Reconstruction of the anterior cruciate ligament using a torn meniscus.

In eighty of 100 patients, reconstruction of the anterior cruciate ligament using a torn meniscus was successful in restoring stability. The length of follow-up ranged from two to six years and the mean interval from injury to operation was two years. Preoperatively, all but one patient had a positive (2+ or 3+) Lachman test and a positive pivot-shift test. Only ten patients had evidence of major instability, as seen on either test. The result was the same regardless of whether the patient had had a concomitant extra-articular (Ellison) procedure. The results of arthroscopic biopsy in eleven p

Sideways dislocation of the temporomandibular joint meniscus: the edge sign.

Seventy temporomandibular joint arthrograms of 49 patients with symptoms of internal joint derangement were reviewed. Twenty-three of these arthrograms (33%) were abnormal but atypical for anteriorly dislocated meniscus. The lower temporomandibular joint space had a well-defined edge located inferiorly, which was attributed to pressure from a sideways-dislocated meniscus. We called this appearance the "edge sign." Seven patients with radiologically apparent sideways dislocation were operated on. The meniscus was dislocated laterally in six and medially in one. The incidence of irreducibility a

Arthroscopic meniscus repair: a safe approach to the posterior horns.

Seventy arthroscopic meniscus repairs were performed using a technique that places posterior horn sutures through spinal needles from safe posterior corner punctures, thus avoiding injury to the posterior neurovascular structures. Follow-up averaged 18 months, with a range from 12 to 28 months. Associated anterior cruciate ligament instability (31%) was either stabilized surgically (13%) or braced postoperatively for pivoting activity (18%). Clinical results were excellent in 98.6% of patients, with only one second tear (1.5%) occurring 2 months postoperatively. Complications (2.8%) included o

Role of the meniscus in the etiology of posttraumatic temporomandibular joint ankylosis.

From an analysis of the factors that could contribute to the development of post-traumatic TMJ ankylosis, it is evident that a number of different conditions are probably operant. Based on clinical and experimental findings, however, it is suggested that of primary importance is the location of the meniscus in relation to the fracture site. It is proposed that the meniscus normally serves as a barrier to prevent fusion of the distal fragment with the glenoid fossa, and that ankylosis generally occurs if this relationship is not maintained. Based on this concept, possible modifications in the m

Effects of Preservation on the Compressive Properties and Microstructure of the Knee Meniscus.

PURPOSE: Meniscus transplantations commonly use tissue preserved by freezing or fresh storage. However, the mechanisms by which preservation affects mechanical properties are not well understood. The objective of the present study was to determine whether structural and mechanical properties of menisci differ after being preserved by freezing or prolonged fresh storage. Preservation is hypothesized to negatively affect the elastic and viscoelastic compressive properties of the meniscus through structural void formation. METHODS: Medial and lateral porcine menisci were obtained on the day of s

Optimization of Arthroscopic All-Inside Meniscus Repair With a Probe.

Meniscal preservation is essential for knee function and long-term joint health. All-inside meniscus repair achieves outcomes comparable to the inside-out technique, but technical challenges remain, including limited working space, suture cut-out in degenerative tissue, unfavorable suture angles, and risk of cartilage injury or arthroscope entanglement. The arthroscopic probe is used to assess tear pattern, tissue quality, stability, and working space, guiding portal selection or alternative approaches. Twirling the suture along the probe allows controlled loop positioning and tightening. Two

Congenital Anomalies of the Knee-Hypoplasia of the Meniscus: A Case Report.

BACKGROUND: Congenital anomalies involving the meniscus and anterior cruciate ligament (ACL) are rare and often discovered incidentally. Discoid meniscus is the most frequently reported congenital variant, whereas medial meniscal hypoplasia and ACL agenesis remain exceptionally uncommon. CASE PRESENTATION: A 16-year-old male presented with right knee pain, swelling, and instability following a noncontact basketball injury. He reported a similar episode weeks earlier. Clinical examination revealed anterior laxity, and magnetic resonance imaging (MRI) demonstrated a nonvisualized ACL consistent

The Interplay of Biomechanical and Biological Changes Following Meniscus Injury.

