Cross-Sectional Imaging Evaluation of Vascular Lesions in the Gastrointestinal Tract and Mesentery.

Gastrointestinal (GI) tract and mesenteric vascular lesions can have various clinical presentations, of which GI bleeding is the most common. This collection of pathology is highly variable in etiology ranging from occlusive disease to vascular malformations to trauma to neoplasms which makes for a challenging workup and diagnosis. The advent of multiple imaging modalities and endoscopic techniques makes the diagnosis of these lesions more achievable, and familiarity with their various imaging findings can have a significant impact on patient management. In this article, we review the gamut of

[Detection of complete common mesentery on CT scan for perforation in acute appendicitis].

Complete common mesentery is a rare abnormality in midgut rotation. Its detection is exceptional during adulthood because, during this period, is very often asymptomatic and therefore not diagnosed. Complete common mesentery may be discovered incidentally in patients with ectopic appendicular syndrome, as in our case study. We here report the case of a 42-year old patient presenting with acute clinico-biological pelvic inflammatory disorder. CT scan allowed the diagnosis of complete common mesentery associated with acute appendicitis. Laparotomy confirmed the diagnosis of perforated acute appe

Lymphangioma of the jejunal mesentery and jejunal polyps presenting as an acute abdomen in a teenager.

Cystic lymphangioma of the small bowel mesentery is a rare clinical entity, especially after childhood. Medical literature reveals a limited number of such cases presenting as acute abdomen due to bowel obstruction, small bowel volvulus and bleeding into the tumour. We present the management experience of an 18-year-old woman who presented with rapid onset diffuse peritonism and raised inflammatory markers. Computed tomography showed a mass in the small bowel mesentery with suspicion of segmental bowel ischaemia. Emergency laparotomy revealed a mass in the mid-jejunal mesentery close to the bo

Clinicopathological and treatment analysis of 13 extragastrointestinal stromal tumors of mesentery and retroperitoneum.

BACKGROUND: Extragastrointestinal stromal tumors (EGISTs) are extremely rare mesenchymal tumors histologically and immunophenotypically similar to GI stromal tumors (GISTs). The aim of this study was to analyze the clinicopathological factors and treatment outcome in 13 patients with EGISTs treated at a tertiary care center. METHODS: Of 109 patients with GISTs treated at our center between April 2002 and December 2012, 13 patients with EGISTs were analyzed for clinicopathological factors and treatment outcome. RESULTS: Mean age was 45.8 (range 30-61) years, and females constituted 62% with a

Navigating the mesentery: a comparative pre- and per-operative visualization of the vascular anatomy.

AIM: Awareness of anatomy is critical for performing safe surgery within the root of the mesentery. Our aim was to investigate the anatomical relationship between the superior mesenteric artery (SMA) and vein (SMV) and their branches within a predefined D3 area of the right colon and to compare preoperatively established three-dimensional (3D) mesenteric vessel anatomy from CT with that found at surgery. METHOD: Prospective data were collected on 139 patients included in the 'Safe Radical D3 Right Hemicolectomy for Cancer Through Preoperative Biphasic Multi-detector Computed Tomography (MDCT)

Primary and secondary disease of the peritoneum and mesentery: review of anatomy and imaging features.

The largest and most complex serosal membrane in the body, the peritoneum, lines the abdominal cavity, and the abdominopelvic viscera. It is frequently involved in a variety of benign and malignant processes. While secondary involvement of the peritoneum is more common, primary tumors can be a diagnostic challenge. Knowledge of the anatomy is crucial in understanding the various pathologic processes. Cross-sectional imaging plays an important role in diagnosing and evaluating the extent of the disease processes. This article reviews the imaging anatomy of the peritoneum and mesentery and the c

A hemangioma of the sigmoid colon mesentery presenting as a retroperitonealtumor: a case report and review.

Hemangiomas of the gastrointestinal tract and mesentery are uncommon benign vascular lesions. While spontaneous bleeding is the hallmark of the gastrointestinal tumor variant, clinical signs of mesenteric hemangiomas are mostly unspecific. Despite the increasing imaging quality of computerized tomography (CT), in most cases the final diagnosis is established through surgery and histopathologic analysis of a macrobiopsy. We present a case report of a 20-year-old female patient who was admitted with progressive abdominal distension and suffered from persistent abdominal pain for 3 months. A large

The mesenterially perfused rat small intestine: A versatile approach for pharmacological testings.

