Indigenous Peoples' responses to evacuation for birth in Ontario: conceptualizing risk through an Indigenous midwifery-led approach.

BACKGROUND: Currently, pregnant Indigenous Peoples living in remote, rural, and northern Indigenous communities in Canada are subjected to evacuation birth policy, whereby they are evacuated out of their community to large, urban hospitals to give birth. Evacuation for birth is assumed to decrease biomedical risk because people are birthing in hospitals. In Canadian health systems, evaluating and mitigating biomedical risk has become a standard in health decision-making but this framework disregards Indigenous ontologies and epistemologies that guide Indigenous people in their evaluation of he

Why are women deciding against birth in alongside midwifery units? A prospective single-center study from Germany.

OBJECTIVES: For healthy women entering labor after an uneventful pregnancy, advantages of birth in midwife-led models of care have been demonstrated. We aimed to study the level of awareness regarding care in alongside midwifery units (AMU), factors involved in the decision for birth in obstetrician-led units (OLU), and wishes for care and concerns about birth in women registering for birth in OLU who would have been eligible for care in AMU. METHODS: Healthy women with a term singleton cephalic fetus after an uneventful pregnancy course booking for birth in OLU were prospectively recruited.

Effect of midwifery students' continuity of care program on women's experiences of maternity care: A randomised controlled trial.

BACKGROUND: Midwifery continuity of care (CoC) benefits women and families. Engaging midwifery students may help extend such care and improve access to quality maternity services. This study evaluated the impact of a student-led CoC on women's maternity care experiences during childbirth in a middle-income country. No trials to date have assessed these effects. METHODS: We conducted a randomized controlled trial with low-risk pregnant women at 26-29 weeks of gestation, intending vaginal delivery. Participants were randomized to intervention (n = 45) or control (n = 48) groups through stratifi

Systemic and political determinants of the global nursing and midwifery shortages: A discussion based on human resources for health data.

BACKGROUND: Nurses and midwives constitute a substantial proportion of the global health workforce and are essential to universal health coverage, maternal and newborn health, and health-system resilience. Despite workforce expansion, persistent shortages, maldistribution, gender inequities, poor working conditions, leadership gaps, and migration pressures continue to undermine both professions. The Global Burden of Disease 1990-2023 Human Resources for Health estimates provide an opportunity to examine these challenges across three decades. AIM: This discussion paper reframes global nursing

A comprehensive systematic review of the use of simulation in the continuing education and training of qualified medical, nursing and midwifery staff.

BACKGROUND: Simulation can be defined as a person, device or set of conditions made to resemble a real life situation. It is used in many high-risk industries particularly when reality is dangerous, critical events are rare and errors are costly in human and/or financial terms. The use of simulation in the UK is now considered an essential component of education programmes designed for healthcare practitioners. However the use of simulation in undergraduate education has been studied in depth but little is known about its use in postgraduate education. OBJECTIVE: The aim of this systematic re

Midwifery care measures in the second stage of labor and reduction of genital tract trauma at birth: a randomized trial.

Genital tract trauma after spontaneous vaginal childbirth is common, and evidence-based prevention measures have not been identified beyond minimizing the use of episiotomy. This study randomized 1211 healthy women in midwifery care at the University of New Mexico teaching hospital to 1 of 3 care measures late in the second stage of labor: 1) warm compresses to the perineal area, 2) massage with lubricant, or 3) no touching of the perineum until crowning of the infant's head. The purpose was to assess whether any of these measures was associated with lower levels of obstetric trauma. After eac

Greener beginnings: Exploring the carbon footprints of midwifery group practice and standard maternity care in regional Australia.

BACKGROUND: Climate change is an escalating concern for maternal and newborn health. Maternity care, particularly through high-intervention, hospital-based practices, generates substantial environmental waste. Midwifery Continuity of Care (MCC) models are known to reduce unnecessary interventions and improve safety and satisfaction of care, whilst suggesting potential reduction in environmental impacts. However, limited evidence exists evaluating the carbon footprint of MCC compared with standard medically led care. AIM: To evaluate whether a Midwifery Group Practice (MGP) model generates low
