What Explains Inequalities in Telehealth Utilization Among North Carolina Medicaid Beneficiaries?

Background: Increased availability of telehealth can improve access to health care. However, there is evidence of persistent disparities in telehealth usage, as well as among people from minoritized racial and ethnic groups and rural residents. The objective of our work was to explore the degree to which disparities in telehealth use for behavioral health (BH) and musculoskeletal (MSK) related services during the COVID-19 pandemic are explained by observed beneficiary- and area-level characteristics. Methods: Using North Carolina Medicaid claims data of Medicaid beneficiaries with BH or MSK co

Thirty-Day Hospital Readmission Among Homeless Individuals With Medicaid in Massachusetts.

BACKGROUND: National efforts are underway to reduce hospital readmissions. Few studies have used administrative data to provide a global view of readmission among people experiencing homelessness, who often utilize multiple hospital systems. OBJECTIVE: To examine the 30-day hospital readmission rate and factors associated with readmission following discharge among homeless Medicaid members in Massachusetts. METHODS: We analyzed medical record and Medicaid administrative data for 1269 hospitalizations between 2013 and 2014 for 458 unique patients attributed to Boston Health Care for the Homel

Does Medicaid payer status affect patient's shoulder outcomes after shoulder arthroplasty?

BACKGROUND: Low socioeconomic status and Medicaid insurance as a primary payer have been shown to influence resource utilization and risk-adjusted outcomes for total joint arthroplasty. The purpose of this study was to assess the effects of Medicaid primary payer status on outcomes following shoulder arthroplasty (SA). MATERIALS AND METHODS: A retrospective review of shoulder surgery patients was undertaken to identify a matched cohort of 51 patients who underwent SA and were stratified based on insurance type into two cohorts: 28 Medicaid (M) patients and 23 non-Medicaid (NM) patients. Potentially Problematic Opioid Prescriptions Among Individuals With Private Insurance and Medicaid.

OBJECTIVE: Opioid analgesics can be safe and effective when used properly. Reducing prescriptions that increase adverse outcomes is a focus for addressing the opioid crisis. In this study, the rate of potentially problematic opioid prescriptions was examined over 11 years in a large sample of U.S. patients. METHODS: Claims from the IBM MarketScan commercial database (about 45 million) and multistate Medicaid database (about 7 million) from 2005 to 2015 were used to calculate rates of the following potentially problematic prescription practices: prescriptions for high-dose opioids for 90 days

Clinical Benefits Associated With Medicaid Coverage Before Diagnosis of Gynecologic Cancers.

PURPOSE: Many low-income patients enroll in Medicaid at the time of cancer diagnosis, which improves survival outcomes. Medicaid enrollment before cancer diagnosis may confer additional benefits. Our objective was to compare stage at diagnosis and overall mortality between women with and without Medicaid enrollment before gynecologic cancer diagnosis. METHODS AND MATERIALS: Women younger than 65 years with a gynecologic cancer (2003 to 2008) were identified through the North Carolina Central Cancer Registry and linked to state Medicaid enrollment files. Those with and without Medicaid enrollm

Validity of Primary Immunodeficiency Disease Diagnoses in United States Medicaid Data.

PURPOSE: Primary immunodeficiency diseases (PID) are a rare group of disorders with a wide array of clinical presentations. The absence of validated methods to identify these diseases in electronic databases has limited understanding of their epidemiology and the impact of drug therapies on outcomes. We measured the positive predictive values (PPVs) of ICD-9 diagnoses for identifying PID within US Medicaid. METHODS: We identified Medicaid patients from California, Florida, New York, Ohio, and Pennsylvania with PID ICD-9 diagnoses (common variable immunodeficiency [279.06], X-linked agammaglob

Health care improvement and cost reduction opportunities in hypertensive Medicaid beneficiaries.

Hypertension and its complications are more frequent and occur about a decade earlier in life among high-risk groups, especially in the Southeast. Moreover, socioeconomic status is inversely related to hypertension and cardiovascular complications. Low-income, young and middle-aged adults living in the Southeast may be at especially high risk. Data on inpatient admissions among hypertensive Medicaid beneficiaries living in this region may provide insights on the burden of hypertension-related disease and on opportunities for successful intervention. A study of hospitalization rates and costs a

Underuse of invasive procedures among Medicaid patients with acute myocardial infarction.

