Riolan's arch: confusing, misnomer, and obsolete. A literature survey of the connection(s) between the superior and inferior mesenteric arteries.

BACKGROUND: There are 2 interpretations of Riolan's arch: (1) Riolan's arch is identical to a central part of the marginal artery (MA), connecting the superior (SMA) and the inferior mesenteric (IMA) arteries; and (2) Riolan's arch represents a rare artery, connecting the SMA and the IMA. The current review aims to emphasize the clinical importance of the colon's vasculature and to show the feasibility of abolishing the terms "Riolan's arch" and "meandering mesenteric artery." METHODS: A literature survey was performed. RESULTS: It appears that no distinct identity can be ascribed to Riolan'

Distance of Peritoneum to Inferior Mesenteric Artery Predicts the Operation Time During Laparoscopic Colectomy for Sigmoid or Rectosigmoid Colon Cancer.

BACKGROUND/AIM: Obesity is a major technical limiting factor for laparoscopic surgery because abundant visceral fat is known to extend the operation time. However, special hardware is needed to assess it. We hypothesized that the depth from the peritoneum to the bifurcation of the inferior mesenteric artery (IMA) defined as 'peritoneum to IMA distance (PID)' might be a simple predictive factor for extended operation time during laparoscopic colectomy. PATIENTS AND METHODS: One hundred twenty-four patients who were diagnosed with sigmoid or rectosigmoid colon cancer and underwent laparoscopic

Endovascular aneurysm repair with inferior mesenteric artery chimney in a high-risk patient with abdominal aortic aneurysm and iliac occlusion.

A 63-year-old man presented for the treatment of abdominal aortic aneurysm in the setting of bilateral internal iliac artery compromise from prior peripheral arterial disease treatments. The inferior mesenteric artery (IMA) measured 5 mm. Patient underwent coronary artery stenting 6 months prior and experienced left leg claudication. He underwent endovascular aneurysm repair with chimney IMA grafting and a femorofemoral bypass with uneventful recovery. At 1 year, computed tomography angiogram shows no flow in the aneurysm sac, and his left leg claudication resolved. Endovascular aneurysm repai

Indocyanine green fluorescence-guided imaging enhances the efficiency and accuracy of inferior mesenteric artery lymph node dissection in rectal cancer.

BACKGROUND: Indocyanine green fluorescence-guided imaging (ICG-FGI) enhances surgical precision in rectal cancer procedures; however, it is technically demanding. This study evaluated the effects of ICG-FGI on the efficacy of lymphadenectomy during radical rectal cancer surgery. METHODS: This multi-center retrospective cohort study analyzed patients who underwent laparoscopic radical resection for rectal cancer between January 2021 and December 2024. The participants were stratified into two cohorts (ICG and non-ICG groups) using 1:1 propensity score matching to ensure balanced baseline chara

Incomplete lymphatic sealing around the inferior mesenteric artery is a risk factor for chylous ascites in robotic rectal cancer surgery.

INTRODUCTION: Compared with laparoscopic surgery (LS), robotic surgery (RS) is considered to have acceptable outcomes in rectal cancer, but few reports have focused on chylous ascites in RS. The aim of this study was to investigate the incidence and etiology of chylous ascites after RS. METHODS: This retrospective study included 291 patients with rectal cancer who underwent RS (n = 165) or LS (n = 126) with high ligation of the inferior mesenteric artery (IMA). Propensity score matching (PSM) was performed to compare the two groups. RESULTS: \Dissection around the IMA was achieved using ultr

Laparoscopic Clipping of the Inferior Mesenteric Artery and Intraoperative Indocyanine Green Angiography for Type II Endoleak Following Endovascular Aneurysm Repair.

Background: Type II endoleaks from a patent inferior mesenteric artery (IMA) occur in up to one-third of patients undergoing endovascular repair of abdominal aortic aneurysms. In the majority of patients, retrograde flow in the aneurysmal sac outside the endograft will seal over time and is rarely associated with sac enlargement or aortic rupture. Intervention is generally recommended in patients with progressively enlarging endoleaks, especially when the sac diameter increases >10 mm during the follow-up, and endovascular IMA embolization has a high rate of treatment failure. Methods: We repo

Total Laparoscopic Inferior Mesenteric Artery Clipping for Persistent Type II Endoleak Following EVAR: A Case Report and Review of Surgical Strategy.

BackgroundType II endoleak is the most common complication following endovascular aneurysm repair (EVAR), typically due to retrograde flow from collateral arteries, most notably the inferior mesenteric artery (IMA). Although usually low-pressure, persistent leaks associated with sac expansion warrant intervention.Case PresentationWe describe an 87-year-old male presenting with progressive aneurysm sac enlargement due to a persistent Type II endoleak post-EVAR. Initial management with intra-sac thrombin injection. Subsequently, a total laparoscopic IMA clipping was performed with intraoperative

Prognostic impact of low versus high inferior mesenteric artery ligation in rectosigmoid cancer: individual patient data meta-analysis of randomized trials.

BACKGROUND: The optimal level of inferior mesenteric artery ligation in rectosigmoid cancer surgery remains a matter of debate. This study evaluated the prognostic impact of low ligation (LL) versus high ligation (HL) after rectal or sigmoid cancer resection. METHODS: A reconstructed individual patient data meta-analysis of randomized clinical trials (RCTs) was performed using restricted mean survival time difference (RMSTD) estimation. PubMed, MEDLINE, Scopus, Web of Science, ClinicalTrials.gov, Cochrane Central Library, and Google Scholar were searched. Primary outcomes were 5-year overall

Preservation of inferior mesenteric artery reduces short- and long-term defecatory dysfunction after laparoscopic colorectal resection for diverticular disease: An RCT.

BACKGROUND: The quality of life after surgery for diverticular disease is mainly linked to the presence and severity of postoperative defecatory disorders. These symptoms are frequently related to the sigmoid and rectal denervation following the arterial ligation. The preservation of Inferior Mesenteric Artery performing colorectal resections seems to reduce denervation, which led to a better defecatory function. This study aimed to assessing the efficacy of inferior mesenteric artery preservation following colonic resections for diverticular disease in terms of reduced defecatory disorders an

[Inferior Mesenteric Artery Embolization with N-Butyl Cyanoacrylate for Life-Threatening Postpartum Hemorrhage: A Report of Two Cases and Literature Review].

The inferior mesenteric artery is a rare source of postpartum hemorrhage. We report two cases of primary postpartum hemorrhage that originated from the inferior mesenteric artery after vaginal delivery. Both patients showed signs of hypovolemic shock, and disseminated intravascular coagulation was suspected. The bleeding continued even after embolization of the uterine artery, a typical source of postpartum hemorrhage. Inferior mesenteric arteriography confirmed contrast extravasation from the superior rectal artery, and selective embolization was performed using N-butyl cyanoacrylate. This re
