Primary pulmonary artery synovial sarcoma: a case report.

BACKGROUND: Primary pulmonary artery synovial sarcoma (PPASS) is an extremely rare cardiac malignancy, often misdiagnosed preoperatively due to its nonspecific clinical and imaging features. This case report aims to enhance diagnostic recognition of this rare entity. CASE DESCRIPTION: A 35-year-old male presented with chest pain after activity for 1 month. Physical examination revealed stable vital signs and no significant cardiac murmurs. Laboratory tests showed negative immunofixation electrophoresis, unremarkable urinary light chains and coagulation function, and an N-terminal pro-B-type n

Diagnostic challenges in synovial sarcoma: a case report.

BACKGROUND: Synovial sarcoma (SS) is a rare, aggressive malignant neoplasm with a high rate of metastasis despite its indolent nature. A distinct SS18::SSX fusion oncogene characterizes SS. The monophasic subtype is the most common and consists of spindle cells. Due to its rarity and subtle clinical presentation, SS can be challenging to identify, leading to misdiagnosis or delays in treatment. The present case highlights the importance of awareness and vigilance among healthcare providers when encountering slow-growing, seemingly benign soft-tissue tumors. CASE PRESENTATION: A 43-year-old Ha

Excellent real-world outcomes of adults with Burkitt lymphoma treated with CODOX-M/IVAC plus or minus rituximab.

Treatment of Burkitt lymphoma (BL) with intensive, multi-agent chemotherapy with aggressive central nervous system (CNS) prophylaxis results in high cure rates, although no regimen is standard of care. We examined population-based survival outcomes of adults with BL treated with a modified combination of cyclophosphamide, vincristine, doxorubicin, prednisone and systemic high-dose methotrexate (MTX) (CODOX-M) with IVAC (ifosfamide, mesna, etoposide, cytarabine and intrathecal MTX) (CODOX-M/IVAC) ± rituximab over a 15-year period in British Columbia. For the 81 patients identified (including 8

Acute lymphoblastic leukemia arising after treatment of Ewing sarcoma was misdiagnosed as bone marrow metastasis of Ewing sarcoma: A case report.

RATIONALE: Both acute lymphoblastic leukemia (ALL) and Ewing sarcoma (ES) are small round cell tumors, and it is difficult to differential diagnose them because of overlapping clinical, radiographic, histologic, and immunophenotypic features. PATIENT'S CONCERNS: A 5-year-old boy was admitted to our hospital because of pains in his left leg without obvious inducement and lameness worsening with walking over a two 2-month period. DIAGNOSES: Based on the comprehensive analysis of radiography, magnetic resonance imaging (MRI), pathology biopsy and immunohistochemistry, the lesion was confirmed t

Off-label drugs in otolaryngological practice against the background of legal conditions of Polish legislation.

Introduction: 'Off-label drug use' refers to the administration of drugs for unapproved indications or age groups, a different dosage or other form of administration. Considering the legal issues, there clearly exists a need to implement rules that would regulate the use of pharmaceuticals outside the scope of a marketing authorisation. The brevity and diversity of Polish laws in the field of health care leads to many interpretative doubts associated with particular legal acts.

Aim: We aimed to present clinical examples from

The utility of the CADISS® system in laparoscopic cholecystectomy for acute cholecystitis.

BACKGROUND: The CADISS® system combines the use of a topical formulation of mesna (sodium 2-Mercaptoethane sulfonate) to facilitate selective dissection of pathological and fibrotic tissues with a non-cutting instrument for a safer dissection. A prospective, multi-site, clinical trial was performed to explore the use of the CADISS® system in laparoscopic cholecystectomy for acute cholecystitis. METHODS: A total of 15 patients were enrolled at different severity stages of pathology according to Tokyo classification. They were operated on prior to, or after 72 h of, the onset of symptoms. Nursing Management of Haemorrhagic Cystitis in Patients Undergoing Haematopoietic Stem Cell Transplantation: a Multicentre Italian Survey.

BACKGROUND: Haemorrhagic cystitis (HC) is a severe complication occurring after haematopoietic stem cell transplantation (HSCT) in 13-40% of patients, caused by infectious and/or non-infectious factors that increase the in-hospital length of stay and the risk of mortality of transplanted recipients. Although different management interventions have been suggested in the literature, available knowledge on interventions performed by Italian nurses in their daily practices has not been documented to date. AIM OF THE STUDY: The aim of this study is to describe HC preventive and treatment intervent

Increased sensitivity of glutathione S-transferase P-null mice to cyclophosphamide-induced urinary bladder toxicity.