OBJECTIVES: The purpose of this study was to determine whether underuse of cardiac procedures among Medicaid patients with acute myocardial infarction is explained by or is independent of fundamental differences in age, race, or sex distribution; income, coexistent illness; or location of care. METHODS: Administrative data from 226 hospitals in New York were examined for 11,579 individuals hospitalized with a primary diagnosis of acute myocardial infarction. Use of various cardiac procedures was compared among Medicaid patients and patients with other forms of insurance. RESULTS: Medicaid pa

Drug prescribing during pregnancy. A controlled study of Tennessee Medicaid recipients.

The effect of pregnancy on prescribing was assessed in a population of pregnant Medicaid recipients and two matched groups of control women. Of the 2,528 gravidas, 62 per cent received systemic legend drugs (excluding dietary supplements) during their pregnancies. White women and women 30 years of age and older were most likely to receive these medications. Systemic anti-infectives were the most frequently prescribed category of drugs (excluding dietary supplements). One fourth of the women received a narcotic-containing drug and 13 percent of the women received psychotropic drugs, most freque

Validation of Medicaid claim-based opioid treatment program retention quality measures.

INTRODUCTION: Understanding the quality of care that opioid treatment programs (OTPs) deliver is a long-standing goal in the United States. In a prior study, this study team developed casemix adjusted OTP-level quality measures of patient retention that revealed a median 180-day OTP retention rate of 26% (25th percentile = 11%; 75th percentile = 37%) in Measurement Year (MY) 2023 (July 1, 2022 - June 30, 2023). The present study evaluated the measures' validity, particularly the association between each OTP's retention and their rate of substance use disorder (SUD) hospitalizations or emergenc

Inpatient utilization among Medicaid members with substance use disorder in Oregon.

BACKGROUND: In the United States, substance use disorders (SUDs) remain a critical public health concern with significant morbidity and mortality. Oregon exemplifies this crisis with high rates of illicit drug use and overdose deaths, compounded by systemic challenges like behavioral health workforce shortages. This study's objective is to characterize patterns of acute care utilization and ED visit and admission reasons among Medicaid enrollees with opioid use disorder, stimulant use disorder, and/or unintentional substance overdoses in the Portland metro area. METHODS: We conducted a cross-

Tobacco cessation treatment in pregnancy: insights from Florida Medicaid claims data.

INTRODUCTION: Tobacco use increases risk for serious pregnancy complications, increasing infant and maternal morbidity and mortality. Cessation treatments are safe during pregnancy, increase the likelihood of successfully quitting, and decrease risk of negative neonatal health outcomes. All Florida Medicaid plans cover evidence-based cessation treatments. This study analyzed Medicaid claims to better understand cessation treatment utilization for pregnant people enrolled in Florida Medicaid. METHODS: We analyzed Florida Medicaid claims from 2018 to 2021 to assess utilization of tobacco cessat

Housing Cost Burden and Outcomes Among Medicaid Beneficiaries With Heart Failure.

IMPORTANCE: Housing cost burden is at an all-time high in the US and may disproportionately affect health outcomes among low-income populations. Medicaid-insured individuals and those diagnosed with cardiovascular (CV) disease, such as heart failure (HF), may be especially at increased risk of adverse health outcomes associated with housing cost burden. OBJECTIVE: To assess the association between area-level housing cost burden and the probability of CV-related hospitalization or emergency department (ED) visits among Medicaid beneficiaries aged 19 to 64 years with HF. Disenrollment Rate For Dual-Eligible Beneficiaries During Medicaid Unwinding, 2023-24.

Twelve million low-income people with Medicare rely on Medicaid for supplemental insurance. We examined Medicaid disenrollment among these dual-eligible beneficiaries after the unwinding of COVID-19 Medicaid maintenance-of-eligibility policies from April 2023 through September 2024. The overall disenrollment rate was 13.9 percent, but it varied widely by beneficiary demographics, state, and baseline coverage characteristics.

Medicaid Retention After Transition to Medicare Among Adults With Expansion Coverage.

IMPORTANCE: When becoming eligible for Medicare, adults previously covered by Medicaid expansion are typically required to meet an asset test for Medicaid coverage of Medicare cost-sharing and/or premiums through the Medicare Savings Program (MSP). It is unclear whether MSP or overall Medicaid coverage is higher in states that waive the MSP asset test than in states that do not. OBJECTIVE: To examine patterns of Medicaid coverage in the first year of Medicare enrollment among beneficiaries who previously enrolled in Medicaid through expansion.
