment effects using a metric that is understandable and meaningful: time. We demonstrate time savings assessments using meta time component tests (TCTs) in the LipiDiDiet multinutrient RCT. Dietary patterns are important for dementia prevention, likely due to individual cumulative nutrient effects. LipiDiDiet used a multinutrient (Fortasyn Connect) formulation in patients wi

Uncommon indications in chronic pain for intrathecal drug delivery systems: a narrative review of the literature.

Intrathecal drug delivery systems (IDDS) have been used successfully for a number of refractory chronic pain conditions. The most common pain indications for the use of IDDS include cancer-related pain and postlaminectomy syndrome. IDDS can also be used to treat a variety of other less common refractory pain conditions, including central neuropathic pain, peripheral neuropathic pain and craniofacial pain. The aim of this article was to review the existing literature on the efficacy and clinical utility of IDDS for the management of uncommon pain indications, including spinal cord injury-relate

A National Survey of Health Care Professionals' Use and Perception of Patient-Reported Outcomes (EORTC QLQ-C15-PAL) in the Patient Trajectory in Specialized Palliative Care.

ObjectivePatient-reported outcome measures (PROMs) and their integration into clinical practice are important topics in health care. However, little is known about health care professionals' (HCPs) perception and use of PROMs. We therefore investigated the use of PROMs by HCPs in specialized palliative care (SPC) and how PROM data are perceived by these HCPs.MethodA survey was sent to the heads of SPC services in Denmark (N = 41), asking them to distribute the survey to their HCPs with patient contact.ResultsThe survey (N = 160) showed that 75% of the HCPs reported that they looked at PROM res

Exploring Virtual Reality as an Intervention to Improve Symptom Severity in Hospice-Eligible Patients.

Virtual reality (VR) as an intervention has appeared in the literature and in clinical settings across many different populations. To expand the use of this care option, it is worth considering the ways in which a VR application may benefit individuals with life-limiting illness in hospice and palliative care settings. The incorporation of VR as a therapy option may aid in symptom management and support people nearing the end of life in focusing on aspects of their overall well-being. The goals of this study were to: 1) explore virtual reality as an intervention to improve symptom severity in

Photoantimicrobials--so what's stopping us?

In the face of increasing multi-drug resistant bacteria, both in healthcare and the community, new drugs or novel methods of decontamination are required. The use of light-activated drugs - photoantimicrobials - has been promoted at various points during the past decade or more but has not yet gained meaningful clinical acceptance. Photoantimicrobials offer advantages over conventional drugs in attacking bacteria at various sites, rather than just one, and in the intermediacy of reactive oxygen species, against which there is little possibility of resistance.

[Testicular torsion--a diagnostic dilemma. Diagnosis confirmed by exploration in only 9 per cent of cases, color ultrasound more accurate].

The diagnosis was confirmed in only 9% of 170 patients surgically explored for suspected testicular torsion over a period of 2 years at four Swedish hospitals. In only every second case of testicular torsion was it considered meaningful to leave the testicle in situ. The high number of explorations, where the preoperative suspicion could not be verified, suggests that the preoperative diagnostic work-up should be improved. The use of power or colour ultrasound is suggested as a way of achieving this.

"Reducing my risks": a phenomenon of older widows' lived experience.

While scholars and practitioners often use the concept of "risk" in relation to older persons who live at home, there has been limited exploration of risk from the perspective of older persons themselves. During a phenomenologic study of the lived experience of seven older widows who were living alone at home, the participants articulated the risks they experienced and described the ways in which they attempted to reduce these risks. The results have meaningful implications for home care providers and policy makers.

Overview and comparison of contemporary Society for Vascular Surgery, American Heart Association/American College of Cardiology, and European Society for Vascular Surgery guidelines for the management of patients with intermittent claudication.

Intermittent claudication (IC) is a phenotype of peripheral artery disease that is characterized by pain in the lower extremity muscles during activity that is relieved by rest. Medical management, risk factor control, smoking cessation, and exercise therapy have historically been the mainstays of treatment for IC, but advances in endovascular technology have led to increasing use of peripheral vascular interventions in this patient population. There are meaningful differences in published society guidelines and appropriate use criteria relevant to the management of IC, especially regarding in

The Population Representativeness of Randomized Controlled Trials and Real-World Evidence Studies for Patients With End-Stage Kidney Disease in the United States.

OBJECTIVE: To assess the population representativeness of randomized controlled trials (RCTs) and real-world evidence (RWE) studies in end-stage kidney disease (ESKD) relative to the United States Renal Data System (USRDS) target population. The primary aim was to evaluate the generalizability of US-based evidence, and the secondary aim to examine the transportability of non-US evidence to the US dialysis population. METHODS: We searched PubMed for RCTs and RWE studies in ESKD dialysis populations (2007-2024). Random-effects models were used to pool effect modifiers and mortality rates separa

Therapeutic Protein Drug Interactions: A White Paper From the International Consortium for Innovation and Quality in Pharmaceutical Development.

Typically, therapeutic proteins (TPs) have a low risk for eliciting meaningful drug interactions (DIs). However, there are select instances where TP drug interactions (TP-DIs) of clinical concern can occur. This white paper discusses the various types of TP-DIs involving mechanisms such as changes in disease state, target-mediated drug disposition, neonatal Fc receptor (FcRn), or antidrug antibodies formation. The nature of TP drug interaction being investigated should determine whether the examination is conducted as a standalone TP-DI study in healthy participants, in patients, or assessed v

Development of a Clinical Global Impression of Change (CGI-C) and a Caregiver Global Impression of Change (CaGI-C) measure for ambulant individuals with Duchenne muscular dystrophy.

BACKGROUND: In clinical trials for rare diseases, such as Duchenne muscular dystrophy, clinical outcome assessments (COA) used to assess treatment benefit are often generic and may not be sensitive enough to detect change in specific patient populations. Thus, there is a need for disease specific COAs that track meaningful change among individuals. When developing such measures, input from clinicians, caregivers and patients is critical for assessing clinically relevant concepts and ensuring validity of the measure. METHOD: The aim of this study was to develop two Duchenne-specific global imp