PURPOSE OF REVIEW: Meniscus injury often leads to joint degeneration and post-traumatic osteoarthritis (PTOA) development. Therefore, the purpose of this review is to outline the current understanding of biomechanical and biological repercussions following meniscus injury and how these changes impact meniscus repair and PTOA development. Moreover, we identify key gaps in knowledge that must be further investigated to improve meniscus healing and prevent PTOA. RECENT FINDINGS: Following meniscus injury, both biomechanical and biological alterations frequently occur in multiple tissues in the j

Biomaterials for meniscus and cartilage in knee surgery: state of the art.

Meniscus and cartilage injuries of the knee joint lead to cartilage degeneration and osteoarthritis (OA). The research on biomaterials and artificial implants as substitutes in reconstruction and regeneration has become a main international focus in order to solve clinical problems such as irreparable meniscus injury, postmeniscectomy syndrome, osteochondral lesions and generalised chronic OA. In this review, we provide a summary of biomaterials currently used in clinical practice as well as state-of-the-art tissue engineering strategies and technologies that are developed for articular cartil

The knee meniscus: management of traumatic tears and degenerative lesions.

Meniscectomy is one of the most popular orthopaedic procedures, but long-term results are not entirely satisfactory and the concept of meniscal preservation has therefore progressed over the years. However, the meniscectomy rate remains too high even though robust scientific publications indicate the value of meniscal repair or non-removal in traumatic tears and non-operative treatment rather than meniscectomy in degenerative meniscal lesionsIn traumatic tears, the first-line choice is repair or non-removal. Longitudinal vertical tears are a proper indication for repair, especially in the red-

[MALDI mass spectrometry of the meniscus. Objectification of morphological findings].

BACKGROUND: The classification of meniscal lesions requires correlation with clinical data. For the standardization of histopathology reports a discrimination between normal, low-grade lesions and high-grade lesions is feasible. This classification can be further specified using other methods. MATERIAL AND METHODS: Formalin-fixed, paraffin-embedded specimens of meniscal tissue from 68 patients were analyzed by matrix-assisted laser desorption ionization (MALDI) imaging. RESULTS: The classification of meniscal lesions and differentiation between low-grade and high-grade and acute versus non-a

Posterior dislocation of a discoid meniscus in a child: a case report.

Discoid meniscus of the knee is a well-known anatomic and congenital variant of the lateral meniscus. It is often asymptomatic, but can be associated with knee pain and other symptoms. Posterior dislocation of the discoid meniscus is an extremely rare finding; to the best of our knowledge, only one case has been reported in the literature. Here, we report a case of 10-year-old girl who presented with right knee pain with no history of trauma. The radiograph of her right knee was negative but MRI showed bilateral lateral discoid menisci with posterior dislocation in the right knee and some subl

Tensile forces on sutures in the human lateral knee meniscus.

Tensile strength is the most often reported parameter in biomechanical investigations of meniscal repair techniques. However, the magnitude of the tensile forces that actually occur on repaired lesions is not clear. The purpose of this study was to investigate if tensile forces occur on repaired lateral meniscal lesions, which could exceed the failure strength of common repair techniques. In human knees (n = 6), vertical-longitudinal lesions 25 mm in length were created in the posterior horn of the lateral meniscus at a distance of 3 mm from the meniscosynovial junction and the popliteal hiatu

Congenital absence of the medial meniscus associated with lipoma arborescens.

Congenital abnormalities of the medial meniscus are extremely rare and have been reported commonly with other deformities. We report an isolated aplasia of the medial meniscus. A 37-year-old man presented with a slow-growing painless swelling, accompanied by intermittent effusion of his left knee. Magnetic resonance imaging demonstrated frond-like proliferations of fatty synovium. He was initially diagnosed with synovial chondromatosis, but later found to have lipoma arborescens. While an arthroscopic synovectomy was being performed, congenital absence of the medial meniscus was discovered as