Hemorrhagic cystitis and diffuse inflammation of the bladder, common side effects of cyclophosphamide (CY) treatment, have been linked to the generation of acrolein derived from CY metabolism. Metabolic removal of acrolein involves multiple pathways, which include reduction, oxidation, and conjugation with glutathione. Herein, we tested the hypothesis that glutathione S-transferase P (GSTP), the GST isoform that displays high catalytic efficiency with acrolein, protects against CY-induced urotoxicity by detoxifying acrolein. Treatment of wild-type (WT) and mGstP1/P2 null (GSTP-null) mice with

The importance of surgery as the first step in multimodality treatment of small cell bronchial carcinoma. The ISC Lung Cancer Study Group.

For patients with small cell lung cancer (SCLC) in their early stages (TNM I, II), surgery for cure was used to eliminate the primary tumour and its regional lymph-nodes followed by intermittent chemotherapy and radiotherapy within the first six postoperative months. After the pathohistological examination of the operation-specimen a two-arm-randomization was performed: standard chemotherapy (1000 mg/m2 cyclophosphamide, 50 mg/m2 doxorubicin, 1.4 mg/m2 vincristine) compared with sequential chemotherapy using three different drug-combinations (A: 1500 mg/m2 cyclophosphamide, 100 mg/m2 lomustine

Signal Mining and Analysis of Drug-Induced Myelosuppression: A Real-World Study From FAERS.

IntroductionDrug-induced myelosuppression (DIM) is a serious side effect of several medications, particularly chemotherapy, immunosuppressants, and targeted therapies, which can lead to infections, anemia, and bleeding. While these drugs are effective, their adverse effects can disrupt treatment plans and reduce quality of life. However, early identification of DIM remains challenging, as many associated drugs do not explicitly list this risk, complicating clinical monitoring.MethodsThis study utilized the FDA Adverse Event Reporting System (FAERS) database to perform signal mining and assess

Intrathoracic Extrapulmonary Pleomorphic Liposarcoma: A Rare Presentation.

Liposarcoma (LPS) is a malignant mesenchymal neoplasm originating from adipocytes. LPS comprises several histological variants, among which pleomorphic liposarcoma (PL) constitutes a rare and highly aggressive subtype. PL is most commonly found in the extremities and presents primarily in individuals aged 50 and above. Primary thoracic presentations of PL are exceedingly rare, particularly in young adults, and are often associated with metastases to the lungs. This case reports a 28-year-old white male with no significant past medical history presenting with shortness of breath, left posterior

Ex vivo depletion of T cells from bone marrow grafts with CAMPATH-1 in acute leukemia: graft-versus-host disease and graft-versus-leukemia effect.

BACKGROUND: Preventing graft-versus-host disease (GVHD) by depletion of T lymphocytes from the stem cell graft for transplantation remains controversial, mainly because of the perceived increase in disease recurrence. METHODS: We retrospectively analyzed the outcome of 50 consecutive individuals in remission of acute lymphoblastic leukemia (n=13; 8 in complete remission [CR]1) or acute myeloblastic leukemia (n=37; 33 in CR1), who had received marrow grafts from HLA-identical siblings. The conditioning regimen included six 2-Gy fractions of total body irradiation, succeeded by cyclophosphamide

Chemoselective protein and peptide immobilization on biosensor surfaces.

Site-specific immobilization of proteins and peptides on a sensor surface represents a significant challenge for bioanalytical applications such as surface plasmon resonance (SPR). The most common protocols for covalent protein immobilization usually result in heterogeneous presentation of the ligand at the surface, which can in some instances yield conflicting results with analogous data obtained in solution. Here, we discuss two complementary and generic bioconjugation methods that allow chemoselective immobilization of peptides and proteins via either their C-terminus (native chemical ligat

Phase II study of induction chemotherapy with paclitaxel, ifosfamide, and carboplatin (TIC) for patients with locally advanced squamous cell carcinoma of the head and neck.

BACKGROUND: This Phase II trial was conducted to determine the response rate, particularly of the primary sites, tolerability, and toxicity of induction chemotherapy of paclitaxel, ifosfamide, and carboplatin for patients with previously untreated locally advanced squamous cell carcinoma of the head and neck (SCCHN). We also hypothesized that improved complete response (CR) rates with the induction chemotherapy may render better survival rates with subsequently delivered definitive local treatment. METHODS: All eligible patients with locally advanced SCCHN received two courses of induction ch