Pharmaceutical compounds enter the body via several major natural gateways; i.e. the lung, the skin and the gastrointestinal tract. Drug application during surgical operations can lead to severe impairment of gastrointestinal motility, which can contribute to a paralytic ileus. Here we investigated an ex vivo perfused small intestine model that allows us to ascertain the influence of pharmaceuticals upon the gut. Corresponding segments from the proximal jejunum of adult rats were used. Their mesenteric arteries and veins were cannulated and the jejunal segment excised. The individual segments

[Desmoid tumor of the mesentery secondary to colectomy. An exceptional cause of ureteric obstruction].

Desmoid tumours are rare lesions with a local invasive potential and a risk of recurrence, considered to be benign due to the absence of metastases. They are classified as fibromatoses and may be associated with Gardner's syndrome. The authors report an unusual case, in a 33-year-old woman, of a desmoid tumour invading the right ureter with upper tract dilatation. Etiologic factors (traumatic, hormonal, auto-immune...) are discussed, together with the treatment of choice to lower the risk of recurrence.

Functional electrical properties of the endothelium in lymphatic vessels of the guinea-pig mesentery.

1. The resting and agonist-stimulated properties of endothelial cells and electrical communication between the endothelium and smooth muscle were investigated in open segments of guinea-pig mesenteric lymphatic vessels using intracellular microelectrodes. 2. Endothelial cells had a mean resting membrane potential (RMP) of -71.5 +/- 0.5 mV (n = 100) which was significantly different from the value of -60.8 +/- 1.1 mV (n = 75) recorded in smooth muscle. 3. Acetylcholine (ACh, 5-10 microM) generally evoked an initial hyperpolarization followed by depolarization (mean 3.4 +/- 0.5 mV and 15.4 +/- 1

Acute small intestinal injury associated with hematomas in the mesentery of four horses.

Four horses were evaluated for clinical signs consistent with small intestinal disease. During exploratory surgery, primary hematomas in the mesentery were found in 2 horses. The third horse developed a hematoma secondary to a mesenteric rent, whereas in the fourth horse, a hematoma developed secondary to strangulation of the bowel by a mesenteric lipoma. Two horses were euthanatized at surgery, because the extensive nature of the hematoma precluded ligation and there was substantial risk of continued hemorrhage postoperatively. The remaining horses recovered from surgery but developed complic

Morphological aspects and observations about the permeability of the rat mesentery to the osmium-amine.

In a previous work we felt that many authors were interested in observing the permeability and structure of the mesentery employing many techniques and analysing the material from different points of view. Our collaboration, at that time, was limited to the study of the permeability of the mesentery to the lanthanum nitrate and we came to a final conclusion that the penetration of the heavy salt is impeded by the presence of a basement membrane, localized between the mesothelial and connective tissue layers and that it was continuous to the basal lamina, located between neighbouring mesothelia

Heterotopic ossification of the intestinal mesentery. Report of a case following intraabdominal surgery.

In a 55-year-old male a nodular mass of heterotopic ossification developed in the intestinal mesentery two weeks after coloproctectomy for severe ulcerative colitis. The lesion was located close to the ileostoma where it encroached upon the intestinal wall and led to luminal obstruction. Histological examination of the tumour mass showed actively proliferating mesenchymal cells forming irregular areas of primitive osteoid and bone. The process appeared consistent with an early stage of heterotopic ossification where the short history explains the absence of the "zone phenomenon" usually charac

Case Report: Multiple extragenital adenomatoid tumors in the rectum and small intestinal mesentery.

Adenomatoid tumors (ATs) are benign tumors commonly found in the genital organs, usually occurring as a single mass with a good prognosis after surgery. However, multiple extragenital ATs are extremely rare, often asymptomatic, and may lead to misdiagnosis. This case report describes a patient with multiple ATs involving the rectum and small intestinal mesentery. The patient was a 39-year-old man who had been suffering from abdominal pain and recurrent loose stools for 6 months. Diagnostic contrast-enhanced computed tomography (DCE-CT) of the abdomen and pelvis showed multiple cystic-solid mas

Imaging features of cecal volvulus complicating a complete common mesentery: A case report.

Cecal volvulus is a rare but serious complication that can occur in association with anomalies of intestinal rotation, particularly in the setting of a complete common mesentery. This congenital anomaly results from improper embryologic rotation of the primitive intestinal loop, leading to abnormal bowel mobility and a predisposition to volvulus. We report the case of a 43-year-old man with a history of goiter, who was admitted for evaluation of a 5-day history of bowel obstruction, marked by cessation of stool and gas passage, persistent vomiting, and unquantified weight loss. Cross-sectional
